The purpose of this text is to tell the story of the implementation of the _Act Respecting End-of-Life Care,_ referred to hereafter as _Law 2_ (Gouvernement du Québec, 2014) with an emphasis on the ambiguous role of ethics in the Interdisciplinary Support Groups (ISGs), created by Quebec's _Ministère de la santé et des services sociaux_ (MSSS). As established, ISGs provide “clinical, administrative and ethical support to health care professionals responding to a request for Medical aid in dying (MAiD)” (Gouvernement du (...) Québec, 2015). It is suggested that their composition includes the participation of a person with an expertise in ethics. These ISGs represent an important innovation for stakeholders involved in MAiD. To date, no scientific research has specifically addressed ISGs and little research has been conducted in other jurisdictions on the roles, operations and practices of MAiD support structures, especially the implication of ethics. Several ISGs have certainly developed promising practices that could benefit all stakeholders in the wider field of ethics and end of life. We will explore the development of ISGs in Quebec as a support structure for MAiD by highlighting the role that ethics has played (and should play) in these morally and humanly challenging situations. To test whether mental activities collected from non-REM sleep are influenced by REM sleep, we suppressed REM sleep using clomipramine 50 mg or placebo in the evening, in a double blind cross-over design, in 11 healthy young men. Subjects were awakened every hour and asked about their mental activity. The marked REM-sleep suppression induced by clomipramine did not substantially affect any aspects of dream recall . Since long, complex and bizarre dreams persist even after suppressing REM sleep either partially or (...) totally, it suggests that the generation of mental activity during sleep is independent of sleep stage. This study shows that double thymidine block treatment efficiently arrests the EO771 cells in the S‐phase without altering cell growth or survival. A long‐term analysis of cell behavior, using 5(6)‐carboxyfluorescein diacetate N‐succinimidyl ester (CFSE) staining, show synchronization to be stable and consistent over time. The EO771 cell line is a medullary breast‐adenocarcinoma cell line isolated from a spontaneous murine mammary tumor, and can be used to generate murine tumor implantation models. Different biological (serum or amino acid deprivation), physical (elutriation, mitotic (...) shake‐off), or chemical (colchicine, nocodazole, thymidine) treatments are widely used for cell synchronization. Of the different methods tested, the double thymidine block is the most efficient for synchronization of murine EO771 cells if a large quantity of highly synchronized cells is recommended to study functional and biochemical events occurring in specific points of cell cycle progression. English summary: This book examines the philosophy of Elisabeth of Bohemia in conjunction with Descartess. She maintained a philosophical dialogue with another philosopher employing pertinent arguments. In addition, she perhaps developed her own theses, truly philosophical theses which can be better understood in consideration with her numerous correspondents. The study seeks to better understand Elisabeths role in Cartesian thought, and to determine her own philosophical thought. French description: Le titre meme de ce volume, sous forme d'interrogation, est a comprendre comme (...) un postulat. Il s'explique comme suit : il s'agit de s'interroger sur la pensee philosophique d'Elisabeth tout autant que sur celle de Descartes. La reflexion d'Elisabeth semble en effet posseder une dimension philosophique a deux titres au moins : d'abord parce que la princesse entretient un dialogue philosophique de haute tenue avec un autre philosophe, en deployant des arguments pertinents; ensuite parce qu'elle developpe peut-etre elle-meme des theses qui lui sont personnelles, qui seraient des theses au sens fort et pleinement philosophique du terme, et dont l'etude pourrait etre affinee par la prise en consideration de ses tres nombreux autres correspondants. Chaque jour, les professionnels de la santé doivent prendre des décisions dans des environnements complexes où s'affrontent de multiples normes qu'il n'est pas toujours facile de départager ni de mettre au clair afin de prendre une décision éclairée. Pourtant, les modèles de prise de décision dont disposent actuellement les éthiciens cliniques ou les comités de bioéthique comportent souvent des lacunes lorsqu'il s'agit d'aborder des problèmes à plusieurs niveaux (micro, méso et macro). Les décisions reposent ainsi trop souvent sur l'intuition plutôt (...) que sur une démarche systématique rigoureuse et facilement reproductible. Afin de pallier la situation, cet ouvrage fournit des outils théoriques et pratiques facilitant la prise de décision. Fondé sur l'étude collective d'un cas complexe ayant trait aux dimensions tragiques des choix éthiques en fin de vie (doit-on ou non administrer un médicament coûteux pour le traitement d'un cancer au stade avancé?), il réunit les contributions de plusieurs experts reconnus travaillant dans le système de santé (cliniciens, éthiciens, chercheurs, gestionnaires et décideurs publics). Background The first COVID-19 wave started in February 2020 in France. The influx of patients requiring emergency care and high-level technicity led healthcare professionals to fear saturation of available care. In that context, the multidisciplinary Ethics- Support Cell (EST) was created to help medical teams consider the decisions that could potentially be sources of ethical dilemmas. Objectives The primary objective was to prospectively collect information on requests for EST assistance from 23 March to 9 May 2020. The secondary aim was (...) to describe the Cell’s functions during that period. Research design This observational, real-time study of requests for Cell consultations concerned ethical dilemmas arising during a public health crisis. The EST created a grid to collect relevant information (clinical, patient’s/designated representative’s preferences and ethical principles strained by the situation), thereby assuring that each EST asked the same questions, in the same order. Participants and research context Only our university hospital’s clinicians could request EST intervention. Ethical considerations The hospital Research Ethics Committee approved this study (no. CER-2020-107). The patient, his/her family, or designated representative was informed of this ethics consultation and most met with EST members, which enabled them to express their preferences and/or opposition. Findings/results 33 requests (patients’ mean age: 80.8 years; 29 had COVID-19: 24 with dyspnea, 30 with comorbidities). 17 Emergency Department solicitations concerned ICU admission, without reference to resource constraints; others addressed therapeutic proportionality dilemmas. Discussion Intervention-request motives concerned limited resources and treatment intensity. Management revolved around three axes: the treatment option most appropriate for the patient, the feasibility of implementation, and dignified care for the patient. Conclusions COVID-19 crisis forced hospitals to envisage prioritization of ICU access. Established decision-making criteria and protocols do not enable healthcare professionals to escape ethical dilemmas. That acknowledgement highlights ethical risks, enhances the added-value of nursing and encourages all players to be vigilant to pursue collective deliberations to achieve clear and transparent decisions. Plato famously promised to complement the Sophist and the Statesman with another work on a third sort of expert, the philosopher--but we do not have this final dialogue. Mary Louise Gill argues that Plato promised the Philosopher, but did not write it, in order to stimulate his audience and encourage his readers to work out, for themselves, the portrait it would have contained. The Sophist and Statesman are themselves members of a larger series starting with the Theaetetus, Plato's investigation (...) of knowledge, and the whole series relies on the Parmenides, the second part of which presents a philosophical exercise, introduced as the first step in a larger philosophical program. Gill contends that the dialogues leading up to the missing Philosopher, though they reach some substantive conclusions, are philosophical exercises of various sorts designed to train students in dialectic, the philosopher's method; and that a second version of the Parmenides exercise, closely patterned on it, spans parts of the Theaetetus and Sophist and brings the philosopher into view. This is the exercise about being, the subject-matter studied by Plato's philosopher. Plato hides the pieces of the puzzle and its solution in plain sight, forcing his students (and modern readers) to dig out the pieces and reconstruct the project. Gill reveals how, in finding the philosopher through the exercise, the student becomes a philosopher by mastering his methods. She shows that the target of Plato's exercise is internally related to its pedagogical purpose. (shrink)

This book explores a fundamental tension in Aristotle's metaphysics: how can an entity such as a living organisma composite generated through the imposition of form on preexisting matterhave the conceptual unity that Aristotle demands of primary substances? Mary Louise Gill bases her treatment of the problem of unity, and of Aristotle's solution, on a fresh interpretation of the relation between matter and form. Challenging the traditional understanding of Aristotelian matter, she argues that material substances are subverted by matter and (...) maintained by form that controls the matter to serve a positive end. The unity of material substances thus involves a dynamic relation between resistant materials and directive ends. Aristotle on Substance offers both a general account of matter, form, and substantial unity and a specific assessment of particular Aristotelian arguments. At every point, Gill engages Aristotle on his own philosophical ground through the detailed analysis of central, and often controversial, texts from the Metaphysics, Physics, On Generation and Corruption, De Anima, De Caelo, and the biological works. The result is a coherent, firmly grounded rethinking of Aristotle's central metaphysical concepts and of his struggle toward a fully consistent theory of material substances. (shrink)

In _Michael Polanyi and His Generation_, Mary Jo Nye investigates the role that Michael Polanyi and several of his contemporaries played in the emergence of the social turn in the philosophy of science. This turn involved seeing science as a socially based enterprise that does not rely on empiricism and reason alone but on social communities, behavioral norms, and personal commitments. Nye argues that the roots of the social turn are to be found in the scientific culture and political (...) events of Europe in the 1930s, when scientific intellectuals struggled to defend the universal status of scientific knowledge and to justify public support for science in an era of economic catastrophe, Stalinism and Fascism, and increased demands for applications of science to industry and social welfare. At the center of this struggle was Polanyi, who Nye contends was one of the first advocates of this new conception of science. Nye reconstructs Polanyi’s scientific and political milieus in Budapest, Berlin, and Manchester from the 1910s to the 1950s and explains how he and other natural scientists and social scientists of his generation—including J. D. Bernal, Ludwik Fleck, Karl Mannheim, and Robert K. Merton—and the next, such as Thomas Kuhn, forged a politically charged philosophy of science, one that newly emphasized the social construction of science. (shrink)

InWisdom, Information and Wonder, Mary Midgley tackles the question at the root of our civilization: What is knowledge for?

Background:Clinical investigation is a growing field employing increasing numbers of nurses. This has created a new specialty practice defined by aspects unique to nursing in a clinical research context: the objectives, setting, and nature of the nurse–participant relationship. The clinical research nurse role may give rise to feelings of ethical conflict between aspects of protocol implementation and the duty of patient advocacy, a primary nursing responsibility. Little is known about whether research nurses experience unique ethical challenges distinct from those experienced (...) by nurses in traditional patient-care settings.Research objectives:The purpose of the study was to describe the nature of ethical challenges experienced by clinical research nurses within the context of their practice.Research design:The study utilized a qualitative descriptive design with individual interviews.Participants and research context:Participating nurses self-identified as having experienced ethical challenges during screening. The majority were Caucasian, female, and worked in outpatient settings. Approximately 50% had > 10 years of research experience.Ethical considerations:The human subjects review board approved the study. Written informed consent was obtained.Findings:Predominant themes were revealed: the inability to provide a probable good, or/do no harm, and dual obligations. The following patterns and subthemes emerged: conflicted allegiances between protocol implementation, needs of the participant, desire to advance science, and tension between the nurse–patient therapeutic relationship versus the research relationship.Discussion:Participants described ethical challenges specific to the research role. The issues are central to the nurse–participant relationship, patient advocacy, the nurse’s role in implementing protocols, and/or advancing science.Conclusion:Ethical challenges related to the specialized role of clinical research nurses were identified. More research is warranted to fully understand their nature and frequency and to identify support systems for resolution. (shrink)

Confucianism conceives of persons as being necessarily interdependent, defining personhood in terms of the various roles one embodies and that are established by the relationships basic to one's life. By way of contrast, the Western philosophical tradition has predominantly defined persons in terms of intrinsic characteristics not thought to depend on others. This more strictly and explicitly individualistic concept of personhood contrasts with the Confucian idea that one becomes a person because of others; where one is never a person independently (...) or in and of oneself but develops into one only in community. This article surveys some differences between Confucian and Western ideas of self and their connection to ethics mainly in light of the relational self of the Confucian Analects and Mencius . A Philosophy Compass article called Confucianism and Ethics in the Western Philosophical Tradition II: A Comparative Analysis of Personhood will follow, that examines how the more individualistic way of conceiving of personhood in the West has had moral and political implications that differ, and even conflict, with those of Confucianism. Debunking arguments against both moral and mathematical realism have been pressed, based on the claim that our moral and mathematical beliefs are insensitive to the moral/mathematical facts. In the mathematical case, I argue that the role of Hume’s Principle as a conceptual truth speaks against the debunkers’ claim that it is intelligible to imagine the facts about numbers being otherwise while our evolved responses remain the same. Analogously, I argue, the conceptual supervenience of the moral on the natural speaks presents (...) a difficulty for the debunker’s claim that, had the moral facts been otherwise, our evolved moral beliefs would have remained the same. This article explains how li 禮 or ‘ritual propriety’ is the ‘body language’ of ren 仁 or the authentic expression of our humanity. Li and ren are interdependent aspects of a larger creative human way (rendao 仁道) that can be conceptually distinguished as follows: li refers to the ritualized social form of appropriate conduct and ren to the more general, authentically human spirit this expresses. Li is the social instrument for self-cultivation and the vehicle of harmonious human interaction. More, li (...) must mean something that is effectively communicated to others for an authentic, human (ren) interaction to occur. Li is the body language of ren in being the ritual vehicle for its’ expression; however, li is underdetermined by ren and so must be distinguished from it in on further grounds: authentic human activity must not just be equivocated with social convention because conclusively establishing whether a particular action is li (or is a truly ren action) is impossible. As a result, li is often confused with social power and privilege that is easier to empirically identify than ren conduct is, but this is a mistake since li has to express ren or it is not li at all. The inescapable ambiguity of li – an ambiguity that attaches to any language – can be critiqued by the Western view that sees something ‘essential’ to the ‘self,’ and that makes one a ‘self’ in and of oneself and not in a way that depends on others. I show that such Western individualism – while resting on a fundamentally different way of thinking of being a person and living a good life – does not reduce Confucian ritual to being an instrument for social discrimination and subordination. My argument is indebted to twentieth-century philosophy of language in the West that offered the idea that some words are actions. (shrink)

"Charming, interesting, thought-provoking and a great read." Rosalind Hursthouse The daughter of a pacifist rector who answered "No!" when his congregation asked him "Is everything in the bible true?", perhaps Mary Midgley was destined to become a philosopher. Yet few would have thought this inquisitive, untidy, nature-loving child would become "one of the sharpest critical pens in the west." This is her remarkable story. Probably the only philosopher to have been in Vienna on the eve of its invasion by (...) Nazi Germany in 1938 and dance in Trafalgar Square on VE day seven year later, she studied philosophy at Oxford in the same year as Iris Murdoch, Elizabeth Anscombe and Philippa Foot, all of whom became close friends. Midgley tells us in vivid and humorous fashion how they cut a swathe through the arid landscape of 1950s British philosophy, writing and arguing - often with each other - about the grand themes of character, beauty and the meaning of rudeness while the spectral figure of Ludwig Wittgenstein hovered in the background. She also charts the highs and lows of philosophy and academia in Britain. On joining the Reading philosophy department on £400 a year in 1949, she doubled its staff complement. But her many years at Newcastle University - where Mike Brearley, who later captained England at cricket, also used to teach - were rewarded with the closure of the philosophy department in the 1980s. The mother of three children, her journey is also one of a woman who in the 1950s and 1960s was fighting to combine a professional career with raising a family. In startling contrast to many of the academic stars of her generation, we learn that Midgley nearly became a novelist and started writing philosophy only when in her fifties, suggesting that Minerva's owl really does fly at dusk. Plainly told like her philosophy, this is an elegiac and moving account of friendships found and lost, bitter philosophical battles and of a profound love of teaching all too rarely acknowledged today. This article revisits the account of magnanimity offered by Thomas Aquinas, in his Commentary on the Nicomachean Ethics of Aristotle and especially in his Summa Theologiae. Recent scholarship has viewed Aquinas' magnanimity as essentially Aristotle's, complemented by the addition of charity and humility to the classical moral horizon. By contrast, I read Aquinas as offering a subtle yet far-reaching critique of important aspects of Aristotelian magnanimity, a critique with roots in Aquinas' theology, yet also comprising a significant philosophic reappraisal of (...) Aristotle's account of human excellence. Against contentions that Aquinas' ethical revisionism is antithetical to civic common sense, the requirements of statesmanship, and the rational foundations of social science, I argue that Aquinas' theory is politically salutary and theoretically enlightening. Moreover, I suggest that recent dissident reflection on twentieth-century totalitarian experience underscores both the humanity and the nobility of a humility-informed magnanimity, such as that advocated by Thomas Aquinas. IN THE GROUNDWORK OF THE METAPHYSICS OF MORALS Kant noted that, while the present work would be concerned only with the supreme principle of morality, he intended some day to write a "metaphysics of morals" in which he would set forth the whole system of man's duties derived from this principle. Twelve years later, in 1797, he published The Metaphysics of Morals in two parts: Metaphysical First Principles of the Doctrine of Right and Metaphysical First Principles of the Doctrine of (...) Virtue. As the Groundwork made clear, the concept of duty contains that of constraint upon an agent who, because of impulses belonging to his sensible nature, does not necessarily do whatever is objectively necessary in accordance with moral laws. The first part of The Metaphysics of Morals is the system of moral principles on which are based laws prescribing duties that one can be coerced to fulfill. The second part is the system of duties for which only self-constraint through the thought of duty is possible. Each part is said to be a system of duties because it is derived from a first principle and remains on the level of principles that can be known a priori to be binding on men generally. Together they form the whole system of man's duties because the first principle of each is derived from the supreme principle of morality, the categorical imperative and the division in terms of coercion and self-constraint is exhaustive. The relationship between Lexical-Functional Grammar (LFG) functional structures (f-structures) for sentences and their semanticinterpretations can be formalized in linear logic in a way thatcorrectly explains the observed interactions between quantifier scopeambiguity, bound anaphora and intensionality.Our linear-logic formalization of the compositional properties ofquantifying expressions in natural language obviates the need forspecial mechanisms, such as Cooper storage, in representing thescoping possibilities of quantifying expressions. Instead, thesemantic contribution of a quantifier is recorded as a linear-logicformula whose use in a proof will establish the (...) scope of thequantifier. "Multiverse" cosmologies imagine our universe as just one of a vast number of others. While this idea has captivated philosophy, religion, and literature for millennia, it is now being considered as a scientific hypothesis--with different models emerging from cosmology, quantum mechanics, and string theory. Beginning with ancient Atomist and Stoic philosophies, Mary-Jane Rubenstein links contemporary models of the multiverse to their forerunners and explores the reasons for their recent appearance. One concerns the so-called fine-tuning of the universe: nature's constants (...) are so delicately calibrated that it seems they have been set just right to allow life to emerge. For some thinkers, these "fine-tunings" are evidence of the existence of God; for others, however, and for most physicists, "God" is an insufficient scientific explanation. Hence the allure of the multiverse: if all possible worlds exist somewhere, then like monkeys hammering out Shakespeare, one universe is bound to be suitable for life. Of course, this hypothesis replaces God with an equally baffling article of faith: the existence of universes beyond, before, or after our own, eternally generated yet forever inaccessible to observation or experiment. In their very efforts to sidestep metaphysics, theoretical physicists propose multiverse scenarios that collide with it and even produce counter-theological narratives. Far from invalidating multiverse hypotheses, Rubenstein argues, this interdisciplinary collision actually secures their scientific viability. We may therefore be witnessing a radical reconfiguration of physics, philosophy, and religion in the modern turn to the multiverse. (shrink)
