Gilles de la Tourette syndrome (TS) is a childhood neuropsychiatric disorder characterised by the presence of motor and vocal tics. Patients with malignant TS experience severe disease sequelae; risking morbidity and mortality due to tics, self-harm, psychiatric comorbidities and suicide. By definition, those cases termed ‘malignant’ are refractory to all conventional psychiatric and pharmacological regimens. In these instances, deep brain stimulation (DBS) may be efficacious. Current 2015 guidelines recommend a 6-month period absent of suicidal ideation before DBS is offered to (...) patients with TS. We therefore wondered whether it may be ethically justifiable to offer DBS to a minor with malignant TS. We begin with a discussion of non-maleficence and beneficence. New evidence suggests that suicide risk in young patients with TS has been underestimated. In turn, DBS may represent an invaluable opportunity for children with malignant TS to secure future safety, independence and fulfilment. Postponing treatment is associated with additional risks. Ultimately, we assert this unique risk-benefit calculus justifies offering DBS to paediatric patients with malignant TS. A multidisciplinary team of clinicians must determine whether DBS is in the best interest of their individual patients. We conclude with a suggestion for future TS-DBS guidelines regarding suicidal ideation. The importance of informed consent and assent is underscored. (shrink)

Background: COVID-19 pandemic has led to heightened moral distress among healthcare providers. Despite evidence of gendered differences in experiences, there is limited feminist analysis of moral distress. Objectives: To identify types of moral distress among women healthcare providers during the COVID-19 pandemic; to explore how feminist political economy might be integrated into the study of moral distress. Research Design: This research draws on interviews and focus groups, the transcripts of which were analyzed using framework analysis. Research Participants and Context: 88 (...) healthcare providers, based in British Columbia Canada, participated virtually. Ethical Considerations: The study received ethical approval from Simon Fraser University. Findings: Healthcare providers experienced moral dilemmas related to ability to provide quality and compassionate care while maintaining COVID-19 protocols. Moral constraints were exacerbated by staffing shortages and lack of access to PPE. Moral conflicts emerged when women tried to engage decision-makers to improve care, and moral uncertainty resulted from lack of clear and consistent information. At home, women experienced moral constraints related to inability to support children’s education and wellbeing. Moral conflicts related to lack of flexible work environments and moral dilemmas developed between unpaid care responsibilities and COVID-19 risks. Women healthcare providers resisted moral residue and structural constraints by organizing for better working conditions, childcare, and access to PPE, engaging mental health support and drawing on professional pride. Discussion: COVID-19 has led to new and heightened experiences of moral distress among HCP in response to both paid and unpaid care work. While many of the experiences of moral distress at work were not explicitly gendered, implicit gender norms structured moral events. Women HCP had to take it upon themselves to organize, seek out resources, and resist moral residue. Conclusion: A feminist political economy lens illuminates how women healthcare providers faced and resisted a double layering of moral distress during the pandemic. (shrink)

We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. Most moral philosophers agree that if a moral agent is incapable of performing some act ϕ because of a physical incapacity, then they do not have a reason to ϕ. Most also claim that if an agent is incapable of ϕ-ing due to a psychological incapacity, brought about by, for example, an obsession or phobia, then this does not preclude them from having a reason to ϕ. This is because the 'ought implies can' principle is usually interpreted as a claim (...) about physical, rather than psychological, capacities. In this paper I argue for an opposing view: if we don't have reasons to do things that we are physically incapable of doing, then neither do we have reasons to do things we are psychologically incapable of doing. I also argue that extending the 'ought implies can' principle to psychological capacities makes the principle more attractive. (shrink)

There is an undercurrent to be detected in Anselm's record of the meditative experience that issued in the Ontological Argument and, although it points to a profound and perennial problem in the interpretation of religion, this undercurrent has been largely ignored. The Argument, as is well known, moves entirely within the medium of reflective meaning focused on the idea of God and, unlike the cosmological arguments of later theologians, it makes no appeal whatever to a principle of causality or to (...) the discovery of a sufficient reason for finite existence. Anselm seems to have had his own sense of what one may call the unadulterated rationalism of the Argument when, in his own words, he wondered, ‘if perhaps it might be possible to find one single argument that for its proof required no other save itself, and that by itself would suffice to prove that God really exists’. Here we are entirely within that inner chamber of the mind so dear to the Augustinian tradition, a mind from which one is to exclude all thought save that of God. The task of the one who reflects is to penetrate the inner meaning of this thought in order to discover what it implies beyond what is evident on the surface. With such an eminently rational or logical aim occupying the centre of attention, it is quite understandable that the presence of another, and quite opposed, concern should have been overlooked - Anselm's concern, namely, to transcend, as it were, the medium of thought itself, and enter into the presence of God. The reason that this concern introduces a tension in the search for a proof is that the realization of presence would seem to render proof superfluous, while the inference in an argument - especially one moving towards existence – inevitably suggests, in some sense and to some degree, the absence of what is sought for. Some decades ago in his intriguing book on Jonathan Edwards, Perry Miller used to great effect the device of supposing a two-fold biography of Edwards, an external one consisting of the historical record embracing the major events of his life and times, and an internal one aimed at an interpretation of the mind of Edwards and the development of his thought.

The theory of Gestalt qualities arose from the attempt to explain how a melody is distinct from the collection of the tones which it comprehends. In this essay from 1890 Christian von Ehrenfels coined the term 'Gestaltqualität' to capture the idea of a pattern which is comprehensible in a single experience. This idea can be applied not only to melodies and other occurrent patterns, but also to continuant patterns such as shapes and colour arrays such as the array of a (...) chess board. Ehrenfel's essay gave birth to the Gestalt movement in psychology. (shrink)

We all think that science is special. Its products—its technological spin-off—dominate our lives which are thereby sometimes enriched and sometimes impoverished but always affected. Even the most outlandish critics of science such as Feyerabend implicitly recognize its success. Feyerabend told us that science was a congame. Scientists had so successfully hood-winked us into adopting its ideology that other equally legitimate forms of activity—alchemy, witchcraft and magic—lost out. He conjured up a vision of much enriched lives if only we could free (...) ourselves from the domination of the ‘one true ideology’ of science just as our ancestors freed us from the domination of the Church. But he told us these things in Switzerland and in California happily commuting between them in that most ubiquitous product of science—the aeroplane. (shrink)

This book offers a defense of the tensed theory of time, a critique of the New Theory of Reference, and an argument that simultaneity is absolute. Although Smith rejects ordinary language philosophy, he shows how it is possible to argue from the nature of language to the nature of reality. Specifically, he argues that semantic properties of tensed sentences are best explained by the hypothesis that they ascribe to events temporal properties of futurity, presentness, or pastness and do not (...) merely ascribe relations of earlier than or simultaneity. He criticizes the New Theory of Reference, which holds that "now" refers directly to a time and does not ascribe the property of presentness. Smith does not adopt the old or Fregean theory of reference but develops a third alternative, based on his detailed theory of de re and de dicto propositions and a theory of cognitive significance. He concludes the book with a lengthy critique of Einstein's theory of time. Smith offers a positive argument for absolute simultaneity based on his theory that all propositions exist in time. He shows how Einstein's relativist temporal concepts are reducible to a conjunction of absolutist temporal concepts and relativist nontemporal concepts of the observable behavior of light rays, rigid bodies, and the like. A series of lectures organized in part by the Society for Applied Philosophy and entitled ‘Philosophy and Practice’ is presumably aimed at displaying the practical implications of philosophical doctrines and/or applying philosophical skills to practical questions. The topic of this paper, the role of interests in science, certainly meets the first condition. For as will be argued there are a number of theses concerning the role of interests in science which have considerable implications for how one should see the scientific (...) enterprise in general and in particular for how one assesses the claim that science ought to be accorded its priviliged position in virtue of its results and/or methods And in view of the respect and resources accorded to science what could be of greater practical interest? It remains the case, however, that my interest may seem the inverse of that of the organizers of this series. For in looking at the role of interest in science, one is examining, so to speak, the extent to which the sphere of the practical determines what goes on in science. One is exploring ways in which the non-scientific impinges on the scientific. While my primary focus will be on the physical sciences, it will be argued that there is a significant difference between them and the social sciences; a difference which renders the social sciences intrinsically liable to penetration from outside. As will be seen, some of the particular arguments for this conclusion make pressing the question: what about philosophy? The answer, it will be concluded, is that philosophy is insulated from external influences to a considerable extent. In that lies both its importance and an explanation as to why much of it has little practical application. (shrink)

The purpose of this study was to determine the development of moral judgement in first-year and senior baccalaureate nursing students. These students were enrolled in three separate nursing programmes, each of which differed significantly in ethical content. The sample totalled 180 students enrolled in three New England programmes. Programme A included an ethics course taught by a professor of ethics. Programme B integrated ethical issues into all nursing theory courses. Programme C did not include ethical content in theory courses. The (...) design was of a developmental cross-sectional study. The dependent variable was the development of moral judgement, as measured by Rest’s Defining Issues Test. The independent variable was the amount of ethics taught in the nursing programmes and the level of academic education. The senior nursing students from programme A scored significantly higher than the other senior groups on the Defining Issues Test. The conclusion is that an ethics course with group participation and a decision-making element significantly facilitated nursing students’ development of moral judgement. (shrink)

In Hegel's Critique of Liberalism , Steven B. Smith examines Hegel's critique of rights-based liberalism and its relevance to contemporary political concerns. Smith argues that Hegel reformulated classic liberalism, preserving what was of value while rendering it more attentive to the dynamics of human history and the developmental structure of the moral personality. Hegel's goal, Smith suggests, was to find a way of incorporating both the ancient emphasis on the dignity and even architectonic character of political life (...) with the modern concern for freedom, rights, and mutual recognition.
