OBJECTIVE: Following two randomized controlled trials that demonstrated reduced mortality and better neurological outcome in cardiac arrest patients, mild therapeutic hypothermia was implemented in many intensive care units. Up to now, no large observational studies have confirmed the beneficial effects of mild therapeutic hypothermia. DESIGN: Internet-based survey combined with a retrospective, observational study. PATIENTS: All patients admitted to an intensive care unit in The Netherlands after cardiac arrest from January 1, 1999 until January 1, 2009. DATA SOURCE: Dutch National (...) Intensive Care Evaluation database. METHODS: The moment of implementation of mild therapeutic hypothermia for each hospital participating in the Dutch National Intensive Care Evaluation database was determined with an Internet survey. To compare mortality before and after implementation of mild therapeutic hypothermia, the odds ratio adjusted for Simplified Acute Physiology Score II score, age, gender, propensity score, and in- or out-of-hospital cardiac arrest was calculated. Patients were excluded if 1) they were admitted to an intensive care unit that did not respond to the survey, 2) they were admitted within 3 months after implementation of mild therapeutic hypothermia, 3) they had a Glasgow Coma Scale score of >8, or 4) they did not satisfy the Simplified Acute Physiology Score II inclusion criteria. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: A total of 13,962 patients were admitted to an intensive care unit following cardiac arrest. In total 8,645 patients were excluded, 5,544 because of a Glasgow Coma Scale score of >8. Of the resultant 5,317 patients, 1,547 patients were treated before and 3,770 patients after implementation of mild therapeutic hypothermia. Patients admitted after implementation of mild therapeutic hypothermia had lower minimal and maximal temperatures (p < .0001) during the first 24 hrs on the intensive care unit compared to patients admitted before implementation of mild therapeutic hypothermia. The adjusted odds ratio of the hospital mortality of patients treated after implementation of mild therapeutic hypothermia was 0.80 (95% confidence interval of 0.65-0.98, p = .029). CONCLUSION: The results of this retrospective, observational survey suggest that implementation of mild therapeutic hypothermia in Dutch intensive care units is associated with a 20% relative reduction of hospital mortality in cardiac arrest patients. In a remarkable development, I have constructed a new definition for a self-delimiting universal Turing machine (UTM) that is easy to program and runs very quickly. This provides a new foundation for algorithmic information theory (AIT), which is the theory of the size in bits of programs for selfdelimiting UTM's. Previously, AIT had an abstract mathematical quality. Now it is possible to write down executable programs that embody the constructions in the proofs of theorems. In “The Sources of Knowledge,” Robert Audi distinguishes what he calls the “four standard basic sources” by which we acquire knowledge or justified belief: perception, memory, consciousness, and reason. With the exception of memory, he distinguishes each of the above as a basic source of knowledge. Audi contrasts basic sources with nonbasic sources, concentrating on testimony. After clarifying the relationship between a source and a ground, or “what it is in virtue of which one knows or justifiedly believes,” (...) Audi evaluates the basic sources’ individual and collective autonomy as well as their vulnerability to defeasibility. The ideas behind open source software are currently applied to the production of encyclopedias. A sample of six English text-based, neutral-point-of-view, online encyclopedias of the kind are identified: h2g2, Wikipedia, Scholarpedia, Encyclopedia of Earth, Citizendium and Knol. How do these projects deal with the problem of trusting their participants to behave as competent and loyal encyclopedists? Editorial policies for soliciting and processing content are shown to range from high discretion to low discretion; that is, from granting unlimited trust to (...) limited trust. Their conceptions of the proper role for experts are also explored and it is argued that to a great extent they determine editorial policies. Subsequently, internal discussions about quality guarantee at Wikipedia are rendered. All indications are that review and ?super-review? of new edits will become policy, to be performed by Wikipedians with a better reputation. Finally, while for encyclopedias the issue of organizational trust largely coincides with epistemological trust, a link is made with theories about the acceptance of testimony. It is argued that both non-reductionist views (the ?acceptance principle? and the ?assurance view?) and reductionist ones (an appeal to background conditions, and a?newly defined??expertise view?) have been implemented in editorial strategies over the past decade. 'Most of us are still groping for answers about what makes life worth living, or what confers meaning on individual lives', writes Charles Taylor in Sources of the Self. 'This is an essentially modern predicament.' Charles Taylor's latest book sets out to define the modern identity by tracing its genesis, analysing the writings of such thinkers as Augustine, Descartes, Montaigne, Luther, and many others. This then serves as a starting point for a renewed understanding of modernity. Taylor argues that modern (...) subjectivity has its roots in ideas of human good, and is in fact the result of our long efforts to define and attain the good. The modern turn inwards is far from being a disastrous rejection of rationality, as its critics contend, but has at its heart what Taylor calls the affirmation of ordinary life. He concludes that the modern identity, and its attendant rejection of an objective order of reason, is far richer in moral sources that its detractors allow. Ethical concepts are, or purport to be, normative. They make claims on us: they command, oblige, recommend, or guide. Or at least when we invoke them, we make claims on one another; but where does their authority over us - or ours over one another - come from? Christine Korsgaard identifies four accounts of the source of normativity that have been advocated by modern moral philosophers: voluntarism, realism, reflective endorsement, and the appeal to autonomy. She traces their history, showing (...) how each developed in response to the prior one and comparing their early versions with those on the contemporary philosophical scene. Kant's theory that normativity springs from our own autonomy emerges as a synthesis of the other three, and Korsgaard concludes with her own version of the Kantian account. Recent research has demonstrated an asymmetry between the origins and endpoints of motion events, with preferential attention given to endpoints rather than beginnings of motion in both language and memory. Two experiments explore this asymmetry further and test its implications for language production and comprehension. Experiment 1 shows that both adults and 4-year-old children detect fewer within-category changes in source than goal objects when tested for memory of motion events; furthermore, these groups produce fewer references to source than (...) goal objects when describing the same motion events. Experiment 2 asks whether the specificity of encoding source/goal relations differs in both spatial memory and the comprehension of novel spatial vocabulary. Results show that endpoint configuration changes are detected more accurately than source configuration changes by both adults and young children. Furthermore, when interpreting novel motion verbs, both age groups expect more fine-grained lexical distinctions in the domain of endpoint configurations compared to that of source configurations. These studies demonstrate that a cognitive-attentional bias in spatial representation and memory affects both the detail of linguistic encoding during the use of spatial language and the specificity of hypotheses about spatial referents that learners build during the acquisition of the spatial lexicon. Demands for public participation in technical decision-making are currently high on the agenda of Science & Technology Studies. It is assumed that the democratisation of technical decision-making processes generally leads to more socially desirable and acceptable outcomes. While this may be true in certain cases, this assumption cannot be generalised. I will discuss the case of the so-called ‘South African AZT debate’. The controversy started when President Thabo Mbeki, after reading some scientific papers on the toxicity of AZT, decided to (...) bar the use of the drug in the public health sector as a means to reduce the transmission of HIV from mothers to children. While the scientific mainstream accepts the effectiveness of AZT in reducing the risk of vertical HIV transmission, a few maverick scientists reject the clinical evidence and argue that the risks of using AZT by far outweigh its benefits. Based on various textual sources and using the ‘Periodic Table of Expertises’ developed by Collins and Evans, Mbeki’s expertise at the time of his intervention into the technical question whether AZT is a medicine or a poison can be classified as primary source knowledge. It is shown that this type of expertise is insufficient for technical decision-making. Mbeki’s primary source knowledge legitimated his presentation of the claims of maverick scientists as a serious contribution to the debate—with tragic consequences for tens of thousands of babies.Keywords: Primary source knowledge; Expertise; Technical decision-making; South Africa; AZT; AIDS. An approach to this issue prevalent in the philosophy of the past half-century seeks to explain the power of directedness in terms of certain items' ability (...) to reliably track things in their environment. A very different approach, with a venerable history and enjoying a recent resurgence, seeks to explain the power of directedness rather in terms of an intrinsic ability of conscious experience to direct itself. Information generally comes from less than fully reliable sources. Rationality, it seems, requires that one take source reliability into account when reasoning on the basis of such information. Recently, Bovens and Hartmann (2003) proposed an account of the conjunction fallacy based on this idea. They show that, when statements in conjunction fallacy scenarios are perceived as coming from such sources, probability theory prescribes that the “fallacy” be committed in certain situations. Here, the empirical validity of their model was assessed. (...) The model predicts that statements added to standard conjunction problems will change the incidence of the fallacy. It also predicts that statements from reliable sources should yield an increase in fallacy rates (relative to unreliable sources). Neither the former (Experiment 1) nor the latter prediction (Experiment 3) was confirmed, although Experiment 2 showed that people can derive source reliability estimates from the likelihood of statements in a manner consistent with the tested model. In line with the experimental results, model fits and sensitivity analyses also provided very little evidence in favor of the model. This suggests that Bovens and Hartmann’s present model fails to explain fully people’s judgements in standard conjunction fallacy tasks. (shrink)

This paper investigates two puzzles in practical reason and proposes a solution to them. First, sometimes, when we are practically certain that neither of two alternatives is better than or as good as the other with respect to what matters in the choice between them, it nevertheless seems perfectly rational to continue to deliberate, and sometimes the result of that deliberation is a conclusion that one alternative is better, where there is no error in one’s previous judgment. Second, there are (...) striking differences between rational agents – some rational agents have most reason to pursue careers on Wall Street while others have most reason to take up a career in teaching, or scuba diving, or working for political causes. These differences aren’t plausibly explained by ‘passive’ facts about our psychology or their causal interaction with our environment; instead, these facts seem in some sense to ‘express who we are’. But what is this sense? These puzzles disappear if we adopt a novel view about the source of the normativity of reasons – some reasons are given to us and others are reasons in virtue of an act of will. We make certain considerations reasons through an act of will and thus sometimes make it true through an act of agency that we have most reason to do one thing rather than another. (shrink)

Certain relationships generate associative duties that exhibit robustness across change. It seems insufficient for friendship, for example, if I am only disposed to fulfil duties of friendship towards you as things stand here and now. However, robustness is not required across all variations. Were you to become monstrously cruel towards me, we might expect that my duties of friendship towards you would not be robust across that kind of change. The question then is this: is there any principled way of (...) distinguishing those variations across which robustness of the disposition to fulfil duties of friendship is required from those across which it is not? In this paper I propose a way of answering this question that invokes distinctions concerning how we value friends and friendships, and how persons and friendships possess value – distinctions that are central to the project of specifying not only the limits of robustness, but also the source of duties of friendship and associative duties more generally. Influential or frequently cited business ethics research does not appear in a vacuum; our study reveals its predominant sources and contributors by discipline. By examining citations from articles published in three top business ethics journals (Journal of Business Ethics, Business Ethics Quarterly and Business Ethics: A European Review) over the period 2004–2008, we document that the preponderance of influential business ethics research comes primarily from the management faculty. In addition, management journals and management books are the predominant sources for influential (...) business ethics research. Further, among the management fields, organizational behavior and organizational structure predominate leadership and strategy as the major subject areas for influential business ethics research, suggesting that this influential body of research is focused on a micro rather than on a macro context. These empirical results lend credence to the perception that there is a silo effect in influential business ethics research and suggest that business ethics research in a micro context might have permeated to the teaching of business ethics. This article argues that political realists have at least two strategies to provide distinctively political normative judgements that have nothing to do with morality. The first ground is instrumental normativity, which states that if we believe that something is a necessary means to a goal we have, we have a reason to do it. In politics, certain means are required by any ends we may intend to pursue. The second ground is epistemic normativity, stating that if something is true, this (...) gives us a reason to believe it. In politics, there are certain empirical regularities that ought to be acknowledged for what they are. Both sources are flawed. Instrumental normativity only requires coherence between attitudes and beliefs, and one can hang on to false beliefs to preserve attitudes incompatible with reality. I may desire to eschew power relations, and accordingly I may imagine politics to be like a camping trip. Epistemic normativity, on the other hand, operates critically, striking down existing normative claims. It shows us that politics is nothing like a camping trip, but it doesn’t tell us what we should do about it. We conclude by showing that if the two are taken together, they remedy each other’s flaws. Why should ‘better than’ be transitive? The leading answer in ethics is that values do not change with context. But this cannot be the entire source of transitivity, I argue, since transitivity can fail even if values never change, so long as they are complex, with multiple dimensions combined non-additively. I conclude by exploring a new hypothesis: that all alleged cases of nontransitive betterness, such as Parfit’s Repugnant Conclusion, can and should be modelled as the result of complexity, not (...) context-relativity. The essays in this collection are concerned with the psychology of moral agency. They focus on moral feelings and moral motivation, and seek to understand the operations and origins of these phenomena as rooted in the natural desires and emotions of human beings. An important feature of the essays, and one that distinguishes the book from most philosophical work in moral psychology, is the attention to the writings of Freud. Many of the essays draw on Freud's ideas about conscience and (...) morality, while several explore the depths and limits of Freud's theories. An underlying theme of the volume is a critique of influential rationalist accounts of moral agency. Although we are morally responsible for things of various kinds, what we bear direct responsibility for are certain exercises of our agency (and perhaps some omissions of these). Theorists disagree about what kind of agency is in this respect the source of our responsibility. Some hold that it is agency the exercises of which are actions. Others say that it is agency exercised in forming reasons-responsive attitudes on the basis of our take on reasons (or value). Either the wrong nominal term (┌ ψ ┐ instead of ┌ φ ┐) is ascribed to the right object or the right nominal term is ascribed to the wrong object. Judgments susceptible to one kind of error are immune to the other. Indexical terms such as ‘here’ and ‘now’ exhibit a corresponding pattern of (...) immunity and susceptibility to error, which suggests that they are complex demonstratives. On Sources of Structural Injustice: A Feminist Reading of the Theory of Iris M. Young The author focuses on a critical theory of justice and democracy by Iris Marion Young. Young's normative approach to justice and the institutional framework of inclusive democracy develops out of her critique of injustice. In the first section the author explains Young's approach to structural injustice, which she conceptualizes in terms of domination and oppression. In the second part the author elucidates Young's concept of the (...) politics of difference and inclusive democracy. In this context Young differentiates between social and cultural groups; this enables her to take into consideration the political significance of group differences. The author goes on to present Young's critical theory of gender based on the notion of women as a social structural group. Young argues that gender refers to social structures that shape relations of subordination and oppression rather than to identity. In the final part the author discusses the application of Young's concept of structural injustice at a transnational level. This paper examines the use of scalar adjectives in two contexts that have played a role in discussions of the subjective/objective distinction: ?faultless disagreement? discourses and the nonfinite complement position of the subjective attitude verb find. I argue that the pattern of distribution and interpretation of scalar adjectives in these contexts provides evidence for two sources for subjectivity, which are distinguished from each other in that one affects the grammatical properties of a predicate and one does not. The first kind, (...) which licenses appearance in the complement position of find, is based in the lexical semantics of predicates that encode qualitative assessments. The second kind, which gives rise to faultless disagreement effects, arises from uncertainty about the dimensions of evaluation that are involved in fixing the extension of a predicate in a context of utterance. This thesis concerns the source of modal truth. I aim to answer the question: what is it in virtue of which there are truths concerning what must have been the case as a matter of necessity, or could have been the case but isn't. I begin by looking at a dilemma put forward by Simon Blackburn which attempts to show that any realist answer to this question must fail, and I conclude that either horn of his dilemma can be (...) resisted. I then move on to clarify the nature of the propositions whose truth I am aiming to find the source of. I distinguish necessity de re from necessity de dicto, and argue for a counterpart theoretic treatment of necessity de re. As a result, I argue that there is no special problem concerning the source of de re modal facts. The problem is simply to account for what it is in virtue of which there are qualitative ways the world could have been, and qualitative ways it couldn't have been. I look at two ways to answer this question: by appealing to truthmakers in the actual world, or by appealing to non-actual ontology. I develop a theory of truthmakers, but argue that it is unlikely that there are truthmakers for modal truths among the ontology of the actual. I look at the main possibilist ontology, David Lewis' modal realism, but argue that warrant for that ontology is unobtainable, and that we shouldn't admit non-actual possibilia into our ontology. I end by sketching a quasi-conventionalist approach to modality which denies that there are modal facts, but nevertheless allows that we can speak truly when we use modal language. (shrink)

In his account of Pyrrhonism, Sextus Empiricus talks about the disturbance concerning matters of opinion that afflicts his dogmatic rivals and that he himself was afflicted by before his conversion to Pyrrhonism. The aim of the present paper is to identify the distinct sources of doxastic disturbance that can be found in that account, and to determine whether and, if so, how they are related. The thesis to be defended is that it is possible to discern three sources of doxastic (...) disturbance and that two of them are to be explained by reference to the third, which is the real cause of mental distress. The paper also considers whether the thesis in question entails that there is no reason for the Pyrrhonist to suspend judgment across the board, but only to suspend judgment about evaluative matters. This paper explores the relation between two ways of thinking about the sources of self-consciousness. We can think about the sources of self-consciousness either in genetic terms (as the origins or precursors of self-conscious thoughts) or in epistemic terms (as the grounds of self-conscious judgements). Using Christopher Peacocke's account of self-conscious judgements in Being Known as a foil, this paper brings out some important ways in which we need to draw upon the sources of self-consciousness in the genetic sense for (...) a proper understanding of the sources of self-consciousness in the epistemic sense.
