The revelation that data obtained for the US-based National Surgical Adjuvant Breast and Bowel Project (NSABP) from subjects enrolled at Hôpital Saint-Luc in Montreal was falsified has eroded public trust in research. Institutions can educate researchers and help prevent unethical research practices by establishing procedures to monitor research involving human subjects. Research monitoring encompasses four categories of activity: annual reviews of continuing research, monitoring of informed consent, monitoring of adherence to approved protocols and monitoring of the integrity of data. The (...) authors describe characteristics of research projects that may call for monitoring procedures in each category. The form taken by such monitoring depends on the nature of the protocol. Although appropriate research monitoring requires substantial investment of personnel and financial resources, it is required under guidelines regulating research involving human subjects in Canada. The clinical trial is the major investigational tool of clinical medicine. Two recent reports highlight the fact that the most often quoted mechanisms for the protection of research subjects, viz., research ethics board review and eligibility criteria, are insufficient to achieve this end. In this paper, we argue that the prime mechanism for the protection of persons in clinical trials should be the clinical judgement of the physician-investigator. The clinical investigator has a duty to protect subjects from both harm and (...) undue risk. It is argued that the clinical investigator has a duty to screen for, and exclude, potential research subjects who may be unduly vulnerable to the risks of a particular clinical trial. In order to fulfill this obligation, the investigator should personally screen each potential research subject at the time of accrual. In larger trials in which this is not feasible, this task could be delegated to another appropriately qualified health care professional, with the principal investigator retaining personal responsibility. To reinforce and make explicit this legal and moral duty, we propose that the investigator sign a statement, appended to each subject's consent form, to attest that this duty has been responsibly discharged. We studied changes in eligibility criteria--the largest impediment to patient accrual--in two samples of clinical trials. Trials from the NSABP (National Surgical Adjuvant Breast and Bowel Program) and POG (Pediatric Oncology Group) were analyzed. After eliminating duplications, the criteria in each protocol were enumerated and classified according to a novel schema. NSABP trials contained significantly more criteria than POG trials, and added precision criteria (making study populations homogeneous) at a faster rate than POG studies. The difference between NSABP studies (explanatory (...) trials) and POG studies (pragmatic trials) suggest that large numbers of eligibility criteria are not necessary for quality studies. We recommend that: (1) the inclusion/exclusion criteria distinction be abandoned; (2) eligibility criteria be explicitly justified; (3) the need for each criterion be assessed when new trials are planned; (4) criteria in phase III trials restricting patient accrual be minimized; and (5) further research be done to assess the impact of criteria on generalizability. OBJECTIVE: Pilot study to characterize treatment differences between patients treated in clinical trials and those treated in a clinical setting. Previous studies have shown higher survival rates for participants in trials of cancer therapy. This difference is observed even after rates are adjusted for important covariates such as age and stage of disease. DESIGN: Retrospective chart review. SETTING: Oncology outpatient department in a tertiary care hospital. PATIENTS: Ninety women 18 to 70 years of age with early-stage breast cancer who were (...) diagnosed in 1990. Fifty-one of the women were treated through clinical trials and 39 were treated outside of clinical trials. OUTCOME MEASURES: Number of blood tests, telephone calls, clinic visits and imaging procedures as well as intensity of chemotherapy and use of radiation therapy. The age of the patient and the stage of disease were important covariates. RESULTS: After the analysis was controlled for patient age and stage of disease, patients treated through a clinical trial were more likely to receive standard-dose chemotherapy (p = 0.020, 95% confidence interval 1.20 to 200.73) and more frequent blood tests (p < 0.001, 95% confidence interval 1.02 to 1.13) than other patients treated in the clinic. CONCLUSIONS: Our results provide a plausible mechanism for the observed survival advantage for participants in clinical trials in oncology. Further study is called for. If these results are confirmed, they have important implications for informed consent to participate in clinical trials and for clinical practice. (shrink)

OBJECTIVE: To investigate whether eligibility criteria that exclude the elderly, persons with psychiatric disease, and persons with substance abuse problems from participation in randomized controlled trials (RCTs) are subjective and hence a source of variability in enrolment decisions and investigator uncertainty. DESIGN: Survey questionnaire. PARTICIPANTS: Cancer investigators from the United States and Canada. INTERVENTIONS: Investigators were presented with clinical vignettes from 3 patient categories--eligible, ineligible and uncertain--for each of 5 eligibility criteria--3 subjective and 2 objective--and were asked whether they would (...) enrol the patient in a trial and how sure they were of this decision. Demographic characteristics of the investigators were also collected. OUTCOME MEASURES: The difference in enrolment decisions between subjective and objective criteria, and the difference in the certainty associated with these decisions. RESULTS: Of 365 questionnaires sent out, 224 usable ones were returned. Compared with the objective criteria, the subjective criteria were associated with more variable enrolment decisions (p = 0.07 for the "eligible" scenario and p = 0.0001 for the "ineligible" and "uncertain" scenarios), and investigators were less sure about the decisions they made (p = 0.0001 for all scenarios). Demographic characteristics of the investigators failed to explain the observed differences. CONCLUSIONS: Subjective eligibility criteria may interfere with the conduct and interpretation of RCTs and, therefore, their use ought to be justified explicitly in the study protocol. RCT designers, funding agencies and research ethics boards have an important role in reviewing eligibility criteria for their necessity. (shrink)

The assessment of student learning is widely regarded with suspicion. Philosophers in particular have been reluctant to take this practice seriously. The essay reviews an ongoing effort to assess the development of philosophical dispositions among undergraduate students at a religiously affiliated liberal arts college. The procedure used in this effort as well as the results obtained so far strongly suggest that the deep learning valued most highly by philosophy teachers can be measured without harm to the teaching enterprise. To what risks may children participating in research be subjected? Institutional review boards can stand surrogate for parents by filtering out studies whose risk is unacceptably high.

Biological evolution provides a creative fount of complex and subtle adaptations, often surprising the scientists who discover them. However, because evolution is an algorithmic process that transcends the substrate in which it occurs, evolution’s creativity is not limited to nature. Indeed, many researchers in the field of digital evolution have observed their evolving algorithms and organisms subverting their intentions, exposing unrecognized bugs in their code, producing unexpected adaptations, or exhibiting outcomes uncannily convergent with ones in nature. Such stories routinely reveal (...) creativity by evolution in these digital worlds, but they rarely fit into the standard scientific narrative. Instead they are often treated as mere obstacles to be overcome, rather than results that warrant study in their own right. The stories themselves are traded among researchers through oral tradition, but that mode of information transmission is inefficient and prone to error and outright loss. Moreover, the fact that these stories tend to be shared only among practitioners means that many natural scientists do not realize how interesting and lifelike digital organisms are and how natural their evolution can be. To our knowledge, no collection of such anecdotes has been published before. This paper is the crowd-sourced product of researchers in the fields of artificial life and evolutionary computation who have provided first-hand accounts of such cases. It thus serves as a written, fact-checked collection of scientifically important and even entertaining stories. In doing so we also present here substantial evidence that the existence and importance of evolutionary surprises extends beyond the natural world, and may indeed be a universal property of all complex evolving systems. The formative period of Latin and Hebrew astrology occurred virtually simultaneously in both cultures. In the second quarter of the twelfth century the terminology of the subject was established and the textbooks which became authoritative were written. The responsibility for this lay almost entirely with two scholars: John of Seville for the Latins, and Abraham ibn Ezra for the Jews. It is unlikely to have been by coincidence that the same developments in astrology occurred in these two cultures. John (...) of Seville and Abraham ibn Ezra were both brought up within the Islamic culture of Spain, and their astrology was Arabic astrology. Moreover, some scholars have thought that John’s origins were Jewish, while Ibn Ezra is known to have collaborated with Latin scholars . It cannot be a coincidence that they forged the science of astrology for their respect co-religionists at almost the same time. Yet, very little research has been done on the possible relations between the two scholars. A detailed analysis is made of Grunbaum's claim that the Abraham-Lorentz (AL) and Dirac-Lorentz (DL) equations have no bearing on causality. It is pointed out that (a) both equations are derived from F = ma, and thus should obey the same causality conditions as Newton's law, (b) independently of what boundary conditions are imposed, non-causal behavior is always along the same straight line as the force, (c) the distinction in status between laws and boundary conditions which Grunbaum imposes is (...) one which is not always useful, especially since what is a law in one formulation of the theory can become a boundary condition in another, and thus it is argued that a complete theory must be such that laws and boundary conditions form a coherent whole, (d) the asymptotic boundary conditions that are applied are in agreement with experiment, (e) the AL equation is such that if the "effect," the acceleration as function of time, is known, then the "cause," the force, can be determined. Why an encyclopedia of the philosophy of religion? Because human beings have been and continue to be religious. Indeed, if one thinks in terms of what it is to be human, what is the essence of a human being, one can reasonably hold that it includes the property of trying to make sense of things and events, and religion, in terms of both belief and practice, is a way of doing this. A religious response to this attempt at sense-making in (...) no way implies that those who are not religious are not trying to make sense of things.They are, but conclude that the sought-after intelligibility, if it can be had, can be had without religion. So if one thinks of philosophy as, at least in part, a rational pursuit of intelligibility about things and events, then the philosophy of religion can plausibly be construed as the pursuit of this intelligibility in religious terms, and an encyclopedia of philosophy of religion as an orderly assemblage of entries about this rational undertaking. While the philosophy of religion has often been thought of as a "Western" activity which is engaged in by those in the Abrahamic faiths (Judaism, Christianity, and Islam), from the inception of this project we have believed it is important to think globally in terms of the scope of entries included in the encyclopedia. Hence, there are not only numerous entries on topics and individuals within the Abrahamic religious traditions, but also equally numerous entries on issues and people in Buddhism, Hinduism, Jainism, and other religious traditions.We have sought to be diverse in terms of the inclusion of entries across the religious world, not because we believe diversity and inclusion are goods in their own right, but because they are instrumental to helping us think more clearly about the overall intellectual project of making sense of things in terms of religion. Some persons might object that because the idea of making sense of things is a rational notion and religion, when properly understood, is at its core a rejection of the notion that things and events are intelligible, the philosophy of religion is itself a deeply incoherent intellectual discipline. Indeed, if we assume a desire tomake sense of things, the attempt to fulfill this desire along with perhaps its eventual frustration is a source of suffering. And who wants to suffer? Thus religion at its core is about giving up on the attempt to make sense of things and to achieve a state of "mind" in which there is nothingness or oneness with all things and, as a result, nothing distinct from other things of which to make sense. While this point is well made and appreciated, it nevertheless is also a fact that anyone reading one or more of the entries in this encyclopedia of the philosophy of religion is a person who is seeking to learn about, and thus make sense of, something or someone related to a particular religion or religions. So at least to this degree the reader is an individual who is pursuing intelligibility about that which is religious, or related to what is religious, in nature. And an initial assumption that the attempt to satisfy the desire for intelligibility through religion must fail and produce suffering is at least questionable. Finally, because this is an encyclopedia of the philosophy of religion, there are many entries about views and peoplewho believe that there are good reasons to reject religion altogether. These entries are warranted because the issues and people they cover occupy an important place in the dialogue that takes place between individuals with differing views who live in a shared global space.
