Background Although medical ethicists and educators emphasise patient-centred decision-making, previous studies suggest that patients often prefer their doctors to make the clinical decisions. Objective To examine the associations between a preference for physician-directed decision-making and patient health status and sociodemographic characteristics. Methods Sociodemographic and clinical information from all consenting general internal medicine patients at the University of Chicago Medical Center were examined. The primary objectives were to (1) assess the extent to which patients prefer an active role in clinical decision-making, (...) and (2) determine whether religious service attendance, the importance of religion, self-rated spirituality, Charlson Comorbidity Index, self-reported health, Vulnerable Elder Score and several demographic characteristics were associated with these preferences. Results Data were collected from 8308 of 11 620 possible participants. Ninety-seven per cent of respondents wanted doctors to offer them choices and to consider their opinions. However, two out of three (67%) preferred to leave medical decisions to the doctor. In multiple regression analyses, preferring to leave decisions to the doctor was associated with older age (per year, OR=1.019, 95% CI 1.003 to 1.036) and frequently attending religious services (OR=1.5, 95% CI 1.1 to 2.1, compared with never), and it was inversely associated with female sex (OR=0.6, 95% CI 0.5 to 0.8), university education (OR=0.6, 95% CI 0.4 to 0.9, compared with no high school diploma) and poor health (OR=0.6, 95% CI 0.3 to 0.9). Conclusions Almost all patients want doctors to offer them choices and to consider their opinions, but most prefer to leave medical decisions to the doctor. Patients who are male, less educated, more religious and healthier are more likely to want to leave decisions to their doctors, but effects are small. (shrink)

BackgroundAs opioid use disorder (OUD) incidence and its associated deaths continue to persist at elevated rates, the development of novel treatment modalities is warranted. Recent strides in this therapeutic area include novel anti-opioid vaccine approaches. This work compares logistical and ethical considerations surrounding currently available interventions for opioid use disorder with an anti-opioid vaccine approach.MethodsThe opinions of student pharmacists and practicing pharmacists assessing knowledge, perceptions, and attitudes toward current and future OUD management strategies were characterized using a staged, multi-modal research (...) approach incorporating a focus group, pilot survey development and refinement, and final survey deployment. Survey responses were assessed using one- and two-way parametric and non-parametric analyses where appropriate, and multi-dimensional matrix profiles were compared using z-tests following an exhaustive combinatorial sum of differences calculation between items within each compared matrix.ResultsFocus group content analysis revealed a high level of agreeableness among participants regarding anti-opioid vaccine technology and a sense of shared ownership regarding solutions to the opioid epidemic at large. Pilot survey results demonstrated subject ability to consider both pragmatic and ethical considerations related to current therapeutics and novel interventions in a single instrument, with high endurance amongst engaged subjects. Access inequality was the most concerning ethical consideration identified for anti-opioid vaccines. Support for anti-opioid vaccine implementation across various clinical scenarios was strongest for voluntary use amongst individuals in recovery, and lowest for mandatory use in at-risk individuals.ConclusionsEthical and logistical concerns surrounding anti-opioid vaccines were largely similar to those for current OUD therapeutics overall. Anti-opioid vaccines were endorsed as helpful potential additions to current OUD therapeutic approaches, particularly for voluntary use in the later stages of clinical progression. (shrink)

This essay aims to show that republicanism does not necessarily preclude the notion of cosmopolitan citizenship. The first part challenges the belief that republican citizenship must be tied to a nationalist reading, therefore reducing its cosmopolitan extension to a mere metaphor. Having argued that the political attributes and philosophical account of the notion of citizenship evolve according to the historical transformation of political communities, our contemporary era renders the notion of cosmopolitan citizenship plausible. Far from being irreconcilable, liberal cosmopolitanism has (...) much to gain from republicanism since a thorough analysis of globalization reveals the limitations of the traditional liberal understanding of electoral and representative democracy. The second part of this article suggests that the republican theory of contestatory democracy enables us to better define the political attributes of cosmopolitan citizenship within a liberal conceptual framework. This study sheds new insight on how historically oppressed and marginalized actors are able to pursue environmental sustainability based on alternative worldviews (e.g., Indigenous worldviews) rather than succumbing to those dominant in the Western society, based on a study of the Syilx (“Okanagan”) people in British Columbia, Canada. We found that the Syilx people enacted the ethical practice of _nʕawqnwixʷ_ (“the reciprocal gentle dropping of thoughts, like water, into everyone’s minds to address the issue at the centre of discussion and (...) to reach collective consensus for action”), anchored in the Syilx worldview, to reconstruct the relationships between humans and nature. Two overlapping processes are involved: developing foundational principles for human–nature relations and carrying out reconstruction work. Ongoing enactment of _nʕawqnwixʷ_ practice provided community-based agency, enabling the Syilx people to shift the conversation around environmental sustainability. From this, we discuss the theoretical potential of community-based agency for the study of environmental sustainability, and the role of Indigenous worldviews for (re)imagining human-nature ethics (and reorienting the theoretical lens of human-animal or human-plant ethics form a firm-centric focus to a community-oriented lens), and important implications for practitioners and policymakers in the field of environmental sustainability. Though 2023 marks the sixtieth anniversary of Dr. Martin Luther King Jr.’s iconic “I Have a Dream” speech, my reflections on the theme of the 2023 annual meeting of the Society of Christian Ethics, “Vision, Imagination, and Dreams in the Work of Ethics,” are inspired by King’s lesser known “Beyond Vietnam” speech. I connect my hopes for the future of Christian ethics to King’s still unrealized vision of social transformation. It is one where the US (and other empires) would affirm—not (...) subvert—each people’s right to self-determination and we scholar-activists would continue to search for connections between and among social issues even if others insist upon their discreteness. We introduce an expected-value theory of linguistic modality that makes reference to expected utility and a likelihood-based confirmation measure for deontics and epistemics, respectively. The account is a probabilistic semantics for deontics and epistemics, yet it proposes that deontics and epistemics share a common core modal semantics, as in traditional possible-worlds analysis of modality. We argue that this account is not only theoretically advantageous, but also has far-reaching empirical consequences. In particular, we predict modal versions of reasoning fallacies from the (...) heuristics and biases literature. Additionally, we derive the modal semantics in an entirely transparent manner, as it is based on the compositional semantics of Korean modal expressions that are morphosyntactically decomposed into a conditional and an evaluative predicate. This paper investigates the concept of missio Dei at Willingen and beyond, and identifies its most remarkable feature which regards God as the initiator and subject of mission, thereby redefining missio ecclesiae with three striking characteristics: first, all places of the world including both the immediate neighborhood and the uttermost parts of the earths; second, all spheres of life such as society, politics, economy and culture; and finally, all events of the time such as catastrophes in the history.In so doing, (...) this paper clearly discovers that missio Dei is here approached primarily in a differentiation from or a sharp contrast to missio ecclesiae from the start, and that, for that reason, the concept of missio Dei at Willingen and beyond has not been fully trinitarian, though it often mentions the triune God. And it also discovers that it goes further either toward an emphasis on culture on the one hand, or toward that on the world on the other hand.Such being the case, this paper suggests that it is necessary to consider the implications of the doctrine of the Trinity for mission more fully to reconfigure the concept of missio Dei. Due to some limits, this paper does not deal with this issue full-fledgedly, but intends to suggest a couple of guidelines for doing so. First, we need to approach missio Dei quite differently, that is, primarily not in relation to missio ecclesiae but in relation to processio Dei, that is, the procession of the triune God. Second, noting that, since the early church, missio Dei has been understood primarily in relation to processio Dei, we need to keep in mind that we could not discuss missio more fully without dealing with processio, and vice versa. If we have these two guidelines in mind, the concept of missio Dei would be much more abundant and fruitful. Further studies on some particular implications of the doctrine of the Trinity for mission need to be done in missiology and also theology in general in the near future. (shrink)

This book takes concepts developed by researchers in theoretical computer science and adapts and applies them to the study of natural language meaning. Summarizing over a decade of research, Chris Barker and Chung-chieh Shan put forward the Continuation Hypothesis: that the meaning of a natural language expression can depend on its own continuation.

This paper presents syntactic and semantic rules for a fragment of English with mixed quotation. The fragment shows that quotation has a recursive and compositional structure. Quoted expressions turn out to denote characters, so the semantics of quotation simulates the pragmatics of speech, including dependence on utterance contexts and reference to mental entities. The analysis also accommodates varieties of unquotation, pure quotation, and causal reference.

We present a general theory of scope and binding in which both crossover and superiority violations are ruled out by one key assumption: that natural language expressions are normally evaluated (processed) from left to right. Our theory is an extension of Shan’s (2002) account of multiple-wh questions, combining continuations (Barker, 2002) and dynamic type-shifting. Like other continuation-based analyses, but unlike most other treatments of crossover or superiority, our analysis is directly compositional (in the sense of, e.g., Jacobson, 1999). In particular, (...) it does not postulate a level of Logical Form or any other representation distinct from surface syntax. One advantage of using continuations is that they are the standard tool for modeling order-of-evaluation in programming languages. This provides us with a natural and independently motivated characterization of what it means to evaluate expressions from left to right. We give a combinatory categorial grammar that models the syntax and the semantics of quantifier scope and wh-question formation. It allows quantificational binding but not crossover, in-situ wh but not superiority violations. In addition, the analysis automatically accounts for a variety of sentence types involving binding in the presence of pied piping, including reconstruction cases such as Whose criticism of hisi mother did each personi resent? It is widely presumed that intuitions about thought experiments can help overturn philosophical theories. It is also widely presumed, albeit implicitly, that if thought experiments play any epistemic role in overturning philosophical theories, it is via intuition. In this paper, I argue for a different, neglected epistemic role of philosophical thought experiments, that of improving some reasoner’s appreciation both of what a theory’s predictions consist in and of how those predictions tie to elements of the theory. I call this role (...) theory clarification. I show that theory clarification does not proceed via intuition, and I argue that it is only in conjunction with theory clarification that intuitions about thought experiments can help overturn philosophical theories. I close by sketching how a more radical view might be true, on which thought experiments help justify the rejection of philosophical theories exclusively by clarifying theories, not by any intuitions those thought experiments might generate. In this article, we examine the effect of corporate social responsibility (CSR) on firms' financial performance (CSR-effect). Two competing hypotheses, social impact hypothesis and shift of focus hypothesis, are proposed to investigate this issue, where the former suggests that CSR has a positive relation with performance and the latter are opposite. In order to ensure the CSR-effect is not contaminated by other faeton or samples are randomly drawn, we employ four matching methods, Nearest, Caliper, Mahala and Mahala Caliper to match (...) the samples of CSR (CSR-firms) and without CSR (NonCSR-firms) with similar characteristics. Although four methods yield slightly different results, firms engaging in CSR activities tend to obtain significantly higher values on pretax income to net sales and profit margin, and adopting CSR at the very least not deteriorate the performance of firms, making our con clusion favors the social impact hypothesis and against shift of focus hypothesis in Taiwan. Thus, ambition and conscience are not conflicting with each other. (shrink)

I develop and defend the view that subjects are necessarily psychologically able to revise their beliefs in response to relevant counter-evidence. Specifically, subjects can revise their beliefs in response to relevant counter-evidence, given their current psychological mechanisms and skills. If a subject lacks this ability, then the mental state in question is not a belief, though it may be some other kind of cognitive attitude, such as a supposi-tion, an entertained thought, or a pretense. The result is a moderately revisionary (...) view of belief: while most mental states we thought were beliefs are beliefs, some mental states which we thought were beliefs are not beliefs. The argument for this view draws on two key claims: First, subjects are rationally obligated to revise their beliefs in response to relevant counter-evidence. Second, if some subject is rationally obligated to revise one of her mental states, then that subject can revise that mental state, given her current psychological mechanisms and skills. Along the way to defending these claims, I argue that rational obligations can govern activities which reflect on one’s rational character, whether or not those activities are under one’s voluntary control. I also show how the relevant version of epistemic ‘ought’ implies ‘can’ survives an objection which plagues other variants of the principle. (shrink)

It has created a sense of alienation, dislocation, exile and subordination for many women. Due to colonialism, globalization and migration, many women are living in other places than their birth places. Asian American women are not exempt from the affects of colonialism and they are experiencing the affect of being the (...) other and the foreigner. In particular, the Asian understanding of Chi can nurture a stronger (...) theological perspective of the Holy Spirit. This paper will examine Chi and illustrate the similarities it shares with the Holy Spirit to work towards a pneumatology which will encourage people to live harmoniously and peacefully with one another. Korean American women are the foundation of the Korean American church. We are devoted, contributing members in the church, but we are seldom given positions of leadership or power. From our subordinate role in the church and wider society, Korean American women have been perpetually subject to racial and gender injustice. To work toward equal empowerment, it is imperative to reimagine historical Christian teaching about God so that it liberates rather than oppresses. As we engage in theological reform, we can (...) begin to experience the wholeness that comes from a Spirit God who embraces all people regardless of race, gender, sexuality, or social status. As a result, Korean American women can finally feel included and contributive to a society which has historically treated them as “perpetual foreigners.” They can also push for multicultural excellence rather than sustaining the dominant white criterion of value. As hybrid spaces proliferate in diversifying America, Korean American women can be an integral source of reimagining the places we inhabit, something that proves to be increasingly necessary to keep the church accessible and contemporary. Traditional Western Christology, focussed on the maleness of Christ, has skewed and limited Christian understandings of God away from female images of the divine and towards masculine language and imagery. The creation of a male norm within the Western Christian tradition has marginalized women of all colours and cultures. Asian North American women have struggled not only to make new sense of 'God-language', but also to harmonize traditional Christian claims with the world-view and culture of the East. Since Christian feminists (...) have attempted to recover a more positive God-language by use of the concept of Sophia Wisdom, this article suggests that an effective strategy would be for Asian women to inculturate the language of Sophia into Eastern philosophical categories. There is a long-standing discussion on the positive interactions between enterprise value creation and business competitiveness. The corporate value can be seen as being created from three major sources within the cycle - from employees, from processes, and from customers or investors through reinvestment. To achieve competitive advantages, a firm must create more value than its competitors in the industry. Emphasizing that, firms should explore the positive drivers of customer value creation, allowing for a true value creation that will lead (...) to increments in competitiveness. In reality, however, there are also barriers that hinder customer value creation. Targeting the above issues that have not yet been explored or analyzed, we have collected related literature at the first stage. Based on these presumable assumptions, this paper then conducts an empirical study by surveying and analyzing the relevance given by the investigated leading machinery measuring equipment firms in Taiwan, regarding the concerns as drivers and barriers in relation to customer value creation. This paper expecially aims to answer several key questions: What drivers revolving around employees and processes can facilitate the organization to create more value for its customers? Conversely, what barriers block the organization from creating value for customers in examining the same dimensions? Does value creation direct an organization's profitability and competitiveness? Our questionnaire survey results show that the most recognized and agreed drivers of customer value creation in consideration of employees are "distinctive skills", "personal experience", "learning and training", and "team work"; and, in regard to the firm's processes, the key drivers are "innovation and evolution", "R&D capability", and "capability for differentiation". Conversely, the most recognized and agreed barriers to customer value creation in relation to employees are a "distrustful environment" and "inadequate knowledge"; and, in terms of processes, they are "short of core technology", "poor resource support", and "bad services and attitudes". Furthermore, our in-depth interview outcomes reveal that "capital sufficiency" and "mergers and acquisitions" are in practice considered to be other important customer value creation drivers; in contrast, "cultural and structural barriers" and "short of mechanisms to measure customer value creation effectively" are viewed as additional critical barriers to customer value creation. Recently, several theorists have proposed that we can perceive a range of high-level features, including natural kind features (e.g., being a lemur), artifactual features (e.g., being a mandolin), and the emotional features of others (e.g., being surprised). I clarify the claim that we perceive high-level features and suggest one overlooked reason this claim matters: it would dramatically expand the range of actions perception-based theories of action might explain. I then describe the influential phenomenal contrast method of arguing for high-level perception (...) and discuss some of the objections that have been raised against this strategy. Finally, I describe two emerging defenses of high-level perception, one of which appeals to a certain class of perceptual deficits and one of which appeals to adaptation effects. A large number of studies have investigated achievement goals and their related antecedents and consequences above elementary school level. However, few studies have implemented achievement goal assessment to investigate achievement goals and their relevance for preschoolers. In particular, no valid measurement has been developed for preschoolers’ self-reporting of their achievement goals. The main purposes of this study were twofold: To develop an innovative achievement goal measurement for preschoolers, and to investigate the best theoretical model for understanding preschoolers’ achievement goal across (...) gender. To examine the effectiveness and efficiency of the pictorial and pure text measurement format and approaches. A total of 364 preschoolers aged 5 years participated in self-report activity, and 32 preschool teachers obtained consent to rate 193 out of 364 preschoolers. Results showed: the developed achievement goal measurement was a valid tool for understanding preschoolers’ achievement goals and was equally suitable for boys and girls. The 6-factor achievement goal model was the best theoretical perspective for understanding preschoolers’ achievement goals for both boys and girls. The pictorial measurement format for preschoolers’ self-reporting of achievement goals was a more effective but less efficient way to investigate preschoolers’ achievement goals, while the opposite was the case for the pure text measurement format for teachers’ ratings. Implications for achievement goal literature and future research are discussed. (shrink)
