Hot-cold empathy gaps are a pervasive phenomena wherein one’s predictions about others tend to skew ‘in the direction’ of one’s own current visceral states. For instance, when one predicts how hungry someone else is, one’s prediction will tend to reflect one’s own current hunger state. These gaps also obtain intrapersonally, when one attempts to predict what one oneself would do at a different time. In this paper, we do three things: We draw on empirical evidence to argue that so-called (...) hot-cold empathy gaps arise when one projects one’s own current state into a simulation about another. Second, we argue that this process does not typically confer knowledge, even when the predictions it produces happen to be accurate. Third, we suggest that these results can be used to develop a challenge for L.A. Paul's view that authentic action sometimes requires subjective knowledge of one’s own values and how these values relate to relevant outcomes. We then sketch an alternative view of the epistemic grounds of authenticity, one on which authenticity requires a kind of understanding. The relevant form of understanding can be achieved by subjective knowledge but can also be achieved elsewise, such as through testimony from a close friend about what one values. (shrink)

Our inability to feel what others feel makes it difficult to understand how they think. Because moral intuitions organize political attitudes, moral empathy gaps can exacerbate political conflict (and other kinds of conflict as well) by contributing to the perception that people who do not share our moral opinions are unintelligent and/or have malevolent intentions.

Empathy of pain as a multi-dimensional process includes sharing and understanding the pain of others in relation to oneself. Subjects in such studies are typically members of western, educated, industrialized, rich and democratic societies. In the literature review that we conducted, we observed that little is known about the empathy for pain in people who are not members of societies with these cultural characteristics. We often understand those who are “similar” to us more easily - ones who belong (...) to “our” cultural circle. However, contact with another culture could help prevent such bias. Group characteristics, such as focus on others, hierarchy preference, or cultural differences in self-constructs, can change the activity of brain regions associated with empathy and compassion. Increasing the diversity of the research participants connected with education level, poverty, industrialization, and respect for basic citizen freedoms seem to be necessary to fully understand the mechanisms that influence the development and operation of empathy. The most critical factor explaining the disjuncture between empathy’s revolutionary potential and today’s empathically-impaired society is the interaction between the brain and our dominant political culture. The evolutionary process has given rise to a hard-wired neural system in the primal brain and particularly in the human brain. This book argues that the crucial missing piece in this conversation is the failure to identify and explain the dynamic relationship between an empathy gap and the hegemonic influence of neoliberal capitalism, (...) through the analysis of the college classroom, the neoliberal state, media, film and photo images, marketing of products, militarization, mass culture and government policy. This book will contribute to an empirically grounded dissent from capitalism’s narrative about human nature. Empathy is putting oneself in another’s emotional and cognitive shoes and then acting in a deliberate, appropriate manner. Perhaps counter-intuitively, it requires self-empathy because we’re all products of an empathy-anesthetizing culture. The approach in this book affirms a scientific basis for acting with empathy, and it addresses how this can help inform us to our current political culture and process, and make its of interest to students and scholars in political science, psychology, and other social sciences. ​ ​. Much of the current debate opposing empathy to rationality assumes that there are no universal standards for rationality. From the postmodern perspective, the “rational” does not just vary according to the different historical stages of a people. It also differs according the social and cultural conditions that define contemporary communities. What counts as reasonable in the Afghan cultural sphere is often considered as irrational in the Western European context. What Americans take to be rational modes of conduct are not (...) considered to be such in various African communities. For postmodern thinkers, these examples point to the failure of the universalist paradigm inherited from the 18th century. We no longer believe that there is a single paradigm of rationality—once seen as exemplified by philosophy—that is capable of crossing our cultural divides.[i] If appeals to reason cannot bridge cultures, what can? We were all moved by the plight of the victims of the tsunami that devastated the Java Peninsula. Showing their empathy, people around the world contributed to their relief. Doesn’t this exhibit a universal empathy, one capable of bridging the gaps between cultures? A similar argument can be made from the universal appeal of certain works of literature. Reading them, we imaginatively participate in their characters’ lives. We feel what they feel, seeing the world through their eyes. We exercise our empathy in its basic etymological sense of feeling in and through another person. If empathy rather than rationality is genuinely universal, then literature, rather than philosophy, becomes our common language. Similarly, empathy rather than rationality owns the public space uniting different cultures. Appeals to our human solidarity, through literature, movies, television reports and so on must be based on it. [ii] As for philosophy, it becomes relegated to the sphere of our private beliefs and personal convictions. Its sphere is reduced to that of the “true for me.” In what follows, I would like to argue against this separation of empathy and rationality. My position will be that their opposition presupposes a limited, Cartesian concept of rationality. Once we abandon this, we find that empathy and rationality are not, in fact, distinct.. (shrink)

Much has been written about Husserl and the famous problems of intersubjectivity and solipsism, and some work has been done regarding Husserl's notion of empathy and its role in the establishment of intersubjectivity. The vast majority of that work, however, focuses on one of Husserl's texts and on the establishment of the possibility of other subjects. What is lacking in the scholarship is an investigation of the role of empathy in Husserl's corpus which address the related issues: validity, (...) the degrees of evidence with which something can be experienced, the different senses of "objective" in Husserl's texts, whether empathy contributes to knowledge or just to the validity of one's knowledge, where in the empathetic-communicative process something becomes objective. This dissertation begins to fill that gap by offering an investigation of the role of empathy in the attainment of objectively valid knowledge in each of Husserl's three introductions to phenomenology: Ideas: A General Introduction to Pure Phenomenology , Cartesian Meditations: An Introduction to Phenomenology , and The Crisis of European Sciences and Transcendental Phenomenology: An Introduction to Phenomenological Philosophy . ;Despite accusations by commentators that Husserl's is a solipsistic philosophy, I show that empathy is related to one's knowledge on the view offered in each of Husserl's introductions to phenomenology. Empathy is significantly related to knowledge in at least two ways and Husserl's epistemology might, consequently, be called a social epistemology: empathy helps to offer evidence for validity and thus solidify one's knowledge and it helps to broaden one's knowledge by affording the ability to gain access to what others have constituted and known. These roles of empathy are not at odds with one another; rather, both are at play in each of the introductions and, given his position in the earlier work, Husserl needed to expand the role of empathy as he did. Such a reliance on empathy, however, calls into question whether Husserl's is a transcendental philosopher in the sense Husserl claimed it is. (shrink)

Nietzsche is not known for his theory of empathy. A quick skimming of the Stanford Encyclopedia of Philosophy entry on empathy demonstrates this. Arthur Schopenhauer, Robert Vischer, and Theodor Lipps are among those whose views are considered representative, but Nietzsche has been simply forgotten in discussion of empathy. Nietzsche’s theory of empathy has not yet aroused sufficient interest among commentators. I believe that his views on this subject merit careful consideration. Nietzsche scholars have been interested in (...) his naturalistic accounts of other phenomena, but there seems to be relatively limited interest in his naturalistic account of a phenomenon that is so central to his disagreement with Schopenhauer, namely, empathic concern for others. This is surprising because Nietzsche makes a valuable contribution; he has views more in keeping with contemporary theories of empathy than others of his time. My goal here is to fill in this gap in the scholarship and provide the first thorough analysis of Nietzsche’s theory of empathy, which appears most clearly in Dawn. (shrink)

In Rita Charon's account of narrative medicine, empathy seems to be an essential element of the clinical relationship. However, empathy has not received much attention, which I believe is problematic. First, I show that not only is there no clear definition of what empathy is, but that this conceptual gap creates ambiguity about its role in the practice of narrative medicine. Second, I argue that certain passages in Charon's work seem to implicitly characterize empathy as a (...) combination of cognitive empathy, affective empathy, and sympathy, and that its inclusion in the practice of narrative medicine can be problematic. Third, following Dan Zahavi, I argue that a phenomenological account of empathy not only seems most compatible with the practices, values, and theoretical framework of narrative medicine, but is also capable of resolving the problems posed by alternative definitions of empathy and the ambiguity about the role of empathy in narrative medicine. This essay is about the relations between two different types of empathy and two different conceptions of oneness. Roughly, the first type of empathy is what is sometimes called “other-focused” or “imagine-other” empathy, in which one reconstructs the thoughts and feelings that someone else has or would have. The second type, “self- focused” or “imagine-self” empathy, is the sort of emotional attitude someone adopts when she imagines how she would think or feel were she in the (...) other person’s place. Some philosophers and psychologists have taken note of this distinction, but none has linked them with one of the more important reemerging religious and philosophical issues of the day, which has to do with the ethical significance of seeing oneself as part of a larger whole, or its proper role in moral motivation and other-directed moral concern. There are some long-overlooked materials in Neo-Confucian philosophy that attend closely to these very linkages, drawing out the morally salient differences between self- and other-focused empathy in light of their implications for the virtue of benevolence. This chapter unearths some of the most important ideas and arguments in this debate, fills in some gaps in the arguments, and draw out their moral implications. (shrink)

Using Kohut's seminal paper "Forms and Transformations of Narcissism" as a springboard, Frank Lachmann updates Kohut's proposals for contemporary clinicians. _Transforming Narcissism: Reflections on Empathy, Humor, and Expectations _draws on a wide range of contributions from empirical infant research, psychoanalytic and psychotherapeutic practice, social psychology, and autobiographies of creative artists to expand and modify Kohut's proposition that archaic narcissism is transformed in the course of development or through treatment into empathy, humor, creativity, an acceptance of transience and wisdom. (...) He asserts that empathy, humor, and creativity are not the goals or end products of transformations, but are an intrinsic part of the ongoing therapist-patient dialogue throughout treatment. The transformative process is bidirectional, impacting both patient and therapist, and their affect undergoes transformation - for example from detached to intimate - and narcissism or self-states are transformed secondarily as a consequence of the affective interactions. Meeting or violating expectations of emotional responsivity provides a major pathway for transformation of affect. For beginning therapists, _Transforming Narcissism_ presents an engaging approach to treatment that incorporates the therapeutic action of these transformations, but also leaves room for therapists to develop styles of their own. For more experienced therapists, it fills a conceptual and clinical gap, provides a scaffold for crucial aspects of treatment that are often unacknowledged, or are dismissed and pejoratively labeled "countertransference." Most importantly, Lachmann offers a balance between therapeutic spontaneity and professional constraint. Focused and engaging, _Transforming Narcissism_ provides a bridge from self psychology to a rainbow of relational approaches that beginning and seasoned therapists can profitably traverse in either direction. Rethinking Feminist Ethics bridges the gap between women theorists disenchanted with aspects of traditional theories that insist upon the need for some ethical principles. The book raises the question of whether the female conception of ethics based on care, trust and empathy can provide a realistic alternative to the male ethics based on duty and rule bound conception of ethics developed from Kant, Mill and Rawls. Koehn concludes that it cannot, showing how problems for respect of the individual arise (...) also in female ethics because it privileges the caregiver over the cared for. Inheriting this ability from religion—the romantic enthusiast Wilhelm Wackenroder considered artistic inspiration to originate in “divine inspiration” in the case of his hero, Raphael 1 —art eventually replaced institutionalized belief in an evolutionary schedule of cultural development (...) determined by German idealism. Hegel, for instance, theorized the evolution of spirit from religion to art as a qualitative advancement over time. Rejecting the metaphysics of German idealism, Friedrich Nietzsche nevertheless contended that “Art raises its head where religions decline.” By assuming the “feelings and moods” of religion which had been dismantled by enlightenment, Nietzsche claimed that art gave new form to the life of feeling and imbued human endeavors with a “loftier, darker coloration.” 2 In the early twentieth century Wassily Kandinsky believed that the “spiritual in art” was the historical manifestation of an “inner necessity” that was in itself timeless and universal. The epistemic and moral worth of empathy is deeply contested. Some doubt the possibility and sufficiency of empathic knowledge. Others question whether knowing how another feels is causally linked to moral actions. Though some have defended empathy as a form of knowing or an important epistemic endeavor, there is a gap in these responses that weakens their force against empathy’s skeptics. On the one hand, the epistemologists tend to focus on individuals. On the other hand, the moral (...) theorists and moral-epistemologists tend to focus on the social. Here, I contend that the first group could learn from the second and, together, form a stronger defense of empathy’s epistemic import. Namely, if, as epistemologists, we can begin from a conceptualization of empathy as a social epistemic practice within which knowledge is formed and confirmed with others, then we can better allow for possibility in understanding how another feels, reliability in empathic processes, and appropriate sensitivity to variance and degrees of empathic knowing. Despite the many contributions of nonhuman animals in history, nonhuman animal representations are seldom crafted with care and accuracy in curricular texts. Because of the anthropocentric vantage point of textbook creation, the nonhuman animal is often portrayed as an object, but as our relationship with the nonhuman world continues to deteriorate, we need now more than ever to consider the agency and subjectivity of nonhuman entities across time and space. In this article, I will use critical contextual analysis as a (...) research method to examine the representation of the horse in a commonly used textbook in middle and high schools in the United States titled World History: Patterns of Interaction to raise larger questions about how to engage with nonhuman animal presence in history with students. Using critical animal studies as a theoretical framework to illuminate the data, the findings reveal that the horse is depicted as agentless and is shown merely as a means for human animals to expand empire and perpetuate social norms. The exploration identifies a gap in the social studies scholarship that neglects the historical contributions of nonhuman animals in curriculum. This article will argue that juxtaposing anthropocentric curricular texts such as the one under investigation with narratives that illuminate nonhuman animal subjectivity is essential for building a more empathetic world and for cultivating a more inclusive historical narrative. This article adopts Husserl’s transcendental phenomenology to explore the complex relationship between patients and physicians. It delves into the coexistence of two distinct voices in the realm of medicine and health: the “voice of medicine” and the “voice of life-world.” Divided into three sections, the article emphasizes the importance of shifting from a scientific-medical attitude to a more personalistic approach in physician–patient interactions. This shift aims to prevent depersonalization and desubjectification. Additionally, it highlights the equal and irreducible nature of patients (...) while acknowledging the vital role physicians hold in the realm of illness. The article stresses the need for a balanced and equitable relationship between both parties, rooted in the shared life-world. Moreover, empathy is underscored as a crucial element in fostering meaningful dialogue, wherein understanding diverse perspectives and attitudes towards illness is paramount. The article argues that differences between patients and physicians are necessary for empathy, while shared similarities form its foundation. Ultimately, a harmonious relationship facilitates empathy and enables the constitution of a new sense of life for both patients and physicians. (shrink)

The paper builds an argument about empathy, kinesthesia, choreography, and power as they were constituted in early eighteenth century France. It examines the conditions under which one body could claim to know what another body was feeling, using two sets of documents – philosophical examinations of perception and kinesthesia by Condillac and notations of dances published by Feuillet. Reading these documents intertextually, I postulate a kind of corporeal episteme that grounds how the body is constructed. The purpose of this study is to examine whether corporate social responsibility activities perceived by consumers affect brand trust and corporate sustainability management. In other words, this study tried to examine whether the compatibility and authenticity of CSR influences brand trust, thereby affecting CSM including economic viability, environmental soundness, and social responsibility. To measure this, an empirical analysis was conducted on 479 consumers who had experience purchasing products from cosmetic companies that are carrying out CSR. As a result of the (...) analysis, it was found that the compatibility and authenticity of CSR have a positive effect on brand trust. Also, it was found that brand trust had a positive effect on social responsibility among the sub-concepts of CSM, but did not affect economic viability and environmental soundness. The results of this study are expected to provide strategic implications for social responsibility performance and brand trust building necessary for cosmetics companies to grow continuously. During the Asian and Pacific War, the Japanese government mobilized approximately 200,000 Asian women to military brothels to sexually serve Japanese soldiers. The majority of these victims were unmarried young women from Korea, Japan’s colony at that time. In the early 1990s, Korean feminist leaders helped more than 200 Korean survivors of Japanese military sexual slavery to come forward to tell the truth, which has further accelerated the redress movement for the women. One major issue in the redress movement and (...) research relating to the so-called “comfort women” issue is whether Japan’s colonization of Korea or gender hierarchy was a more fundamental cause of the Korean women’s suffering. Using an intersectional perspective, this article analyzes how colonial power, gender hierarchy, and class were inseparably tied together to make the victims’ lives miserable. By doing so, it shows that a one-sided emphasis on colonization or gender hierarchy will misrepresent the feminist political issue and misinterpret the “comfort women’s” experiences. (shrink)

Motivationally unconscious (M-unconscious) states are unconscious states that can directly motivate a subject’s behavior and whose unconscious character typically results from a form of repression. The basic argument for M-unconscious states claims that they provide the best explanation to some seemingly non rational behaviors, like akrasia, impulsivity or apparent self-deception. This basic argument has been challenged on theoretical, empirical and conceptual grounds. Drawing on recent works on apparent self-deception and on the ‘cognitive unconscious’ I assess those objections. I argue that (...) (i) even if there is a good theoretical argument for its existence, (ii) most empirical vindications of the M-unconscious miss their target. (iii) As for the conceptual objections, they compel us to modify the classical picture of the M-unconscious. I conclude that M-unconscious states and processes must be affective states and processes that the subject really feels and experiences —and which are in this sense conscious— even though they are not, or not well, cognitively accessible to him. Dual process psychology and the literature on cold-hot empathy gaps partly support the existence of such M-unconscious states. (shrink)

Individuals often fail to accurately predict others’ decisions in a risky environment. In this paper, we investigate the characteristics and causes of this prediction discrepancy. Participants completed a risky decision-making task mixed with different domains (gain vs. loss) and probabilities (small vs. large), with some participants making decisions for themselves (the actor) and the others predicting the actors’ decisions (the predictor). The results demonstrated a prediction discrepancy: predictions were more risk-averse than the actual decisions over small-probability gains and more risk-seeking (...) over large-probability gains, while these patterns were reversed in the loss domain. Reported and predicted levels of emotional stimulation revealed a pattern that is consistent with the notion of risk-as-feelings and empathy gaps. Mediation analysis provided strong evidence that such prediction discrepancy is driven mainly by the predictor’s underestimate of the intensity (not the impact) of the actor’s emotional state. One of the aims of Aesthetics is to understand aesthetic experience, that of our own and that of others. Yet, the underlying question of how_ _we can get information about other people’s aesthetic experience has not been granted enough attention. This article contributes to bridging this gap. The main argument is that by resorting to aesthetic judging, we can get information about other people’s aesthetic experience without sharing it. This article outlines how aesthetic judging works as an interface to aesthetic (...) experience. Aesthetic judging allows us to access aesthetic experience indirectly: with it, we can get some information about aesthetic experience. Aesthetic judging thus positions us in relation to someone else’s aesthetic experience. In a nutshell, learning about aesthetic experience happens via aesthetic judging in at least three ways proposed and analyzed here: “aesthetic participation”, “affective appropriation”, and “distanced aesthetic empathy”. Or a rat who refused to push a lever for food (...) when he saw that doing so caused another rat to be shocked? Aren’t these clear signs that animals have recognizable emotions and moral intelligence? With _Wild Justice_ Marc Bekoff and Jessica Pierce unequivocally answer yes. Marrying years of behavioral and cognitive research with compelling and moving anecdotes, Bekoff and Pierce reveal that animals exhibit a broad repertoire of moral behaviors, including fairness, empathy, trust, and reciprocity. Self-knowledge seems to be radically different from the knowledge of other people. However, rather than focusing on the gap between self and others, we should emphasize their commonality. Indeed, different mirror matching mechanisms have been found in monkeys as well as in humans showing that one uses the same representations for oneself and for the others. But do these shared representations allow one to report the mental states of others as if they were one''s own? Scholarly interest in employees’ voluntary pro-environmental behavior has begun to emerge. While this research is beginning to shed light on the predictors of workplace pro-environmental behavior, our understanding of the psychological mechanisms linking the various antecedents to employees’ environmentally responsible behavior and the circumstances under which any such effects are enhanced and/or attenuated is incomplete. The current study seeks to fill this gap by examining: the effects of perceived corporate social responsibility on employees’ voluntary pro-environment behavior; an underlying mechanism that (...) links CSR perceptions to these behaviors; and a boundary condition to these relationships. Data from 183 supervisor-subordinate dyads employed in large- and medium-sized casinos and hotels in Guangdong China and Macau revealed that employees’ corporate social responsibility perceptions indirectly affect their engagement in voluntary pro-environmental behavior through organizational identification, and these effects are stronger for employees high in empathy. Situations of fundamentally opposing interests are such that, without some degree of trust, they will fail to advance. In this paper I consider war and the achievement of peace through a series of reciprocal steps as the primary setting in which to examine trust, its terms, and its limits. A surprising point that transpires is that, in the attempt to resolve situations that are fundamentally conflictive, there is often a tendency to take steps to promote something else, superficially resembling trust, (...) sometimes counterproductively at the expense trust. An analysis of trust consisting of three elements is offered: (1) Vulnerability: the truster expresses an inclination to put himself in a vulnerable position with respect to the trusted. (2) Benefit: for the purpose of some gain that will befall the truster if the trusted does not exploit this vulnerability. (3) Reciprocity: with the expectation that the trusted will be sufficiently motivated to reciprocate at least partly due to the fact that truster’s behaviour is informed by such an expectation. The problem of trust is then presented as the idea of overcoming the “trust gap”. It is this: given the trust gap, and the rational-prudential basis to distrust – what could after all enable trust? Overcoming the gap may be achieved in one of two ways. Metaphorically speaking, we can narrow the gap, making the risks involved less worrisome, even if not eliminating them all together. Thus, encouraging cooperation on the basis of self-regarding prudential reasons. Or we can bridge the gap, leaving the risks as they are while procuring non-prudential resources to encourage cooperation. Only the latter can be truly classified as trust. Three ways in which the gap may be bridged are specified: Non-prudential reasons to reciprocate trust (Empathy, Fairness, and Reciprocity) correspond to the three components of trust (Vulnerability, Benefit, Reciprocity). The three reasons are introduced with the help of a minor thought experiments the results of which get further support from psychological experimental literature. It is then shown how these features of the situation provide a moral reason and, therefore, an expectation to act trustworthily, and as such facilitate trust. The possibility of trust is thus explained on the basis of an idea of reasonableness, rather than instrumental rationality, sentiment, or faith. Finally, it is argued that narrowing the gap might detrimentally affect bridging the gap. Confidence building measures (guarantees, incentives, and sanctions), designed to narrow the trust gap and facilitate cooperation, may unwittingly demolish the foundations for bridging the gap. Some policy implications are tentatively proposed: (1) Don’t focus exclusively on narrowing the gap. (2) Manufacture “common knowledge”. (3)Take care when narrowing the gap not to thwart bridging the gap. (shrink)

BackgroundThe issue of stigma is very important in the battle against HIV/aids in Africa since it may affect patient attendance at healthcare centres for obtaining antiretroviral medications and regular medical check-ups. Stigmatization creates an unnecessary culture of secrecy and silence based on ignorance and fear of victimization. This study was designed to determine if there is external stigmatization of people living with HIV and AIDS by health care workers at a tertiary hospital in KwaZulu-Natal province, South Africa. The study investigated (...) the impact of knowledge of HIV/aids by HCWs on treatment of patients, as well as the comfort level and attitude of HCWs when rendering care to PLWHA.MethodsA descriptive cross sectional study was designed to collect data using an anonymous self-administered structured questionnaire from 334 HCWs. The study was conducted in clinical departments of a large multidisciplinary 922-bed tertiary care teaching hospital in Durban, KZN.ResultsOverall HCWs had an above average knowledge about HIV/aids although some gaps in knowledge were identified. Tests of statistical significance showed that there was association between level of education and knowledge of HIV/aids ; occupation and knowledge of HIV/aids ; and gender and knowledge of HIV/aids. Test for comfort level was only significant for gender, with males showing more comfort and empathy when dealing with PLWHA. The study also revealed that patients were sometimes tested for HIV without informed consent before surgery, due to fear of being infected, and there was some gossiping about patients' HIV status by HCWs, thereby compromising patient confidentiality. The majority of HCWs showed a willingness to report incidents of stigmatization and discrimination to higher authorities, for better monitoring and control.ConclusionsAlthough knowledge, attitude and comfort level of HCWs taking care of PLWHA was above average, enforcement of existing antidiscrimination laws and continuing education in medical ethics and healthcare law, would greatly improve the performance of HCWs taking care of PLWHAs. More psychological support and counselling should be provided to HCWs, to further reduce the impact of stigmatization and discrimination against PLWHA. (shrink)

Background Little is known about communication between patients, families, and healthcare providers regarding ethical concerns that patients and families experience in the course of illness and medical care. To address this gap in the literature, we surveyed patients and family members to learn about their ethical concerns and the extent to which they discussed them with their healthcare providers. Methods We surveyed adult, English-speaking patients and family members receiving inpatient care in five hospitals in the Washington DC-Baltimore metropolitan area from (...) July 2017 to March 2020. Descriptive statistics were used to determine the frequency, comfortableness, and helpfulness of discussions regarding ethical concerns experienced when sick or receiving medical care. Univariable and multivariable stepwise logistic regression models were used to identify associations between healthcare provider and respondent characteristics and attitudes and (1) the likelihood of speaking to a healthcare provider about their ethical concern and (2) their level of comfort during these discussions. Results Of 468 respondents who experienced ethical issues, 299 (64%) reported discussing the situation with a member of their healthcare team; 74% (197/265) of respondents who had such a discussion found the discussion comfortable, and 77% (176/230) of respondents found the discussion helpful. To make discussions more comfortable and helpful, respondents proposed suggestions in open-ended responses involving (1) content and quality of communication; (2) positive healthcare provider qualities such as empathy, open-mindedness, knowledge, honesty, and trustworthiness; and (3) other contextual factors including having adequate time and available resources. Conclusions Patients and families often have ethical concerns that they discuss with clinicians, and they want clinicians to be routinely receptive and attentive to such discussions. (shrink)
