Martin Heidegger a vu dans l'avènement grec de la vérité comme identification de l'être et de la raison l'origine du dispositif technologique planétaire qui rend aujourd'hui possible l'annihilation de l'homme et du monde. Cette catastrophe met en évidence la finitude de la vérité ontologique, ainsi que son opacité à l'originaire, d'emblée renvoyé dans le néant. L'affaire de la pensée exige aujourd'hui de surmonter la décision qui a inauguré notre destin. Heidegger a tenté de le faire à partir de Hölderlin, dans (...) une lecture qui identifie le mystère de l'originaire à la Terre et subordonne le réenracinement de la vérité à l'avènement d'une ultime figure du divin. Jean Vioulac propose une alternative au néopaganisme heideggérien. En suivant la voie ouverte par saint Paul qui pense le Logos comme mystère, ou celle de Maître Eckhart, qui identifie l'originaire avec l'abîme de la déité, c'est l'incarnation de la vérité, et non son enracinement, qui s'avère susceptible de sauvegarder le mystère. L'incarnation du Logos s'impose ainsi comme un "autre Commencement", antagoniste au Commencement grec bien qu'en étroite connexion avec lui. Dans cette perspective, le christianisme constitue non plus un obstacle au projet philosophique de Heidegger. We inhabit a time of crisis-totalitarianism, environmental collapse, and the unquestioned rule of neoliberal capitalism. Philosopher Jean Vioulac is interested in and worried by all of this, but his main concern is how these phenomena all represent a crisis within-and a threat to-thinking itself. In his first book to be translated into English, Vioulac radicalizes Heidegger's understanding of truth as disclosure through the notion of "the apocalypse of truth." La Révolution industrielle a fait basculer l'humanité tout entière dans une période critique qui semble aujourd'hui la destiner à une catastrophe globale. Penser cette catastrophe impose de la situer dans le processus tragique dont elle est le dernier acte, et d'assumer ainsi la tâche d'une philosophie de l'Histoire. Mais la catastrophe n'a pas épargné la philosophie elle-même, qui dans le même moment s'est confrontée à l'obsolescence de toute sa tradition en laquelle elle a reconnu une mystification : d'où la nécessité (...) de sa réinstitution radicale, qui renverse sa structure théologique pour la réélaborer dans une structure archéologique. Cette archéologie cependant découvre l'absence de tout fondement et n'accède qu'à l'abîme : elle ne peut établir qu'un principe d'anarchie, et se développer comme anarchéologie. Il est alors possible de concevoir l'Histoire en la ressaisissant depuis son inauguration par la Révolution néolithique, non plus en postulant à son commencement un Universel en puissance dont l'Histoire serait la réalisation de droit, mais en constatant à son achèvement un Universel en acte dont l'Histoire est la genèse de fait : l'Un numérique parvenu à l'hégémonie mondiale, devenu puissance démiurgique et pouvoir cybernétique, c'est-à-dire Capital. Notre époque est ainsi révélation apocalyptique de l'essence nihiliste de l'Histoire, processus d'universalisation par consumation de toute particularité, et finalement processus d'annihilation. Deux principes phénoménologiques s’opposent : l’un qui réduit tout donné au pôle singulier de la subjectivité vivante (Husserl), l’autre qui le réduit au pôle universel du concept (Hegel), et cette opposition définit la crise de la modernité occidentale. La revendication du sujet à la plénitude du sens impose donc une prise de position par rapport à tout le destin de la métaphysique occidentale : la subjectivité ne peut être posée comme source de droit que si est frappé d’illégitimité l’objectivisme en (...) tant que tel. The issue of phenomenological reduction proceeds from the methodological requirement of the absence of presupposition : however, transcendental subjectivity as an actor of the reduction constitutes a presupposition that is inherent to Husserl’s approach. Radicality would then impose a reduction the subject submits to, not a reduction the subject chooses. That is how disease was considered by Nietzsche. Indeed, disease is the event through which subjective flesh shows itself from itself, in pathological phenomena, and shows itself as the ground and (...) origin of any sense and value. Disease encloses the subject in the sphere of his immanent lived-experiences, where the essence of being reveals itself as pathos : it operates what is to be called a pathological reduction. Inaugurée par la révolution copernicienne, la Révolution française, la révolution industrielle et la révolution technologique, notre époque est à tous égards révolutionnaire : elle est la seconde révolution connue par l'humanité après la révolution néolithique, qui inaugura l'histoire il y a une centaine de siècles. Concevoir un tel bouleversement pose des problèmes considérables, puisque les principes logiques, concepts et catégories jusqu'ici en vigueur tendent à devenir eux-mêmes obsolètes. Seule une pensée révolutionnaire est à la mesure de notre époque, et la (...) pensée de Marx est fondamentalement révolutionnaire. Mais sa nouveauté fut longtemps dissimulée par des interprétations idéologiques qui l'ont ravalée à un matérialisme, un scientisme, un positivisme ou un naturalisme. La pensée de Marx peut pourtant se définir clairement : elle est un communisme, qui reconnaît la communauté historique des sujets vivants comme sol ontologique et fondement premier. La mise au jour du communisme comme position philosophique peut se conduire à partir des ultimes développements de la phénoménologie, qui mènent Husserl à reléguer comme superficiel et dérivé le niveau théorique et donc à lui reconnaître un statut idéologique - pour approfondir la rétrocession transcendantale en direction de la communauté intersubjective de corps vivants oeuvrant sur le terrain de la praxis à partir d'un héritage historique. C'est donc sur ce fondement de droit qu'il devient possible de critiquer l'autonomisation de l'objectivité qui définit tout à la fois le capitalisme, la science et la technique modernes. Mais cette critique appelle alors elle-même une révolution, seule à même de conjurer le danger qu'une automatisation totale ferait peser sur l'humanité. (shrink)

_The VaKE Handbook: Theory and Practice of Values and Knowledge Education_ presents a theoretical model and many examples in various fields of education and training for the realization of the principle "Values without knowledge are blind, while knowledge without values is irresponsible".

In this paper, we examine recent critiques of the debate about defining disease, which claim that its use of conceptual analysis embeds the problematic assumption that the concept is classically structured. These critiques suggest, instead, developing plural stipulative definitions. Although we substantially agree with these critiques, we resist their implication that no general definition of “disease” is possible. We offer an alternative, inductive argument that disease cannot be classically defined and that the best explanation for this is that the concept (...) is structured as a cluster. We further argue that we do not need to reject the possibility of defining the general concept “disease” to legitimate developing stipulative definitions of disease that are relative to particular purposes. A cluster definition of disease is compatible with contextually motivated definitions, which may be considered précisifications of the more general cluster concept. (shrink)

Along with practical, ethical, and scientific questions, overdiagnosis raises questions about our concept of disease. In this paper, we analyse overdiagnosis as an epistemic problem and show how it challenges many existing accounts of disease. In particular, it raises ques- tions about conceptual links drawn between disease and dysfunction, harm, and risk. We argue (...) that ‘disease’ should be considered a vague concept with a non-classical structure. On this view, overdiagnosed cases are ‘borderline’ cases of disease, falling in the zone between cases that are clearly disease, and cases that are clearly not disease. We then develop a pre ́cising definition of disease designed to provide practical help in preventing and limiting overdiagnosis. We argue that for this purpose, we can define disease as dysfunction that has a significant risk of causing severe harm to the patient. (shrink)

Personalised medicine has been discussed as a medical paradigm shift that will improve health while reducing inefficiency and waste. At the same time, it raises new practical, regulatory, and ethical challenges. In this paper, we examine PM strategies epistemologically in order to develop capacities to address these challenges, focusing on a recently proposed strategy for developing patient-specific models from induced pluripotent stem cells so as to make individualised treatment predictions. We compare this strategy to two main PM strategies—stratified medicine and (...) computational models. Drawing on epistemological work in the philosophy of medicine, we explain why these two methods, while powerful, are neither truly personalised nor, epistemologically speaking, novel strategies. Both are forms of correlational black box. We then argue that the iPSC models would count as a new kind of black box. They would not rely entirely on mechanistic knowledge, and they would utilise correlational evidence in a different way from other strategies—a way that would enable personalised predictions. In arguing that the iPSC models would present a novel method of gaining evidence for clinical practice, we provide an epistemic analysis that can help to inform the practical, regulatory, and ethical challenges of developing an iPSC system. People are motivated by shared social values that, when held with moral conviction, can serve as compelling mandates capable of facilitating support for ideological violence. The current study examined this dark side of morality by identifying specific cognitive and neural mechanisms associated with beliefs about the appropriateness of sociopolitical violence, and determining the extent to which the engagement of these mechanisms was predicted by moral convictions. Participants reported their moral convictions about a variety of sociopolitical issues prior to undergoing functional (...) MRI scanning. During scanning, they were asked to evaluate the appropriateness of violent protests that were ostensibly congruent or incongruent with their views about sociopolitical issues. Complementary univariate and multivariate analytical strategies comparing neural responses to congruent and incongruent violence identified neural mechanisms implicated in processing salience and in the encoding of subjective value. As predicted, neuro-hemodynamic response was modulated parametrically by individuals’ beliefs about the appropriateness of congruent relative to incongruent sociopolitical violence in ventromedial prefrontal cortex, and by moral conviction in ventral striatum. Overall moral conviction was predicted by neural response to congruent relative to incongruent violence in amygdala. Together, these findings indicate that moral conviction about sociopolitical issues serves to increase their subjective value, overriding natural aversion to interpersonal harm. (shrink)

Neil Levy argues that while addicts who believe they are not addicts are self-deceived, addicts who believe they are addicts are just as self-deceived. Such persons accept a false belief that their addictive behaviour involves a loss of control. This paper examines two implications of Levy's discussion: that accurate self-knowledge may be particularly difficult for addicts; and that an addict's self-deceived belief that they cannot control themselves may aid their attempts at self-control. I argue that the self-deceived beliefs of addicts (...) in denial and of self-described addicts differ in kind. Unlike the self-deception of an addict in denial, that of the self-described addict allows them to acknowledge their behaviour. As such, it may aid an addict to develop more self-control. A paradoxical implication is that this self-deception may allow an addict more self-knowledge. (shrink)

An increasing number of patients receive diagnoses of disease without having any symptoms. These include diseases detected through screening programs, as incidental findings from unrelated investigations, or via routine checks of various biological variables like blood pressure or cholesterol. In this article, we draw on narrative identity theory to examine how the process of making sense of being diagnosed with asymptomatic disease can trigger certain overlooked forms of harm for patients. We show that the experience of asymptomatic disease can involve (...) ‘mismatches’ between one’s beliefs about one’s health status on the one hand, and bodily sensations or past experience on the other. Patients’ attempts to integrate these diagnoses into their self-narratives often involve either forming inaccurate beliefs about bodily sensations and/or past experience, or coming to believe that feelings and experience do not necessarily track or predict health status, leading to an ongoing sense of vulnerability to ill health. These resulting alterations in self-understanding can sometimes be considered harmful, in view of their implications for ascriptions of responsibility and ongoing anxiety. (shrink)

One of the most perceptive and ambidextrous social commentators of our day, Augustinian scholar Jean Bethke Elshtain furnishes in ever fresh ways through her writings a bridge between the ancient and the modern, between politics and ethics, between timeless moral wisdom and cultural sensitivity. To read Elshtain seriously is to take the study of culture as well as the "permanent things" seriously. But Elshtain is no mere moralist. Neither is she content solely to dwell in the domain of the (...) theoretical. For it is Elshtain the citizen - the creatively engaged and contributing citizen - whom the reader encounters on virtually every page of her writings. But reader beware: Elshtain does not shy away from controversy. At the same time, she is anything but a controversialist. In the essay that follows, several prominent themes that emerge from Elstain's writings - civic responsibility, justice, gender, and war - are considered afresh. Whether one agrees with her positions or not, one is forced to confess in the end that she cares deeply about the common good. Programs of drug testing welfare recipients are increasingly common in US states and have been considered elsewhere. Though often intensely debated, such programs are complicated to evaluate because their aims are ambiguous – aims like saving money may be in tension with aims like referring people to treatment. We assess such programs using a proportionality approach, which requires that for ethical acceptability a practice must be: reasonably likely to meet its aims, sufficiently important in purpose as to outweigh harms incurred, (...) and lower in costs than feasible alternatives. In the light of empirical findings, we argue that the programs fail the three requirements. Pursuing recreational drug users is not important in the light of costs incurred, while dependent users who may require referral are usually identifiable without testing and typically need a broader approach than one focussing on drugs. Drug testing of welfare recipients is therefore not ethically acceptable policy. (shrink)
