Background: The American Medical Association, the British Medical Association and the Canadian Medical Association have guidelines that specifically discourage physicians from self-prescribing or prescribing to family members, but only the BMA addresses informal prescription requests between colleagues. Objective: To examine the practices of paediatric providers regarding self-prescribing, curbsiding colleagues, and prescribing and refusing to prescribe to friends and family. Methods: 1086 paediatricians listed from the American Academy of Paediatrics 2007 web-based directory were surveyed. Results: 44% of eligible survey respondents returned (...) usable surveys. Almost half of respondents had prescribed for themselves. An equal number had informally requested a prescription from a colleague. Three-quarters stated they had been asked to prescribe a prescription drug for a first-degree or second-degree relative, and 51% had been asked by their spouse. Eighty-six per cent stated that they had refused to write a prescription on at least one occasion for a friend or family member. The following reasons “strongly influenced” their decision to refuse a prescription request: outside of provider’s expertise ; patient’s need for his or her own physician ; not medically indicated ; need for a physical examination. Conclusion: These data confirm that most physicians have engaged in self-prescribing or curbside requests for prescriptions. It can be argued that curbsiding is more morally problematic than self-prescribing because it implicates a third party, and should be discouraged regardless of whether the requester is a colleague, family member or friend. Developmental coordination disorder is a neurodevelopmental disorder that significantly impairs a child’s ability to learn motor skills and to perform everyday activities. The cause of DCD is unknown; however, evidence suggests that children with DCD have altered brain structure and function. While the cerebellum has been hypothesised to be involved in developmental coordination disorder, no studies have specifically examined cerebellar structure in this population. The purpose of our study was to examine cerebellar differences in children with DCD compared to typically-developing (...) children. Using voxel-based morphometry, we assessed cerebellar morphology in children 8–12 years of age. Forty-six children were investigated using high resolution T1-weighted images, which were then processed using the spatially unbiased atlas template of the cerebellum and brainstem toolbox for a region of interest-based examination of the cerebellum. Results revealed that children with DCD had reduced grey matter volume in several regions, namely: the brainstem, right/left crus I, right crus II, left VI, right VIIb, and right VIIIa lobules. Further, Pearson correlations revealed significant positive associations between the total motor percentile score on the Movement Assessment Battery for Children-2 and regions that had reduced grey matter volume in our cohort. These findings indicate that reductions in cerebellar grey matter volume are associated with poorer motor skills. Given the cerebellum’s involvement in internal models of movement, results of this study may help to explain why children with DCD struggle to learn motor skills. (shrink)

Medical assistance in dying was legalized in Canada in 2016. As of July 2017, approximately 2149 patients have accessed MAID. There remains no national-level data on the perspectives of future physicians about MAID or its changing legal status. We provide evidence from a national survey of Canadian medical students about their opinions, intentions, and concerns about MAID. From October 2016 to July 2017, we distributed an anonymous online survey to all students at 15 of Canada’s 17 medical schools. The survey (...) collected data on respondent socio-demographic characteristics, features of their medical education, intentions for medical practice, and perspectives on MAID. We analyzed responses using univariate descriptive and stepwise multivariate logistic regression. In 1210 completed surveys, 71% of respondents reported being willing to provide MAID under a legal framework that permits it. Non-religious respondents reported greater willingness to participate in MAID than respondents of any religious affiliation. Frequency of religious attendance was inversely associated with willingness to provide MAID. Medical students born in Québec were more willing to provide MAID than respondents from other provinces. Age, sex, socioeconomic status, year of medical study, previous academic major, and rural/urban city of birth were not associated with willingness to provide MAID. As the current class of medical students becomes the first cohort of new physicians to enter Canada’s changing medical and legal landscape around MAID, our findings inform the public debate by examining attributes associated with support or opposition to the practice. This systematic review aimed to provide an overview of earlier research on the relationships between age conceptualizations and indicators of employability. We have conducted a systematic literature search using PsycINFO, Academic Search Premier, Business Source Complete, CINAHL, ERIC, MEDLINE, and Science Direct. Two raters evaluated the articles and subsequently distinguished k = 41 studies that met the inclusion criteria for this systematic review. Our review revealed that many researchers adopted different operationalizations to measure employability. Moreover, most studies included calendar age (...) as indicator of aging at work, and were based on a cross-sectional design. Based on the Standardized Index of Convergence method, different types of evidence were found for the relationships between age and the employability measures. For relationships between psychosocial age and lifespan age, on the one hand, and employability measures, on the other hand, too few studies were found to draw conclusions. Yet, for relationships between calendar age and labor market-based measures strong consistent negative relationships were found across the studies, and moderately strong positive relationships were found for functional age and labor market- based measures. For organizational age and both competence-based as well as labor market-based measures moderately strong negative relationships were found. We discuss the implications of these results and propose a research agenda for future studies. (shrink)

Let K be a family of structures, closed under isomorphism, in a fixed computable language. We consider effective lists of structures from K such that every structure in K is isomorphic to exactly one structure on the list. Such a list is called a computable classification of K, up to isomorphism. Using the technique of Friedberg enumeration, we show that there is a computable classification of the family of computable algebraic fields and that with a 0'-oracle, we can obtain similar (...) classifications of the families of computable equivalence structures and of computable finite-branching trees. However, there is no computable classification of the latter, nor of the family of computable torsion-free abelian groups of rank 1, even though these families are both closely allied with computable algebraic fields. K.L. Reinhold’s interpretation of deduction plays a seminal role in the development of German Idealism. However, as yet Reinhold’s concept of deduction has not been sufficiently explained. The goals of this paper are to analyze Reinhold’s notion of deduction in his Neue Darstellung der Hauptmomente der Elementarphilosophie, to reconstruct his deduction of a priori forms of representation and to discuss his deduction critically. In doing so, I will present Reinhold’s objections against Immanuel Kant’s method in the Critique of Pure Reason.

Darwins bahnbrechendes Werk 'Über die Entstehung der Arten' (1859) löste eine neue Debatte aus, die weniger durch wissenschaftliche Forschung als durch weltanschauliche Proklamationen geprägt war. Sie nahm in Deutschland einen anderen Verlauf als in England: Die dort noch starke Physikotheologie war in Deutschland bereits einflußlos geworden, und der vorangegangene Materialismus-Streit hatte den Boden für die Rezeption der Lehre Darwins vorbereitet. Von Seiten des weltanschaulichen Materialismus wurde der Darwinismus wegen seiner Eliminierung eines zwecksetzenden göttlichen Verstandes als unverhoffte Bestätigung angesehen und als (...) eine neue und zudem wissenschaftlich abgesicherte "Schöpfungstheorie" rezipiert und zum Schlüssel für die Lösung aller wissenschaftlichen und philosophischen Rätsel stilisiert. Kurt Walter Zeidler darf als einer (...) jener selten gewordenen Denker gelten, die in unermüdlichem Reflexionseinsatz seit Jahrzehnten sich für die seit längerem in Gefahr geratene klassische Philosophie auf einem ihrer Hochplateaus, der Transzendentalphilosophie, genauer noch: deren postneukantianischer Erscheinungsform einsetzen, dies im lebendigen Wechselgespräch mit einer Reihe von Meisterdenkern von Platon und Aristoteles an über Leibniz, die neuzeitliche Aufklärung hin zu Kant, dann über Fichte, Hegel, Schelling zu ausgewählten Neukantianern sowie z. T. auch polemisch zu den ‚Modernen‘ und dem neo-nominalistischen Zeitgeist weithin. – Die Position Zeidlers dem allen gegenüber ist im Grunde sehr einfach zu kennzeichnen: Es ist ein Denken einer ihre eigene Geschichte in ihren ‚großen‘ Gestalten einbeziehenden Vernunft, die in ihrem Prinzipien-Selbst als theoretischer wie praktischer Erst- und Gesamtgrund menschliche Würde wie auch diesheitliche Ganzheit zu fundieren und zu garantieren vermag. – Zeidler darf in diesem Panorama mit Fug und Recht als eine Art Leuchtturm im rasch wechselnden Gewölk modernen und nachmodernen Gedankenlebens erscheinen. A book reading between k.d. lang's interpretation of Leonard Cohen's 'Hallelujah,' male and female desire, today's network culture, Adorno on radio and Nietzsche on the Greeks. The working of music is transformed by digital media, broadcast and recording dynamics. Given the sexual/religious core of Cohen's Hallelujah, this study poses a phenomenological reading of the objectification of both men and women, raising the question of desire, including gender issues and both homosexual and heterosexual desire. A review of critical thinking about (...) musical performance as 'currency' and consumed commodity takes up Adorno's reading of Benjamin's analysis of the work of art in the age of mechanical reproduction as applied to music/radio/sound and the persistent role of 'recording consciousness'. Ultimately, the question of what Nietzsche called the becoming-human-of-dissonance is explored in terms of both ancient tragedy and Beethoven's striking deployment of dissonance as Nietzsche analyses both as playing with suffering, discontent, and pain itself, musically, as joy. De Marco et al 1 argue that the standard account of medical ‘invasiveness’ (as ‘incision’ or ‘insertion’) fails to capture three aspects of its existing use, namely that invasiveness can come in degrees, often depends on features of alternative medical interventions and can be non-physical. They propose a new schematic account that suggests that medical interventions can possess ‘basic invasiveness’ (which can come in degrees and of which they suggest at least two types: physical and mental), and ‘threshold invasiveness’ which (...) can depend on the alternatives a specific intervention is being compared with. While we agree that the standard account of invasiveness fails to capture existing use and are supportive of their aim, here we express scepticism about the practical utility of De Marco et al ’s revised account. Our scepticism concerns the application of De Marco et al’s revised account in the narrow context of clinician–patient communication and deliberation about medical interventions within clinical practice, which we see as a crucial context for its use. While De Marco et al do not specify the contexts in which their revised account could be used, we assume from their discussion of treatment interventions (e.g. non-invasive prenatal testing, ventilation) and their comparison to ‘more or less invasive’ options, that at least one of the purposes in mind for …. Causal reductionism is the widespread assumption that there is no room for additional causes once we have accounted for all elementary mechanisms within a system. Due to its intuitive appeal, causal reductionism is prevalent in neuroscience: once all neurons have been caused to fire or not to fire, it seems that causally there is nothing left to be accounted for. Here, we argue that these reductionist intuitions are based on an implicit, unexamined notion of causation that conflates causation with prediction. (...) By means of a simple model organism, we demonstrate that causal reductionism cannot provide a complete and coherent account of ‘what caused what’. The essay includes a discussion of transmedial expression, including dramatization, or television and social media, text and subtext, as well as the inspiration of Anders’s poem as a work of art continuing in our times: the ongoing exclusion(s) of certain names and certain thinkers as of certain musical modes, including electronic musical works, as of voices and (...) of collective memory, or oblivion. Reading Raymond Williams along with Anders and Adorno on television updated in today’s era of screen-being, this essay reads the challenges of on-line music magazines, Leonard Cohen and k.d.lang, between modes of memorialization, including a reading of Anders’s poetic memorial on the violence of Walter Benjamin’s death to conclude with Ivan Illich on the ongoing expropriation of death (and health) today. Philosophy of religion has been the object of penetrating critiques concerning its continued near-complete blindness to all but a single religion. The need for philosophy of religion to open up so as to include more than merely occasional and tokenistic treatments of ‘Other’ religions is clearly evident from the slew of recently published titles concerned with diversifying the field. In this light, a book such as Victoria Harrison’s Eastern Philosophy of Religion should surely come as a welcome addition. And yet, (...) unfortunately, this book turns out to be a case study in how not to diversify philosophy of religion in the twenty-first century. (shrink)

Luck, Value, and Commitment comprises eleven new essays which engage with, or take their point of departure from, the influential work in moral and political philosophy of Bernard Williams (1929-2003).

On the basis of the archival materials, first identified by the authors, and the published historical sources that have not yet come to the attention of historians of science, this article reconstructs the biography of Johann Christian Weltzien (1767–1829), doctor of medicine and surgery. In 1785, Weltzien became a court physician. In 1799, in the retinue of Grand Duke Konstantin Pavlovich, he participated in Italian and Swiss military campaigns. After that, Weltzien was assigned to the Сourt of Grand Duke Konstantin (...) Pavlovich of Russia and stayed at his court until 1810. He started his charitable activities in the Medico-Philanthropic Committee. From May 1810 to the middle of 1825, Weltzien was a physician at the Imperial Page Corps. It turned out that around the same time, in 1804–1825, he was a member of the Medical Council. Furthermore, as there were two medical councils under two ministries at the time, in 1811–1822, he was a member of both. Since 1825, Weltzien lived on his estate in Somel, now Vaiatu Manor in Estonia, and died in St. Petersburg. Weltzien’s work as a provincial physician prompted him to write a book on medical police, which was published in 1785. It has been established that this remained his only book on the topic. The article shows that Weltzien became one of the first ideologists of state care for public health in the Russian Empire. Weltzien’s ideas, expressed over 200 years ago, regarding the state having mechanisms to impact public health and the importance of not excluding it from solving health problems have not lost their significance even today. Die Revision des Wahrheitsbegriffs seitens Nietzsches kommt so ans Tageslicht. Forscher wie J. Simon und F. Kaulbach haben unterstrichen, daß Nietzsche die schon von Kant angelegte Übertragung der Wahrheit aus dem Theoretischen ins Praktische vollzogen hat. In seiner geplanten Doktorarbeit im Jahr 1868 Die Teologie seit Kant greift Nietzsche F.A. Langes und K. Fischers Auslegungen (...) auf, deren Einfluß auch in seinen späteren Werken noch wahrnehmbar ist. Insbesondere entnimmt er Kant die Gegenüberstellung von menschlicher und tierischer Welt, der dann seine offene Kritik am kantischen "Anthropomorphismus" folgt. Nietzsche nimmt so mit Kant Stellung.In the last decades, many Nietzsche scholars have begun to attend Kant's influence on Nietzsche's philosophy, particularly on his criticism of science and of knowledge. This is how Nietzsche's reexamination of the concept of truth emerges. Scholars such as J. Simon and F. Kaulbach have stressed how Nietzsche has completed the displacement of truth - already formulated by Kant - from the level of theoretical reflection to the level of pragmatical reflection. In the planned Doktorarbeit of 1868, Die Teleologie seit Kant, Neitzsche uses Kant interpretations of F. A. Lange and K. Fischer, whose influence can still be found in the works of the maturity. It is mainly from Kant that Nietzche derives the opposition between human world and animal world. Such an opposition turns into an open criticism of what he considers the Kantian anthropomorphism. Thus, Nietzsche's position can be summarised as a movement mit Kant gegen Kant. (shrink)
