Diagnosis of dementia in primary care is both difficult and important. The recommendations by several authors to improve the diagnosis of dementia by general practitioners are important, but insufficient. It is argued that perhaps the disease concept in itself is a cause of confusion for clinicians. Primary care physicians need an adapted procedure, gradually leading to the final diagnosis of dementia. It has to be a stepwise labelling strategy, using global descriptions and non-disease specific labels in the beginning, ending up (...) with well-defined disease criteria. In this process, there is circularity: previous diagnoses have to be kept in mind because symptoms and signs may gradually change during the progression of the disease, leading to reconsideration of previous deleted options. To frame this properly, the primary care physician needs to adopt a broad frail elderly geriatric concept. Implementation of this concept not only helps the diagnostic process, but also stimulates the care for dementing patients and their caregivers. Relevant arguments for early diagnostic involvement of primary care physicians can be put forward on condition that a new concept, adapted procedures and adapted instruments are used. (shrink)

Throughout the biological and biomedical sciences there is a growing need for, prescriptive ‘minimum information’ (MI) checklists specifying the key information to include when reporting experimental results are beginning to find favor with experimentalists, analysts, publishers and funders alike. Such checklists aim to ensure that methods, data, analyses and results are described to a level sufficient to support the unambiguous interpretation, sophisticated search, reanalysis and experimental corroboration and reuse of data sets, facilitating the extraction of maximum value from data sets (...) them. However, such ‘minimum information’ MI checklists are usually developed independently by groups working within representatives of particular biologically- or technologically-delineated domains. Consequently, an overview of the full range of checklists can be difficult to establish without intensive searching, and even tracking thetheir individual evolution of single checklists may be a non-trivial exercise. Checklists are also inevitably partially redundant when measured one against another, and where they overlap is far from straightforward. Furthermore, conflicts in scope and arbitrary decisions on wording and sub-structuring make integration difficult. This presents inhibit their use in combination. Overall, these issues present significant difficulties for the users of checklists, especially those in areas such as systems biology, who routinely combine information from multiple biological domains and technology platforms. To address all of the above, we present MIBBI (Minimum Information for Biological and Biomedical Investigations); a web-based communal resource for such checklists, designed to act as a ‘one-stop shop’ for those exploring the range of extant checklist projects, and to foster collaborative, integrative development and ultimately promote gradual integration of checklists. The past 50 years have seen an accumulation of evidence suggesting that associative learning depends on high-level cognitive processes that give rise to propositional knowledge. Yet, many learning theorists maintain a belief in a learning mechanism in which links between mental representations are formed automatically. We characterize and highlight the differences between the propositional and link approaches, and review the relevant empirical evidence. We conclude that learning is the consequence of propositional reasoning processes that cooperate with the unconscious processes involved (...) in memory retrieval and perception. We argue that this new conceptual framework allows many of the important recent advances in associative learning research to be retained, but recast in a model that provides a firmer foundation for both immediate application and future research. In this response, we provide further clarification of the propositional approach to human associative learning. We explain why the empirical evidence favors the propositional approach over a dual-system approach and how the propositional approach is compatible with evolution and neuroscience. Finally, we point out aspects of the propositional approach that need further development and challenge proponents of dual-system models to specify the systems more clearly so that these models can be tested.

This article contributes to the contemporary debate on land and water grabbing through a detailed, qualitative case study of horticultural agribusinesses which have settled in Tanzania, disrupting patterns of land and water use. In this paper we analyse how capitalist settler farms and their upstream and downstream peasant neighbours along the Nduruma river, Tanzania, expand and defend their water use. The paper is based on 3 months of qualitative field work in Tanzania. We use the echelons of rights analysis framework (...) combined with the concept of institutional bricolage to show how this contestation takes place over the full spectrum of actual abstractions, governance and discourses. We emphasise the role different (inter)national development narratives play in shaping day-to-day contestations over water shares and rule-making. Ultimately, we emphasise that water grabbing is not a one-time event, but rather an on-going struggle over different water resources. In addition, we show how a perceived beneficial development of agribusinesses switching to groundwater allows them to avoid peasant-controlled institutions, avoiding further negotiation between the different actors and improving their image among neighbouring communities. This development illustrates how complex and obscured processes of water re-allocation can be without becoming illegal per se. (shrink)

Jan-Werner Müller’s Democracy Rules advances the debate over populism beyond politicised arguments over definitions. His refusal to blame populism on unenlightened masses points to the need for a coalition of democrats and liberals if liberal democracy is to be saved. But are these enough? Or does the uneven history of democracy in the twentieth century, recounted in Müller’s other works, suggest the need for coalitions that also reach moderates and other blocs less moved by either democratic or liberal ideals? Though (...) distorting media and unresponsive parties may share some blame, what if committed liberals and democratic constitutionalists sometimes really aren't in the majority? And when is polarising between ‘true’ and ‘false’ democrats, and daring to ‘fight fire with fire’, more likely to succeed than to further the hyper-polarisation populists rightly see as their own best strategy? In searching for an appropriate utility function in the expected utility framework, we formulate four properties that we want the utility function to satisfy. We conduct a search for such a function, and we identify Pareto utility as a function satisfying all four desired properties. Pareto utility is a flexible yet simple and parsimonious two-parameter family. It exhibits decreasing absolute risk aversion and increasing but bounded relative risk aversion. It is applicable irrespective of the probability distribution relevant to the prospect (...) to be evaluated. Pareto utility is therefore particularly suited for catastrophic risk analysis. A new and related class of generalized exponential (gexpo) utility functions is also studied. This class is particularly relevant in situations where absolute risk tolerance is thought to be concave rather than linear. (shrink)

Background Parental refusal of routine childhood vaccination remains an ethically contested area. This systematic review sought to explore and characterise the normative arguments made about parental refusal of routine vaccination, with the aim of providing researchers, practitioners, and policymakers with a synthesis of current normative literature. Methods Nine databases covering health and ethics research were searched, and 121 publications identified for the period Jan 1998 to Mar 2022. For articles, source journals were categorised according to Australian Standard Field of Research (...) codes, and normative content was analysed using a framework analytical approach. Results Most of the articles were published in biomedical journals (34%), bioethics journals (21%), and journals that carry both classifications (20%). Two central questions dominated the literature: (1) Whether vaccine refusal is justifiable (which we labelled ‘refusal arguments’); and (2) Whether strategies for dealing with those who reject vaccines are justifiable (‘response arguments’). Refusal arguments relied on principlism, religious frameworks, the rights and obligations of parents, the rights of children, the medico-legal best interests of the child standard, and the potential to cause harm to others. Response arguments were broadly divided into arguments about policy, arguments about how individual physicians should practice regarding vaccine rejectors, and both legal precedents and ethical arguments for vaccinating children against a parent’s will. Policy arguments considered the normative significance of coercion, non-medical or conscientious objections, and possible reciprocal social efforts to offset vaccine refusal. Individual physician practice arguments covered nudging and coercive practices, patient dismissal, and the ethical and professional obligations of physicians. Most of the legal precedents discussed were from the American setting, with some from the United Kingdom. Conclusions This review provides a comprehensive picture of the scope and substance of normative arguments about vaccine refusal and responses to vaccine refusal. It can serve as a platform for future research to extend the current normative literature, better understand the role of cultural context in normative judgements about vaccination, and more comprehensively translate the nuance of ethical arguments into practice and policy. In this paper we have two related aims. First, we aim to present an account of what it is to treat women as sex-objects.1 Like other philosophical writers in the field, we hold that the central idea in an account of such treatment is the failure to treat women with proper respect in sexual behavior. This idea has been cashed out in terms of using,2 and in terms of dehumanization or failure to accord equal rights to freedom and welfare.3 However, (...) we believe that there is a central deficiency in most such philosophical accounts of treating women as sex-objects, namely, a failure to outline in any detail a theory of interpersonal norms so that one can grasp in more concrete terms what it is to avoid treating a woman as a sex-object. We aim to rectify this deficiency by presenting a model of interpersonal norms based on the work of the social psychologist Chris Argyris. (shrink)

Open-minded people should endorse dogmatism because of its explanatory power. Dogmatism holds that, in the absence of defeaters, a seeming that P necessarily provides non-inferential justification for P. I show that dogmatism provides an intuitive explanation of four issues concerning non-inferential justification. It is particularly impressive that dogmatism can explain these issues because prominent epistemologists have argued that it can’t address at least two of them. Prominent epistemologists also object that dogmatism is absurdly permissive because it allows a seeming to (...) provide justification even if the seeming was caused in some apparently inappropriate way. It charts the history of the relation and its relevance to contemporary debates in normative and metanormative philosophy and proceeds to survey issues to do with fittingness covered in the volume’s chapters, including the nature and epistemology of fittingness, the relations between fittingness and reasons, the normativity of fittingness, fittingness and value theory, and the role of fittingness in theorizing about responsibility. The chapter concludes with a brief discussion of issues to (...) do with fittingness that aren’t covered extensively by the volume’s chapters in order to indicate avenues for further research. The impact of neoliberal reforms of education systems on the work of teachers and school leaders, particularly in relation to high-stakes accountability frameworks, has been extensively studied in recent decades. One significant aspect of neoliberal schooling is the emergence of quasi-autonomous public schools (such as Academies in England, Charter Schools in the USA and Independent Public Schools in Australia), characterised by heterarchical governance models, the promotion of entrepreneurial leadership cultures, and the promotion of a discourse of pursuing educational equity by (...) means of ‘achievement for all’. This paper explores the emergence of a mode of teacher professionalism characteristic of these quasi-autonomous schools, and conceptualises this as being ‘neo-performative’. The neo-performative profession is shaped by the shift in the focus of the regulation and management of schools from ‘governing to governance’, and the consequential rise of the ‘responsibilised profession’, and marked by the emergence of an entrepreneurial model of school leadership. The paper argues that this new conceptualisation of teacher professionalism requires further, more focused, empirical study in order to explore how neoperformative teachers and school leaders articulate their vision of educational equity and social justice, and how they enact this vision in an increasingly intensified high-stakes accountability culture. Any attempt to construct a realist interpretation of quantum theory founders on the Kochen-Specker theorem, which asserts the impossibility of assigning values to quantum quantities in a way that preserves functional relations between them. We construct a new type of valuation which is defined on all operators, and which respects an appropriate version of the functional composition principle. The truth-values assigned to propositions are (i) contextual; and (ii) multi-valued, where the space of contexts and the multi-valued logic for each context (...) come naturally from the topos theory of presheaves. The first step in our theory is to demonstrate that the Kochen-Specker theorem is equivalent to the statement that a certain presheaf defined on the category of self-adjoint operators has no global elements. We then show how the use of ideas drawn from the theory of presheaves leads to the definition of a generalised valuation in quantum theory whose values are sieves of operators. In particular, we show how each quantum state leads to such a generalised valuation. A key ingredient throughout is the idea that, in a situation where no normal truth-value can be given to a proposition asserting that the value of a physical quantity A lies in a set D of real numbers , it is nevertheless possible to ascribe a partial truth-value which is determined by the set of all coarse-grained propositions that assert that some function f(A) lies in f(D), and that are true in a normal sense. The set of all such coarse-grainings forms a sieve on the category of self-adjoint operators, and is hence fundamentally related to the theory of presheave. (shrink)

The Covid-19 pandemic creates an unprecedented threatening situation worldwide with an urgent need for critical reflection and new knowledge production, but also a need for imminent action despite prevailing knowledge gaps and multilevel uncertainty. With regard to the role of research ethics in these pandemic times some argue in favor of exceptionalism, others, including the authors of this paper, emphasize the urgent need to remain committed to core ethical principles and fundamental human rights obligations all reflected in research regulations and (...) guidelines carefully crafted over time. In this paper we disentangle some of the arguments put forward in the ongoing debate about Covid-19 human challenge studies and the concomitant role of health-related research ethics in pandemic times. We suggest it might be helpful to think through a lens differentiating between risk, strict uncertainty and ignorance. We provide some examples of lessons learned by harm done in the name of research in the past and discuss the relevance of this legacy in the current situation. (shrink)

One can often encounter an opinion that Polish scientific philosophy deserves to be much better known than actually is. This book is thought as a response to such a claim. The papers collected in this volume are divided into two parts: Background and Influence and History and Systematics. However, there is no sharp borderline between themes which are touched in both parts. Generally speaking, all papers of the first part relate the Lvov-Warsaw School to some philosophical movements external to it (...) whereas the papers collected in the second one focus on internal issues connected with the school . Since the Polish school of mathematical logic is much better known than the Polish analytic philosophy we decided to omit here any treatment of the former. Thus, this collection centers on purely philosophical matters. We projected this volume not as an exhaustive panorama of Polish analytic philosophy but rather as a series of essays on particular persons or topic. As a result one can find here papers on Twardowski. Ajdukiewicz, Kotarbinski, Tarski and Lukasiewicz as well as on ethics on science, nominalism, and the methodology of psychology. We hope that this book will contribute to a better knowledge and evaluation of Polish achievements in analytic philosophy. We would like to express our gratitude to Professor Leszek Nowak, the editor-in-chief of Poznan Studies in the Philosophy of the Sciences and the Humanities, who initiated the idea of the collection and helped in its preparation. This article returns to the debate about the relative importance of the productive forces and the relations of production in the feudal mode of production. It argues, using western medieval evidence, that this relation is an empirical one and varies between modes, maybe also inside modes; and that, in the specific case of feudalism, not only were the relations of production the driving force, but developments in the productive forces actually depended upon them.

This Element provides an opinionated introduction to the debate in moral philosophy over identifying the basic elements of well-being and to the related debate over the nature of happiness. The question of the nature of happiness is simply the question of what happiness is, and the central philosophical question about well-being is the question of what things are in themselves of ultimate benefit or harm to a person, or directly make them better or worse off.

The so-called `no-thesis' view is without a doubt one of the most immediately puzzling philosophical features of Nāgārjuna's thought and also largely responsible for ascribing to him either sceptical or mystical leanings (or indeed both). The locus classicus for this view is found in verse 29 of the Vigrahavyāvartanī: “If I had some thesis the defect [just mentioned] would as a consequence attach to me. But I have no thesis, so this defect is not applicable to me.” That this absence (...) of a thesis is to be regarded as a positive feature is stressed in a passage from the Yuktiṣaṣṭikā, where Nāgārjuna remarks about the Buddhas: “For these great beings there is no position, no dispute. How could there be another's [opposing] position for those who have no position?” Now it is important to observe that when considered in isolation it is very hard to make any coherent sense of these passages. For even if we assume that the Buddhas do not hold any philosophical position anymore (having perhaps passed beyond all conceptual thinking), how are we to make sense of the first quotation which, in the middle of a work full of philosophical theses claims that there is no such thesis asserted at all?
