There is a need to increase the number of practicing medical doctors in South Africa. We examine the ethical implications of patients’ rights being affected in medical education in a South African context.The South African legal framework advocates public healthcare access. Yet, the State’s ethical obligations when it comes to guaranteeing public healthcare access, conflict with its utilitarian policy, that allows for medical education to help achieve the State’s public healthcare commitments, at the cost of eroding patients’ rights, and accepts (...) that certain actions are imperative, in line with Ubuntu, which is tenable yet nuanced. A patient treated by a licenced doctor today, benefits because other patients have allowed themselves to be used as hands-on learning material for medical students yesterday.Healthcare institutions need to take cognisance of the numbers of medical students that patients can reasonably be expected to endure. There is a need for the Health Professions Council of South Africa and medical schools to adopt guidelines on reasonable levels of medical student-patient interaction, and medical student-to-patient ratios in healthcare delivery. A book on human communication that is worthy of its subject must introduce the reader to the dynamic interaction of a number of diverse fields. Colin Cherry's book, over successive editions, has served for twenty years as perhaps the most literate and readable introduction to this interaction available. Readers have consistently found that fields within their specialty are covered with authority; that fields far removed are covered with clarity; and that the connections among them are shown to be close (...) and natural. This third edition includes a new chapter, "Feeling, Knowledge, and Understanding," that extends the book's reach into philosophy, and in particular communicates some existential points-of-view. In it the author undertakes to demonstrate that "The language of physical science is inadequate for discussion of what is essentially human about human communication." A notion of Education for Active Citizenship is identified in the pronouncements of certain politically influential individuals. Key elements in this are seen to include action, the citizen, appreciating the benefits of democracy and freedom, respect for the rule of law, a due balance between rights and duties, participation and service to the community. These are shown to be systematically ambiguous, simultaneously capable of evoking critical, independent-minded, socially effective citizens and docile conforming subjects. The prima facie linguistic evidence of everyday experience suggests that human beings are bewilderingly different from each other: adults frequently complain that it is impossible to master a foreign language; indeed, many communities despise languages or dialects other than their own; serious translation is often a painful struggle, producing results that are felt to be inadequate, even by the translators themselves; and even within our own communities, most of us have had the despairing experience of not understanding a single word (...) of what someone else is saying. In What the Body Commands, Colin Klein proposes and defends a novel theory of pain. Klein argues that pains are imperative; they are sensations with a content, and that content is a command to protect the injured part of the body. He terms this view "imperativism about pain," and argues that imperativism can account for two puzzling features of pain: its strong motivating power and its uninformative nature. Klein argues that the biological purpose of pain is homeostatic; like hunger (...) and thirst, pain helps solve a challenge to bodily integrity. It does so by motivating you to act in ways that help the body recover. If you obey pain's command, you get better. He develops his account to handle a variety of pain phenomena and applies it to solve a number of historically puzzling cases. Klein's intent is to defend the imperativist view in a pure form -- without requiring pain to represent facts about the world.Klein presents a model of imperative content showing that intrinsically motivating sensations are best understood as imperatives, and argues that pain belongs to this class. He considers the distinction between pain and suffering; explains how pain motivates; addresses variations among pains; and offers an imperativist account of maladaptive pains, pains that don't appear to hurt, masochism, and why pain feels bad. In this essay, McGinn argues for a version of reliabilism, contending that a belief counts as knowledge just in case ‘it is produced by a method capable of yielding true beliefs in a range of relevant cases’. This view diverges from other versions of reliabilism, notably Robert Nozick's counter‐factionalist tracking theory, which, McGinn argues, ‘localizes the conditions for knowledge into a relation between the knower and a unique proposition’. Against this local analysis of propositional knowledge, McGinn presents a global reliabilism (...) in which the notion of discrimination is basic. On McGinn's view, global reliability consists in a subject's capacity to distinguish the true from the false within a relevant class of propositions. In this precursor to his Mental Content, McGinn defends and explores the implications of his dual‐component theory of mental and linguistic content. According to McGinn, our concept of belief combines two components: the first consists in a mode of representation of things in the world, the second involves the semantic relations between such representations and the things represented. The case is analogous for linguistic content, which is also structurally duplex; meaning, for McGinn, consists in both reference and mode of presentation. (...) Background In recent years, patient advocacy has emerged as a prominent concept within healthcare. How nursing students decide what is best for their patients is not well understood. Objective The objective is to examine nursing students' views on doing what is best for patients during their clinical experiences and how they seek to establish patient interests when providing care. Research questions guiding the interview were as follows: (1) What are nursing students' perceptions of patient interests? (2) What factors influence nursing (...) students' perceptions of advocating for patient’s interests? Research design Qualitative descriptive research using thematic analysis. Participants and research context Data was collected through individual online interviews with nine nursing students with clinical experience. Ethical considerations The study was approved by the University Research Ethics Committee. Participants provided digital informed consent. Results The students asserted that they are able to understand the patients' interests by placing emphasis on the patients' needs. They believe that it is crucial to adopt a collaborative strategy for the provision of care to meet these requirements. In addition, some of them expressed concern over the most effective methods of advocating for the interests of patients. Three themes were identified. (1) Focussing on patient needs first, (2) taking a collective approach, and (3) learning how to advocate. Conclusions Students understand and value the ethical commitments associated with advocating for the patient's best interest by considering factors such as prioritising the patient's needs, adopting a strategy that involves everyone, and acquiring the ability to undertake the advocacy role. Additionally, nursing education strategies in clinical contexts require additional study to inspire students to do what is in their patient’s best interests. (shrink)

Colin McGinn presents his latest work on consciousness in ten interlinked papers, four of them previously unpublished. He extends and deepens his controversial solution to the mind-body problem, defending the view that consciousness is both ontologically unproblematic and epistemologically impenetrable. He also investigates the basis of our knowledge that there is a mind-body problem, and the bearing of this on attempted solutions. McGinn goes on to discuss the status of first-person authority, the possibility of atomism with respect to consciousness, (...) extreme dualism, and the role of non-existent objects in constituting intentionality. He argues that traditional claims about our knowledge of our own mind and of the external world can be inverted; that atomism about the conscious mind might turn out to be true; that dualism is more credible the more extreme it is; and that all intentionality involves non-existent objects. These are all surprising positions, but he contends that what the philosophy of mind needs now is 'methodological radicalism' - a willingness to consider new and seemingly extravagant ideas. Pragmatism is America's best-known native philosophy. It espouses a practical set of beliefs and principles that focus on the improvement of our lives. Yet the split between classical and contemporary pragmatists has divided the tradition against itself. Classical pragmatists, such as John Dewey and William James, believed we should heed the lessons of experience. Neopragmatists, including Richard Rorty, Hilary Putnam, and Jürgen Habermas, argue instead from the perspective of a linguistic turn, which makes little use of the idea of experience. (...) Can these two camps be reconciled in a way that revitalizes a critical tradition? -/- Colin Koopman proposes a recovery of pragmatism by way of "transitionalist" themes of temporality and historicity which flourish in the work of the early pragmatists and continue in contemporary neopragmatist thought. "Life is in the transitions," James once wrote, and, in following this assertion, Koopman reveals the continuities uniting both phases of pragmatism. Koopman's framework also draws from other contemporary theorists, including Michel Foucault, Pierre Bourdieu, Bernard Williams, and Stanley Cavell. By reflecting these voices through the prism of transitionalism, a new understanding of knowledge, ethics, politics, and critique takes root. Koopman concludes with a call for integrating Dewey and Foucault into a model of inquiry he calls genealogical pragmatism, a mutually informative critique that further joins the analytic and continental schools. Explains Popper's views on natural and social science, ranging in Part I from metaphysical considerations to his interpretation of the formalism of quantum mechanics, and in Part II from the errors of historicism and holism to the roles of theoretical models, institutions, traditions and history.

I argue that by incorrectly translating Wittgenstein's remarks into a set of philosophical theses of the sort Wittgenstein explicitly denies making, Colin McGinn systematically misinterprets Wittgenstein's later claims about meaning and rules. Once this sort of mistake is corrected, it becomes implausible to claim, as McGinn does, that Wittgenstein rejects a social conception of rules and meaning.

Empirical evidence of self-deception's propositional duality is not sought; philosophically relevant links between propositions proper and mind are explored instead. Speech in unison ably indicates the social grounding of such attitudinal structures. An extra-theoretical eye – with regard to cognitivism – is cast on a case of “illusory communication.” The reinforcing of lexical analysis shows Mele's approach to be in need of non-ego concepts, wherefore it lacks soundness with respect to reference.

Since the appearance of a widely influential book, Self-Knowledge and Self-ldentity, Sydney Shoemaker has continued to work on a series of interrelated issues in the philosophy of mind and metaphysics. This volume contains a collection of the most important essays he has published since then. The topics that he deals with here include, among others, the nature of personal and other forms of identity, the relation of time to change, the nature of properties and causality and the relation between the (...) two, dualism and immortality, and the nature of mental states. All the essays show the same care and precision in argument as the earlier book, but they also reveal a substantial shift in Professor Shoemaker's position to a form of materialism. It is not immediately clear from Wittgenstein’s Tractatus how to connect his idea there of an object with the logical ontologies of Frege and Russell. Toward clarification on this matter, this paper compares Russell’s and Wittgenstein’s versions of the thesis of an atomic fact that it is a complex composition. The claim arrived at is that whilst Russell (at times at least) has one particular of the elements of a fact – the relation – responsible for the unity of the (...) whole, for Wittgenstein the unity of a fact is the product of copulative powers inherent in all its elements. All kinds of constituents of Tractarian facts – all kinds (forms) of object – are, to use Fregean terminology, unsaturated. (shrink)

Wnt proteins are involved in a large number of events during development and disease. The crucial element in the transduction of the signal elicited by Wnt is the state and activity of β-catenin. There are two pools of β-catenin, one associated with cadherins at the cell surface and a soluble one in the cytolasm, whose state and concentration are critical for Wnt signalling. In the absence of Wnt, the cytoplasmic pool is low due to targetted degradation of β-catenin. Upon Wnt (...) signalling, β-catenin is stabilized. As a consequence, it can access the nucleus where it interacts with members of the Tcf family of transcription factors to modulate the expression of defined targets. Recent reports indicate that, in addition to Tcfs, β-catenin can interact with other nuclear proteins raising the possibility that Wnt signalling has a wider modulatory effect on transcription than is mediated by its interactions with Tcfs. In this piece the author takes issue with Mario Bunge’s claims that conceptual and semiotic systems have "compositions, environments and structures, but no mechanisms." Structures, according to Bunge, can never be mechanisms in conceptual and semiotic systems. Contra this the author argues that in social systems, social structures (which are concept-dependent and reproduced and/or transformed, at least in part, semiotically), can be mechanisms in the sense that such structures are one of the processes in a concrete system that makes itwhat (...) it is. As such, not only may conceptual and semiotic systems have mechanisms, but they may also themselves be considered some of the mechanisms that make the social what it is. As such, they can be said to possess powers and liabilities that neither reside at lower levels nor are explainable in terms of the lower level. To hold out the prospect of social explanation by conceptual and/or semiotic mechanisms does not represent an attempt to decouple these systems from material factors. What it does mean is that conceptual and/or semiotic systems can, potentially at least, play a role in social explanation; and themselves be subject to explanation. Mapping from acoustic signals to lexical representations is a complex process mediated by a number of different levels of representation. This paper reviews properties of the phonetic and phonological levels, and hypotheses about how category structure is represented at each of these levels, and evaluates these hypotheses in light of relevant electrophysiological studies of phonetics and phonology. The paper examines evidence for two alternative views of how infant phonetic representations develop into adult representations, a structure-changing view and a structure-adding view, (...) and suggests that each may be better suited to different kinds of phonetic categories. Electrophysiological results are beginning to provide information about phonological representations, but less is known about how the more abstract representations at this level could be coded in the brain. The need to integrate ethics into professional life, from the grassroots up, has been recognized, and a comprehensive ethics program has been proposed as a model. The model includes the four dimensions of: consensus building, ethics guidelines development and review, education, and implementation. The activities of the International Society for Environmental Epidemiology (ISEE) are presented as examples and compared with the proposed model. Several innovative activities are described and incentives for ethical professional conduct are highlighted. During development of the Xenopus embryo, the formation of the nervous system depends on an inductive interaction between mesoderm and ectoderm. The result is a neural tube that is regionally differentiated along the anterior–posterior axis from forebrain to spinal cord (Fig. 1). The discovery of genes whose transcripts can be used as molecular markers for different regions of the nervous system has permitted reassessment of the existing theories of neural tissue formation. It has been much debated whether Tractarian objects are what Russell would have called particulars or whether they include also properties and relations. This paper claims that the debate is misguided: there is no logical category such that Wittgenstein intended the reader of the Tractatus to understand his objects either as providing examples of or as not providing examples of that category. This is not to say that Wittgenstein set himself against the very idea of a logical category: quite the (...) contrary. However, where Russell presents his logical variety of particulars and the various types of universal, and Frege presents his of objects and the various types of function, Wittgenstein denies the propriety of such a priori expositions. Se tomarmos o mestre como o artífice e o discípulo como objeto deste ensino, devemos ter em mente que a (...) arte de ensinar é conveniente ao discípulo e não ao mestre. O mestre, portanto, é instrumento para que se possa ensinar tal arte ao discípulo. O objetivo deste trabalho é refletir o ensino, seu instrumento e seu objeto com o intuito de repensar o caminho da filosofia na educação.
