BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. In this paper we show that in every |T |+-resplendent model N , for every A ⊆ N such that |A | ≤ |T |, the group Autf of strong automorphisms is the least very normal subgroup of the group Aut and the quotient Aut/Autf is the Lascar group over A . Then we generalize this result to every |T |+-saturated and strongly |T |+-homogeneous model.

A Steiner triple system (STS) is a set S together with a collection B of subsets of S of size 3 such that any two elements of S belong to exactly one element of B. It is well known that the class of finite STS has a Fraïssé limit M_F. Here, we show that the theory T of M_F is the model completion of the theory of STSs. We study the groups Gal L and Gal KP, and the associated equivalence relations EL and EKP, attached to a first order theory T. An example is given where EL≠ EKP. It is proved that EKP is the composition of EL and the closure of EL. Other examples are given showing this is best possible.

A model $M$ of countable similarity type and cardinality $\kappa$ is expandable if every consistent extension $T_{1}$ of its complete theory with $|T_{1}|\leq \kappa$ is satisfiable in $M$ and it is compactly expandable if every such extension which additionally is finitely satisfiable in $M$ is satisfiable in $M$ . In the countable case and in the case of a model of cardinality $\geq 2^{\omega}$ of a superstable theory without the finite cover property the notions of saturation, expandability and compactness for (...) expandability agree. The question of the existence of compactly expandable models which are not expandable is open. Here we present a test which serves to prove that a compactly expandable model of cardinality $\geq 2^{\omega}$ of a superstable theory is expandable. It is stated in terms of the existence of a certain elementary submodel whose corresponding theory of pairs of models satisfies a weak elimination of Ramsey quantifiers. (shrink)

The "space" of Lascar strong types, on some sort and relative to a given complete theory T, is in general not a compact Hausdorff topological space. We have at least three aims in this paper. The first is to show that spaces of Lascar strong types, as well as other related spaces and objects such as the Lascar group Gal L of T, have well-defined Borel cardinalities. The second is to compute the Borel cardinalities of the known examples as well (...) as of some new examples that we give. The third is to explore notions of definable map, embedding, and isomorphism, between these and related quotient objects. We also make some conjectures, the main one being roughly "smooth if and only if trivial". The possibility of a descriptive set-theoretic account of the complexity of spaces of Lascar strong types was touched on in the paper [E. Casanovas, D. Lascar, A. Pillay and M. Ziegler, Galois groups of first order theories, J. Math. Logic1 305–319], where the first example of a "non-G-compact theory" was given. The motivation for writing this paper is partly the discovery of new examples via definable groups, in [A. Conversano and A. Pillay, Connected components of definable groups and o-minimality I, Adv. ' I am surprised at how little he is read — at least in the United States. He is a splendid writer, often entertaining, always a skilled (...) craftsman. His earlier novels {Afternoon Men, 1931; Venusberg, 1932; From a View to a Death, 1933; Agents and Patients, 1936; and What's Become of Waring, 1939) are short satiric and ironic comedies dealing mainly with London bohemia in the 1920s. His later fictional work is a twelve volume tour de force (A Question of Upbringing, 1951; A Buyer's Market, 1952; The Acceptance World, 1955; At Lady Molly's, 1957; Casanova's Chinese Restaurant, 1960; The Kindly Ones, 1962; The Valley of Bones, 1964; The Soldier's Art, 1966; The Military Philosophers, 1968; Books Do Furnish a Room, 1971; Temporary Kings, 1973; Hearing Secret Harmonies, 1975) 2 in which the narrator, NicholasJenkins, attempts, with varying degrees of success, to make sense of life as A Dance to the Music of Time. Both the early and later works are populated by many characters. More recently Hilary Spurling has compiled a book-length guide to the plots and characters ofDance. It also includes an index of books, paintings, and places referred to.4 Even with these guides there are loose ends, and it is not always clear what functions certain minor characters serve. Nonetheless, the popping-up in unexpected places is, as I hope to show, essential to the theme of the work. Powell is by no means universally acclaimed; his novels have been described as "contrived,"5 and "pseudo-Proustian" and "blandly snobbish."6 The early novels have been said to "tell about 'twentyish emancipation, silly parties at which drunks behave stupidly, ambitious young men on the make for the daughters of fox-hunting families, all with an air of slightness and couldn't-care less." 7 And Dance has likewise been scorned: To begin with, each character is introduced as he first appears to the narrator, who immediately speculates for three or four pages on the probable nature of a character who is blond, has small ears, and wears a black overcoat. The overcoat then says a few desultory words, whereupon the narrator takes several pages of pain to modify his original opinion in the light of this additional evidence.8 Like all parodies, these have some truth in them. But it is simplistic to describe his work in these terms — it amounts to painting in colors far too crude what is an enormously subtle picture of life. Powell's "easy modulations," as John Russell has called his novels,9 capture intricacies of human behavior and interaction in a profound and original way. And Powell's focus on ritual and the attempts of people to play chosen roles, ultimately provides a genuinely metaphysical, epistemological, ethical, and aesthetic interpretation of life. It aims to answer two questions central to Aristotle's psychology and biology: why does Aristotle think we have sense-organs, and why does he describe the sense-organs in the way he does? The author looks at all the Aristotelian evidence for the five senses and shows how pervasively Aristotle's accounts of the sense-organs are motivated by his interest in form and function. J. T. Ismael's monograph is an ambitious contribution to metaphysics and the philosophy of language and mind. She tackles a philosophical question whose origin goes back to Descartes: What am I? The self is not a mere thing among things--but if so, what is it, and what is its relationship to the world?

Why should Aristotle reject his own criteria for a science to admit this puzzling science of being? Or does he really reject them? Perhaps the science of being is not intended to be a universal science of the type rejected elsewhere. The Metaphysics and the Organon are not concerned with exactly the same questions; and verbal differences may not reflect real or important doctrinal conflicts.

Recent work on extended mind theory has considered whether the material realizers of phenomenally conscious states might be distributed across both body and world. A popular framework for understanding perceptual consciousness in world-involving terms is sensorimotor enactivism, which holds that subjects make direct sensory contact with objects by means of their active, exploratory skills. In this paper, I consider the case of emotional experience, and argue that although the enactivist view does not transfer neatly to this domain, there are elements (...) of emotional consciousness whose physical underpinnings include parts of the extra-bodily environment. Beginning with a review of Michael Pollan's latest book about the renaissance of research into the use of psychedelics to treat addiction, depression, and end-of-life anxiety, this essay considers wisdom and insight that might be gained by examining the psychedelic practices of primitive people. Pollan finds that almost all who begin using psychedelics to treat the ill eventually come to the conclusion that they should be made available for the broader purpose of 'the betterment of well people'. By considering both (...) scientific and spiritual perspectives on these compounds, the book offers insight into the toxicity not only of our mental environment, but at the same time our relationship to fellow humanity and Mother Nature. However, Pollan does not consider what is perhaps the greatest source of wisdom regarding the pharmacological potential of psychedelics: primitive societies who have long incorporated these compounds into their ways of being. By casting anthropological accounts of primitive psychedelic practices against those of contemporary research, this essay seeks to deepen enquiry into the pathologies of modern personhood as they bear upon the ecological crisis, together with the potential value of entheogens for understanding and responding to these problems. Augustine defends three claims about the passions: The Stoic position differs only verbally from the Platonic-Aristotelian position. The Stoic positionis wrong and the Platonic-Aristotelian position is right. The will is engaged in the different passions; indeed the different passions are different expressionsof the will. The first two claims, properly understood, are defensible. But the most plausible versions of them give us good reason to doubt the third claim.
