BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. Mining social network data and developing user profile from unstructured and informal data are a challenging task. The proposed research builds user profile using Twitter data which is later helpful to provide the user with personalized recommendations. Publicly available tweets are fetched and classified and sentiments expressed in tweets are extracted and normalized. This research uses domain-specific seed list to classify tweets. Semantic and syntactic analysis on tweets is performed to minimize information loss during the process of tweets classification. After (...) precise classification and sentiment analysis, the system builds user interest-based profile by analyzing user’s post on Twitter to know about user interests. The prime objective of this study is to discuss the economic impact of the COVID-19 crisis concerning Pakistan and globally across South Asian developing countries. It also demonstrates those economic indicators through which the economic performance of the countries is influenced considering the prospective global economic costs of COVID-19 under different scenarios. The study systematically reviewed the literature on historical pandemic crises and the most recent Covid-19 fallout economic disorder in South Asian countries. It is observed that the prevailing outbreak of COVID-19 has eventually affected large scale manufacturing, service sectors and arose supply side constraint to an extent. On the contrary, GDP and consumption declined sharply. Furthermore, sudden loss of service sector jobs and the rise in food prices have created an economic crisis for developing countries of South Asia. The study attempts to discuss the policy responses given by the World Bank, International Monetary Fund, Asian Development Bank, The State Bank of Pakistan, and the Ministry of finance, Pakistan encountered due to the deteriorating international environment, fiscal stress, and difficulties in financial liquidity markets and provides relevant policies for the better prospect of Pakistan's economy after comparing it with other South Asian countries affected by this pandemic vulnerabilities. This paper addresses the implementation of semantic markedness as a subtle tool of ideology in U.S. mainstream newspapers The New York Times, Washington Post, and The Wall Street Journal in the context of the Israeli raids on Nablus between 2022–2023. Existing research on this topic suggests that international media associate lexemes connoting negative images with Palestinians as a part of its control over the worldview of the Israeli occupation of Palestine. Previous research also touches upon semantic markedness as a tool (...) that indicates gender-based discrimination against women. However, there is no critical scholarship directed towards the use of semantic markedness in media discourse and critical discourse analysis (CDA). This paper adheres to Jakobson and Lyon’s (1971; 1997) definitions of markedness and builds on discussion of semantic discourse and political ideology, respectively, as its framework. Using Halliday’s transitivity and semantic markedness as a tool of analysis, the findings of the paper reveal that the selected newspapers employ semantically marked lexemes to portray Palestinian resistance as disorganised militant units and hence justify the mass murder of Palestinians. If there were no such thing as feeling ill, there would be no such thing as suffering from a physical illness. Yet there is no logical connection between feeling ill and being mentally ill. Mental illness is manifested in various forms of behaviour, for example, suspiciousness, elation, depression, etc.; if a form of behaviour is to (...) count as mental illness, it has to be an insane form; yet it is possible to be mentally ill without being insane and it is possible to be insane without being mentally ill. Two groups of scientists have just announced what is being described as a leap forward in human stem cell research.1–3 Both have found ways of producing what are being called “induced pluripotent cells” , stem cells that they hope will demonstrate the same key properties of regeneration and unrestricted differentiation that human embryonic stem cells possess, but which are derived from skin cells not from embryos. In simple terms, these scientists have succeeded in reprogramming skin cells to behave like hESCs.Stem (...) cell research has been hailed as one of the most important and exciting areas of science, because it is believed that these types of cells will not only play an important part in regenerative medicine, but also yield valuable scientific information. These latest developments in cell reprogramming represent a milestone for stem cell science. No longer does the paradigm of irreversible cell specialisation hold true; instead, almost any type of cell might have the potential to become any other.The advent of techniques for producing these iPCs has also been hailed as an ethical breakthrough. Up until now, the production of hESCs has required a process which involves the destruction of embryos. Many of these embryos are available as by-products of IVF; but in addition, the process of somatic cell nuclear transfer to produce cloned embryonic stem cells requires a supply of human oocytes, which must currently be harvested from female donors at no insignificant cost. In a nutshell, iPCs seem to enable us to produce “embryo-free” human pluripotent stem cells, and in a “gender-neutral” way—that is, without the need for human oocytes.The response from the scientific community, ethical commentators and the public has, however, reflected an inherent confusion over the ethical significance of this research. This paper is an historical study of Tarski's methodology of deductive sciences (in which a logic S is identified with an operator Cn S, called the consequence operator, on a given set of expressions), from its appearance in 1930 to the end of the 1970s, focusing on the work done in the field by Roberto Magari, Piero Mangani and by some of their pupils between 1965 and 1974, and comparing it with the results achieved by Tarski and the Polish school (...) (Łoś, Suszko, Słupecki, Pogorzelski, Wójcicki). In the last section of the paper we will then compare these works with some recent developments in algebraic logic: this will lead to a better understanding of the results of the methodology of deductive science, but at the same time will show some intrinsic limits to such an approach to logic. Even if Magari's work on diagonizable algebras and universal algebra and Mangani's axiomatization of MV-algebras and results in model theory are rather famous, the articles on closure operators, published in the 1960s, are almost totally unknown outside Italy (mainly because of a linguistic limitation, the papers we analyse having been written and published in Italian). This paper aims to fill the gap in the literature and to enable the international community to get acquainted with this part of Italian logic. Today, nurses are required to have knowledge and awareness concerning professional values as standards to provide safe and high-quality ethical care. Nurses’ perspective on professional values affects decision-making and patient care. Therefore, the present study aimed to investigate the importance of professional values from clinical nurses’ perspective. The present cross-sectional study was conducted in 2016 in four educational hospitals of Kerman University of Medical Sciences, Iran. Data were collected via the Persian version of Nursing Professional Values Scale-Revised by Weis and (...) Schank. Sampling was conducted through the use of stratified random sampling method and 250 clinical nurses participated in the study. Results indicated that the total score of the nurses’ professional values was high.. From nurses’ perspective items such as “Maintaining confidentiality of patients” and “Safeguarding patients’ right to privacy” had more importance; however, “Recognizing role of professional nursing associations in shaping healthcare policy” and “Participating in nursing research and/or implementing research findings appropriate to practice had less importance. A statistically significant relationship was observed between NPVS-R mean scores and nurses’ age, work experience as well as participation in professional ethical training. Although the total score related to the clinical nurses’ perspective on professional values was high, the importance of certain values was at a lower level. Owing to the emerging ethical challenges, it is indispensable to design educational programs in order to improve nurses’ awareness and understanding of the comprehensive importance of professional values. Furthermore, it is recommended that mixed methods studies should be conducted in order to design an instrument to evaluate the use of values in nursing practice. (shrink)

Point-of-care testing (POCT) is a sensitive, specific and rapid form of testing for the presence of HIV antibodies. Post-exposure prophylaxis for HIV infection can reduce seroconversion rates by up to 80%. Needlestick injuries are the second commonest cause of occupational injury in the NHS and 20% of these occur during operations. In the NHS, in order to protect staff and patients from the risk of bloodborne viruses such as HIV, it is mandatory to report such injuries; however, numerous studies have (...) shown that many groups, particularly doctors, are reluctant to do so. This article outlines the arguments for and against the introduction of preoperatively seeking consent from patients to have their blood tested for HIV via POCT in order to improve the reporting rates of needlestick injuries incurred during surgery and to protect staff from infection. The aim of this book is “to point how far the Arabian philosophers succeeded in their attempt to integrate the Semitic and Muslim conception of revelation with Greek wisdom.” The problem is precisely that which confronted the Catholic theologians of the 13th century. Seeing the efforts of the Arabian philosophers side by side with the achievement of St. Thomas we can better appreciate what St. Thomas succeeded in doing.

This paper analyzes Kent Bach’s representational theory of action, one of the causal theories of action. Bach’s theory sets requirements not only for the cause of an action, but also for how it unfolds in time and transitions into another action. These requirements suggest a sequential emergence of two components of the agent’s action awareness: the representation of the prepared movement and the perception of its sensory consequences. Bach introduces the concepts of “effective representation” (ER) and “receptive representation” (RR) to (...) denote these components of awareness. According to representational theory, action has a cyclic three-step causal structure, where ER is the cause of a movement, the movement is the cause of RR, and RR is the cause of ER of subsequent movement. In constructing his theory, Bach tries to take into account the problem of deviant causal chains and to introduce the so-called minimal actions into the purview of the philosophy of action. Relying on the behavioral data on blindfolded deafferentated patients, in this paper I argue that RRs are not a necessary element of action. I also analyze Bach’s distinction between ER and RR and compare it with J. Searle’s approach, placing Bach’s theory within the context of studies of the structure of intentional states. I show that Bach’s theory occupies a unique position among views on the structure of action awareness. At the same time, I conclude that the frameworks of Bach and Searle are insufficient for describing the metaphysical difference between the two kinds of intentional states, and I also point out the difficulties facing the notion of ER. Based on this reasoning and the data on deafferentated patients, I propose a modification of the causal structure of action proposed by Bach, making a remark about the inadequate definition of ER. In the proposed modification, RR and the agent’s prior intention serve as alternative components of action awareness. (shrink)

The purpose of the present study was to address the shortcomings of Cronbach’s alpha concerning the semantic overlap between items. Using an example from a motivational measure, the correction of Cronbach’s alpha was applied by partialing out the effects due to conceptual overlap. The significance of Cronbach’s alpha was tested using simulated random data derived from the measure and by estimating the confidence intervals with known and unknown distributions. The results indicated that the uncorrected conceptual overlap coefficient alpha was equal (...) to 0.89 and 0.66 following the correction. After simulating the corrected statistical results, the distribution of alpha with random numbers had an estimate of 95%, equal to 0.41. The lower bound of the corrected alpha distribution was equal to 0.41, suggesting that the corrected alpha could easily belong to the distribution of alpha developed from simulated random numbers. Thus, the semantic overlap between items on a measure represents a significant threat to the validity of the alpha coefficient. (shrink)

Introduction to Sexology [Vvedenie v seksologiiu] by I. S. Kon has finally been published after having made the rounds of publishing houses for many years. This is the first Soviet publication devoted to a description and an analysis of the genesis, development, and state of a new branch of scientific knowledge about man-sexology-which affects every one of us. To be sure, General Sexual Pathology [Obshchaia seksopatologiia], a textbook for physicians edited by G. S. Vasil'chenko, which came out in 1977, has (...) an extensive introductory chapter on sexology, but it is limited to medical problems. Kon's book is an example of an interdisciplinary approach that provides an idea of the most essential components of sexology; it is thus addressed not only to the medical profession, but also to sociologists, philosophers of culture, psychologists, pedagogues, etc. The book takes into account the latest publications in this area of science.
