Epilepsy surgery is the most effective therapeutic approach for children with drug resistant epilepsy. Recent advances in neurosurgery, such as the Laser Interstitial Thermal Therapy, improved the safety and non-invasiveness of this method. Electric and magnetic source imaging plays critical role in the delineation of the epileptogenic focus during the presurgical evaluation of children with DRE. Yet, they are currently underutilized even in tertiary epilepsy centers. Here, we present a case of an adolescent who suffered from DRE for 16 years (...) and underwent surgery at Cook Children's Medical Center. The patient was previously evaluated in a level 4 epilepsy center and treated with multiple antiseizure medications for several years. Presurgical evaluation at CCMC included long-term video electroencephalography, magnetoencephalography with simultaneous conventional EEG and high-density EEG in two consecutive sessions, MRI, and fluorodeoxyglucose - positron emission tomography. Video long-term EEG captured nine focal-onset clinical seizures with a maximal evolution over the right frontal/frontal midline areas. MRI was initially interpreted as non-lesional. FDG-PET revealed a small region of hypometabolism at the anterior right superior temporal gyrus. ESI and MSI performed with dipole clustering showed a tight cluster of dipoles in the right anterior insula. The patient underwent intracranial EEG which indicated the right anterior insular as seizure onset zone. Eventually LITT rendered the patient seizure free. Retrospective analysis of ESI and MSI clustered dipoles found a mean distance of dipoles from the ablated volume ranging from 10 to 25 mm. Our findings highlight the importance of recent technological advances in the presurgical evaluation and surgical treatment of children with DRE, and the underutilization of epilepsy surgery in children with DRE. ObjectivesWorking-age cancer patients face barriers to resuming work after treatment completion. Those resuming work contend with reduced productivity arising from persisting residual symptoms. Existing studies of return to work after cancer diagnosis were done predominantly in Western countries. Given that employment and RTW in cancer survivors likely vary regionally due to healthcare provision and social security differences, we documented rates and correlates of RTW, work productivity, and activity impairment among Chinese cancer survivors in Hong Kong at one-year post-treatment.MethodsOf 1,106 cancer (...) patients assessed at six-months post-cancer treatment, 593 previously worked; detailed work status, psychological distress, physical symptom distress, supportive care needs, health-related quality of life, and illness perception were assessed. Six months later, work productivity and activity impairment were assessed. Descriptive analyses examined RTW rate. Fully adjusted regressions determined RTW, work productivity, and activity impairment predictors.ResultsAt baseline, 39% were working, 26% on sick leave, and 35% were unemployed. Compared to patients returning to work, unemployed participants were older, likely manual/service-oriented workers, and had lower family income, chemotherapy, fewer unmet health system and information needs, poorer physical functioning, and negative illness perceptions. Sick leave participants were likely service-oriented workers, who had head and neck cancer, chemotherapy, and poor physical functioning. At FU, baseline depressive symptoms, physical symptom distress, and negative illness perceptions predicted presenteeism and work productivity loss; gynecological cancer, fewer unmet health system and information needs, and greater unmet sexuality needs predicted absenteeism; physical symptom distress, negative illness perception, and poor physical functioning predicted activity impairment.ConclusionCancer survivors who had more physically demanding jobs and poorer physical functioning delayed RTW. Unmanaged physical symptom and psychological distress hindered work productivity. (shrink)

The present essay, here published for the first time, is part of a group of four texts on the history of the early phenomenological movement that Theodor Conrad wrote right after World War II. One of these texts known as “Conrads Bericht” was edited by Eberhard Avé-Lallemant and Karl Schuhmann and published in Husserl Studies in 1992. The four original typescripts are preserved in the archive of the Munich Circle of phenomenology at the Bavarian State Library. As the reader (...) will immediately realize, at the center of the present text is a peculiar account of the beginning of the phenomenological tradition, namely, of its first schism. Contrary to the usual thesis according to which the first schism revolved mainly, if not exclusively around the so-called “idealism-realism controversy,” Conrad points out that this was only part of it, and that the first controversy between Husserl and some of his early disciples or students bore on the nature of phenomenology itself. This paper investigates Hedwig Conrad-Martius’ theory of representation, which is unique in that it introduces a method of ideation that is completely different from the one of transcendental phenomenology. Instead of separating the essence from the individual real entity through reduction, Conrad-Martius’ method of representation elucidates what constitutes the reality of the actual individual. In a representation, we can explore and play with our receptivity to the self-emergence of reality by observing the different ways in which our representations (...) themselves appear as real. I will discuss this access to reality as a realistic phenomenological method not just in terms of lived experience, but also as eidetic reduction. My thesis is that Conrad-Martius’ theory of representation allows for conceiving how essences, as the immutable features of reality beyond surface perception, can be addressed by intuition, viz. representation. In a second step, a shift in Conrad-Martius’ thinking towards the constitution of reality in itself is discussed based on her second major work, Realontologie. Thirdly, Conrad-Martius’ theory of representation is roughly contrasted with Husserl’s to further elucidate what constitutes the “really real” in her phenomenology beyond the simple alternative between idealism and realism. In later Edith Stein and Hedwig Conrad-Martius, finite existence appears to be necessarily intertwined with infinite being. In response to this observation, this paper puts particular focus on the experience of finite being in order to address the specifically phenomenological (i.e., experiential) aspects of Stein’s and Conrad-Martius’ metaphysics. As a consequence, instead of pointing to eternal or infinite being, finite experience is understood to – less specifically – transcend itself. Using the notion of actus essendi (priority of existence (...) over essence), I identify two ideas as specifically characterizing this transcendence: non-ownership of time (in Conrad-Martius), by which is questioned the coherence of inner time consciousness, as well as non-ownership of sense (in Stein), stipulating that the sense one intuitively and intellectively experiences in reference to objects is discovered, rather than made. Subsequently, the paper discusses how Stein’s and Conrad-Martius’ metaphysics of finite existence is reflected in their critical assessments of Heidegger’s existential finitude. (shrink)

Initially seen as a simple master (...) mariner or at most as a technical device, a way to get a tale told, with the work of Albert Guérard he began to receive his due, becoming lionized as the true subject at least of Heart of Darkness, and of more than technical interest in the other stories. 1 In recent years, however, his reputation has slumped hard as his racism and sexism have come to the fore. In this article, I attempt no full-scale rehabilitation but simply point out that, despite his failings, Marlow possesses a virtue worth remarking and that his failings involve a failure to extend that virtue far enough.I call that virtue attentiveness, and more specifically, attentiveness to the other. It has been neglected, at least by philosophers, although as a virtue it is utterly obvious. This is not surprising. The moral life is generally the sphere of the obvious and progress here often consists merely in noting what has been overlooked. The presence or absence of this virtue is revealed less by what the agent feels or doesn't feel or does or doesn't do than by her stance or orientation toward others. And as orientations are slippery things to describe (and this one more than most) there is an advantage in looking at literary characters: they show palpably the virtue (or corresponding vice) at issue. The horror!" but "The last words he pronounced was—your name" (HD, p. 161). 2 Marlow says that the truth "would have been too dark, too dark altogether" (p. 162). But what doesn't Marlow want the Intended to know? That her beloved went mad in the jungle, became evil? And why shouldn't she know this? Because it would bring her pain? But Marlow insists that the Intended has "a mature capacity" for suffering (p. 157; a capacity to handle it, presumably, not a proneness to it). Anyway, Marlow's lie will perpetuate suffering, her mourning for her image of Kurtz: smash her idol and she might be able to get on with her life. In this essay I discuss one way in which a narrative might be political. My proof text (...) will be Joseph Conrad's novel Nostromo.2Let's start by noting several ways in which a narrative might criticize the present.(1). It highlights a shortcoming, say, an injustice, of the present.(2). It shows the possibility of a different and better society, usually inhabited by people with a different and better psychology.(3). It shows nothing positive, and yet it elicits a desire for a radically different and radically better future, that is, for something very different from the present.The first option divides into:(1a) The narrative shows a particular injustice, the sort of thing that concrete institutional change might eliminate or ameliorate. Many classic works do this, such as Middlemarch and Madame Bovary. Of course, precisely because such works criticize large and pervasive phenomena, they usually do not point to any specific solution. They say, "This (something fairly general) is bad."When we move to narratives that try to present better worlds, things get more complicated. We can see the issue by noting a distinction in (2) and between (2) and (3). With (2a), we are in the realm of what John Rawls calls a "realistic utopia."3 This is supposed to be (i) sufficiently specifiable, in terms of, say, principles of distribution, a conception of the person, and a conception of justification; that is, in terms of several central organizing ideas. It is also supposed to be (ii) sufficiently realistic in terms of the moral psychology needed for its instantiation. Public attitudes concerning cognitive enhancements are significant for a number of reasons. They tell us about how socially acceptable these emerging technologies are considered to be, but they also provide a window into the ethical reasons that are likely to get traction in the ongoing debates about them. We thus see Conrad et al’s project of empirically investigating the effect of metaphors and context in shaping attitudes about cognitive enhancements as both interesting and important. We sketch what we suspect (...) is a central theme that runs through these public attitudes, but that Conrad el al’s paper elides. We were disappointed that they did not more directly explore the efficacy of frames and metaphors associated with the values of authenticity and self-expression. This seems like a missed opportunity. Based on the premise that individualistic values enjoy centrality in Western and especially North American culture (e.g. Taylor 1989), we hypothesize that metaphors and frames informed by those values will be especially effective in shaping public attitudes. That is, when various kinds of novel enhancement are described as allowing people to more fully express themselves, or as helping people overcome obstacles to being authentic and true to their inner sense of themselves, those enhancements will be considered justified, and their use more likely to be viewed as socially acceptable by the public. We support our contention by drawing on work by Elliott (2004, 2011, c.f. Kadlac 2018), and discuss how this study, and others modeled on it, might shed light on our hypothesis. (shrink)

Popular sovereignty - the doctrine that the public powers of state originate in a concessive grant of power from 'the people' - is perhaps the cardinal doctrine of modern constitutional theory, placing full constitutional authority in the people at large, rather than in the hands of judges, kings, or a political elite. Although its classic formulation is to be found in the major theoretical treatments of the modern state, such as in the treatises of Hobbes, Locke, and Rousseau, this book (...) explores the intellectual origins of this doctrine and investigates its chief source in late medieval and early modern thought. Long regarded the principal source for modern legal reasoning, Roman law had a profound impact on the major architects of popular sovereignty such as François Hotman, Jean Bodin, and Hugo Grotius. Adopting the juridical language of obligations, property, and personality as well as the model of the Roman constitution, these jurists crafted a uniform theory that located the right of sovereignty in the people at large as the legal owners of state authority. This paper describes a plausible view of the nature of physical objects, their mereological connections to each other, and their relation to spacetime. As well as being parsimonious, the view provides a plausible context for denying all of the following: (1) A theory that objects endure through time (and do not have temporal parts, as normally conceived) cannot claim that material objects are identical to space-time regions they occupy. (2) At least one of the family of mereological connections (part-whole, overlap (...) etc.) is to be taken as primitive. (3) Claims entirely in the language of quantifiers and identity and mereology are not semantically vague. It is thus an example showing that the there are more options for the metaphysics of objects, spacetime and mereology than many metaphysicians ordinarily assume. (shrink)

Students and teachers of Chinese history and philosophy will not want to miss Daniel Gardner's accessible translation of the teachings of Chu Hsi —a luminary of the Confucian tradition who dominated Chinese intellectual life for centuries. In this book Daniel Smyth offers a comprehensive overview of Immanuel Kant's conception of intuition in all its species – divine, receptive, sensible, and human. Kant considers sense perception a paradigm of intuition, yet claims that we can represent infinities in intuition, despite the finitude of sense perception. Smyth examines this heterodox combination of commitments and argues that the various features Kant ascribes to intuition are meant to remedy specific cognitive shortcomings that arise from the discursivity of our intellect (...) Intuition acting as the intellect's cognitive partner to make knowledge possible. He reconstructs Kant's conception of intuition and its role in his philosophy of mind, epistemology, and philosophy of mathematics, and shows that Kant's conception of sensibility is as innovative and revolutionary as his much-debated theory of the understanding. Spatial attention and eye-movements are tightly coupled, but the precise nature of this coupling is controversial. The influential but controversial Premotor theory of attention makes four specific predictions about the relationship between motor preparation and spatial attention. Firstly, spatial attention and motor preparation use the same neural substrates. Secondly, spatial attention is functionally equivalent to planning goal directed actions such as eye-movements (i.e. planning an action is both necessary and sufficient for a shift of spatial attention). Thirdly, planning a goal (...) directed action with any effector system is sufficient to trigger a shift of spatial attention. Fourthly, the eye-movement system has a privileged role in orienting visual spatial attention. This article reviews empirical studies that have tested these predictions. Contrary to predictions one and two there is evidence of anatomical and functional dissociations between endogenous spatial attention and motor preparation. However, there is compelling evidence that exogenous attention is reliant on activation of the oculomotor system. With respect to the third prediction, there is correlational evidence that spatial attention is directed to the endpoint of goal-directed actions but no direct evidence that this attention shift is dependent on motor preparation. The few studies to have directly tested the fourth prediction have produced conflicting results, so the extent to which the oculomotor system has a privileged role in spatial attention remains unclear. Overall, the evidence is not consistent with the view that spatial attention is functionally equivalent to motor preparation so the Premotor theory should be rejected, although a limited version of the Premotor theory in which only exogenous attention is dependent on motor preparation may still be tenable. A plausible alternative account is that activity in the motor system contributes to biased competition between different sensory representations with the winner of the competition becoming the attended item. Recently, William Hasker and Cheryl Chen have argued that James Sterba’s argument for the non-existence of God based on the existence of horrendous evil consequences fails. Hasker, among other things, contends that eliminating horrendous evil consequences will result in a moral kindergarten. It is unclear, however, whether the elimination of horrendous evil consequences will result in a moral kindergarten. Moreover, if Hasker is right, then it may be that most people in the actual world live in a moral kindergarten. Chen (...) argues that eliminating horrendous evil consequences may lead to a morally worse world. While Chen is ultimately right about this, it is not fatal to the basic intuition at the heart of Sterba’s argument. (shrink)

What are the metaphysical and metapsychological boundaries of a person? How do we draw our borders? This much is clear: personal identity without thought experiments is impossible. I develop a new way of conceptualizing physiological and psychological borders leading to a re-evaluation of the problem of personal identity within the contemporary literature, especially Parfit, arguing that we must, necessarily, turn to the conceptual analysis of metaphysical and metapsychological borders. I offer an explanation of the persistence of common sense against philosophical (...) analysis, criticize Wilkes's position, and provide a foundation for the proper use of thought experiments in analytic metapsychology and metaphysics. In this addition to the award-winning Church and Postmodern Culture series, respected theologian Daniel Bell compares and contrasts capitalism and Christianity, showing how Christianity provides resources for faithfully navigating the postmodern global economy. Bell approaches capitalism and Christianity as alternative visions of humanity, God, and the good life. Considering faith and economics in terms of how desire is shaped, he casts the conflict as one between different disciplines of desire. He engages the work of two important postmodern philosophers, Deleuze (...) and Foucault, to illuminate the nature of the postmodern world that the church currently inhabits. Bell then considers how the global economy deforms desire in a manner that distorts human relations with God and one another.
