BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. Purpose: Several accounts like Ernst von Glasersfeld's Who Conceives of Society? (2008) locate empirical research in the social sciences and radical constructivism in almost parallel universes. The main purpose of this paper is to argue for more inter-active relations and to stress the importance of establishing weak, medium and strong ties between radical constructivism and empirical social research in general. Findings: The article shows that that weak, medium and strong ties between radical constructivism and empirical research in the social sciences (...) can be established. Moreover, these weak, medium and strong ties play a crucial role for increasing and maintaining the relevance of radical constructivism within the social sciences and for contributing to the viability of radical constructivism in the long run. Implications: The most important consequence for radical constructivism lies in a new research agenda. In the course of this article weak, medium and strong ties to empirical research have been specified specifically and exclusively for the social sciences. But these specific links cannot be simply generalized or transferred to other disciplinary fields outside the social sciences. Since interactive links between radical constructivism and empirical research are currently missing in practically all disciplines, these weak, medium and strong links, as a new RC-agenda, have to be developed separately for each major scientific research arena. Han Fei was a famous Legalist in the late Warring States period. During the struggle to criticize the Confucian school, he developed the theory of the "rule of law," which laid a theoretical groundwork on which the newly emerging landlord class could build a centralized feudal state. His works had been appreciated by Ch'in Shih-huang. When Ch'in Shih-huang read the book Han Fei Tzu, he sighed and said, "I would feel no regret about dying if I could meet this person (...) and become his friend." Later Ch'in Shih-huang applied this progressive theory of Han Fei in practice and developed it through practice. (shrink)

What “physical” means is sometimes clarified by appealing to paradigmatically physical objects, properties, or phenomena. This move is not entirely unmotivated. The most basic intuition behind physicalism can be identified as that we, as conscious beings, are not ontologically special: we are, ultimately, like all these inanimate and unconscious things; we do not exemplify any mysterious properties that are categorically over and above all the properties that are exemplified by ordinary things like chairs or rocks or their constituents. And, according (...) to the dualists, we are, in terms of substance or property, metaphysically different from chairs, rocks, and the like. The kind of conception of the physical that refers to paradigm cases of the physical is in line with this disagreement in intuition between the physicalist and the dualist. Trying to conceptualize the physical based on some paradigmatically physical objects or phenomena, I argue, however, is a dead-end. (shrink)

The author refers to his book “Die Selbstgestaltung des Lebendigen, synoptische Theorie des Lebens als ein Beitrag zu den philosophischen Grundlagen der Naturwissenschaft” and comments some basic theses of it, namely the body-soul-relation, the gestalt and the problem of the “soul” of the plant and the significance of these conceptions for the description and the understanding of the realizing itself of the living being. The Author prefers not to call a soul the transcendent order which is propre to a plant, (...) as to all what lives, and which is the consequence of their quality as a gestalt. He designs with J.Reinke the transcending order of the plant with the name “diaphysis”. He names his theory of life autotelism and has some hope that it can reconcile mecanism and vitalism in the idea of identity of the so called parallel processes in different ontological dimensions. The duality of body or organism and soul or “innen” is a result only of human thinking, really they are a whole together and do not work upon another, but what happens in the one of them, happens at the same moment in the other one like in a mirror. The unity of both has been stated so often, that it sounds rather trivial, but in the book cited above it is described and argued exactly in that sense that phenomene and gignomene in one and the same life process are correspondent structures in different ontological dimensions, coordinated to one another, as it were, like two mathematical co-ordinates.The author replies to a critic who has doubts about the possibility of our thinking to approximate to the problems in question. In addition to the neglect of philosophy by medicine, emphasized in a recent editorial in this journal, there has been an equally important neglect of medicine by philosophy. Philosophy stands to gain from medicine in three respects: in materials, the conceptual difficulties arising in the practice of medicine being key data for philosophical enquiry; in methods, these data, through their problematic character, being ideally suited to the technique of linguistic analysis; and in results, the practical requirements of medicine placing a (...) direct demand for progress on philosophical theory.
