Although many studies have previously examined medicalisation, we add a new dimension to the concept as we explore how contemporary oncological medicine shapes the dying self as predominantly medical. Through an analysis of multiple case studies collected within a comprehensive cancer centre in Ontario, Canada, we examine how people with late‐stage cancer and their healthcare providers enacted the process of medicalisation through engaging in the search for oncological treatments, such as experimental drug trials, despite the incurability of their disease. (...) The seven cases included 20 interviews with patients, family, physicians and nurses, the analysis of 30 documents and 5 hr of field observation. A poststructural perspective informed our study. We propose that searching for life extension enacts medicalisation by shaping the dying person afflicted with terminal cancer into new medical subjectivities that are knowledgeable, active, entrepreneurial and curative. Participants initially took up medical thinking from the formal oncology system, but then began to apply and internalise medical rationalities to alter their personhood, thereby generating new curative possibilities for themselves. For people seeking life extension, the embodied and day‐to‐day experiences of suffering and being close to death became expressed and moderated in fundamentally medicalised terms. (shrink)

Analyzing the linguistic, rhetorical, and affective aspects of these metaphors opens the range of issues available for bioethical scrutiny and increases public accountability. This article shows how such a multidisciplinary approach can be useful by looking at a set of texts about one issue, the use of a newly developed technique for genetic modification, CRISPRcas9.

I call this “the value problem.” I have previously argued that most forms of reliabilism cannot handle the value problem. The aim of this article is to use a model from the origin of life studies to provide some depth and detail to our understanding of exploratory models by suggesting that some of these models should be understood as indeterminate. Models that are indeterminate are a type of exploratory model and therefore have extensive potential and can prompt new lines of research. They are distinctive in that, given the current state of scientific understanding, we cannot specify how and where (...) the model will be useful in understanding the natural world: in this case, the origin of life on Earth. The purpose of introducing indeterminacy is to emphasize the epistemic uncertainty associated with modeling, a feature of this practice that has been under emphasized in the literature in favor of attempts to understand the more specific epistemic successes afforded by models. Theodor W. Adorno and Jürgen Habermas both champion the goal of a rational society. However, they differ significantly about what this society should look like and how best to achieve it. While theories of animal rights maintain that nonhuman animals possess prima facie rights, such as the right to life, the dominant philosophies of animal rights permit the killing of nonhuman animals for reasons of self-defense. I argue that the animal rights discourse on defensive killing is problematic because it seems to entail that any nonhuman animal who poses a threat to human beings can be justifiably harmed without question. To avoid this human-privileged conclusion, I argue that the animal rights (...) position needs to both (1) deploy a new criterion of liability to defensive harm, and (2) seriously consider whether human beings themselves are liable to defensive harm in human-animal conflicts. By shifting the focus to whether humans are liable to defensive harm, we will find that in many situations of human-animal conflict, human beings are actually the ones liable to be harmed because they are often culpable or, to some degree, morally responsible for posing an unjust threat to nonhuman animals. William James’s historic fascination with psychic phenomena, including the possibility of life after death, has become more widely known with the publication of recent books and articles on this controversial aspect of his scientific legacy. However, little is known about the emerging evidence suggesting the possibility that James’s scientific interest in these topics has not waned since he died. This paper reviews preliminary observations, including two exploratory double-blinded mediumship investigations, which are consistent with the hypothesis that James (with others) (...) may be continuing his lifelong quest to address the question of the survival of consciousness after physical death ‘from the other side’. These proof-of-concept investigations illustrate how future systematic laboratory research is possible. The limitations of current neuroscience methods are explicated in terms of investigating the hypothesis of the brain as a possible antenna-receiver for consciousness. If James’s tentative conclusions about the nature of the relationship between consciousness and the brain turn out to be accurate, then it is logically plausible (if not essential) to posit the possibility that his efforts have persisted in the recent past and present, and may even continue in the future. Scientific integrity plus the pursuit of verity require our being open to this important theoretical and empirical possibility. (shrink)

Background Infertility is an increasingly prevalent disease in society and is considered by the World Health Organization to be a public health problem. An important ethical issue arises from the clarification of reproductive rights in a fair and equal way. The objective of this study was to deepen and update the knowledge and discussion about the difficulty of accessing infertility treatments in Brazil. Methods A cross-sectional observational study was carried out through the application of an online questionnaire that collected the (...) socioeconomic characteristics of couples and identify how barriers to infertility care affect the most vulnerable populations. We included couples who sought medical assistance to achieve pregnancy at two clinics in the states of São Paulo and Minas Gerais. Results A total of 201 questionnaires were analyzed. Most couples self-declared as white and the average age of wives was 36 years and husbands 38 years. 65% (65%) of couples would proceed with the treatment in a different city to which they lived, 37% evaluated as having easy access to a medical specialist only after indication, and more than half of the participating have thought about giving up the treatment due to some difficulty in accessing it. 39% of participants sought more than one medical service to find better reception, 42% of couples sought more than one medical service to define where it would be better financially, and 67.2% referred to the high cost of treatments, that is, financial issues, as a great difficulty in accessing medical services and/or treatment. Although 72.6% of couples considered having a good quality of life, 54.2% admitted that infertility and the search for treatment generated anxiety/stress in the couple’s life. Conclusion There is a need for public education on reproductive health and for policymakers to raise awareness of the importance of the difficulty that many couples face in seeking treatment to become pregnant, especially in countries with less financial resources. Indeed, it is commonly accepted that there is a universal human right to access healthcare of appropriate quality as a matter of justice. Discussion of access to reproductive technologies should be considered taking into account the longstanding ethical debate regarding fertility, fecundity, and infertility, as well as reproductive care. In recent memory, the following theatrical and dance performances "Contact" by Susan Stroman and John Weidman, "Art" by Yasmina Reza, (...) "Sunday in the Park with George" by Stephen Sondheim, book by James Lapine, "Les Miserables" by Claude-Michel Schfonberg/music and others, "Picasso at the Lapin Agile" by Steve Martin, and "The Sleep of Reason," by Antonio Buero-Vallego have taken the not-so-dead-life (natura morta) or still life of painting into the vital arena of the performing arts. The quest to seek intra-discipline aesthetics among dance, music, theater, and the visual arts is not the charge, but instead a turn to establish a narrative that has brought about this invasion of interactivity between the arts. Clearly defined boundaries between the arts have beenfading since Fluxus, Happenings, and Performance Art for over half a century. In the early part of the twentieth century, Marcel Duchamp's chess games and use of the fourth dimension of time in his numerous produced and pondered pieces all speak to the interrelatedness of the arts.Contemporary novels based on historical themes and subjects have created characters based on the lives and works of famous and intriguing artists such as Michelangelo Caravaggio, Georges de La Tour, and Jan Vermeer. The silence and pensive qualities of the figures of Jan Vermeer, the seventeenth-century Dutch painter, have been the catalyst for two books, The Girl in Hyacinth Blue by Susan Vreeland and the Girl with the Pearl Earring by Tracy Chevalier.1 Vreeland combines the mystic of the Vermeer paintings' characters with the impoverished history of the refugee status of its owners. In various parts of Vreeland's book the authenticity of the Vermeer painting becomes a subtext for describing the work. [End Page 107] Just the lament of some Dutch art historians. Where has such a treasure gone, or some such thing? He turned to pour us both a brandy. So why could this not be? It's his same window opening inward at the left that he has used so often, the same splash of pale yellow light. Take a look at the figures in the tapestry on the table. Same as in nine other paintings. Same Spanish chair with lion's head finials that he used in eleven canvases, same brass studs in the leather. Same black and white tiles placed diagonally on the floor.2A few pages later the interest in the authenticity of the painting continues: No, no signature. But that was not unusual. He often failed to sign his work. Besides, he had at least seven styles of signature. For Vermeer, signatures are not definite evidence. Technique is. People, and also religious and humanist communities, in a post-sec¬ular world are challenged to develop a new existential, ethical and spiritual language that fits to their global and pluralistic surroundings. This new world-viewing language could rise out of the reflection on contrast experiences, positive and negative disruptive experiences that question the everyday inter pretations of life. The connection of these articulated reflections on contrast experiences with former world-viewing (...) sources and practices with regard to precarious life could provide new meaning and orientation for individuals and communities. The problem of teleology is one of the major challenges for theoretical biology and, in general, for every philosophy of life. The debate about the nature of the organism and its difference from inorganic matter cannot come to terms with, at one point or another, the problem that arises from the unavoidable impression of teleology exhibited by the living itself. In this respect, the advantages that a systematic thinker such as Hartmann can offer to the philosophical analysis of (...) class='Hi'>life are remarkable. The first discussion by Hartmann on the problem of causality can be found in Philosophische Grundfragen der Biologie (1912), a work which, however accurate, takes a predominantly Kantian and neo-Kantian approach, and cannot rely on the original and fecund tools of Hartmann’s mature stratified ontology. Therefore, the present contribution will focus on Hartmann’s ontological works, in particular his Philosophie der Natur: Abriss der speziellen Kategorienlehre (1950), in order to highlight and discuss the theoretical advantages they present. In particular, the validity of Hartmann’s ontological approach will be revealed by three key points: 1) a careful categorial distinction between different forms of determination, and even of causality, present in nature; 2) the refusal of vitalism, with the resolute limitation of finality in the proper sense to human conscious action; and 3) a great openness to the possibility that the living could be a third realm, governed by an autonomous modality of determination whose nature has yet to be discovered. Modern medicine provides unprecedented opportunities in diagnostics and treatment. However, in some situations at the end of a patient’s life, many physicians refrain from using all possible measures to prolong life. We studied the incidence of different types of treatment withheld or withdrawn in 6 European countries and analyzed the main background characteristics.

A new design principle is discussed for making a sufficiently complex cell for the creation of the first wet artificial life in the laboratory. The current approach is to attempt a minimal cell, which consists of a liposome that contains a minimal metabolic cycle for self-maintenance and self-replication. Given the lack of success with the minimal cell to date, the authors suggest it is possible to take an alternative approach to building the first wet artificial life form that (...) they have called the first cell. In this article, the concept of the first cell versus the minimal cell is discussed. The new design principle is supported by the following observations that are examined and developed from an Origins of Life perspective: That human development shows a U-shaped curve where the skills and developmental patterns of human infants are proficient at the first phase then decline and subsequently recover to a qualitatively better level. A review of spatial temporal dynamics using cellular automata studies shows that complexity generated at both the initial states and the time evolution dynamics can synthesize rich evolutionary processes. A chemical experiment exhibiting self-organizing dynamics results in the emergence of a self-propelling oil droplet that demonstrates the self-sustaining properties of a non-equilibrium state. Finally, the important design question Is life a contingent or deterministic phenomenon? is examined which asks whether a life form is a dynamic product of its surroundings or whether it is the inevitable outcome of prior events, and is discussed making reference to the characteristics of an art installation The Way Things Go. This contemporary masterpiece, which exhibits a long-lasting chain of unrelated causal events, serves as a theoretical model for discussion of evolutionary processes, suggesting that when it comes to designing wet artificial life forms in the laboratory, both contingent and deterministic processes should be taken into account. The discovery of a second genesis of life besides the one on Earth, this time on Mars, would have profound scientific and philosophical implications. Scientifically, it would provide a second example of biochemistry and of evolutionary history. Many important biological questions may be answerable through the comparison of biochemistry between the life forms on the two planets. Philosophically, the discovery of a second genesis of life in our solar system would suggest that the phenomenon of life (...) is distributed throughout the universe. We could finally be confident that we are not alone. To protect a second genesis as we search for it, the robotic and human exploration of Mars should be done in a way that is biologically reversible, i.e., we must be able to undo our contamination of Mars if we discover a second genesis of life there. It is important to note that human exploration can be done in a way that is biologically reversible. Further, the discovery of a second genesis of life on Mars poses new questions in ethics. One question is: what ethical consideration is due to an alien life form when that life is distinctly different from Earth life, and the members of that life are no more advanced than microorganisms? Will we choose to terraform Mars to enhance the richness and diversity of the indigenous life we find there?
