The revelation that data obtained for the US-based National Surgical Adjuvant Breast and Bowel Project (NSABP) from subjects enrolled at Hôpital Saint-Luc in Montreal was falsified has eroded public trust in research. Institutions can educate researchers and help prevent unethical research practices by establishing procedures to monitor research involving human subjects. Research monitoring encompasses four categories of activity: annual reviews of continuing research, monitoring of informed consent, monitoring of adherence to approved protocols and monitoring of the integrity of data. The (...) authors describe characteristics of research projects that may call for monitoring procedures in each category. The form taken by such monitoring depends on the nature of the protocol. Although appropriate research monitoring requires substantial investment of personnel and financial resources, it is required under guidelines regulating research involving human subjects in Canada. Research monitoring is a step forward in re-establishing public confidence in medical research. (shrink)

OBJECTIVE: To investigate whether eligibility criteria that exclude the elderly, persons with psychiatric disease, and persons with substance abuse problems from participation in randomized controlled trials (RCTs) are subjective and hence a source of variability in enrolment decisions and investigator uncertainty. DESIGN: Survey questionnaire. PARTICIPANTS: Cancer investigators from the United States and Canada. INTERVENTIONS: Investigators were presented with clinical vignettes from 3 patient categories--eligible, ineligible and uncertain--for each of 5 eligibility criteria--3 subjective and 2 objective--and were asked whether they would (...) enrol the patient in a trial and how sure they were of this decision. Demographic characteristics of the investigators were also collected. OUTCOME MEASURES: The difference in enrolment decisions between subjective and objective criteria, and the difference in the certainty associated with these decisions. RESULTS: Of 365 questionnaires sent out, 224 usable ones were returned. Compared with the objective criteria, the subjective criteria were associated with more variable enrolment decisions (p = 0.07 for the "eligible" scenario and p = 0.0001 for the "ineligible" and "uncertain" scenarios), and investigators were less sure about the decisions they made (p = 0.0001 for all scenarios). Demographic characteristics of the investigators failed to explain the observed differences. CONCLUSIONS: Subjective eligibility criteria may interfere with the conduct and interpretation of RCTs and, therefore, their use ought to be justified explicitly in the study protocol. RCT designers, funding agencies and research ethics boards have an important role in reviewing eligibility criteria for their necessity. (shrink)

In contrast to attempts that have been made to measure the clarity of reporting of the methods of clinical trials in journal articles, we report here an attempt to measure the accuracy of methods reporting. We focus in this article on eligibility criteria as a test case for the reporting of clinical trial methods. We examined the reporting of eligibility criteria in the protocol, methods paper (if applicable), journal article, and Clinical Alert for articles appearing in print between January 1988 (...) and September 1994 for which a Clinical Alert had been issued. Eligibility criteria were further classified into five categories in order to examine the content of information loss, if any. On average, 82% of protocol eligibility criteria were reported in methods papers. Journal articles and Clinical Alerts fared somewhat worse: 63% of criteria were reported in journal articles, 19% in Clinical Alerts. In all three categories of medical communication, the reporting of criteria that defined the study disease tended to be complete; reporting of criteria relating to trial precision, patient safety, legal and ethical concerns, and administrative considerations, was not complete. We found that criteria for clinical trial eligibility are frequently under-reported in medical communications. Moreover, some of the criteria omitted are of considerable clinical importance. We suggest that in the design phase of clinical trials, proposed eligibility criteria be scrutinized closely. Those criteria that survive this scrutiny and that have clinical import must be reported upon fully and accurately when communicating trial results. (shrink)

We studied changes in eligibility criteria--the largest impediment to patient accrual--in two samples of clinical trials. Trials from the NSABP (National Surgical Adjuvant Breast and Bowel Program) and POG (Pediatric Oncology Group) were analyzed. After eliminating duplications, the criteria in each protocol were enumerated and classified according to a novel schema. NSABP trials contained significantly more criteria than POG trials, and added precision criteria (making study populations homogeneous) at a faster rate than POG studies. The difference between NSABP studies (explanatory (...) trials) and POG studies (pragmatic trials) suggest that large numbers of eligibility criteria are not necessary for quality studies. We recommend that: (1) the inclusion/exclusion criteria distinction be abandoned; (2) eligibility criteria be explicitly justified; (3) the need for each criterion be assessed when new trials are planned; (4) criteria in phase III trials restricting patient accrual be minimized; and (5) further research be done to assess the impact of criteria on generalizability. OBJECTIVE: Pilot study to characterize treatment differences between patients treated in clinical trials and those treated in a clinical setting. Previous studies have shown higher survival rates for participants in trials of cancer therapy. This difference is observed even after rates are adjusted for important covariates such as age and stage of disease. DESIGN: Retrospective chart review. SETTING: Oncology outpatient department in a tertiary care hospital. PATIENTS: Ninety women 18 to 70 years of age with early-stage breast cancer who were (...) diagnosed in 1990. Fifty-one of the women were treated through clinical trials and 39 were treated outside of clinical trials. OUTCOME MEASURES: Number of blood tests, telephone calls, clinic visits and imaging procedures as well as intensity of chemotherapy and use of radiation therapy. The age of the patient and the stage of disease were important covariates. RESULTS: After the analysis was controlled for patient age and stage of disease, patients treated through a clinical trial were more likely to receive standard-dose chemotherapy (p = 0.020, 95% confidence interval 1.20 to 200.73) and more frequent blood tests (p < 0.001, 95% confidence interval 1.02 to 1.13) than other patients treated in the clinic. CONCLUSIONS: Our results provide a plausible mechanism for the observed survival advantage for participants in clinical trials in oncology. Further study is called for. If these results are confirmed, they have important implications for informed consent to participate in clinical trials and for clinical practice. (shrink)

ABSTRACTIn this paper I investigate how conceptual engineering applies to mathematical concepts in particular. I begin with a discussion of Waismann’s notion of open texture, and compare it to Shapiro’s modern usage of the term. Next I set out the position taken by Lakatos which sees mathematical concepts as dynamic and open to improvement and development, arguing that Waismann’s open texture applies to mathematical concepts too. With the perspective of mathematics as open-textured, I make the case that this allows (...) us to deploy the tools of conceptual engineering in mathematics. I will examine Cappelen’s recent argument that there are no conceptual safe spaces and consider whether mathematics constitutes a counterexample. I argue that it does not, drawing on Haslanger’s distinction between manifest and operative concepts, and applying this in a novel way to set-theoretic foundations. I then set out some of the questions that need to be engaged with to establish mathematics as involving a kind of conceptual engineering. I finish with a case study of how the tools of conceptual engineering will give us a way to progress in the debate between advocates of the Universe view and the Multiverse view in set theory. Some authors have claimed that ante rem structuralism has problems with structures that have indiscernible places. In response, I argue that there is no requirement that mathematical objects be individuated in a non-trivial way. Metaphysical principles and intuitions to the contrary do not stand up to ordinary mathematical practice, which presupposes an identity relation that, in a sense, cannot be defined. In complex analysis, the two square roots of –1 are indiscernible: anything true of one of them is true of (...) the other. I suggest that ‘i’ functions like a parameter in natural deduction systems. I gave an early version of this paper at a workshop on structuralism in mathematics and science, held in the Autumn of 2006, at Bristol University. The Phenomenology of Spirit has been in rich and equal measures a source of both frustration and fascination to its readers. Coming to it from the more conventional texts of our tradition readers have been puzzled, first, by the structure of the Phenomenology. Despite his suggestions that he is following an actual historical development of some sort Hegel will pass from the Terror of 1793–94 to prehistoric religions of nature, or from Kantian universality in morality to the life of the (...) Greek polis. In addition the Phenomenology contains a vast number of allusions to particular texts and authors which seems disproportionate to its claim to have followed a necessary path to absolute knowledge. One may have the impression that the highway of despair has been so named because of its constant and confusing detours into wildernesses which have only the most peripheral connection with the promised land of spirit. All of this has prompted an amazing quantity of ingenious hermeneutical activity. The leading directions in such exegesis can be sorted out into the logical, the existential, and the poetic; each is governed by the intention of saving and preserving the integral value of the text. There is also, of course, a skeptical reading of the Phenomenology, often appealing to philological evidence, which attempts to suggest, that what we are dealing with is at best a patchwork of essays on various subjects and with different purposes, hurriedly put together to meet the demands of the printer. I regard the patchwork theory as a last resort and will pass over it in silence here, since I have not yet been reduced to its level of desperation or irony. The logical reading such as that given in recent years by Stanley Rosen claims that the Phenomenology presupposes the Logic rather than serving as an introduction to it; but Hegel repeatedly says that the Phenomenology is such an introduction or ladder to the standpoint of science. An existential approach to the text finds Hegel to have surrendered joyously to the drama and possibilities of the Lebenswelt, both contemporary and historical. The authors developed this textbook in response to an increasing interest in ethics, and a growing number of courses on this topic that are now being offered in educational leadership programs. It is designed to fill a gap in instructional materials for teaching the ethics component of the knowledge base that has been established for the profession. The text has several purposes: First, it demonstrates the application of different ethical paradigms (the ethics of justice, care, critique, and the profession) through (...) discussion and analysis of real-life moral dilemmas that educational leaders face in their schools and communities. Second, it addresses some of the practical, pedagogical, and curricular issues related to the teaching of ethics for educational leaders. Third, it emphasizes the importance of ethics instruction from a variety of theoretical approaches. Finally, it provides a process that instructors might follow to develop their own ethics unit or course. * Part I provides an overview of why ethics is so important, especially for today's educational leaders, and describes a multiparadigm approach essential to practitioners as they grapple with ethical dilemmas. * Part II deals with the dilemmas themselves. Stewart Shapiro's ambition in Vagueness in Context is to develop a comprehensive account of the meaning, function, and logic of vague terms in an idealized version of a natural language like English. It is a commonplace that the extensions of vague terms vary according to their context: a person can be tall with respect to male accountants and not tall (even short) with respect to professional basketball players. The key feature of Shapiro's account is that the extensions of (...) vague terms also vary in the course of conversations and that, in some cases, a competent speaker can go either way without sinning against the meaning of the words or the non-linguistic facts. As Shapiro sees it, vagueness is a linguistic phenomenon, due to the kinds of languages that humans speak; but vagueness is also due to the world we find ourselves in, as we try to communicate features of it to each other. We examine George Boolos's proposed abstraction principle for extensions based on the limitation-of-size conception, New V, from several perspectives. Crispin Wright once suggested that New V could serve as part of a neo-logicist development of real analysis. We show that it fails both of the conservativeness criteria for abstraction principles that Wright proposes. Thus, we support Boolos against Wright. We also show that, when combined with the axioms for Boolos's iterative notion of set, New V yields a system equivalent to (...) full Zermelo-Fraenkel set theory with a principle of global choice. This advances Boolos's longstanding interest in the foundations of set theory. (shrink)

The dialogue has been central in the ongoing debate about the theory of forms, and it remains a crucial text for Plato scholars in both the analytical and the phenomenological traditions. Stanley Rosen's book is the first full-length study of the Sophist in English and one of the most complete in any language. (...) The revisionary approach to semantic paradox is commonly thought to have a somewhat uncomfortable corollary, viz. that, on pain of triviality, we cannot affirm that all valid arguments preserve truth (Beall2007, Beall2009, Field2008, Field2009). We show that the standard arguments for this conclusion all break down once (i) the structural rule of contraction is restricted and (ii) how the premises can be aggregated---so that they can be said to jointly entail a given conclusion---is appropriately understood. In this insightful and controversial book, the eminent philosopher Stanley Rosen takes a new look at the famous 'quarrel' that the moderns have with the ancients, analyzing and comparing ancient philosophers and modern Continental and analytical thinkers from Plato, Descartes, and Kant to Fichte, Nietzsche, and Rorty. He urges that we do not dismiss the classical heritage but appropriate it, for this appropriation is an indispensable step in the process of legitimizing our historical experience.

Cells are cognitive entities possessing great computational power. DNA serves as a multivalent information storage medium for these computations at various time scales. Information is stored in sequences, epigenetic modifications, and rapidly changing nucleoprotein complexes. Because DNA must operate through complexes formed with other molecules in the cell, genome functions are inherently interactive and involve two-way communication with various cellular compartments. Both coding sequences and repetitive sequences contribute to the hierarchical systemic organization of the genome. By virtue of nucleoprotein complexes, (...) epigenetic modifications, and natural genetic engineering activities, the genome can serve as a read-write storage system. An interactive informatic conceptualization of the genome allows us to understand the functional importance of DNA that does not code for protein or RNA structure, clarifies the essential multidirectional and systemic nature of genomic information transfer, and emphasizes the need to investigate how cellular computation operates in reproduction and evolution. Barth consistently comments on Kant's importance for his early thought in his autobiographical sketches, letters, and even more explicitly in his 1930 lectures on Kant in his Protestant Theology in the Nineteenth Century. Interestingly, however, little attention has been paid to these latter lectures in the secondary literature. In part, this oversight is due to the manner in which Barth's theology has been thought to overcome Kant's influence much earlier on in his intellectual development. Hence, although commentators such as Merold (...) Westphal, Simon Fisher and Bruce McCormack have developed keen interest in Kant's influence upon Barth's early work, even engaging Barth's Neo-Kantian context in great detail, my contention is that Barth's later interpretation of Kant is crucial and gives further insight into Barth's legacy for contemporary thought today. After Kant, Barth did not abandonment or disregard the metaphysical question of being, but rather, faced it all the more rigorously. (shrink)

I respond to Fishkin?s critique of my book The State of Democratic Theory. I reiterate my defense of a competitive model of democracy geared to reducing domination, rather than Fishkin?s deliberative model that deploys structured discussion to enlighten mass preferences. In light of the literatures on framing effects and the value of mutually independent judgments, I question whether the procedures Fishkin recommends would produce outcomes that are better informed rather than differently informed. Recognizing that deliberation might sometimes be helpful in (...) reducing domination, I note that sometimes it will not, and I fault Fishkin for his indiscriminate embrace of exceedingly costly deliberative mechanisms that promise dubious benefits? notably his and Bruce Ackerman?s?Deliberation Day? Building on recent work, I present sequent systems for the non-classical logics LP, K3, and FDE with two main virtues. First, derivations closely resemble those in standard Gentzen-style systems. Second, the systems can be obtained by reformulating a classical system using nonstandard sequent structure and simply removing certain structural rules (relatives of exchange and contraction). I clarify two senses in which these logics count as “substructural.”.

This is the first full-length study of the Symposium to be published in English, and one of the first English works on Plato to take its bearings by the dramatic form of the Platonic dialogue, a thesis that was regarded as heterodox at the time but which today is widely accepted by scholars of the most diverse standpoint. Rosen was also one of the first to study in detail the philosophical significance of the phenomenon of concrete human sexuality, as it (...) is presented by Plato in the diverse characters of the main speakers in the dialogue. His analysis of the theoretical significance of pederasty in the dialogue was highly controversial at the time, but is today accepted as central to Plato's dramatic phenomenology of human existence. Rosen discusses a variety of topics that had previously been neglected in the secondary literature, including the problem of the hybristic nature of the philosopher, the poetical dimension of Plato's conception of philosophy, and the theoretical implication of the difference between Platonic writing and Socratic conversation. Over the course of the seventeenth to eighteenth centuries, an interior private notion of religion gained wide public recognition. It then spread through settler colonial contexts around the world. It has since been criticized for its abstract, immaterial nature as well as its irrelevance to traditions beyond the European context. However, such critiques obscure the contradiction between religion’s definition as a matter of interior privacy and its public visibility in various printed publications. Firstly, this monograph responds by re-evaluating the cultural (...) impact of the exterior forms in which religious texts were printed, such as pamphlets, broadsheets, books and journals. Secondly, it applies that evidence to critical studies of religion shaped by the crisis of representation in the human sciences. While Jacques Derrida is oft-cited as a progenitor of that crisis, the opposite case is made. Lastly, it draws on Derrida’s thought to reframe the relation between a religious text’s internal hermeneutic interests and its external forms. In sum, a new model is provided of how religion was printed in ways that can be compared to other material cultures around the world. (shrink)

Religion after Deliberative Democracy responds to gaps exposed by the case of religion in deliberative democratic theory. Religion's persistent visibility in political life has called for new solutions for healing deeply divided societies. In response, the author begins with Jeffrey Stout’s pragmatist vision of democracy before providing a series of supplements in subsequent chapters.
