The growing prevalence of health care ethics consultation (HCEC) services in the U.S. has been accompanied by an increase in calls for accountability and quality assurance, and for the debates surrounding why and how HCEC is evaluated. The objective of this study was to evaluate the effectiveness of HCEC as indicated by several novel outcome measurements in East Asian medical encounters.

This study uses a rational ethical decision-making framework to examine the influence of moral intensity (potential harm and social pressure) on whistleblowing intention and behaviour using organisational commitment as a moderator. A scenario was developed, and an online questionnaire was used to conduct an empirical analysis on the responses of 533 participants. The mean age and years of work experience of the respondents were 31 and 8.2 years, respectively. The results show, first, that while moral intensity is correlated with whistleblowing (...) intention, only the potential harm is positively correlated with such intention. Second, potential harm and social pressure differentially affect whistleblower choice of using an internal or external channel. Third, organisational commitment has a moderated mediation effect among moral intensity, whistleblowing intention and behaviour. Fourth, whistleblowers may be grouped into four conceptual types: indifferent, rebel, mature and spoil. Finally, theoretical and managerial implications of the findings are discussed. (shrink)

We investigated the implicit learning of a linguistically relevant variable in a natural language context . Trial by trial subjective measures indicated that exposure to a form–animacy regularity led to unconscious knowledge of that regularity. Under the same conditions, people did not learn about another form–meaning regularity when a linguistically arbitrary variable was used instead of animacy . Implicit learning is constrained to acquire unconscious knowledge about features with high prior probabilities of being relevant in that domain.

Using Robert Merton’s perspective on social structure [Social theory and structure. Free Press, New York, 1968], this study tested the individual-level association between job position and ethical reasoning. Anomie theory was employed to examine how country-level factors moderate that individual-level association. The hierarchical linear modeling (HLM) method was used to analyze 22,359 subjects from 28 nations. The statistical results proved that workers are more likely to justify ethically suspect behaviors, and that this individual-level relationship is moderated by the country-level factors (...) of power distance, masculinity, social inequality, and education accessibility. These results imply that Merton’s view of social structure and contemporary anomie theorists’ perspective of anomie [Messner and Rosenfeld (Crime and the American dream. Wadsworth, Belmont, 2001); Rosenfeld and Messner (The future of anomie theory. Northeastern University Press, Boston, 1997)] are mutually complementary rather than exclusive. (shrink)

This study examines whether human gaze lacking the confounding factor of eye whites can be processed unconsciously and explores the critical aspects for such process. Utilizing the continuous flash suppression paradigm, a schematic face—with direct or averted gaze, and with neutral, fearful or happy expressions—was presented to one eye while dynamic masks rendered it invisible to the other eye. Schematic faces were used to avoid unwanted influence from salient eye whites. Participants’ detection time of anything other than the masks was (...) used as an index of unconscious processing time. Faster detection was found for faces with direct gaze than those with averted gaze. However, there was no difference between detection times for different facial expressions and upright-face, inverted-face, and eyes-only conditions . These results confirm that, with schematic faces, gaze can be processed unconsciously regardless of facial expression, and eyes alone are sufficient for such process. (shrink)

This essay illustrates what the Chinese family-based and harmony-oriented model of medical decision making is like as well as how it differs from the modern Western individual-based and autonomy-oriented model in health care practice. The essay discloses the roots of the Chinese model in the Confucian account of the family and the Confucian view of harmony. By responding to a series of questions posed to the Chinese model by modern Western scholars in terms of the basic individualist concerns and values (...) embedded in the modern Western model, we conclude that the Chinese people have justifiable reasons to continue to apply the Chinese model to their contemporary health care and medical practice. Previous studies have identified the critical role of the left fusiform cortex in visual word form processing, learning, and memory. However, this so-called visual word form area's other functions are not clear. In this study, we used fMRI and the subsequent memory paradigm to examine whether the putative VWFA was involved in the processing and successful memory encoding of faces as well as words. Twenty-two native Chinese speakers were recruited to memorize the visual forms of faces and Chinese words. Episodic (...) memory for the studied material was tested 3. h after the scan with a recognition test. The fusiform face area and the VWFA were functionally defined using separate localizer tasks. We found that, both within and across subjects, stronger activity in the VWFA was associated with better recognition memory of both words and faces. Furthermore, activation in the VWFA did not differ significantly during the encoding of faces and words. Our results revealed the important role of the so-called VWFA in face processing and memory and supported the view that the left mid-fusiform cortex plays a general role in the successful processing and memory of different types of visual objects. © 2010 Elsevier Inc. As science has grown in size and scope, it has also presented a number of ethical and moral challenges. Approaching these challenges from an ethical framework can provide guidance when engaging with them. In this article, I place science within a virtue ethics framework, as discussed by Aristotle. By framing science within virtue ethics, I discuss what virtue ethics entails for the practicing scientist. Virtue ethics holds that each person should work towards her conception of flourishing where the virtues enable (...) her to realize that conception. The virtues must become part of the scientist’s character, undergirding her intentions and motivations, as well as the resulting decisions and actions. The virtue of phronêsis, or practical wisdom, is critical for cultivating virtue, enabling the moral agent to discern the appropriate actions for a particular situation. In exercising phronêsis, the scientist considers the situation from multiple perspectives for an in-depth and nuanced understanding of the situation, discerns the relevant factors, and settles upon an appropriate decision. I examine goods internal to a practice, which are constitutive of science practiced well and discuss the role of phronêsis when grappling with science’s ethical and moral features and how the scientist might exercise it. Although phronêsis is important for producing scientific knowledge, it is equally critical for working through the moral and ethical questions science poses. (shrink)

In this article I explain why scientists cannot always resolve their disagreements about experiments even if they do not hold conflicting theoretical assumptions, and how incommensurability in experiments can occur even if experiments are not deeply encumbered by theoretical assumptions. On the basis of recent discoveries in cognitive psychology and an extended analysis of a historical case, I explore a cognitive mechanism that may generate incommensurability in experiment appraisal. I find that, because of the involvement of goal-derived categories, incommensurability in (...) experiments may result from the conflict of goals that scientists pursue in their researches, from the differences of goal-derived classification schemata that they employ in analyzing experiments, and from discrepancies between skills that they have developed in their practices. This account differs from the conventional interpretation of Kuhn’s thesis, which attributes the cause of incommensurability solely to theoretical differences. In the conclusion, I further discuss the implications of this new account of incommensurability for both philosophical and historical studies of science. (shrink)

High-tech industries often regard workers as their main source of value creation. In order to stimulate their employees' willingness to innovate and their innovative behavior and reduce the turnover intention, companies are now seeking to establish employer–employee relationships in which their employee's willingness to stay is not simply driven by extrinsic motivations. Therefore, it is an important topic in human resources for companies to implement measures that encourage employees to willingly devote themselves to their jobs and consider organizational growth as (...) a component of their career development. This study aimed to investigate the effect of person–organization fit and person–job fit on employees' innovative behavior and turnover intention via the mediators including job satisfaction and organizational commitment. Six hundred ninety-seven employees from China's eight major high-tech industries were examined in this study, and the empirical results were analyzed using partial least squares. Based on the results, it is suggested that the person–organization fit and person–job fit are both crucial factors affecting employees' job satisfaction and organizational commitment, which, in turn, increase employees' willingness to innovate in their jobs and reduce their turnover intentions. Furthermore, this study could serve as a reference for companies in selecting employees, promoting job satisfaction, and developing strategies for sustainable development. (shrink)

Kuhn regards local incommensurability as an unavoidable result of changes in worldview, but his account fails to explain both historical cases in which rivals with different paradigms obtained consensus, and psychological experiments in which people with different cultural backgrounds accurately presented other points of view. Although the conditions required to generate local incommensurability were present in the dispute between Brewster and Herschel on light absorption, they succeeded in communicating. Ultimately Brewster understood his opponent's position, in the same way that subjects (...) in Barsalou's recent psychological experiments proved able to comprehend alien conceptual structures. Building upon recent cognitive theories of graded conceptual structures, I offer a new account of incommensurability, which accommodates these historical cases and psychological results. By correcting and extending Kuhn's account I show that local incommensurability may be a matter of degree. (shrink)

In the process of rapid urbanization, urban heat island effect has been showing more and more significant impacts on human well-being. Therefore, a more detailed understanding of the impact of three-dimensional building morphology on UHI effect across a continuum of spatial scales will be necessary to guide and improve the human settlement.This study selected 31 provincial capital cities of mainland China to analyze the impacts of the 3D building morphology, including the number, area, height, volume, and the surface area of (...) the buildings, on the land surface temperature. By exploring how the influence of 3D building morphology on LST changes with the increase of spatial scale, this study finally recognized which 3D building morphology index is the most significant index affecting LST in different cities, and which spatial scale these 3D building morphology indexes have the most significant impact on LST. The results showed that the building area is the most important 3D building morphology parameter affecting the LST, while the building height has the slightest influence on the LST. These effects are more significant in the spatial scale of 150 m–540 m, and the spatial scale increases with the increase of building areas in developed cities. These results highlight the necessity of considering fine-grained management in the governance and alleviating of the urban thermal environment through urban planning and urban renewal strategies. Background: Previous systematic review indicated the prevalence of prenatal anxiety as 14–54%. Pregnant women are a high-risk population for COVID-19. However, the prevalence of anxiety symptoms and related factors is unknown in Chinese pregnant women during COVID-19 outbreak.Objective: To investigate the prevalence of anxiety symptoms and the related factors in Chinese pregnant women who were attending crisis intervention during the COVID-19 pandemic.Methods: The data of this cross-sectional study were collected in about 2 months. Data analysis was performed from April to (...) May 2020. Participants completed a set of questionnaires via the Wechat Mini-program before starting the online self-help crisis intervention for COVID-19 epidemic. A total of 2,120 Chinese pregnant women who were attending a self-help crisis intervention participated in this study. A survey was developed to address possible stress-related factors in pregnant women during the COVID-19 outbreak, including demographic, socioeconomic, and pregnancy-related factors, as well as COVID-19 related factors. Generalized Anxiety Disorder-7 scale and the 10-item perceived stress scale were, respectively, employed to measure anxiety and stress-related factors.Results: A total of 21.7% of pregnant women reported at least mild anxiety, and only 82 women reported moderate to severe anxiety. Factors associated with at least mild anxiety included living in Hubei province, nobody providing everyday life support, pelvic pain or vaginal bleeding, and higher perceived stress. Having relatives or neighbors with a diagnosis of COVID-19 was not associated with anxiety.Conclusions and Relevance: Our findings indicate that evaluation and intervention for maternal and infant health are necessary in pregnant women with anxiety during COVID-19 epidemic, especially those with higher perceived stress, less everyday life support, or vaginal bleeding. Interactions among these related medical, social and psychological factors need to be investigated in future studies. (shrink)

This paper addresses the Basic Argument in favour of incompatibilism, both in its Strawsonian form and in its weakened form (the CDA). After examining the worries raised by this argument, I will defend a version of semi-compatibilism that is motivated by a narrative theory of the self, arguing that moral responsibility is possible even if the thesis of determinism is taken to be incompatible with the thesis of freedom of will. The semi-compatibilist argument that I provide lowers the standard of (...) proof, stops the regress of causes, finds a buck-stopper in the form of a self who can be held morally responsible for a choice or an action, and avoids committing the Strawsonian fallacy of circular reasoning by not setting impossible demands on moral responsibility. Most studies have examined the outcomes of patients supported by extracorporeal membrane oxygenation as a life-sustaining treatment. It is unclear whether significant social events are associated with the use of life-sustaining treatment. This study aimed to compare the trend of extracorporeal membrane oxygenation use in Taiwan with that in the world, and to examine the influence of significant social events on the trend of extracorporeal membrane oxygenation use in Taiwan.

This investigation analyzed Islamic spirituality as measured by a Muslim Experiential Religiousness Scale. Iranian university and seminary students responded to this instrument along with the Psychological Measure of Islamic Religiousness and Perceived Stress and Self-Esteem scales. Muslim Experiential Religiousness correlated predictably with all PMIR sub-scales, Perceived Stress, and Self-Esteem, and mediated almost all relationships of the PMIR Islamic Beliefs subscale with religious functioning. When evaluated by participants, Muslim Experiential Religiousness items proved to be “rational” relative to their Muslim religious ideals. (...) Women in an Islamic seminary scored higher on Muslim Experiential Religiousness than Islamic women in a more general university, and Muslim Experiential Religiousness also mediated the many other religious differences observed between these two student groups. These data most importantly identified the Muslim Experiential Religiousness Scale as a useful instrument for testing hypotheses about the dynamics of Muslim spirituality. (shrink)

Over the last quarter-century, China has experienced dramatic changes associated with its development of a market economy. The character of clinical practice is also profoundly influenced by the ways in which reimbursement scales are established in public hospitals. The market distortions that lead to the over-prescription of drugs and the medically unindicated use of more expensive drugs and more costly high-technology diagnostic and therapeutic interventions create the most significant threat to patients. The payment of red packets represents a black-market attempt (...) to circumvent the non-market constraint on physicians' fees for services. These economic and practice pattern changes are taking place as China and many Pacific Rim societies are reconsidering the moral foundations of their professional ethics and their bioethics. The integrity of the medical profession and the trust of patients in physicians can only be restored and protected if the distorting forces of contemporary public policy are altered. (shrink)

Liver transplantation (LT) activities during the COVID-19 pandemic have been curtailed in many countries. The impact of various policies restricting LT on outcomes of potential LT candidates is unclear. We studied all patients on the nationwide LT waitlists in Hong Kong and Singapore between January 2016 and May 2020. We used continuous time Markov chains to model the effects of different scenarios and varying durations of disruption on LT candidates.

Though Congress passed the American Recovery and Reinvestment Act earlier this year, debate continues to swirl around the provision of funds for comparative effectiveness research. Critics warn that government bodies could use such research to dictate “appropriate” care and impose third‐party oversight so intrusive it would impinge upon the interaction between doctors and their patients. On the contrary, I believe that comparative effectiveness has the potential to strengthen the patient‐doctor relationship. How? Examples from Chinese, Thai, and Finnish illustrate why researchers cannot always be confident about the precise nature of the word unit. Understanding ambiguities regarding where a word begins and ends, and how to model word recognition when many derivations of a word are possible, is essential for universal theories of reading applied to both developing and expert readers.

While translation competence assessment has been playing an increasingly facilitating role in translation teaching and learning, it still failed to offer fine-grained diagnostic feedback based on certain reliable translation competence standards. As such, this study attempted to investigate the feasibility of providing diagnostic information about students’ translation competence by integrating China’s Standards of English with cognitive diagnostic assessment approaches. Under the descriptive parameter framework of CSE translation scales, an attribute pool was established, from which seven attributes were identified based on (...) students’ and experts’ think-aloud protocols. A checklist comprising 20 descriptors was developed from CSE translation scales, with which 458 students’ translation responses were rated by five experts. In addition, a Q-matrix was established by seven experts. By comparing the diagnostic performance of four widely-used cognitive diagnostic models, linear logistic model was selected as the optimal model to generate fine-grained information about students’ translation strengths and weaknesses. Furthermore, relationships among translation competence attributes were discovered and diagnostic results were shown to differ across high and low proficiency groups. The findings can provide insights for translation teaching, learning and assessment. (shrink)

On the Definition of Religion in Hobbes’ Leviathan It has long been a controversial topic whether Hobbes was an atheistic philosopher or a sincere Christian. A clear understanding of the issue requires an investigation into Hobbes’ view of religion. This paper tends to provide such an examination. First, it analyses the legal point of view by which Hobbes distinguishes religion from superstition. Secondly, it examines the two elements by which Hobbes defines religion: fear and power. Thirdly, it explores what true (...) religion is for Hobbes. Fourthly, after dealing with Hobbes’ rational theology, it raises certain questions about difficulties and contradictions in Hobbes’ definition of religion and true religion. Fifthly and finally, drawing on the double teaching theory and taking the purpose of Hobbes’ political theory into account, it concludes that there is a double doctrine in Hobbes on the one hand and, on the other hand, Hobbes desires to publicize the esoteric philosophic teaching in accordance with the purpose of his political philosophy, i.e. with the pursuit of a rational and free world state. The structure of Au layers deposited by sputtering on oxidized p-type Si substrates is investigated by a combination of scanning electron microscopy and scanning probe microscopy. The effect of the temperature on the grain structure of the layers has been determined, revealing that an annealing temperature of 300° C results in a larger grain size and smoother surfaces but generates some cracks in the film surface. At an annealing temperature of 500° C, further grain growth is observed, but a high (...) density of cracks and voids also results while there is little enhancement regarding the smoothness of the grain surfaces. Objective To determine the knowledge and attitudes of healthcare personnel regarding the provision of non-beneficial futile care in the intensive care units at the major public hospitals in Trinidad and Tobago. Method Prospective data collection was done using a questionnaire administered to the medical and nursing staff of the intensive care units. The questionnaire was designed to capture the opinions regarding the futile care offered to terminally ill patients at the intensive care units. The responses were based on a five-point (...) Likert scale. The influence of factors such as age, gender, duration of work experience, religious belief, ethnicity, occupational category and educational status on the responses were analysed. Results A total of 274 completed responses were obtained from doctors and nurses. The frequency with which the respondents encountered ethical or legal problems in the intensive care unit varied widely from ‘daily’ to ‘yearly’. The majority felt that knowledge of ethics is important, and only 32% knew the legal issues pertaining to their work. Eighty percent of doctors and nurses had no knowledge of an existing Hospital Ethics Committee and its role in ethical dilemmas. Although 62% said their decisions regarding futile care will be influenced by their scientific knowledge, only 32% agreed to withdraw care. Eighty percent said that the government should pass appropriate laws regarding withdrawal of futile care. Conclusions Most healthcare providers in intensive care unit are not knowledgeable in the ethical and legal issues of non-beneficial futile care. There is a need to devise means to bring awareness and educate intensive care unit healthcare providers in this subject. (shrink)

Paul Ricoeur develops a hermeneutics of tradition centered on a threefold conception of tradition which involves the notions of traditionality, traditions, and Tradition. These refer to form, content, and truth-claims within the framework of the hermeneutics of historical consciousness. This hermeneutics of tradition is treated in a panchronic and ternary way. Both methods operate at the levels of past, future and present, while the ternary method also consists in the rhetoric of truth-claims, the dialectic of remoteness vs. de-distanciation and that (...) of question vs. answer. This article attempts to study three aspects of the panchronic and ternary approach: (1) a living and ternary tradition with an ongoing dialectic interweaving the ecstases of time; (2) tradition in relation to historical consciousness and the spatialization of historicity; (3) temporalizing history and historicizing schematism in threefold mimesis. Morin (1921) founded the complex mode of thinking in order to remedy the defects of the Western classic simple mode of thinking. In doing so, he approached to some degree the mode of thinking inherent to the Eastern civilization. This article elucidates that for some principles of Morin's complex thinking, such as correlation of opposites, recursive causality, and union of unity of multiplicity, there were similar ideas in Chinese classic philosophy. This shows that the complex paradigm of thinking, in a (...) certain point of view, will be the fusion of Western and Eastern cultural factors. Besides, in integrating his theory of complexity with the reality of our time to find out solutions to the issues of globalization, Morin has become one of the representative thinkers of the planetary age of humankind.
