Clinicians at quaternary centers see part of their mission as providing hope when others cannot. They tend to see sicker patients with more complex disease processes. Part of this mission is offering longshot treatment modalities that are unlikely to achieve their stated goal, but conceivably could. When patients embark on such a treatment plan, it may fail. Often treatment toward an initial goal continues beyond the point at which such a goal is feasible. We explore the progression of care from (...) longshot to fantasy using two pediatric cases. This progression may be differentiated into four distinct stages of care related to the potential of achieving the initial goals of care. Physicians are often ill prepared for the progression of treatments from a longshot hope to an unfeasible and, therefore, typically unjustified intervention. We present a structured approach to guide clinicians at referral institutions where these situations may be common. The transition of care from “longshot” to “fantasy” is an inherent part of quaternary care for the sickest of patients that has been underexplored. Physicians are often poorly equipped to approach that transition. We advocate this approach to the shift from longshot to fantasy with the belief that such a structured method will have multiple benefits, including: reduced suffering for the patient; decreased emotional burden on patient and family; decreased provider moral distress; increased likelihood of seeking high quality palliative care earlier; and provision of honest and straightforward information to patients and their families. Although there is wide agreement that ethics consults are at risk for conflicts of interest, ethics consultants have limited guidance with regard to how to identify and approach COIs. We aim to address these concerns and provide practical guidance. We will define and consider four categories of COIs: consult type, team composition, dual clinical roles and other concerns. We will define and consider six actions available for ECs to take in response to COIs: no action, disclosure only, obtaining a second (...) opinion, referring to another EC, referring to an institutional ethics committee or seeking an outside consult. We will then propose a points-based algorithm for ECs to use to determine the appropriate response to COI. Finally, we will discuss the strengths and limitations of our proposed algorithm. (shrink)

In this commentary, the formation of “pre-iconic” visual-prime persistence is described in the context of prime-specific, independent-component activation at prefrontal and posterior EEG-recording sites. Schizophrenia is associated with a series of visual perception impairments, which might impact on the patients’ every day life and be related to clinical symptoms. However, the heterogeneity of the visual disorders make it a challenge to understand both the mechanisms and the consequences of these impairments, i.e., the way patients experience the outer world. Based on earlier psychiatry literature, we argue that issues regarding time might shed a new light on the disorders observed in patients with schizophrenia.We will briefly (...) review the mechanisms involved in the sense of time continuity and clinical evidence that they are impaired in patients with schizophrenia.We will then summarize a recent experimental approach regarding the coding of time-event structure in time, namely the ability to discriminate between simultaneous and asynchronous events. The use of an original method of analysis allowed us to distinguish between explicit and implicit judgments of synchrony.We showed that for SOAs below 20ms neither patients nor controls fuse events in time. On the contrary subjects distinguish events at an implicit level even when judging them as synchronous. In addition, the implicit responses of patients and controls differ qualitatively. It is as if controls always put more weight on the last occurred event, whereas patients have a difficulty to follow events in time at an implicit level. In patients, there is a clear dissociation between results at short and large asynchronies, that suggest selective mechanisms for the implicit coding of time-event structure. These results might explain the disruption of the sense of time continuity in patients.We argue that this line of research might also help us to better understand the mechanisms of the visual impairments in patients and how they see their environment. The purpose of this study was to investigate how the subjective impression of English speech would change when pause duration at punctuation marks was varied. Two listening experiments were performed in which written English speech segments were rated on a variety of evaluation items by both native-English speakers and non-native speakers. The ratings were then subjected to factor analysis. In the first experiment, the pauses in three segments were made into the same durations, from 0.075 to 4.8 s. Participants rated (...) the segments on 23 evaluation items on a rating scale from 1 to 10. A varimax rotation after PCA led to two factors that were related to speech style. These two factors could be interpreted as representing speech naturalness and speech rate. Speech segments with a pause duration of 0.6 s received the highest naturalness evaluation, while perceived speech rate decreased as the physical pause duration increased, without any changes in utterance segments. In the second experiment, a full-factorial design of pause durations within and between sentences, i.e., for commas and for periods, was implemented in two speech segments. The original speech segments and speech segments without any pauses were also included as control conditions. From ratings on 12 evaluation items, similar to Experiment 1, two factors representing speech naturalness and speech rate were obtained. The results showed again that the perceived speech rate decreased with an increase only in pause duration. As for speech naturalness, the highest evaluations occurred when pause durations were 0.6 s within sentences, and either 0.6 or 1.2 s between sentences. This recommends fixing all pause durations to 0.6 s as a practical way to train non-native speakers to make their spoken English appear more natural. (shrink)

In a recent study, Becker and Elliott [Becker, C., & Elliott, M. A. . Flicker induced color and form: Interdependencies and relation to stimulation frequency and phase. Consciousness & Cognition, 15, 175–196] described the appearance of subjective experiences of color and form induced by stimulation with intermittent light. While there have been electroencephalographic studies of similar hallucinatory forms, brain activity accompanying the appearance of hallucinatory colors was never measured. Using a priming procedure where observers were required to indicate the presence (...) of one of eight target colors we compared electrophysiological correlates of hallucinatory color with brain states associated with other visual phenomena. Different target colors were accompanied by different patterns of EEG activation. However, in general, we found that the appearance of hallucinatory colors is preceded by a power decrease in the lower alpha band alongside an increase in gamma band frequencies. We argue that decreasing activity in the lower alpha band acts as a gating mechanism, inducing a switch in perception between different colors. The increasing gamma activation may correlate with the formation of a coherent conscious percept. (shrink)

Patient RP suffers a unilateral right homonymous quadrant anopia but demonstrates better than chance discrimination for stimuli presented in the blind field at temporal frequencies between 33 and 47 Hz . Examination of her reports of visual experience during blind-field discrimination suggests a more complex picture in which experiences particular to correct discrimination are not found at low-mid-gamma frequencies, but are significantly more likely than average at a lower frequency at which blindsight is not observed. We believe that visual experience (...) may serve to support blindsight if discrimination tasks are generally impaired at frequencies outside of the low-mid-gamma band. If this is so, although generally experienced as non-specific and unstructured light, the visual experience that accompanies discrimination performance must be based upon a neural representation which includes information on the visual features present in the stimulus. Catholic healthcare institutions live amidst tension between three intersecting primary values, namely, a commitment of service to the poor and vulnerable, promoting the common good for all, and financially sustainability. Within this tension, the question sometimes arises as to whether it is ever justifiable, i.e., consistent with Catholic identity, to place limits on charity care. In this article we will argue that the health reform measures of the Affordable Care Act do not eliminate this tension but actually increase the urgency (...) of addressing it. Moreover, we will conclude that the question of limiting charity care in a manner that is consistent with the obligations of Catholic identity around serving the poor and vulnerable, promoting the common good, and remaining financially sustainable is not a question of if, but of how such limits are established. Such limits, however, cannot be established in light of one overriding moral consideration or principle, but must be established in light of a multitude of principles guiding us to a holistic understanding of the interrelatedness of the moral dimensions of Catholic identity. Late in 1990, the Center for the Study of Ethics in the Professions at Illinois Institute of Technology (lIT) received a grant of more than $200,000 from the National Science Foundation to try a campus-wide approach to integrating professional ethics into its technical curriculum.! Enough has now been accomplished to draw some tentative conclusions. I am the grant's principal investigator. In this paper, I shall describe what we at lIT did, what we learned, and what others, especially philosophers, can learn (...) from us. That some propositions are testable, while others are not, was a fundamental idea in the philosophical program known as logical empiricism. That program is now widely thought to be defunct. Quine’s (1953) “Two Dogmas of Empiricism” and Hempel’s (1950) “Problems and Changes in the Empiricist Criterion of Meaning” are among its most notable epitaphs. Yet, as we know from Mark Twain’s comment on an obituary that he once had the pleasure of reading about himself, the report of a death (...) can be an exaggeration. I agree that this obsession needs to be overcome. But Rosenberg repeats some of the old mis- takes and invents epicycles on others. Where Western philosophy ends, with the limits of language, marks the beginning of Eastern philosophy. The Tao de jing of Laozi begins with the limitations of language and then proceeds from that as a starting point. On the other hand, the limitation of language marks the end of Wittgenstein's cogitations. In contrast to Wittgenstein, who thought that one should remain silent about that which cannot be put into words, the message of the Zhuangzi is that one can speak about that (...) which cannot put into words but the speech will be strange and indirect. Through the focus on the monstrous character, No-Lips in the Zhuangzi, this paper argues that a key message of the Zhuangzi is that the art of transcending language in the Zhuangzi is through the use of crippled speech. The metaphor of crippled speech, speech which is actually unheard, illustrates that philosophical truths cannot be put into words but can be indirectly signified through the art of stretching language beyond its normal contours. This allows Eastern philosophy, through the philosophy of the Zhuangzi to transcend the limits of language. (shrink)

The need to prove the existence of the external world has been a subject that has concerned the rationalist philosophers, particularly Descartes and the empiricist philosophers such as John Locke, George Berkeley and David Hume. Taking the epoché as the key mark of the phenomenologist—the suspension of the question of the existence of the external world—the issue of the external world should not come under the domain of the phenomenologist. Ironically, however, I would like to suggest that it could (...) be argued that the founder of the phenomenological school of thought, Edmund Husserl, also did not avoid the question of the existence of the external world. What I would like to suggest further is that Immanuel Kant grants himself illicit access to the external world and thus illustrates that the question of the external world is vital to the argument structure of the first Critique. In an unsung yet excellent paper, W.Z. Harvey set out to explain how both Maimonides and Spinoza have similarly problematic views on the nature of the knowledge of good and evil. In it, he proposed an answer to solving the problem. In the many decades since, debates surrounding this topic have flourished. A recent paper by Joshua Parens, his conclusions mark a distinction between Spinoza and Maimonides that threaten to undermine Harvey’s solution to the problem. Lawrence Stenhouse was one of the most distinguished, original and influential educationalists of his generation. His theories about curriculum, curriculum development, pedagogy, teacher research, and research as a basis for teaching remain compelling and fresh and continue to be a counterpoint to instrumental and technocratic thinking in education. In this book, renowned educationalists describe Stenhouseâe(tm)s contribution to education, explore the contemporary relevance of his thinking and bring his work and legacy to the attention of a wide range of students, teachers, (...) teacher educators and others involved in education. Stenhouse saw the primary aim of education as the development of individuality through a creative and critical engagement with culture. He was an early advocate of inclusive education and was committed to making available to all pupils an education that was challenging and empowering. For Stenhouse many of the problems of education stemmed not so much from its content as from the terms and conditions under which students were required to access it. Consequently he pioneered an approach to curriculum reform that stressed the quality of the educational process and the values that defined it, as opposed to âe~rational curriculum planningâe(tm), which stressed the pre-specification of measureable learning outcomes. Stenhouse devised the curriculum reform movementâe(tm)s most ambitious strategy, âe~the process modelâe(tm), and was its principal theorist. His idea of âe~the teacher as researcherâe(tm) lay at the heart of this strategy as the means by which the values that define a worthwhile educational process could be progressively realized by teachers in concrete forms of action within their classrooms and schools. What marked out Stenhouseâe(tm)s unique contribution to the field of curriculum was his distinctive conceptualisation of the relationship between the teacher (authority), the learner (autonomy) and the subject matter (understanding). Founded on his epistemological scepticism and forged in his encounters with expertly discerning teachers who valued and nurtured the intellectual independence of students, Stenhouse acquired an acute appreciation of the ways in which teaching enhances or inhibits, develops or displaces the potential for autonomous thinking of students. He changed the relationship between curriculum theory, educational research and teachers; placing teachers right at the heart of the curriculum development process and the teacher as researcher at the heart of teacher professionalism. The Quine-Putnam indispensability argument urges us to place mathematical entities on the same ontological footing as (other) theoretical entities of empirical science. Recently this argument has attracted much criticism, and in this paper I address one criticism due to Elliott Sober. Sober argues that mathematical theories cannot share the empirical support accrued by our best scientific theories, since mathematical propositions are not being tested in the same way as the clearly empirical propositions of science. Ostensible contradictions between Augustine's account of the two cities are resolved by his concealed claim to the privileged epistemic status of a Christian prophet. Faith and grace provide the mobility between this quasi-divine and the fallen human position. Such mobility is impossible in a pluralist and secular system of thought. McDonald's thesis is that the chief villain in the unfolding tragedy of Canada is Pierre Elliott Trudeau and, to a lesser extent, his predecessor Lester B. Pearson. Goldwin Smith's well-known late-19th century comment that Canada was “rich by nature, but poor by policy” seems to hold true today. The secondlargest country in the world, rich in resources, Canada has been virtually bankrupted by government profligacy. Canada has been called “the impossible country,” and many have considered it “an impossible country to (...) govern.” The accumulated national debt is now reaching 600 billion in Canadian dollars (one Canadian dollar equals about $.72 US), which per capita is six times that of the US. The book features contributions from some of the most respected contemporary Nietzsche scholars, and each essay displays rigorous analysis while being written in an engaging style. -/- Many of these contributions are evidently written in response to Young’s own (...) provocative reading of Nietzsche as a communitarian thinker in Nietzsche’s Philosophy of Religion (Cambridge: Cambridge University Press, 2006), and Young presents aspects of this reading in the opening essay. He argues that Nietzsche’s “liberal communitarianism” (8) stems from his intellectual inheritance from Wagner of a distinctly Hegelian project, that of synthesizing the need for common meaning with the desire for liberal individual rights. Young provides a close reading of Wagner, identifying the liberal aspect of Wagner’s communitarianism in a form of “soft power” (9)—namely, the capacity to inspire, rather than coerce, the collective toward a shared ideal. This soft power opens the space for reconciliation between the desires of the individual and the goals of the community at large. Young contends that Wagner’s early Hegelianism “appears virtually word for word” in Nietzsche’s Birth of Tragedy (16). More controversially and less convincingly, he extends this liberal communitarianism across Nietzsche’s entire corpus. In particular, for Young, we should understand Nietzsche’s free spirits as “agents of change” (25), exemplary individuals who both establish and question the evaluative boundaries of the collective ethical imperative. -/- Hans Sluga’s chapter investigates Nietzsche’s call for a new “great politics” (32). Drawing comparisons with Plato, Sluga argues that Nietzsche views the lack of a socially recognized order of rank, a corollary of modern democratic pluralism, as the inception of political nihilism. Sluga locates this diagnosis in Human, All Too Human, where Nietzsche’s critique of modern democracy inaugurates his concern with the tensions between political processes and the prospect of cultural flourishing. For Sluga, however, it is in Beyond Good and Evil that Nietzsche floats the possibility of reconciliation between the two, with the prospect of exceptional individuals instigating a new political relationship. Sluga argues that Nietzsche sees the European notion of the nation-state as something that should and will be abolished. While taking Nietzsche’s recommendation to be problematic, Sluga concludes that his diagnosis is profound. This would accord with Ken Gemes and Chris Sykes’s approach in the following essay. Like Young, Gemes and Sykes emphasize Nietzsche’s kinship with Wagner, identifying the problem of meaning as central to both. Nietzsche is identified as conducting an anthropology of communities’ answers to the problem of meaning, or, rather, their creations of illusions to act as semblances of meaning. In his early works they take Nietzsche to hold that the foundations for the kind of cultural consecration necessary for the provision of collective meaning can be provided through myth, in the place of metaphysics. But, contra Young, Gemes and Sykes argue that Nietzsche develops away from his early communitarianism: first, toward an (ultimately pessimistic) investigation into the possibility of a scientific culture that provides meaning, and then by advocating a narrower individualism. For Gemes and Sykes, then, Nietzsche’s aspiration for communitarian cultural flourishing died at Bayreuth, and with Thus Spoke Zarathustra, Nietzsche makes a final attempt to provide a life-affirming “mythology” (72) for the flourishing of his intended recipients, now of a far more limited scope—namely, only truly exemplary individuals. -/- Here one might wonder whether Nietzsche’s thought of the eternal recurrence might better fit this need for the provision of a mythology. Background This analysis is about practical living bioethics and how law, ethics and sociology understand and respect children’s consent to, or refusal of, elective heart surgery. Analysis of underlying theories and influences will contrast legalistic bioethics with living bioethics. In-depth philosophical analysis compares social science traditions of positivism, interpretivism, critical theory and functionalism and applies them to bioethics and childhood, to examine how living bioethics may be encouraged or discouraged. Illustrative examples are drawn from research interviews and observations in two (...) London paediatric cardiac units. This paper is one of a series on how the multidisciplinary cardiac team members all contribute to the complex mosaic of care when preparing and supporting families’ informed consent to surgery. Results The living bioethics of justice, care and respect for children and their consent depends on theories and practices, contexts and relationships. These can all be undermined by unseen influences: the history of adult-centric ethics; developmental psychology theories; legal and financial pressures that require consent to be defined as an adult contract; management systems and daily routines in healthcare that can intimidate families and staff; social inequalities. Mainstream theories in the clinical ethics literature markedly differ from the living bioethics in clinical practices. Conclusion We aim to contribute to raising standards of respectful paediatric bioethics and to showing the relevance of virtue and feminist ethics, childhood studies and children’s rights. (shrink)

Elliott Sober is both an empiricist and an instrumentalist. His empiricism rests on a principle called actualism, whereas his instrumentalism violates this. This violation generates a tension in his work. We argue that Sober is committed to a conflicting methodological imperative because of this tension. Our argument illuminates the contemporary debate between realism and empiricism which is increasingly focused on the application of scientific inference to testing scientific theories. Sober’s position illustrates how the principle of actualism drives a wedge between (...) two conceptions of scientific inference and at the same time brings to the surface a deep conflict between empiricism and instrumentalism. Adaptationism is a doctrine that has meant different things to different people. In this essay, I want to isolate and discuss a reading of adaptationism that makes it a non-trivial empirical thesis about the history of life. I'll take adaptationism to be the following claim: natural selection has been the only important cause of most of the phenotypic traits found in most species. I won't try to determine whether adaptationism, so defined, is true. Rather, my task will be one of (...) clarification. What does this statement mean, and how is it related to various remarks that biologists have made about adaptationism, pro and con? (shrink)

In ‘Two Dogmas of Empiricism’, Quine attacks the analytic/synthetic distinction and defends a doctrine that I call epistemological holism. Now, almost fifty years after the article’s appearance, what are we to make of these ideas? I suggest that the philosophical naturalism that Quine did so much to promote should lead us to reject Quine’s brief against the analytic/synthetic distinction; I also argue that Quine misunderstood Carnap's views on analyticity. As for epistemological holism, I claim that this thesis does not follow (...) from the logical point that Duhem and Quine made about the role of auxiliary assumptions in hypothesis testing, and that the thesis should be rejected. It is a common occurrence that basic physical principle instructs us to construct some syntactically simple set of differential equations, but it then proves almost impossible to extract salient information from them. As Charles Peirce once remarked, you can’t get a set of such equations to divulge their secrets by simply tilting at them like Don Quixote. As a consequence, applied mathematicians are often forced to pursue (...) roundabout and shakily rationalized expedients if any useful progress is to be made. Often these provisional and quasi-empirical procedures within applied mathematics loom large in motivating the “anti-realist” or “anti-unificationist” conclusions of Nancy Cartwright and allied authors.
