When Stanford M. Lyman authored The Seven Deadly Sins: Society and Evil in 1978 it was hailed by Alasdair MacIntyre as 'a book of absorbing interest and importance_[that] places us all in his debt.' By Nelson Hart as 'a masterful and thought-provoking book_[that] is the only scholarly treatment of sin that is so well-informed by the best of ancient through modern perspectives.' By James A. Aho as a work whose 'abstract hardly does justice to the scholarly (...) and detailed analysis of sin.' And by Harry Cohen as a 'book_[that] stands as a beautiful illustration of what holistic, idiosyncratic, interdisciplinary, and creative thinking and writing can bring to bear on the age-old problem of society and evil.' The American Sociological Association's section on the Sociology of the Emotions selected this book as one of the works that laid the foundations for the study of pride, lust, envy, and anger—basic sentiments embedded in the social process. This work is a "prolegomena to the study of evil," the beginning of a more ambitious project designated by the author an "ideational critique of society." Such an endeavor would include a "rhetoric that grasps the structures of consciousness, the phenomenology of history, and the dramaturgy of contemporary scenes." The bulk of the present study constitutes an essay in phenomenological sociology. Each of the seven deadly sins is insightfully described in terms of its dominant features as well (...) as in relation to and distinction from allied or similar phenomena. Significantly, the presiding intellectual presences are as often Homer, Augustine, Gregory the Great, and Aquinas as more contemporary ones such as Simmel, Marx, Weber, and Freud. The scope and sensitivity of the author is further manifested in his illuminating use of literary figures including Dante, Shakespeare, Chekhov, and Artaud. Lyman maintains that "sin has been neglected by sociology" and views this work as a first step in remedying such neglect. At the outset he states that he is not offering a way out of or elimination of evil nor the promise of good. Fortunately, much of the book and in particular its concluding chapter serves as at least a strong qualification of this posture. Though holding out no hope for redemption or a final release from sin and evil, Lyman’s effort would seem to have a commendable melioristic aim. He is concerned, however, to avoid what is for him the besetting sin of contemporary sociology—the attempt to present a sociological blueprint for earthly happiness. Lyman would like to see sociology respect "the social visions that the reason of ordinary people produces," while neither applauding nor opposing them. "The sociological task would then be to describe the processes of social architectonics, and not to build the new society in advance of them." It is evident that Lyman wishes to avoid explaining away sin or reducing it to psychological or societal deviance but it would have been helpful to have had some suggestion of his ontology or metaphysics of sin. The "reality" of sin and evil remains unclarified throughout these analyses. The citations from Homer, Augustine, Gregory the Great, Dante, and other such figures are presented without irony or condescension. Now it may be possible to distill psychological and sociological insights from these thinkers while rejecting or ignoring their theological dimensions. To do so, however, involves a process of abstraction and a contextualism which hardly leaves the phenomena as presented by these thinkers untouched and unchanged. Hence, it would seem that some justification for the "sin" character of these actions in their contemporary milieu is called for. Lyman hints that he is unhappy with the neutralization of sin resulting from contemporary social science. What is not clear is whether he rejects the naturalistic assumptions which undergird and indeed make inevitable such neutralizing interpretations. The background assumption of Lyman’s study is a Nietzschean-like "sociology of the absurd" which presupposes "a world that is ultimately and essentially without meaning" but is made meaningful "by the actions and beliefs of the people who participate in it." This is, of course, a radically different world than the one within which the sin-evil language emerged and gave this language its depth and significance. One aspect of Lyman’s continuing task, therefore, would be to show how it is possible to speak meaningfully of sin and evil when the belief in a transcendent which constituted these phenomena in their classical modes is absent. Nietzsche’s challenge must be faced: after the "death of God" we can still speak of "good and bad" but can we any longer speak of "good and evil"?—E.F. (shrink)

Chronic obstructive pulmonary disease (COPD) is one of the most deadly and costly chronic diseases in the world characterized by many breathing problems. The management of COPD and the prevention of exacerbations are a priority goals to improve the quality of life in patients affected by this illness. In addition, it is also crucial to improve the patients’ adherence to care which, in turn, depends on their knowledge and understanding of some factors such as the prescribed medical treatment, (...) changes in dailylife, and the process of breathing. In turn, the adherence to care leads to greater autonomy for the patient who is thus able to better manage his illness. Here we presented the application of the Model IARA in patients affected by COPD in order to achieve their autonomy in illness management which, in turn, leads to a better quality of life. IARA is an intervention program which improve the awareness and knowledge of patients with respect to both the disease and symptoms through health education. Moreover, through IARA the patients are encouraged to become more actively involved in COPD care process, also regarding drug therapy adherence. Using St. George's Respiratory Questionnaire combined with qualitative analysis, we demonstrated that IARA could be considered a useful approach in COPD management. (shrink)

You will typically write a paper or an essay in answer to a prompt or a question. The sin of the Minimal Answer is to write the absolute minimum that could possibly be considered a complete answer to the question you have been asked. This strategy might be perfectly appropriate for exams that test factual recall, for example, because providing extra information is just an extra opportunity to be wrong about something, but essay writing (in papers or exams) is not (...) like that. An essayÕs quality is determined in large part by the depth of understanding of the issues that it exhibits, the comprehensiveness with which it treats them (within the limits specified by the assignment), and by how thorough and convincing a case the essay makes for its thesis. Thus, essay writing is always an open-ended affair in which the quality of the finished product is determined by how much you are able to do with the question or prompt, not how little you can put down that technically constitutes an answer to it. There are many similarities between the Christian concept of the Seven Deadly Sins, the Stoic concept of internal events, the Hebrew concept of 'awon (sin, punishment), and the existential concept of bad faith. All of these evidence a concern for an internalization of sin or bad faith, which is more common in Eastern than Western religions. These four views are contrasted with the English concept synne which represents the Western extemalization of sin or bad faith. The Water Margin is a great Chinese classical novel; Wu Song’s 武松 killing of his sister-in-law, Pan Jinlian 潘金蓮, is one of the most popular episodes of the novel. It depicts Wu as the hero and defender of traditional values, and Pan as the adulterous woman. In contemporary discussion, there has been a dearth of ethical analyses regarding Wu’s killing of Pan. How should we judge the moral status of his action? Does the killing signify Wu Song’s ethical achievement or (...) his ethical failure? What does the killing tell us about Wu’s character or his virtues? Does our appraisal of Wu’s action square with our modern belief regarding the treatment of women? For over half a century, scholars have laboured to show that C. S. Lewis's famed but apparently disorganised Chronicles of Narnia have an underlying symbolic coherence, pointing to such possible unifying themes as the seven sacraments, the seven deadly sins, and the seven books of Spenser's Faerie Queene. None of these explanations has won general acceptance and the structure of Narnia's symbolism has remained a mystery. -/- Michael Ward has finally solved the enigma. In Planet (...) Narnia he demonstrates that medieval cosmology, a subject which fascinated Lewis throughout his life, provides the imaginative key to the seven novels. Drawing on the whole range of Lewis's writings (including previously unpublished drafts of the Chronicles), Ward reveals how the Narnia stories were designed to express the characteristics of the seven medieval planets - - Jupiter, Mars, Sol, Luna, Mercury, Venus, and Saturn - - planets which Lewis described as "spiritual symbols of permanent value" and "especially worthwhile in our own generation". Using these seven symbols, Lewis secretly constructed the Chronicles so that in each book the plot-line, the ornamental details, and, most important, the portrayal of the Christ-figure of Aslan, all serve to communicate the governing planetary personality. The cosmological theme of each Chronicle is what Lewis called 'the kappa element in romance', the atmospheric essence of a story, everywhere present but nowhere explicit. The reader inhabits this atmosphere and thus imaginatively gains connaître knowledge of the spiritual character which the tale was created to embody. -/- Planet Narnia is a ground-breaking study that will provoke a major revaluation not only of the Chronicles, but of Lewis's whole literary and theological outlook. The article presents the results of an interview in a student’s group on the problem of the fall into sin based on the discussion at the seminar of Dante's «divine Comedy». The authors consider human as an image and likeness of God, who creates himself, choosing between good and harm. There were changes in the perception of the structure of Inferno: the number of circles of hell in the minds of young people interviewed decreased: all respondents do not see (...) sin in unbelief, three-quarters of respondents − in gluttony, half − in lust. The inter-pretation of heresy and fraud as ways to promote their projects is relevant. The effect of absolute rejection of violence, contemptuous attitude to treachery is revealed. The authors believe that secularization has touched only the first five circles of Hell, but at the bottom everything is still the same. (shrink)

Besides commentaries on the āgamas, his writings also cover almost all other aspects of Jaina philosophy. The Yogadḍḍḍisamuccaya (hereafter YDS), as the title suggests, discusses aspects of yoga in Jainism with reference to other schools of Indian philosophy. The book under review here, Reconciling Yogas: Haribhadra's Collection of Views on Yoga, by Christopher Key Chapple, consists of seven chapters of essays on the YDS and its author, a new English translation with Sanskrit text, and notes, a bibliography, and an index.In the first chapter, which deals with the writings of Haribhadra, especially the YDS, the author reviews opinions on his dates and agrees with the view that he lived [End Page 681] in the eighth century. These are identified, as the chapter title indicates, as Vedānta yoga and Buddhist yoga.In chapter 4, on the philosophical background of the various yogas, the author points out that although Haribhadra was familiar with the thought of other schools, he proclaimed the Jaina tenet of realism and criticized Buddhist momentariness and Vedāntin monism. In the last chapter Christopher Chapple examines Haribhadra's thoughts on yoga from a sociological standpoint; Chapple also assigns contemporary yoga movements to four groups in accordance with the types of yogins mentioned in the YDS.The author points out that in the Jaina tradition the word "yoga" fundamentally stands for any actions by living beings and the behavior required of pious Jainas (p. 28). In the YDS, however, Haribhadra uses this word to denote activities involved in reaching higher stages of spiritual awareness. This unique usage of the word shows Haribhadra's attitude toward "reconciling [the] Yoga" of Jainism and that of other schools, especially that of Patanñjali.Among the contributions made by this book to Jaina studies and Indology are (1) a clear explanation of the relationship between guḍasthāna in Jainism and Patañjali's Yoga system and (2) an attempt to recover the lost yogas, that is, the Buddhist yoga and Vedāntist yoga mentioned in the YDS. These two topics will be examined in more detail below.Guḍasthāna and YogaPatañjali's yoga system in essence is regarded as a course of spiritual purification. The Jainas also have a similar system, known as guḍasthāna, which consists of fourteen stages of spiritual development. This idea of guḍasthāna does not appear in the earlier canons, so it must be a comparatively new development in the history of Jainism. By the time of Haribhadra, guḍasthāna must have been popular among the Jainas as he explicitly uses the term in the YDS and tries to allot stages to each of eight limbs or aḍgas in Patañjali's system. Cognitive Linguistics is an approach to language study based on three central premises: that the function of language is to convey meaning, that linguistic description must rely on constructs that are psychologically real, and that grammar emerges from usage. Over the last 40 years, this approach to studying language has made enormous strides in virtually every aspect of linguistic inquiry, achieving major insights as well as bringing about a conceptual unification of the language sciences. However, it has also faced problems, (...) which, I argue, must be addressed if the approach is to continue to flourish. Some of these are shared with generative linguistics, while some are peculiar to the cognitive approach. The former include excessive reliance on introspective evidence; paying only lip service to the Cognitive Commitment; too much focus on hypothesis formulation (and not enough on hypothesis testing); ignoring individual differences; and neglecting the social aspects of language. The latter include assuming that we can deduce mental representations from patterns of use and equating distribution with meaning. I conclude by sketching out how these pitfalls could be avoided. Most donated organs in the United States come from brain dead donors, while a small percentage come from patients who die in “controlled,” or expected, circumstances, typically after the family or surrogate makes a decision to withdraw life support. The number of organs available for transplant could be substantially if donations were permitted in “uncontrolled” circumstances–that is, from people who die unexpectedly, often outside the hospital. According to projections from the Institute of Medicine, establishing programs permitting “uncontrolled donation after circulatory (...) determination of death,” or uDCDD, throughout the United States has the potential to provide 22,000 more donation opportunities annually. In contrast, U.S. controlled donation after circulatory determination of death, or cDCDD, cases have increased progressively over the past decade from 87 to 848 donors, but currently account for only 10.6 percent of all deceased donors. Following the IOM recommendations, several projects exploring the feasibility of uDCDD were funded by the federal government, including a grant from the Health Resources and Services Administration that supported a pilot project in New York City in which the authors of this article participated.A key feature of our protocol, and indeed of many uDCDD protocols, is the initiation of preservation methods such as chest compressions and extracorporeal membrane oxygenation shortly after death in order to perfuse and preserve the donor's organs. Critics of uDCDD argue that the means of determining death deviates from generally ascribed principles. They assert that reinstituting circulation in order to preserve organs has the effect of “undoing” the prior determination of death. The result is that cDCDD is widely accepted and practiced routinely even though it only marginally increases the number of organs available for transplantation, and uDCDD is widely considered unacceptable despite being ethically embraced and proven to significantly increase organ donation opportunities in other countries. This article explores the evolution of this counterintuitive state of affairs and calls for a policy that, in line with the IOM report, allows for both cDCDD and uDCDD protocols. (shrink)

The author attempts here to sketch the beginnings of an adequate interpretation of Plato's treatment of the tall and the equal in the "Phaedo". The paper consists of seven sections (roman numerals). In I-II, he (a) argues that any attempt to solve the puzzle stated at "Phaedo" 102 bc within the parameters there set down would "eo ipso" be an attempted theory of relational statements; (b) formulates that puzzle; and (c) shows that Frege solved it by denying its (...) presuppositions. In IV the author proposes an interpretation of Plato's solution: that the "than y" in "x is taller than y" modifies the "is." In V-VI he shows how this theory fits in with and sheds light on other aspects of the theory of forms: the connection to Non-Contradiction, The doctrines of degrees of reality and of immanent characters. Finally in VII he shows that the puzzle of relational statements as formulated in the "Phaedo" need not extend to "equal." the paper also includes (iii) a discussion of H-N Castañeda's interpretation of Plato's theory. In this paper I will argue that a profile of the pseudo-sciences can be gained from the scientific pretensions of the pseudo-scientist. These pretensions provide two yardsticks which together take care of the charge of scientific prejudice that any suggested demarcation of pseudo-science has to face. To demonstrate that my analysis has teeth I will apply it to Freud and modern-day Bach-kabbalists. Against Laudan I will argue that the problem of demarcation is not a pseudo-problem, though the discussion will bear (...) out that Laudan's replacement question, namely the question whether someone's theory is well-confirmed, is not, as Lugg claimed, independent of the question as to whether that person is a pseudo-scientist. I further argue that my prototype pseudo-scientists do not have the shortcomings highlighted in Thagard's recent analysis of pseudo-science. (shrink)

Disparities in access to, and utilization of, treatment for depression among African-American and Caucasian elderly adults have been well-documented. Less fully explored are the multidimensional factors responsible for these disparities. The intersection of cultural constructs, socioeconomic factors, multiple levels of racism, and stigma attending both mental health issues and older age may help to explain disparities in the treatment of the depressed elderly. Personalized medicine with its promise of developing interventions tailored to an individual's health needs and genetically (...) related response to treatment might seem a promising antidote to the documented underutilization of standard depression treatments by African Americans. However, this paper examines the multidimensional factors associated with disparities in effective treatment of depression among African-American and Caucasian elderly adults and argues the scientific and ethical importance of pursuing various paths to address multiple levels and sources of stigma and mistrust if pharmacogenomics is to help, rather than exacerbate, disparities in depression treatment. Seven recommendations are offered to increase the likelihood that developments in pharmacogenomics will reduce disparities in depression treatment. (shrink)

OBJECTIVES: Life-sustaining treatment at the end of life gives rise to many ethical problems in Japan. Recent surveys of Japanese physicians suggested that they tend to treat terminally ill patients aggressively. We studied why Japanese physicians were reluctant to withhold or withdraw life-support from terminally ill patients and what affected their decisions. DESIGN AND PARTICIPANTS: A qualitative study design was employed, using a focus group interview with seven physicians, to gain an in-depth understanding of attitudes and rationales in (...) Japan regarding medical care at the end of life. RESULTS: Analysis revealed that physicians and patients' family members usually make decisions about life-sustaining treatment, while the patients' wishes are unavailable or not taken into account. Both physicians and family members tend to consider withholding or withdrawing life-sustaining treatment as abandonment or even killing. The strongest reason to start cardiopulmonary resuscitation- and to continue it until patients' family members arrive-seems to be the family members' desire to be at the bedside at the time of death. All physicians participating in our study regarded advance directives that provide information as to patients' wishes about life-sustaining treatment desirable. All expressed concern, however, that it would be difficult to forego or discontinue life-support based on a patient's advance directive, particularly when the patient's family opposed the directive. CONCLUSION: Our group interview suggested several possible barriers to death with dignity and the appropriate use of advance directives in Japan. Further qualitative and quantitative research in this regard is needed. (shrink)

The number of women living with HIV/AIDS is increasing worldwide, and there is an urgent public health need to develop new user-initiated HIV prevention methods, including microbicides. Although funding for microbicide development has increased since 2000, financial support is provided predominantly by governmental agencies and private foundations. Many donors, including the US Agency for International Development (USAID) and the US National Institutes of Health (NIH), have policies that restrict how research funds may be used. Among these are the now-rescinded Mexico (...) City Policy, elements of the US Foreign Assistance Act, and restrictions on non-study-related care. The effect of these restrictions on the design and conduct of clinical research is poorly understood. As part of a recent mapping exercise conducted by the Global Campaign for Microbicides, we reviewed the impact of donor restrictions on seven HIV prevention trials. We found considerable confusion within the HIV prevention field as to whether and how Mexico City and other policies affect the use of research funds. We also found that these donor-imposed policies limited the level of care provided to trial participants and the types of capacity building projects undertaken. (shrink)

While tracing the principal streams (...) of Western thought which irrigate Dante’s fertile genius, Mazzotta reconstructs the intellectual topography of Dante’s Europe and show how “l’altissimo poeta” reorders and refines it and arrives at a radical reformulation of the foundation of knowledge in contrast to the standard philosophical and theological currents of the times. This new vision of the foundation of knowledge is the central concern of The Divine Comedy which for Mazzotta provides a world view no less monumental than the synthesis of the classical and Judeo-Christian traditions of Thomas Aquinas. Perhaps, more so, for he argues Dante’s great epic provides not only an alternative epistemology but one which captures much more adequately the human historical experience.Vico’s insights, a thorough familiarity of Dante’s sources and his alert appreciation of the unique treatment these sources receive in the context of “la commedia divina” supply Mazzotta with a theoretical framework for his own brilliant critical, historical, and etymological analyses. He marshals these researches around his overriding concern to show that inherent within the Comedy is the view of “poetry as nothing less than the foundation of all possible knowledge” (p. 3). Dante’s radical claim is designed to provide an account which accords with the educational and intellectual development of the individual within history. Consequently for Mazzotta, Dante’s poetic (metaphorical) mode attempts to reconcile what must have been an intellectual and moral landscape of hopeless contradictory systems and projects. Dante’s poem [End Page 194] provides a vivid survey of the breakdown of human bonds, human projects, and human justice and suggests a corrective that escaped the methods of reason, as well as those of the mystic’s. Since both of these are at bottom antihistorical, they attempted to deny conflict and uncertainty (Dewey’s ultimate traits of experience).The above comments should suggest both the general appeal as well as, and I must emphasize, the high level of scholarship Mazzotta exhibits in his project which is arranged into three parts. The first part (Chapter 1–5) is devoted to “poetry and the arts of language,” and a quick glance at these chapters reveals that it is concerned with that part of the liberal arts called the trivium (grammar, rhetoric, and dialectic). In addition to their educational role, the author examines the role and intricate connections these disciplines have to the central social structures of the times. Consequently, these are not mere sterile academic exercises but serious attempts that show the intimate connections between methods of knowing (discourses) and social structures, especially authority in all of its modes (moral, intellectual, political, etc.). Further, by virtue of this, he shows the need for a reexamination of knowledge which does justice to the lived experiences of the individual and to which the second part (Chapters 6–8) is devoted. Briefly, this means coming to terms with language, memory, imagination, dreams and visions, and that great nightmare, human history. It also invokes the aesthetic (poetic/metaphorical) foundations of all knowledge in contrast to the Cartesian dismissal. The claim, however, enlightening for the Comedy, is controversial as a general tenet and demands will be forthcoming for an evaluation, even though there is much good work in philosophy which lends support to it, e.g., Stephen Pepper’s World Hypotheses, R. G. Collingwood’s The Principles of Art, and Lewis E. Background Discussions on ethical aspects of life-and-death decisions within the hospital are often made in plenary. The prehospital physician, however, may be faced with ethical dilemmas in life-and-death decisions when time-critical decisions to initiate or refrain from resuscitative efforts need to be taken without the possibility to discuss matters with colleagues. Little is known whether these considerations regarding ethical issues in crucial life-and-death decisions are documented prehospitally. This is a review of the ethical considerations documented in the prehospital medical records (...) of patients in a Danish prehospital setting for whom the decision to resuscitate or not was made at the scene. Methods The study is based on discharge summaries of all patients subjected to crucial life-and-death decisions by the Mobile Emergency Care Unit in Odense in the years 2010 to 2014. The medical records with possible documentation of ethical issues were independently reviewed by two philosophers in order to identify explicit ethical or philosophical considerations pertaining to the decision to resuscitate or not. Results In total, 1275 patients were either declared dead at the scene without exhibiting layman’s reliable signs of death or admitted to hospital following resuscitation. In a total of 62 patients, 85 specific ethical issues related to resuscitation were documented. The expressions of the ethical considerations were generally vague or unclear and almost exclusively concerned the interests of the patient and not the relatives. In the vast majority of cases where an ethical content was identified, the ethical considerations led to a decision to terminate treatment. Conclusions A strengthened practice of documenting ethical considerations in prehospital life-and-death decision-making in the patient’s medical records is required. We suggest that a template be implemented in the prehospital medical records describing the basis for any ethical decisions. This template should contain information regarding the persons involved in the deliberations and notes on ethical considerations. The documentation should include considerations concerning the patient’s end-of-life wishes, the estimations of the quality of life before and after the incident, and a summary of other ethical concerns taken into account, such as the integrity of the patient and frame of mind of relatives. ObjectiveTo assess the scientific and ethical basis for clinical innovation in psychopharmacology.MethodsWe conducted a literature review, utilizing MEDLINE search and bibliographic cross-referencing, and historical evidence regarding the discovery and development of new medications in psychiatry. Clinical innovation was defined as use of treatments in a clinical setting which have not been well-proven in a research setting.ResultsEmpirical data regarding the impact of clinical innovation in psychopharmacology are lacking. A conceptual and historical assessment of this topic highlights the ethical and scientific importance (...) of clinical innovation. Ethically, it touches a borderline that, in our judgment, is not adequately framed in contemporary mainstream bioethics. Currently, research is viewed as not at all benefiting the patients who participate in it, while clinical care is viewed as being solely for the benefit of patients. Clinical innovation straddles these two worlds, uncomfortably at times. While many argue that clinical innovation should either be avoided or folded into research projects, we argue that clinical innovation is necessary for progress in psychopharmacology research, and that it can prosper best when guided by the following ethical principles: 1.) The treatment should be based on a viable hypothesis. 2.) Whenever possible, one's clinical observations should be reported so they can be evaluated by the scientific community. 3.) One should be willing to report unexpected observations of drug effects. 4.) A high standard of informed consent should be maintained. Again, this proposal goes against the standard view among bioethicists that research and clinical care are categorically opposed activities, as made clear by the either-or dichotomy of the Belmont Report on bioethics. This approach has so polarized our profession into clinicians versus researchers, that many clinicians will not apply new knowledge produced by clinical research until it eventually gets incorporated into formal treatment guidelines, while researchers have little to guide them as to what kind of new knowledge it is most important to provide.SummaryClinical innovation brings out the ambiguities in our current ethical conceptions of research versus clinical care. Yet, historically, clinical innovation has been an important contributor to progress in psychopharmacology.
