BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. It has been hypothesized that psychedelic experiences elicit lasting psychological benefits by altering narrative selfhood, which has yet to be explicitly studied. The present study investigates retrospective reports (n = 418) of changes to narrative self that participants believe resulted from, or were catalysed by, their psychedelic experience(s). Responses to open-ended questions were analysed using inductive and deductive thematic coding and interpreted within agent-centred approaches to development and well-being. Themes include decentred introspection, greater access to self-knowledge, positive shifts in self-evaluation (...) processes, greater psychological and behavioural autonomy, and enhanced connectedness with others and the world. While this explorative qualitative study offers some initial support for the explanation that changes to narrative self are a cornerstone of psychedelics' therapeutic and transformative potential, methodological and recruiting limitations preclude the ability to make objective claims and generalizations. Future scientific research is necessary to further elucidate this hypothesized mechanism. (shrink)

The relationship between ethics and trust is ambiguous as ethics can promote trust, whilst trust can simultaneously be abused resulting in unethical behaviour. In this contribution to the debate on trust and ethics the focus is specifically on the role that ethics can play in facilitating trustworthiness. The article starts with a definition of the concept trustworthiness. It then reports on an empirical longitudinal study on trustworthiness that was conducted in a South African company in the insurance industry. The facilitators (...) of trustworthiness that were identified in this study and their relative contributions to trustworthiness are then discussed. Finally the implications of these findings for the ethical conduct of managers are discussed. It is demonstrated how ethical managerial conduct can enhance the trustworthiness of managers. (shrink)

In Morality and Socially Constructed Norms, Valentini searches for a unifying principle that underlies whatever genuine obligations we might have to obey the norms of any and all social practices, ranging from line queueing norms, through offsides rules in soccer, to obligations not to break the law. I argue that this search is driven, and distorted, by a commitment to what Bernard Williams labeled the ‘morality system’. Once we see this, we should question the value of the unifying project. Most (...) social norms can be considered on their own terms, as part of a rich and variegated ethical life, without subsuming them under the question of whether others’ moral rights are at stake. Similarly, political obligation needs to be understood in relation to the practical activity of politics, not as an abstract moral quandary. (shrink)

Problem reports result from several misunderstandings about the nature and functions of phenomenal consciousness. I discuss some philosophical and scientific correctives that, taken together, can make the hard problem seem less hard.

Background: Patients today interact with physicians, physician groups, and health plans, each of which may follow distinct ethical guidelines.Method: We systematically compared physician codes of ethics with ethics policies at physician group practices and health plans, using the 1998–99 policies of 38 organisations—18 medical associations , nine physician group practices , and 12 health plans —selected using random and stratified purposive sampling. A clinician and a social scientist independently abstracted each document, using a 397-item health care ethics taxonomy; a reconciled (...) abstraction form was used for analysis. This study focuses on ethics policies regarding professional obligation towards patients, resource allocation, and care for the vulnerable in society.Results: A majority in all three groups mention “fiduciary obligations” of one sort or another, but associations generally address physician/patient relations but not health plan obligations, while plans rarely endorse physicians’ obligations of advocacy, beneficence, and non-maleficence. Except for occasional mentions of cost effectiveness or efficiency, ethical considerations in resource allocation rarely arise in the ethics policies of all three organisational types. Very few associations, groups, or plans specifically endorse obligations to vulnerable populations.Conclusions: With some important exceptions, we found that the ethics policies of associations, groups, and plans are narrowly focused and often ignore important ethical concerns for society, such as resource allocation and care for vulnerable populations. More collaborative work is needed to build integrated sets of ethical standards that address the aims and responsibilities of the major stakeholders in health care delivery. (shrink)

But the core of the Buddhist religion is the sangha , the community of bhikkhus around whose corporate life the religion is moulded. It is the existence and structure of the sangha which has shaped the history of Buddhism, enabled it to take (...) root in new countries, and given it the customs and rituals which have made it a religion rather than a small sect. Constituent power is an idea with a long tradition in modern political thought but has been largely abandoned since the middle of the twentieth century. Here I offer a new account of constituent power that avoids problems of the classical account, including the paradox of constitutionalism, and clarifies how individuals contribute to creating their shared political order. I argue that constituent power should be understood as an individual power-with: the agential power to constitute a legal order with others. Our individual, (...) banal acts of law-abidingness each partially effect the collective outcome. We generally exercise constituent power unconsciously and automatically, guiding our actions to succeed as defined by law and relying on the legal system to take up our contributions and effectively combine them with others. Essodina BAMAZE N’GANI | : En raison de fortes tensions ethniques qui le mettent en branle, l’État en Afrique vit un délitement du tissu collectif. Dans un contexte de pluralisme ethnique fort, la négation de ce pluralisme par la politique d’assimilation a eu pour effet contraire l’édification d’un sectarisme ethnique donnant lieu à une instabilité socio-politique. En réaction, la prise en considération institutionnelle de l’ethnie n’a pu empêcher cette instabilité. Oscillant ainsi entre politique d’assimilation et naturalisation normative de l’ethnie, l’État (...) y vit une crise du bien commun. Le présent article vise à montrer que la gestion du pluralisme ethnique en Afrique ne saurait se réduire à de simples dispositions politico-juridiques. Il s’agit bien plutôt de promouvoir la perspective de la postcommunauté qui vient fédérer trois types de mobilisations enchevêtrées : l’une relative à la conscience, l’autre à la participation de toutes les composantes communautaires au bien commun, enfin celle relative aux nouvelles responsabilités de l’État. | : Due to high ethnic tensions at play, the State in Africa is witnessing a disintegration of the social fabric. Against the background of a pervasive ethnic pluralism, the weakening of this pluralism through the policy of assimilation had the unwanted effect of establishing ethnic sectarianism, culminating into social and political disruptions. Subsequently, the institutional consideration of ethnicity could not prevent this instability. Torn between the policy of assimilation and the normative naturalisation of ethnicity, the State is undergoing a crisis in terms of the common good. This paper seeks to demonstrate that the management of ethnic pluralism in Africa cannot be watered down to simple political and legal provisions. It has far more to do with promoting the perspective of post-community which merges three types of interrelated concepts : one related to conscience, the other to the involvement of all community components working for the common good, and lastly the one dealing with new duties of the State. The period between 1945 and 1952 marked the development of Professor Ryle's conception of the principles of inference as performance rules. This development has paralleled that of his now well-known distinction between knowing-how and knowing-that. Indeed, the former is a corollary to the latter. Beginning with the inaugural address to the Aristotelian Society in 1945 and reaching full fruition in The Concept of Mind in 1949, it finds its most detailed and illuminating expression in “‘If’, ‘So’, and ‘Because’,” which appeared (...) in 1950. The present essay will attempt to explore this conception of rules in some detail, in the hope of clarifying the nature of Professor Ryle's views on classical logic and on certain problems within the philosophy of logic. Professor Ryle belongs, if his views may be so labeled, to the school of informal language analysts. On the other hand, we incline more toward formalism. We tend to agree with those philosophers who consider certain schemata indispensable tools for the solution of philosophical problems. Logic and the philosophy of logic are, as everyone knows, the areas in which the formalists’ technique originated. In these areas it is accepted by virtually everybody. The only recent exception is a group of English writers, many of whom take their inspiration from Ryle. One may hope, then, that the present criticism of Ryle's view from the other standpoint will throw some light on both positions. What follows falls naturally into two parts. The first deals mainly with Ryle's objections to current logical theory, the second with his own philosophy of logic. (shrink)
