This article argues from a South African perspective that national experience in attempting to fulfil the right to health supports the need for an international framework. Secondly, we suggest that this framework is not just a matter of good choice or even of justice but of a direct legal duty that falls on those states that have consented to operate within the international human rights framework by ratifying key treaties such as the International Covenant on Economic Social and Cultural Rights (...) (ICESCR), the Convention on the Elimination of all forms of Discrimination Against Women (CEDAW), and the Convention on the Rights of the Child (CRC). States can either accept this duty or face with growing pressure from those who believe in global social justice to find lasting solutions to the terrible inequities in global health standards. This is evident not only in unresolved debates that are raging about where to allocate health aid or how to sustain and expand funding for AIDS treatment, but also in challenges facing national health systems that are incapable of purely domestic resolution. But there is some irony and much opportunity in this situation. Not only have the last 20 years seen an unprecedented growth in funding for health, (...) mainly through funding for AIDS, but there has also been a range of initiatives and ideas that have generated better knowledge not only of the determinants of health, but also of how to attain it. A new proof of the impossibility of reconciling realism and locality in quantum mechanics is given. Unlike proofs based on Bell's inequality, the present work makes minimal and transparent use of probability theory and proceeds by demonstrating a Kochen-Specker type of paradox based on the value assignments to the spin components of two spatially separated spin-1 systems in the singlet state of their total spin. An essential part of the argument is to distinguish carefully two commonly confused types of contextuality; (...) we call them ontological and environmental contextuality. These in turn are associated with two quite distinct senses of nonlocality. We indicate the relevance of our treatment to other related discussions in recent literature on the philosophy of quantum mechanics. (shrink)

Theories of discourse bring to realism new ideas about how knowledge develops and how representations of reality are influenced. We gain an understanding of the conceptual aspect of social life and the processes by which meaning is produced. This collection reflects the growing interest realist critics have shown towards forms of discourse theory and deconstruction. The diverse range of contributions address such issues as the work of Derrida and deconstruction, discourse theory, Eurocentrism and poststructuralism. What unites all of the contributions (...) is a sense that it is essential to provide a realist alternative to the hitherto dominance of social constructionism, hermeneutics and postmodernism, over many of the issues discussed. By developing a realist perspective the different authors attempt to embed discourse within the structured nature of the reality of the world. Realism can situate language, discourse and ideology within context specific, or 'causally efficacious' circumstances. Realism can help to uncover issues of power, representation, and subjectivity and how discursive and other social practices produce real effects. This can help us understand the manner in which social structures are reproduced through various forms of ideology and discourse. At the beginning of the sanatorium era, 80 percent of the prison deaths were attributed to TB. As the mountain air was “commonly known” to be healthful, the first prison sanatorium was opened in the mountains near Dannemora, N.Y. in 1904. It served to isolate contagious prison inmates until the advent of effective chemotherapy for the disease (...) in the 1950’s. Early antibiotic therapy for TB was such a great success that the public health aspects of TB in prisons remained dormant for the next 40 years.In 1991, a correctional officer from Auburn Correctional Facility in Auburn, New York died as a result of multidrug-resistant TB. He had been posted to care for hospitalized patients, from whom he acquired his disease. This death, and the transmission of TB infection to health care workers in the same hospital, brought the nature and extent of modern inmate medical care into finer focus. (shrink)

John Dupre warns that our understanding of human nature is being distorted by two faulty and harmful forms of pseudo-scientific thinking. Not just in the academic world but in everyday life, we find one set of experts who seek to explain the ends at which humans aim in terms of evolutionary theory, while the other set uses economic models to give rules of how we act to achieve those ends. Dupre demonstrates that these theorists' explanations do not work and (...) that, if taken seriously, their theories tend to have dangerous social and political consequences. For these reasons, it is important to resist scientism: an exaggerated conception of what science can be expected to do for us. Dupre restores sanity to the study of human nature by pointing the way to a proper understanding of humans in the societies that are our natural and necessary environments. Integrity ought logically to be a particularly important concept within political science. If those acting within the political system do not have integrity, our ability to trust them, to have confidence in their actions, and perhaps even to consider them legitimate can be challenged. Indeed, the very concept of integrity goes some way towards underwriting positive views of political actors. Yet, despite this importance, political science as a discipline has perhaps focused too little on questions of integrity. Where political science (...) has looked at the subject of integrity, it has often done so without using the specific linguistic formulation “integrity”. Most commonly, the focus has instead been on “corruption”—a strand of research which has produced results that cannot always be translated into discussions of integrity, by virtue of its narrower focus upon the “negative pole” of public ethics. Other measures, such as “Quality of Government”, focus on positive attributes, notably impartiality, but this also fails fully to capture the notion of integrity: dishonesty can be impartial. Specific formal “codes” used within public life and among political practitioners can be much more nuanced than the most widely used measures, and can be much closer to what we understand—academically—as “integrity”. This paper argues that the hard conceptual and empirical work of elaborating integrity into a fully operationalizable concept offers the potential reward of an analytical concept that is more closely aligned with political reality. Intentionality is taken to be the crucial mental phenomenon, and its analysis involves wide-ranging discussions of perception, action, causation, meaning, and reference. Cortical color blindness, or cerebral achromatopsia, has been likened by some authors to ''blindsight'' for color or an instance of ''covert'' processing of color. Recently, it has been shown that, although such patients are unable to identify or discriminate hue differences, they nevertheless show a striking ability to process wavelength differences, which can result in preserved sensitivity to chromatic contrast and motion in equiluminant displays. Moreover, visually evoked cortical potentials can still be elicited in response to chromatic stimuli. We suggest (...) that these demonstrations reveal intact residual processes rather than the operation of covert processes, where proficient performance is accompanied by a denial of phenomenal awareness. We sought evidence for such covert processes by conducting appropriate tests on achromatopsic subject M.S. An ''indirect'' test entailing measurement of reaction times for letter identification failed to reveal covert color processes. In contrast, in a forced choice oddity task for color, M.S. was unable to verbally indicate the position of the different color, but was surprisingly adept at making an appropriate eye movement to its location.
