This paper explores the notions of hope and how individual patient autonomy can trump carefully reasoned ethical concerns and policies intended to regulate stem cell transplants. We argue that the same limits of knowledge that inform arguments to restrain and regulate unproven treatments might also undermine our ability to comprehensively dismiss or condemn them. Incautiously or indiscriminately reasoned policies and attitudes may drive critical information and data underground, impel patients away from working with clinical researchers, and tread needlessly on hope, (...) the essential motivator of patients, advocates and researchers alike. We offer recommendations to clinicians and health care providers to help balance the discourse with individuals seeking treatment while guarding against fraud, misconception, and patient harm. Questions of whether anything exceeds reasonable sense and meaning have persisted throughout the history of philosophy. These questions have even continued in postmodern thought as well as in liberatory philosophies in which many kinds of events and lineages are experienced and seen as beyond philosophy. In this cowritten text, distinguished philosophers Nancy Tuana and Charles Scott pay particular attention to lineages and their dynamism as they develop the idea of things beyond philosophy, beyond norms. This is not a history of (...) philosophy or a critical study of a particular philosopher but a way to engage experience around dimensions of events that are beyond measuring, counting, meaning, and value. These attunements, they assert, are vitally important for the ways people orient themselves in the world and comport themselves in it. Tuana and Scott build on the alternatives to normative ethics that they find in the work of Nietzsche, Foucault, and Anzaldúa. They urge attunement to the world as a way to speak about what is impossible to give voice to, to live in the spaces between speech and the unspeakable, and to conceptualize and articulate the boundaries of rational sensibility. In the coming decades, highly treated wastewater, known as reclaimed water, is slated to be a major element of municipal water supplies. In particular, planners propose supplementing drinking water with reclaimed water as a sustainable solution to the growing challenge of urban water scarcity. Public opposition is currently considered the primary barrier to implementing successful potable water reuse projects; nonetheless, public responses to reclaimed water are not well understood. Based on a survey of over 250 residents of Tucson, Arizona, this (...) article assesses the relationship between trust in the professional institutions responsible for municipal water development and willingness to drink reclaimed water. Results demonstrate that public acceptance of potable reuse is contingent on trust in the authorities who influence design of sociotechnical systems for water supply and reuse—including water and wastewater utilities, regulators, consultants, academics, and elected local officials. Findings emphasize the highly interdependent social and political factors that inform personal decisions to support or reject potable reuse. The authors suggest that achieving greater acceptance of potable reuse will require bringing local and regional water policy in line with public values, as well as finding ways to incorporate these values into the planning process. This concluding essay offers reflections on core components of the faculty fellowship program, its outcomes and results, and program design and administration. Amid the current calls for reform in legal and other professional education, the lessons we learned and perspectives we gained during this fellowship program may be relevant to any faculty members and university administrations that are seeking to create more effective and engaged professional and graduate school programs, whatever may be their subject-matter discipline.

Scientistic conceptualisations hold to the positivistic positions that science is limitless in its potential representations of material phenomena and that it is the only sure path to knowledge. In recent popular scientific literature, these presuppositions have been reaffirmed to the detriment of both philosophy and theology. This article argues for the contrary position by a meta-analysis of empirical science from a Thomist perspective. Identifying empirical science as limited in its method and bound to the material sphere of being alone, we (...) posit that rather than standing as the sole path to the knowledge of being, empirical science is constrained at its frontiers. It is subsequently contended that far from empirical science having the explanatory ability to respond to all presenting scientific problems in principle, fundamentals without the grasp of the methodology of empirical science exist. The social ecological model (SEM) is a conceptual framework that recognizes individuals function within multiple interactive systems and contextual environments that influence their health. Medical Legal Partnerships (MLPs) address the social determinants of health through partnerships between health providers and civil legal services. This paper explores how the conceptual framework of SEM can be applied to the MLP model, which also uses a multidimensional approach to address an individual’s social determinants of health.

National attention on issues of public health preparedness necessarily brings into sharp focus the question of how to assure adequate, community-wide health care financing for preventive, acute care, and long-term medical care responses to public health threats. In the U.S., public and private health insurance represents the principal means by which medical care is financed. Beyond the threshold challenge of the many persons without any, or a stable form of, coverage lie challenges related to the structure and characteristics of health (...) insurance itself, particularly the commercial industry and its newly emerging market of consumer-driven health plans. States vary significantly in how they approach the regulation of insurance and in their willingness to support various types of insurance markets. This variation is attributable to the size and robustness of the insurance market, the political environment, and regulatory tradition and custom. Reconciling health insurance markets with public health-related health care financing needs arising from public health threats should be viewed as a major dimension of national health reform. Constitutes a thoughtful survey of contemporary hermeneutics in its historical context.

I critically analyse two causal analyses of seeing, by Frank Jackson and Michael Tye. I show that both are unacceptable. I argue that Jackson's analysis fails because it does not rule out cases of non-seeing. Tye's analysis seems to be superior to Jackson's in this respect, but I show that it too lets in cases of non-seeing. I also show that Tye's proposed solution to a problem for his theory -- which involves a robot that mimics another (unseen) robot -- (...) fails. Finally I show that his 'variability' requirement is not necessary, because there are cases where someone can see an object even though the variability that Tye requires does not exist. (shrink)

This paper compares the cases of female ailments recorded in the Epidaurian Miracles Cures with the theory and therapeutics of the Hippocratic gynecological texts as a means of testing the extent of the assumptions shared between temple and Hippocratic medicine. I argue that where temple and Hippocratic practice hold common ground, it is readily explicable through widely circulating and historically rooted cultural presuppositions regarding female physiology and pathology, rather than through scientific borrowings. Rather than representing complementary outlets of medical care (...) in which Asklepios specialized in “hopeless” cases, I suggest that the iamata permit us to observe a process in which parallel medical traditions branched out from a common cultural substratum, and that more nuance is required in framing the relationship between Greek naturalist and religious medicine. African Philosophy and St Thomas Aquinas have both been taught in African universities, but the engagement between the continent’s indigenous philosophical tradition and the Catholic intellectual tradition’s preeminent strand, has not been thorough. Presupposing that plural philosophical traditions contribute to the search to better understand, this research embarks upon a comparative analysis of the perspectives of the African ubuntu philosophy and Thomist philosophical conceptualisations of human becoming and being. Through analysis of dimensions of both traditions, it is contended that human (...) fulness arises through relationality. It is argued that in centring on the interpersonal encounter and the consequent recognition of another’s being through mutual engagement, these philosophical traditions open to each other. Further, both traditions contribute toward the ontology of personhood in ubuntu and the good of mutual indwelling, respectively. (shrink)

This is a volatile time for health insurance policy. Medicare and Medicaid are in turmoil, as is the private health insurance market. Public and private health insurance costs constitute eighty percent of healthcare spending in the United States. Public health professionals depend on the insurance system to behave in ways that are responsive to public health in prevention and crisis management.Seventy-five percent of the American population, excluding the elderly, has coverage through the private health insurance system. Ninety percent of this (...) group receives their insurance through employer-sponsored programs, and the remaining ten percent buy their own coverage. Approximately ten percent of the non-elderly population has insurance through a government program, and fifteen percent of the non-elderly population, almost forty-one million Americans, is uninsured. Foucault's genealogies and archeologies provide occasions in which one may come to know the powers, accidents, and influences that have structured a particular knowledge or discipline. The Birth of the Clinic shows the development of modern medicine in a process by which rational inference and emphasis on the history of a disease are replaced by pathological anatomy. In modern anatomy, the corpse, not reason, became the “space” of modern medical knowledge. In this “space” developed a confederation of dead body, knowledge, (...) trained and noninferential gaze, organic unity, and the increased importance of bodily space instead of physical time. The discussion shows how Foucault's genealogy of modern medicine is influenced by and overcomes the knowledge that it describes. In this context, his genealogy is also shown to be self-overcoming and to raise important questions for medical ethics when ethical thought is conceived within the structures of knowledge that have been exposed by the genealogy. Human desire usually has an object of longing or hope. The more intense the desire, the more singularly prominent its object. Sides, after all, means “heavenly body.” When people desire, they want, crave, and even covet the desired, whether the desired is ice cream, a professorship, or another’s body. What is intensely desired, even if it is not heavenly, has the status of an object with exceptional and immediate meaning and draw. We begin at the site of borders, the demarcations between us, between: my body and your body, humans and nonhuman animals, habits of thought and institutional structures, nature and culture, subject and object. We find ourselves between the devil and the deep blue sea. Differences, distinctions, and borders are key to knowing and acting responsibly. Yet we are “held captive” by particular habits of understanding that police such borders with unbecoming fervor. We desire to trouble these borders with the aim (...) of transforming habits of thought and embodied dispositions, to give rise to new appreciations of difference through the deep, ongoing play of the in-between. Background Forward-looking, democratically oriented governance is needed to ensure that human genome editing serves rather than undercuts public values. Scientific, policy, and ethics communities have recognized this necessity but have demonstrated limited understanding of how to fulfill it. The field of bioethics has long attempted to grapple with the unintended consequences of emerging technologies, but too often such foresight has lacked adequate scientific grounding, overemphasized regulation to the exclusion of examining underlying values, and failed to adequately engage the public. Methods (...) This research investigates the application of scenario planning, a tool developed in the high-stakes, uncertainty-ridden world of corporate strategy, for the equally high-stakes and uncertain world of the governance of emerging technologies. The scenario planning methodology is non-predictive, looking instead at a spread of plausible futures which diverge in their implications for different communities’ needs, cares, and desires. Results In this article we share how the scenario development process can further understandings of the complex and dynamic systems which generate and shape new biomedical technologies and provide opportunities to re-examine and re-think questions of governance, ethics and values. We detail the results of a year-long scenario planning study that engaged experts from the biological sciences, bioethics, social sciences, law, policy, private industry, and civic organizations to articulate alternative futures of human genome editing. Conclusions Through sharing and critiquing our methodological approach and results of this study, we advance understandings of anticipatory methods deployed in bioethics, demonstrating how this approach provides unique insights and helps to derive better research questions and policy strategies. BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. The five books of Artemidorus of Ephesus’ Oneirocritica (c. second century CE) constitute the largest collection of divinatory dream-interpretations to survive from Graeco-Roman antiquity. This article examines Artemidorus’ contribution to longstanding medico-philosophical debates over the ontological and epistemic character of such dreams. As with wider Mediterranean traditions concerning premonitory dreams, Greeks and Romans popularly understood them as phenomena with origins exterior to the dreamer (e.g. a visitation of a god). Presocratic and Hippocratic thinkers, however, initiated an effort to bring at (...) least some dream events within the body as interior processes of a physical soul, a bodily turn which ultimately engendered a split in thinking about the possibility and scope of prophetic and diagnostic dreams (e.g. Hippocrates Regimen 4, Aristotle On Prophecy in Dreams, Rufus of Ephesus, and Galen). Here, I examine the way that Artemidorus meant the Oneirocritica to address seriously questions about the origins of dreams and the powers of empirical science in two, related ways. We will see that his defense of oneirocriticism as a techne unfolds along two main axes. First, Artemidorus constructs a necessary veneer of scientific credibility by mobilizing the tropes and organizational strategies characteristic of contemporary technical literature. Secondly, he anchors that authority upon a materialist account of prophetic dreams as “semiotic movements of the soul.” As Price, S. 1986. “The Future of Dreams: From Freud to Artemidorus.” Past & Present 113: 3–36 and Harris-McCoy, D. 2012. Artemidorus’ Oneirocritica: Text, Translation, and Commentary. Oxford: Oxford University Press observed, this explicit commitment to an empirical method of dream-interpretation resembles that of the Hellenistic medical ‘sect’ known as ‘Empiricists,’ who eschewed interest in theoretical etiology in favor of observation, comparison, and compilation of symptoms and outcomes. Pushing beyond similarity, I argue that Artemidorus not only deployed this method to defend empirically grounded accounts of dream-divination (cf. Cicero De div. 1), but endeavored to show that such a scientific method can be used to stabilize and corral productively the potentially infinite number of interpretations any dream-sign might bear without recourse to an underlying, causal theory. In his 2010 work, The Grand Design, Stephen Hawking, argues that ‘… philosophy is dead’. While not a Philosopher, Hawking provides strong argument for his thesis, principally that philosophers have not taken science sufficiently seriously and so Philosophy is no longer relevant to knowledge claims. In this paper, Hawking’s claim is appraised and critiqued, becoming a meta-philosophical discussion. It is argued that Philosophy is dead, in some sense, due to particular philosophers having embarked on an intellectual path no longer in (...) keeping with the ancient definition of Philosophy. Philosophy as the seeking of wisdom necessarily includes the consideration of findings of other intellectual pursuits, including physical and natural science. While Philosophy has justifiably evolved through its long history, is it unrecognisable in the terms by which it historically defined itself? Seeking consistency, Hawking is critiqued for appearing to practise ‘dead’ Philosophy. Indeed, Hawking’s appeal to multiverse theory and his core discussion of the metaphysical problem of being are philosophical. The question of the death of Philosophy has contemporary relevance for the discipline which is particularly under threat for its survival in the academy, oftentimes assumed to be irrelevant. What does living well look like in the Anthropocene? Despite our brief tenure on planet Earth, we have reached an epoch--the Anthropocene--that is characterized by our species' uncanny ability to spoil our own nest. In the face of this somber reality of ecological degradation, The Book of Ecological Virtues asks the all-important question, "What does living well look like in the Anthropocene?" It is vitally important that we turn towards the cultivation of ecovirtues, a new set of values by which (...) to live, if there is to be hope for us and other species to continue. This essay is written in two sections. The first, following a short introduction, is made up of three scenarios drawn from the life and work of Martin Buber. As well as demonstrating his obvious interest in human relationships with the other-than-human, each scenario describes an encounter between either Buber himself or a stand-in character and a member of the other-than-human world. Together, these scenes not only suggest that I/Thou encounters are possible with the other-than-human, and that they are important for (...) the development of the human I, but also outline characteristics that make such encounters possible. The second section focuses on Buber's description of the “great actor” in order to offer a way teachers might engage their own students with the other-than-human world. Just as the actor embodies both the character and his or her own person in a dramatic role, so the teacher might embody the I/Thou relationship with the natural world, becoming Teacher/Tree, and exemplifying that relationship with the students.
