With the outbreak of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), global researchers were confronted with major challenges. The German National Pandemic Cohort Network (NAPKON) was launched in fall 2020 to effectively leverage resources and bundle research activities in the fight against the coronavirus disease 2019 (COVID-19) pandemic. We analyzed the setup phase of NAPKON as an example for multicenter studies in Germany, highlighting challenges and optimization potential in connecting 59 university and nonuniversity study sites. We examined the ethics application (...) process of 121 ethics submissions considering durations, annotations, and outcomes. Study site activation and recruitment processes were investigated and related to the incidence of SARS-CoV-2 infections. For all initial ethics applications, the median time to a positive ethics vote was less than two weeks and 30 of these study sites (65%) joined NAPKON within less than three weeks each. Electronic instead of postal ethics submission (9.5 days (Q1: 5.75, Q3: 17) vs. 14 days (Q1: 11, Q3: 26), p value = 0.01) and adoption of the primary ethics vote significantly accelerated the ethics application process. Each study center enrolled a median of 37 patients during the 14-month observation period, with large differences depending on the health sector. We found a positive correlation between recruitment performance and COVID-19 incidence as well as hospitalization incidence. Our analysis highlighted the challenges and opportunities of the federated system in Germany. Digital ethics application tools, adoption of a primary ethics vote and standardized formal requirements lead to harmonized and thus faster study initiation processes during a pandemic. (shrink)

Lassen sich dennoch angemessene Strategien des Umgangs entwickeln, die weder die prekare Faktizitat des Bosen noch seine historische Tiefendimension ausblenden? Ausgehend von dieser Leitfrage nahert sich der vorliegende, interdisziplinar breit angelegte Band dem vielschichtigen Phanomen des Bosen in praxeologischer Perspektive. Social epistemologists should be well-equipped to explain and evaluate the growing vulnerabilities associated with filter bubbles, echo chambers, and group polarization in social media. However, almost all social epistemology has been built for social contexts that involve merely a speaker-hearer dyad. Filter bubbles, echo chambers, and group polarization all presuppose much larger and more complex network structures. In this paper, we lay the groundwork for a properly social epistemology that gives the role and structure of networks their due. In particular, (...) we formally define epistemic constructs that quantify the structural epistemic position of each node within an interconnected network. We argue for the epistemic value of a structure that we call the (m,k)-observer. We then present empirical evidence that (m,k)-observers are rare in social media discussions of controversial topics, which suggests that people suffer from serious problems of epistemic vulnerability. We conclude by arguing that social epistemologists and computer scientists should work together to develop minimal interventions that improve the structure of epistemic networks. (shrink)

Spatial region concepts such as “front,” “back,” “left,” and “right” reflect our typical interaction with space, and the corresponding surrounding regions have different statuses in memory. We examined the representation of spatial directions in the auditory space, specifically in how far natural response actions, such as orientation movements toward a sound source, would affect the categorization of egocentric auditory space. While standing in the middle of a circle with 16 loudspeakers, participants were presented acoustic stimuli coming from the loudspeakers in (...) randomized order, and verbally described their directions by using the concept labels “front,” “back,” “left,” “right,” “front-right,” “front-left,” “back-right,” and “back-left.” Response actions varied in three blocked conditions: (1) facing front, (2) turning the head and upper body to face the stimulus, and (3) turning the head and upper body plus pointing with the hand and outstretched arm toward the stimulus. In addition to a protocol of the verbal utterances, motion capture and video recording generated a detailed corpus for subsequent analysis of the participants’ behavior. Chi-square tests revealed an effect of response condition for directions within the left and right sides. We conclude that movement-based response actions influence the representation of auditory space, especially within the sides’ regions. Moreover, the representation of auditory space favors the front and the back regions in terms of resolution, which is possibly related to the physiological characteristics of the human auditory system, as well as to the ecological requirements of action control in the different regions. The use of placebos in randomised controlled trials is a subject of considerable ethical debate. In this paper we present a set of considerations to evaluate the ethics of placebo controlled trials that includes: social value of the study; need for a randomised controlled trial and placebo; standards of care; risks of harm due to administration of placebo and the harm benefit balance; clinical equipoise; and double standards. We illustrate the application of these considerations using a case study of a (...) large ongoing multicentre, placebo-controlled, double-blinded, randomised trial to determine primaquine anti-relapse efficacy in vivax malaria. There is an urgent need for primaquine anti-relapse studies in order to rationalise the management of a potentially fatal disease. An ethical justification for the use of the placebo arm is provided on the grounds that the actual current applied standard of care in most endemic places does not include primaquine. It has also been argued that there is clinical equipoise among the primaquine study arms and that the risk of harms of being in the placebo arm is the risk of having relapse, which is no more than not being included in the trial, and that there are no double standards. Based on our set of considerations, we conclude that a placebo arm is not only justified but imperative in this study. We propose that similar considerations should be prospectively applied to other placebo controlled trials and observational control arms where no treatment is offered. (shrink)

Die demografische Entwicklung und der medizinische Fortschritt werden die Problematik der Ressourcenknappheit im Gesundheitswesen in Zukunft weiter verschärfen. Auch Deutschland steht eine Prioritätensetzung im Gesundheitswesen bevor. Diese sollte in möglichst transparenter Weise nach klaren Kriterien erfolgen. Ein nur selten in diesem Kontext besprochenes Kriterium der Verteilung von Mitteln in der Gesundheitsversorgung ist die marginale Wirksamkeit medizinischer Leistungen. Im vorliegenden Beitrag wird dieses Kriterium vorgestellt und auf seine Fairness hin untersucht. Nach der kritischen Diskussion einiger Argumente gegen den offenen Einsatz von (...) Hendrik Lorenz presents a comprehensive study of Plato's and Aristotle's conceptions of non-rational desire. They see this as something that humans share with animals, and which aims primarily at the pleasures of food, drink, and sex. Lorenz explores the cognitive resources that both philosophers make available for the explanation of such desires, and what they take rationality to add to the motivational structure of human beings. Mhealth technology is mushrooming world-wide and, in a variety of forms, reaches increasing numbers of users in ever-widening contexts and virtually independent from standard medical evidence assessment. Yet, debate on the broader societal impact including in particular mapping and classification of ethical issues raised has been limited. This article, as part of an ongoing empirically informed ethical research project, provides an overview of ethical issues of mhealth applications with a specific focus on implications on autonomy as a key notion in (...) the debate. A multi-stage model of references to the potential of mhealth use for strengthening some or other form of self-determination will be proposed as a descriptive tool. It illustrates an assumed continuum of enhanced autonomy via mhealth broadly conceived: from patient to user autonomy, to improved health literacy, and finally to the vision of supra-individual empowerment and democratised, participatory health and medicine as a whole. On closer examination, however, these references are frequently ambivalent or vague, perpetuating the at times uncritical use of established autonomy concepts in medical ethics. The article suggests zooming in on the range of autonomy-related aspects against the backdrop of digital innovation and datafied health more generally, and on this basis add to existing frameworks for the ethical evaluation of mhealth more specifically. Their relation, however, is one of considerable complexity. Levinas' philosophical project stems directly from his situation as a Jewish thinker in the twentieth century and takes its particular form from his study of the Torah and the Talmud. It is, indeed, a hermeneutics of biblical experience. If inspired by Judaism, Levinas' ethics are not eo ipso confessional. What his ethics takes from Judaism, rather, is a particular way of conceiving transcendence and the other human being. It owes to the philosophy of Franz Rosenzweig and Martin Buber a logos of the world and of the holy, which acknowledges their incom mensurability without positing one as fallen and the other as supernal. The term ‘locked-in’ syndrome (LIS) describes a medical condition in which persons concerned are severely paralyzed and at the same time fully conscious and awake. The resulting anarthria makes it impossible for these patients to naturally communicate, which results in diagnostic as well as serious practical and ethical problems. Therefore, developing alternative, muscle-independent communication means is of prime importance. Such communication means can be realized via brain–computer interfaces (BCIs) circumventing the muscular system by using brain signals associated with preserved cognitive, (...) sensory, and emotional brain functions. Primarily, BCIs based on electrophysiological measures have been developed and applied with remarkable success. Recently, also blood flow–based neuroimaging methods, such as functional magnetic resonance imaging (fMRI) and functional near-infrared spectroscopy (fNIRS), have been explored in this context. After reviewing recent literature on the development of especially hemodynamically based BCIs, we introduce a highly reliable and easy-to-apply communication procedure that enables untrained participants to motor-independently and relatively effortlessly answer multiple-choice questions based on intentionally generated single-trial fMRI signals that can be decoded online. Our technique takes advantage of the participants’ capability to voluntarily influence certain spatio-temporal aspects of the blood oxygenation level–dependent (BOLD) signal: source location (by using different mental tasks), signal onset and offset. We show that healthy participants are capable of hemodynamically encoding at least four distinct information units on a single-trial level without extensive pretraining and with little effort. Moreover, realtime data analysis based on simple multi-filter correlations allows for automated answer decoding with a high accuracy (94.9%) demonstrating the robustness of the presented method.. Beauchamp and Childress’ definition of autonomous decision‐making includes the conditions of intentionality, understanding, and non‐control. In genetics, however, a relational conception of autonomy has been increasingly recognized. This article aims to empirically assess aspects of social influence in genetic testing decision‐making and to connect these with principlist and relational theories of autonomy. We interviewed 18 adult genetic counsellees without capacity issues considering predictive genetic testing for cancer predisposition for themselves and two counselling physicians in Switzerland. We conducted a qualitative analysis, (...) building on a grounded theory study about predictive genetic testing decision‐making. We found that some participants agreed to predictive genetic testing predominantly because relatives wanted them to do it, with some even acting contrary to their own convictions. Others, in contrast, based their decision on purely individualistic reasons but expressed difficulties in explaining their decision to their social environment. Healthcare professionals had a critical influence on decision‐making in many cases without being manipulative, as perceived by counsellees. Still, cases of coercion and social pressure occurred within social relationships. In conclusion, predictive genetic testing decision‐making includes relational and individualistic aspects, and both are compatible with autonomous decision‐making. While the principlist and relational notions of autonomy compete on a theoretical level, they are two sides of the same coin when used as analytical lenses for genetic testing decision‐making. Social acceptance of refusal of testing should be improved to mitigate social pressure. Individuals should be encouraged to decide for themselves how much their social environment influences their decision regarding predictive genetic testing. (shrink)

This paper studies the Lockean thesis from the perspective of contemporary epistemic logic. The Lockean thesis states that belief can be defined as ‘sufficiently high degree of belief’. Its main problem is that it gives rise to a notion of belief which is not closed under conjunction. This problem is typical for classical epistemic logic: it is single-agent and static. I argue that from the perspective of contemporary epistemic logic, the Lockean thesis fares much better. I briefly mention that it (...) can successfully be extended from single-agent to multi-agent settings. More importantly, I show that accepting the Lockean thesis (and a more sophisticated version for conditional beliefs) leads to a significant and unexpected unification in the dynamic behavior of (conditional) belief and high (conditional) probability with respect to public announcements. This constitutes a methodological argument in favor of the Lockean thesis. Furthermore, if one accepts Baltag’s Erlangen program for epistemology, this technical observation has even stronger philosophical implications: because belief and high probability display the same dynamic behavior, it is plausible that they are indeed one and the same epistemic notion. In recent years, a number of authors have started studying Aristotelian diagrams containing metalogical notions, such as tautology, contradiction, satisfiability, contingency, strong and weak interpretations of contrariety, etc. The present paper is a contribution to this line of research, and its main aims are both to extend and to deepen our understanding of metalogical diagrams. As for extensions, we not only study several metalogical decorations of larger and less widely known Aristotelian diagrams, but also consider metalogical decorations of another type (...) of logical diagrams, viz. duality diagrams. At a more fundamental level, we present a unifying perspective which sheds new light on the connections between new and existing metalogical diagrams, as well as between object- and metalogical diagrams. In this paper, we consider certain cardinals in ZF (set theory without AC, the axiom of choice). In ZFC (set theory with AC), given any cardinals C and D, either C ≤ D or D ≤ C. However, in ZF this is no longer so. For a given infinite set A consider $\operatorname{seq}^{1 - 1}(A)$ , the set of all sequences of A without repetition. We compare $|\operatorname{seq}^{1 - 1}(A)|$ , the cardinality of this set, to |P(A)|, the cardinality of (...) the power set of A. What is provable about these two cardinals in ZF? The main result of this paper is that $ZF \vdash \forall A(|\operatorname{seq}^{1 - 1}(A)| \neq|\mathscr{P}(\mathscr{A})|)$ , and we show that this is the best possible result. Furthermore, it is provable in ZF that if B is an infinite set, then $|\operatorname{fin}(B)| <|\mathscr{P}(B)|$ even though the existence for some infinite set B* of a function f from $\operatorname{fin}(B^\ast)$ onto P(B*) is consistent with ZF. Ancient philosophical theories of soul are in many respects sensitive to ways of speaking and thinking about the soul psuchê] that are not specifically philosophical or theoretical. We therefore begin with what the word ‘soul’ meant to speakers of Classical Greek, and what it would have been natural to think about and associate with the soul. We then turn to various Presocratic thinkers, and to the philosophical theories that are our primary concern, those of Plato (first in the Phaedo, then (...) in the Republic), Aristotle (in the De Anima or On the Soul ), Epicurus, and the Stoics. These are by far the most carefully worked out theories of soul in ancient philosophy. Later theoretical developments — for instance, in the writings of Plotinus and other Platonists, as well as the Church Fathers — are best studied against the background of the classical theories, from which, in large part, they derive. This paper explores the interplay between logic-sensitivity and bitstring semantics in the square of opposition. Bitstring semantics is a combinatorial technique for representing the formulas that appear in a logical diagram, while logic-sensitivity entails that such a diagram may depend, not only on the formulas involved, but also on the logic with respect to which they are interpreted. These two topics have already been studied extensively in logical geometry, and are thus well-understood by themselves. However, the precise details of their (...) interplay turn out to be far more complicated. In particular, the paper describes an elegant and natural interaction between bitstrings and logic-sensitivity, which makes perfect sense when bitstrings are viewed as purely combinatorial entities. However, when we view bitstrings as semantically meaningful entities (which is actually the standard perspective, cf. the term ‘bitstring semantics’!), this interaction does not seem to have a full and equally natural counterpart. The paper describes some attempts to address this situation, but all of them are ultimately found wanting. For now, it thus remains an open problem to capture this interaction between bitstrings and logic-sensitivity from a semantic (rather than merely a combinatorial) perspective. (shrink)

He was that rare beast, a brilliant scientist who could write beautifully. He did more than any other person to establish and popularize the study of how animals behave, receiving a Nobel Prize for his work. This paper studies John Buridan's octagons of opposition for the de re modal propositions and the propositions of unusual construction. Both Buridan himself and the secondary literature have emphasized the strong similarities between these two octagons (as well as a third one, for propositions with oblique terms). In this paper, I argue that the interconnection between both octagons is more subtle than has previously been thought: if we move beyond the Aristotelian relations, and also take Boolean considerations into account, then (...) the strong analogy between Buridan's octagons starts to break down. These differences in Boolean structure can already be discerned within the octagons themselves; on a more abstract level, they lead to these two octagons having different degrees of Boolean complexity (i.e. Boolean closures of different sizes). These results are obtained by means of bitstring analysis, which is one of the key tools from contemporary logical geometry. Finally, I argue that this historical investigation is directly relevant for the theoretical framework of logical geometry, and discuss how it helps us to address certain open questions in this framework. Goal-setting theory states that challenging, specific, and concrete goals (i.e., subordinate goals) are powerful motivators and boost performance in goal pursuit more than vague or abstract goals (i.e., superordinate goals). Goal-setting theory predominantly focuses on single, short-term goals and less on broad, long-term challenges. This review article extends goal-setting theory and argues that superordinate goals also fulfill a crucial role in motivating behavior, particularly when addressing broad, long-term challenges. The purpose of this article is to show theoretically that people pursue (...) long-term goals more successfully when they focus on subordinate and superordinate goals simultaneously than when they focus on either subordinate or superordinate goals alone.
