Our research extends the current understanding of cognitive moral reasoning research by considering the often‐overlooked element of context, specifically the issue presented in the ethical dilemma, and the issue of gender agreement between the subject and the dilemma actor. We rely on gender identity and cognitive moral reasoning theories to provide the theoretical underpinnings of our exploration to deepen our understanding of the contextual forces affecting cognitive moral reasoning. Our results generally confirm earlier research findings and provide (...) valuable information regarding our understanding of the impact of the dilemma context and the gender agreement or disagreement between the subject and the actor in an ethical dilemma, with significance found for four of our five hypotheses, with half of the significant results in the direction predicted. Telling the truth is one of the most respected virtues in medical history and one of the most emphasized in the code of medical ethics. Health care providers are frequently confronted with the dilemma as to whether or not to tell the truth. This dilemma deepens when both choices are critically vicious: The choice is no longer between “right and right” or “right and wrong,” it is between “wrong and wrong.” In the case presented and discussed in this (...) paper, a research team in Saudi Arabia unintentionally uncovered information regarding misattributed paternity. In such a situation and in the context of a tribal cultural system, what should the team do with this information? This case analysis demonstrates the joint application of ethical resources originating from within and outside the Saudi Arabian context. The article analyses the case based on the moral problems involved, relevant medical application, and the impact of such information in the Saudi tribal and Islamic domains. The most pertinent relevant values and secular debates on similar matters are discussed. Finally, the article aims to provide an Islamic dimension of family, fatherhood, and adultery. (shrink)

In their update to Intentionality All-Stars, Hutto and Satne claim that there is currently no satisfactory account for a naturalised conception of content. From this the pair suggest that we need to consider whether content is present in all aspects of intelligence, that is, whether it is content all the way down. Yet if we do not have an acceptable theory of content such a question seems out of place. It seems more appropriate to question whether content itself is the (...) problematic assumption. This paper will explore this question, with the analysis hinging on whether those trying to naturalise content are facing a tough challenge as Haugeland has suggested, or whether it is a genuine dilemma as Hutto has suggested elsewhere. If those attempting to naturalise content are facing a genuine dilemma, a dilemma in which the non-existence of content is one of the horns, then it would seem that the issue is not at what stage you posit content, but whether you should posit content at all. This also has the added benefit of shifting the debate to a more global discussion, that is, the best way to explain intelligence, period. (shrink)

The article is devoted to certain fundamental and discussed threads defined as dilemmas of political anthropology. Starting from specific rudimentary descriptions of human nature, the natural state or natural man, initiated by thinkers described by Barnard as “precursors of anthropology,” as well as referring to the problems of contemporary political philosophy, the papers aims to bring closer the issues concerning the fall of human and his “regeneration,” the “mask regime,” tensions between a human being and society, conflict and cooperation; dialogue (...) and antagonism. The proposed interpretations of the thoughts of Hobbes, Machiavelli, or Rousseau have the character of “retroactive reading.” It means that the references made to historical-philosophical examples activate the contexts of contemporary thought, or even give them new meanings, and at the same time trigger a thought that leans towards the future. Three inevitable realities instigate this study. Primarily, the goal of security in the region will most likely be accomplished just if as a minimum some level of cooperation is achieved between the major countries of the region. Second, Afghanistan cannot advance economically or enhance its security and administration independently without some cooperation from India and Pakistan. Third, although many strategists view the idea of Pak-India cooperation with distrust, there are many common grounds where both can gain considerable security, governance, and (...) economic advantages. Most of the existing studies are focused on the assumptions that how peace in the region will come. First view is about involvement of USA and European countries on one side and stake holders in Afghanistan on the other. Some believe in a trilateral pattern i.e. Western World, Afghan stake holders and Pakistan. Many also highlight the importance of support from neighboring countries like Iran, the Central Asian Republics, and China. This study highlights the significance of a trilateral solution in Afghanistan and the steps forward from this particular aspect. The possible answer to security dilemma of Pakistan in the context of Afghanistan is a solution within the region. (shrink)

Background:Nurses are in a key position for reproduction health service delivery. Therefore, it is thought that it would be important to inspect opinions of student nurses, who will be health employees in the future, about self-induced abortion to develop women health and public health.Objectives:The goal of this study is to inspect opinions of nursing students with different sociocultural specialties, about self-induced abortions.Research design:It is qualitative type and planned with ethnographic research pattern.Participants and research context:The study was conducted with 20 (...) last-term students of Kırşehir Ahi Evran University, Faculty of Health Sciences, Department of Nursing, who were chosen by maximum diversity sampling technique. Interviews were made with semi-structured interview form and voice records during the study; data were analyzed with content analyzing method.Ethical considerations:Permission from the organization, university ethics committee, and personal approvals were taken from participants to conduct the research.Findings:Based on the analysis, two major themes on self-induced abortion in Turkish nursing students were found: dilemma of traditional perspective and professional identity and occupational awareness.Discussion and conclusion:It is seen that there is a dilemma between traditional point of view and professional identities about self-induced abortion for nursing students, but they had occupational awareness. It should be recommended to give information about national and international licit legislations for reproductive health, self-induced abortion, and setting up ethical discussion environments for nursing students. Much has been written about the ethics and values of today's business student, but this research has generally been characterized by a variety of methodological shortcomings — the use of convenience samples, a failure to establish the relevance of comparison groups employed, attempts to understand behavior in terms of unidimensional values preselected by the researcher, and the lack of well-designed longitudinal studies. The research reported here addresses many of these concerns by comparing the values and ethical decision making behavior of (...) a large cohort of students entering an M. B. A. program to students entering law school. Using the Rokeach value survey and several ethical decision making vignettes, significant differences were found between the two groups which have important implications for both the business and legal professions and the education of their future leaders. The first half of the 17th century was a time of intellectual ferment when wars of natural philosophy were echoes of religious wars, as we illustrate by a case study of an apparently innocuous mathematical technique called adequality pioneered by the honorable judge Pierre de Fermat, its relation to indivisibles, as well as to other hocus-pocus. André Weil noted that simple applications of adequality involving polynomials can be treated purely algebraically but more general problems like the cycloid curve cannot be (...) so treated and involve additional tools–leading the mathematician Fermat potentially into troubled waters. Breger attacks Tannery for tampering with Fermat’s manuscript but it is Breger who tampers with Fermat’s procedure by moving all terms to the left-hand side so as to accord better with Breger’s own interpretation emphasizing the double root idea. We provide modern proxies for Fermat’s procedures in terms of relations of infinite proximity as well as the standard part function. (shrink)

DeRose famously argued that, given that assertability varies with practical context, we cannot combine a biconditional knowledge norm of assertion with classical invariantism. The first aim of this paper is to show that De Rose's argument is ultimately unsuccessful. Second, I develop a view, entitled Assertion Functionalism, which combines the knowledge norm with classical invariantism and at the same time offers an appealing account of the intuitive variability of proper assertion.

This article relates the evolving relationship between republicanism and the problem of ‘empire’ to the changing social contexts within which republican political theory emerges in the early modern period. It is argued that the initial antagonism between republicanism and empire was a politically constituted dilemma that related to the specific configuration of economic and political power characteristic of pre-capitalist societies. With the development of capitalism in England in the early modern period, the problem of empire becomes partially resolved due (...) to the way in which the separation of economic and political power under capitalism reconstitutes the nature of empire itself. This new social context — characterized by new social, economic and political relationships specific to an emerging capitalist context — laid the foundations for the resolution of the ‘republican dilemma’ of empire and the ideological establishment of the first ‘Republican Empire’ in 18th century America. The order of influence from thesis to hypothesis, and from philosophy to the social sciences, has historically governed the way in which the abstraction and significance of language as an empirical object is determined. In this article, an argument is made for the development of a more reflexive intellectual relationship between ordinary language philosophy and the social sciences that it helped inspire. It is demonstrated that, and how, the social scientific traditions of ethnomethodology and conversation analysis press OLP to re-consider (...) the variety of problematic abstractions it has previously made for the sake of philosophical clarity, thereby self-reinvigorating. The study presented in this article is based on field observations over one year on a critical care unit in Norway. Data were analysed according to Glaser’s grounded theory and generated a theory of hidden and emerging drama in the context of ambiguity while the nurses routinized the handling of complex technology. To the untrained eye the unit presented a picture of calm competence, while under the surface one finds hidden drama full of difficult interacting clinical and ethical problems. (...) The nurses identified six ethical dilemmas: (1) end of life issues; (2) whether there should be an age limit for coronary surgery; (3) distributing limited resources; (4) resource allocation in terms of better staffing; (5) situations in which is it more harmful than beneficial to continue treatment; and (6) transferring patients to other facilities. The six problems overlap and have clinical as well as ethical components, but it was the ethical dimensions that the nurses identified and discussed. The significance for nursing is the importance of well-educated and well-qualified nurses and how they find viable solutions to complex ethical and clinical problems. (shrink)

This article examines how coronavirus disease 2019 impacted on religion–state relations in South Africa. By examining the models of religion–state relations the article highlights the implications of these relations in the context of the South African government’s national response to COVID-19 and critically engages a public theology of ‘immense trust and authority’ assigned to leaders. The article identifies ‘separation with interaction’ as a model, which informs the South African government’s state–religion engagement. Although this model is constitutional and promotes religious (...) freedom, the article identifies the government’s failure to act decisively on religious leaders who exploit the poor as a major obstacle to socio-economic and religious transformation. The article contends that the dark part of South African history presents a dilemma to church–state relations in South Africa and suggests that life-affirming practices of political and religious leaders should be tested through the values of goodness, kindness, justice and obedience as a demonstration that they are essential workers who have an important transformational role to play in the context of COVID-19.Contribution: This article represents a systematic and practical reflection within a paradigm in which the intersection of philosophy, religious studies, social sciences, humanities and natural sciences generates an interdisciplinary, multidisciplinary and transdisciplinary contested discourse. OBJECTIVES: To identify ethical dilemmas experienced by occupational and physical therapists working in the UK National Health Service (NHS). To compare ethical contexts, themes and principles across the two groups. DESIGN: A structured questionnaire was circulated to the managers of occupational and physical therapy services in England and Wales. SUBJECTS: The questionnaires were given to 238 occupational and 249 physical therapists who conformed to set criteria. RESULTS: Ethical dilemmas experienced during the previous six months were reported by 118 occupational and (...) 107 physical therapists. The two groups were similar in age, grade, and years of experience. Fifty of the occupational therapy dilemmas occurred in mental health settings but no equivalent setting emerged for physical therapy. Different ethical themes emerged between the two groups, with the most common in occupational therapy being difficult/dangerous behaviour in patients and unprofessional staff behaviour, and for physical therapists resource limitations and treatment effectiveness. No differences were found in the ethical principles used. CONCLUSION: The ethical dilemmas reported by the therapists were primarily concerned with health care ethics, rather than the more dramatic ethics reported in much of the biomedical ethics literature. Differences were found between the two professional groups when ethical contexts and themes were compared but not when ethical principles were compared. This suggests that educators and researchers need to be aware of work settings and the interdisciplinary nature of employment as well as ethical principles held by individual therapists. (shrink)

This article addresses dilemmas of agency for feminism through reflections on social psychological research on the role of representations in the construction of identity by Muslim women. Engaging first with Saba Mahmood’s account of religious subjectivities in Politics of Piety, the author argues that feminist research requires a social conception of agency that addresses dialogical dynamics of representation and identity. Drawing on research concerning veiling and identity among Muslim women in the UK and Denmark, the author shows how a social (...) conception of agency may be elaborated through the analysis of how competing representations of gender are negotiated in the self–other encounter. The author argues that a dialogical approach to researching identity in context defies blunt political judgement while shedding light on the intersecting socio-cultural forces and relations of power that engage processes of resistance and re-presentation. It draws attention to the original characteristics that made up the idea and essence of university, which should not be forgotten at a time when higher education institutions are being diversified. It analyzes the development of higher education in terms of its functions and value in the context of societal change in the era of industrialization and (...) globalization. It contemplates the university’s loss of identity at a time when financial capital prevails over science and technology, and educational preferences favor the market and practice. It considers paradigmatic change in university education as well as the future role of universities. The gamer’s dilemma offers three plausible but jointly inconsistent premises: (1) Virtual murder in video games is morally permissible. (2) Virtual paedophelia in video games is not morally permissible. (3) There is no morally relevant difference between virtual murder and virtual paedophelia in video games. In this paper I argue that the gamer’s dilemma can be understood as one of three distinct dilemmas, depending on how we understand two key ideas in Morgan Luck’s (2009) original formulation. The two (...) ideas are those of (1) occurring in a video game and (2) being a virtual instance of murder or paedophelia. Depending on the weight placed on the gaming context, the dilemma is either about in-game acts or virtual acts. And depending on the type of virtual acts we have in mind, the dilemma is either about virtual representations or virtual partial reproductions of murder and paedophelia. This gives us three dilemmas worth resolving: a gaming dilemma, a representation dilemma, and a simulation dilemma. I argue that these dilemmas are about different issues, apply to different cases, and are susceptible to different solutions. I also consider how different participants in the debate have interpreted the dilemma in one or more of these three ways. (shrink)

Background Recently, palliative care is increasingly important, with an emphasis on the process of dying with dignity. However, nurses who care for such patients experience the associated ethical dilemmas. Objective To explore the meaning of nurses’ experiences in dealing with ethical dilemmas in relation to palliative sedation. Research design A qualitative research design was employed with a thematic analysis approach. Participants and research context Using purposive sampling, 15 nurses, working at palliative care units for at least 1 year, were (...) recruited as participants. Data were collected using unstructured in-depth interviews, and data collection and analysis was performed simultaneously. Ethical considerations Ethical approval was obtained from the authors’ institutional review board. All participants provided informed consent. For the face-to-face interview, the South Korean standard COVID-19 quarantine guidelines, such as mandatory masking and social distancing, were followed. Results Dilemmas raised by patients, were related to concerns about appropriate drug dose; dilemmas raised by nurses, were related to passive care, sense of guilt for failure to predict death, and colleague’s disrespectful attitudes toward patients; dilemmas from patients’ families were related to demands for palliative sedation and reversal of those demands. Care actions to deal with ethical dilemmas comprised evidence-based care, person-centered thinking, reflecting on the death situation, compassion, providing explanation and help to family members. Conclusion Nurses’ ethical dilemmas were pre-dominantly influenced by themselves, rather than by the patients or their families, especially if they felt they could not do their best for patients. The core concept of care actions to deal with the ethical dilemmas, was person-centered care and compassion. Then, how patients and their family members perceive person-centered care and compassion, should be further explored to improve palliative sedation. (shrink)

Undergraduate social work students are exposed to ethical and legal dilemmas during their fieldwork training. This article presents a study that examined these ethical dilemmas in an Israeli sample of undergraduate social work students. 117 students who participated in a course in ethics submitted 31 written presentations of ethical-dilemma analysis. Their oral presentations were recorded and transcribed. Using a qualitative analysis, three major themes emerged: 1. The tension between the duty to maintain client's confidentiality and its violation under certain (...) conditions; 2. The students’ conflict of loyalty toward the client vs. the organization; 3. The question of the clients’ right to self-determination when, in the student's view, this is against their best interests.Our findings point to the vital role of an ethics course in the social work undergraduate curriculum that can enhance students’ knowledge of specific laws relevant to the limits of confidentiality in social work and their ability to analyze ethical dilemmas as future professionals. Furthermore, it is essential to raise awareness in educators and supervisors concerning moral distress that may afflict students coping with ethical dilemmas at this early professional developmental phase. ObjectivesIn 2015, the Province of Quebec, Canada passed a law that allowed voluntary active euthanasia. Palliative care stakeholders in Canada have been largely opposed to euthanasia, yet there is little research about their views. The research question guiding this study was the following: How do palliative care physicians in Quebec position themselves regarding the practice of VAE in the context of the new provincial legislation?MethodsWe used interpretive description, an inductive methodology to answer research questions about clinical practice. A total (...) of 18 palliative care physicians participated in semistructured interviews at two university-affiliated hospitals in Quebec.ResultsParticipants positioned themselves in opposition to euthanasia. Their justifications were framed within their professional commitment to not hasten death, which sat in tension with the value of patients’ autonomy to choose how to die. Participants described VAE as unacceptable if it impeded opportunities to evaluate and alleviate suffering. Further, they contested government rhetoric that positioned VAE as a way to improve end-of-life care. Participants felt that VAE would diminish the potential of palliative care to relieve suffering. Dilemmas were apparent in their narratives, about reconciling respect for patient autonomy with broader palliative care values, and the value of accompanying and not abandoning patients who make requests for VAE while being committed to neither prolonging nor hastening death.ConclusionsThis study provides insight into nuanced positions of experienced palliative care physicians in Quebec and confirms expected tensions between an important stakeholder and the practice of VAE as guided by the new legislation. This article examines conceptualisations of violence against women developed in Brazilian feminism, and in legal and institutional measures against violence, from the 1980s to the present. Based on ethnographic studies carried out at the Women’s Police Stations and Special Criminal Courts, and the controversies surrounding the 2006 Brazilian Law on domestic and familial violence, the authors map the meanings of expressions such as ‘violence against women’, ‘marital violence’, ‘domestic violence’, ‘family violence’ and ‘gender violence’. The article reveals that the discourse (...) that transforms violence into crime, in the Brazilian context, leads to semantic and institutional developments that replace an interest in politicising justice for the defence of women with the judicialisation of family relations. The rise of social-media-mediated celebrity culture raises several philosophical concerns. Therefore, it is not uncommon to see, for example, Hollywood actors being placed in the same bracket as YouTube artists and Instagram influencers. The increased perceived ‘connectivity’ afforded by social media allows online celebrities to reach more fans and increases the perceived engagement or intimacy in the fan-celebrity relationship. In this paper I argue that this online relationship, which is beneficial to celebrities (for brand development) and social media companies (in (...) profiting from the high engagement), has the potential to cause harm to both the celebrities as well as their fanbases (and sometimes innocent third parties). Firstly, I argue that celebrities’ attempts to be more intimate or authentic with their fanbase can turn out to be – more often than not – acts of Sartrean bad faith. Celebrities who tend to project a certain self-styled ‘online authenticity’, given the context of social media’s attention economy, can run the risk of treating a fan as a means-to-an-end, rather than an autonomous for-itself. Secondly, I argue that this phenomenon can catalyse real-world harms, by drawing upon network theory and social psychology. I argue that the connectedness of social media, ‘blending into the crowd’, and social media technologies’ ethos of optimising-for-engagement, leads to real harms to human well-being when fans do not engage critically and mindfully with online celebrities. Examples include Trump-esque celebrity politicians trying to drum up populist influence; celebrities feuding online encouraging fans’ conduct of harmful trolling and harassment; and vilification of those who do not subscribe to the same perceived worldview of a celebrity’s fandom. Following my arguments within, I shall outline an online celebrity’s ethical duties in their social media engagement, with emphasis on the duties that celebrity status comes with, particularly in preventing harms that transcend the online into the offline. Gaining ethical approval for qualitative health research and implementing all the planned research processes in a proposed study are not straightforward endeavours. The situation becomes more complex when qualitative research is conducted in a cross-national healthcare and academic context. Also, it is even exhausting when the study is student-based, as student researchers may be considered novices and inexperienced researchers, especially for field-based research. Our aim in this reflective paper is to present, reflect, and discuss the experiences of a doctoral (...) researcher in dealing with two independent institutional review boards in Canada and Ghana during an interdisciplinary Ph.D. project and the ethical dilemmas encountered while collecting data in Ghana. Based on the researcher’s experiences, it became apparent that consent and its documentation can have cultural implications in different settings; hence, institutional review boards must exercise reflexivity in their protocol review practice. Also, sharing research data with participants and institutional leaders while maintaining participant confidentiality and privacy in institutional ethnographic research requires sensitivity to bi-lateral ethical values. With the experiences shared in this paper, we advocate for a dialogic ethical review process in qualitative research where researchers and research ethics boards engage in ongoing dialogue rather than the usual prescriptive format research ethics reviews often assume. In this paper, we are interested in the effects of institutional context on public attitudes towards climate policies, where institutions are defined as the conventions, norms and formally sanctioned rules of any given society. Building on a 2014 survey experiment, we conducted thirty qualitative interviews with car-owners in Oslo, Norway, to investigate the ways in which institutional context and political-value orientation affect public attitudes towards emissions policies. One context (presented as a text treatment) highlighted individual rationality, emphasising (...) the ways in which local pollution impacts the individual citizen; the other highlighted social rationality, emphasising the wider significance of carbon emissions and global responsibility for climate change. We analysed the effects of these contexts on attitudes, finding that institutional context influenced individuals' perspectives as well as their attitudes towards climate policies. Groups with different value orientations differed in terms of their evaluations but not their interpretations of these contexts. (shrink)

What on earth are economic theorists like me trying to accomplish? This paper discusses four dilemmas encountered by an economic theorist: The dilemma of absurd conclusions: Should we abandon a model if it produces absurd conclusions or should we regard a model as a very limited set of assumptions that will inevitably fail in some contexts? The dilemma of responding to evidence: Should our models be judged according to experimental results? Respect for patient autonomy can apply at two levels: ensuring that patient care reflects their considered values and wishes and honoring patient preferences about how to make momentous decisions. Caregivers who seek to respect patient autonomy in the context of some end-of-life decisions face a dilemma. Because these decisions are fraught, patients may prefer to approach them sequentially, only making decisions at the time they arise. However, respecting patients’ preferences for a sequential approach can increase the likelihood that (...) surrogates and care teams wind up in situations in which they lack information needed to ensure patients receive care that conforms to their considered values after they are no longer competent to make decisions for themselves. Sequential decision-making can thus conflict with the goal of ensuring care reflects the wishes of patients. After illustrating how this dilemma can arise in the use of life-sustaining “bridge” technologies, we argue that care teams may be warranted in requiring patients to articulate their wishes in an advance care plan before treatment begins. In some cases, care teams may even be permitted to refuse to undertake certain courses of care, unless patients articulate their wishes in an advance care plan. (shrink)

This paper argues that for moral degradation to occur, it necessarily involves one’s commissions or omissions that transgresses one’s personal morality, and hence, distinguishes sufferers of moral injury from PTSD sufferers who were witnesses to traumatic and morally abhorrent events. To this end, it clarifies how some of the situations surrounding moral injury are misunderstood, (...) by discussing the process of moral reasoning in the context of moral dilemmas, dirty hands, and moral blind alleys. Finally, it concludes that when we conceptualise moral injury as being caused by one’s commissions and omissions in moral dilemmas, we find that shame and guilt are situation-appropriate responses with a role to play in what ethics mean. Finally, it concludes that when we conceptualise moral injury as being caused by one’s commissions and omissions in moral dilemmas, we find that shame and guilt are situation-appropriate responses with a role to play in what ethics mean. The article charts my trajectories as a feminist activist/academic seeking to research, write and talk about gender-based violence in relation to the Middle East. More specifically, I am drawing on research and activism in relation to Iraq, Turkey and Lebanon to map the discursive, political and empirical challenges and complexities linked to scholarship and activism that is grounded in both feminist and anti-racist/anti-islamophobic politics. Given that care duties are central to the definition of motherhood across contexts, an extended separation from the woman’s family due to migration presents a major threat to her social identity as a mother and wife. Drawing on West and Zimmerman’s notion of “doing gender” and ethnographic research on Vietnamese low-waged contract workers in Taiwan, I provide vital insights into the discursive processes and everyday practices that underlie migrant women’s negotiations of motherhood and femininity. Specifically, I examine the various ways (...) migrant women perform and negotiate meanings of hy sinh and chịu đựng that are core values of Vietnamese womanhood. Combating the stigma of bad motherhood and failed femininity, I emphasize, is not just about reasserting one’s sense of gendered self but also about reassuring her access to the future support and care of the family. This article presents an interpretive phenomenological study that explores how physicians experience dilemmas of conscience in their day-to-day practice. Eighteen physicians of various ages and professional backgrounds were interviewed and asked to identify and discuss three instances when they experienced a dilemma of conscience. Preliminary findings from narrative analyses of these physician interviews suggest that dilemmas of conscience are ubiquitous, temporal and context-dependent; they cannot be reduced and understood as a focal phenomenon. Moral development appears to parallel acquisition (...) of medical knowledge; participants' specific concerns and their clinical contexts evolved as they gained experience and insight. Participants learned how to negotiate dilemmas of conscience through time, mistakes, examples, actualization, rehearsals and struggles. Remaining engaged in care, developing partnerships and protecting spaces for dialogue can help create practices of accountability when dilemmas of conscience occur. (shrink)

Médecins Sans Frontières (MSF) is an independent medical humanitarian organisation working in over 70 countries. It has provided medical assistance for over 35 years to populations vulnerable through conflict, disease and inadequate health systems. Medical ethics define the starting point of the relationship between medical staff and patients. The ethics of humanitarian interventions and of research in conflict settings are much debated. However, less is known about the ethical dilemmas faced by medical humanitarian staff in their daily work. Ethical dilemmas (...) can be intensified in humanitarian contexts by insecure environments, lack of optimum care, language barriers, potentially heightened power discrepancies between care providers and patients, differing cultural values and perceptions of patients, communities and medical staff. Time constraints, stressful conditions and lack of familiarity with ethical frameworks can prevent reflection on these dilemmas, as can frustration that such reflection does not necessarily provide instant solutions. Lack of reflection, however, can be distressing for medical practitioners and can reduce the quality of care. Ethical reflection has a central role in MSF, and the organisation uses ethical frameworks to help with clinical and programmatic decisions as well as in deliberations over operational research. We illustrate and discuss some real ethical dilemmas facing MSF teams. Only by sharing and seeking guidance can MSF and similar actors make more thoughtful and appropriate decisions. Our aim in sharing these cases is to invite discussion and dialogue in the wider medical community working in crisis, conflict or with severe resource limitations. (shrink)

Background:Caring for a partner with primary malignant brain tumour can be a dramatic life-changing event. Primary malignant brain tumour is known to give poor life expectancy and severe neurological and cognitive symptoms, such as changed behaviour and personality, which demand greater caring responsibilities from spouses.Aim:The aim of the study is to explore ethical dilemmas spouses experience in the everyday care of a partner in treatment for primary malignant brain tumour.Research design, participants and research context:A phenomenological and hermeneutic qualitative descriptive (...) design was adopted as a method for collecting and analysing data. Ten spouses were interviewed twice using an in-depth, semi-structured interview guide. The interviews took place at the spouses’ homes or at the hospital.Ethical consideration:Ethical matters were considered throughout the research process. Permission from The National Committee on Health Research Ethics and the Danish Data Protection Agency was obtained.Findings:The analysis showed that the spouses perceived daily ethical dilemmas in caring for a partner with primary malignant brain tumour. Their life as well as their partner’s life had changed considerably. The main theme that emerged therefore was ‘oscillating in a changing relationship’. This theme was further elaborated in three subthemes that in more detail demonstrated the dilemmas: ‘doing the right thing in unpredictable daily situations’; ‘torn between patience and guilt’; and ‘living in a time of uncertainty, hope and despair’.Conclusion:Caring for a partner with changed behaviour and personality due to primary malignant brain tumour may involve exhausting ethical caring dilemmas. Spouses’ married life may change to a semi-professional asymmetrical relationship, which is challenged by the oscillation between acting responsibly for their partners’ well-being and caring dilemmas with no answer for what the right thing to do is. Mixed feelings of right and wrong, patience and guilt, hope and despair seem to be spousal companions through their partners’ progressing illness. (shrink)

Here the collected papers explore the whole question of the relation between the mythopoetic and the moral in the context of the Mahabharata. Here we have a story of extreme complexity, characters that are unforgettable, and a cosmic context in which gods and men alike grapple with destiny. The obligations of kinship and friendship jostle with each other. The women characters, as in everyday life, seem to bear a very heavy load of the burden of life and to (...) stand in a key position in almost every conflict. We are presented with predicaments at every turn. At times these predicaments seem to be aggravated by social structure. At other times they are cushioned by it. Philosophical tangles tied up with karma and dharma are interwoven with the mythopoetic material. Perhaps philosophical issues are pinpointed rather more than they are in Greek epic literature. The essays in this book treat the Mahabharata from an unusual angle, fastening on the moral dilemmas it presents. How universal are the dilemmas faced by the characters in the story, and are the dilemmas in fact resolved? In dealing with these questions, the discussions range over the meaning of the purusarthas, the institutions of marriage and the family, the concept of action in the Gita and the special predicaments faced by Draupadi, Arjuna and others. These studies invite the scholar to reflect afresh on the text and encourage the general reader to find in epic literature much that is relevant to life today. (shrink)

In this article we discuss the ethical dilemmas facing performance evaluators and the "evaluatees" whose performances are measured in a business context. The concepts of role morality and common morality are used to develop a framework of behaviors that are normally seen as the moral responsibilities of these actors. This framework is used to analyze, based on four empirical situations, why the implementation of a performance measurement system has not been as effective as expected. It was concluded that, in (...) these four cases, unethical behavior (i.e. deviations from the ethical behaviors identified in the framework) provided, at least to some extent, an explanation for the lower than expected effectiveness of the performance measurement procedures. At the end of the paper we present an agenda for further research through which the framework could be further developed and systematically applied to a broader set of cases. (shrink)

Dilemma discussions have been proven to be one of the most effective methods to enhance students’ moral reasoning in ethics education. Dilemma discussions are increasingly arranged online, but research on the topic has remained sparse, especially in the context of continuing professional education. The aim of the present paper was to develop a method of dilemma discussions for professional ethics. The method was based on asynchronous discussions in small groups. Health and social care students raised work-related (...) dilemmas from their experiences and discussed them in terms of professional values, ethical guidelines and theories. Participants in this quasi-experimental study were 87 first-term graduate students at a Finnish university of applied sciences. Health and social care students in two consecutive ethics courses constituted two experiment groups, whereas health and social care students and business students in other programmes served as control groups. Students filled in a Defining Issues Test (DIT2) at the beginning of their studies and three months apart. Statically significant increase in moral reasoning was evidenced for experiment group 2, when discussion groups were purportedly composed to maximise differences in initial levels of moral reasoning. Findings suggest that online dilemma discussions can advance students’ moral reasoning development, especially when students’ exposure to higher-level arguments is ensured through complementary means, such as instructions, examples and plenary discussions. Online real-life dilemma discussions may also serve other important goals of ethics education, especially acquiring ethical concepts, and they can promote other components of ethical decision making: ethical sensitivity and motivation, and acquisition of implementation skills. Working as clinical ethicists in an academic hospital, we find that practitioners tend to take a principle‐based approach to moral dilemmas when it comes to (not) treating patients who feel like a burden, in which respect for autonomy tends to trump other principles. We argue that this approach insufficiently deals with the moral doubts of professionals with regard to feeling that you are a burden as a motive to decline or withdraw from treatment. Neither does it take into adequately account (...) the specific needs of the patient that might underlie their feeling of being a burden to others. We propose a care ethics approach as an alternative. It focuses on being attentive and responsive to the caring needs of those involved in the care process—which can be much more specific than either receiving or withdrawing from treatment. This approach considers these needs in the context of the patient's identity, biography and relationships, and regards autonomy as relational rather than as individual. We illustrate the difference between these two approaches by means of the case of Mrs K. Furthermore, we show that a care ethics approach is in line with interventions that are found to alleviate feeling a burden and maintain that facilitating moral case deliberation among practitioners can supports them in taking a care ethics approach to moral dilemmas in (not) treating patients who feel like a burden. The core of the paper consists of dialogue from a true case where an employee experienced moral dilemmas following a disquieting directive from his manager. The case is considered from the perspective of Dancy's particularistic theory of moral reasons. This case was chosen not to illustrate the theory, but rather to test the assumption that an approach to moral judgement based on Ross and Dancy has general applicability. It is suggested that, in its simplest form, that approach approximates to the (...) manner in which people in organisations, without prior knowledge of ethical theorising, would ordinarily deal with comparable situations, and so it can be relatively easy to learn given practice. Based on this case, some insight is also offered into individuals' moral learning, including their need for personal qualities such as assertiveness and independence of mind. In that context a reciprocal relationship between Dancy's approach and Werhane's thinking on moral imagination is suggested. (shrink)
