Recently the confrontation has grown more intense. The ap pointment of clinical ethicists in hospitals and other health care settings is an accelerating trend in North America. Concomitantly, those institutions involved in training peo ple in clinical ethics have added organized exposure to the world of practice, in the form of placement requirements, to the normal academic course load. Theory and Practice is a series of nine lectures that Jacques Derrida delivered at the École Normale Supérieure in 1976 and 1977. The topic of “theory and practice” was associated above all with Marxist discourse and particularly the influential interpretation of Marx by Louis Althusser. Derrida’s many questions to Althusser and other thinkers aim at unsettling the distinction between thinking and acting. Derrida’s investigations set out from Marx’s “Theses on Feuerbach,” in particular the eleventh thesis, which (...) has often been taken as a mantra for the “end of philosophy,” to be brought about by Marxist practice. The opponents of Epicureanism in antiquity, including Cicero, Plutarch and Lactantius, succeeded in establishing a famous cliché: the theoretical and practical disinterest of Epicurus and the Epicureans in political communities. However, this anti-Epicurean literature did not provide considerations of Epicurean political theory or the testimonies about Epicurean lifestyle. Therefore, the purpose of this book is to shed light on the contribution of Epicurean thought to political life in the ancient world. Sidgwick's seminal textThe Methods of Ethicsleft off with an unresolved problem that Sidgwick referred to as the dualism of practical reason. The problem is that employing Sidgwick's methodology of rational intuitionism appears to show that there are reasons to favour both egoism and utilitarianism. Katarzyna de Lazari-Radek and Peter Singer offer a solution in the form of an evolutionary debunking argument: the appeal of egoism is explainable in terms of evolutionary theory. I argue that like rational prudence, rational benevolence (...) is subject to debunking arguments and so problematic, but also – and more importantly – that debunking arguments are irrelevant in the debate over the dualism of practical reason on the view of reason and rational intuitionism that Lazari-Radek and Singer embrace. Either both egoism and utilitarianism are debunked, or neither are. If I am right, Sidgwick's dualism is left standing. (shrink)

In this paper we introduce narrative and hermeneutical perspectives to clinical ethics support services (CESS). We propose a threefold consideration of ‘theory’ and show how it is interwoven with ‘practice’ as we go along. First, we look at theory in its foundational role: in our case ‘narrative ethics’ and ‘philosophical hermeneutics’ provide a theoretical base for clinical ethics by focusing on human identities entangled in stories and on moral understanding as a dialogical process. Second, we consider the (...) role of theoretical notions in helping practitioners to understand their situation in clinical ethics practice, by using notions like ‘story’, ‘responsibility’, or ‘vulnerability’ to make explicit and explain their practical experience. Such theoretical notions help us to interpret clinical situations from an ethical perspective and to foster moral awareness of practitioners. And, thirdly, we examine how new theoretical concepts are developed by interpreting practice, using practice to form and improve our ethical theory. In this paper, we discuss this threefold use of theory in clinical ethics support services by reflecting on our own theoretical assumptions, methodological steps and practical experiences as ethicists, and by providing examples from our daily work. In doing so, we illustrate that theory and practice are interwoven, as theoretical understanding is dependent upon practical experience, and vice-versa. (shrink)

Criticism of the welfare state is mostly economic and administrative, relating to the resultant national debt and state bureaucracy. Budget cuts and privatization may help but not eliminate the difficulty. Yet, the primary concern of the welfare system is neither economic nor administrative; so, the force of this criticism is limited. To restrict the discussion to the defunct free-markets and centralized economies is to distort and to obstruct clear thinking on national priorities. This innovative study focuses on the Kantian theory of international relations, a subject which has frequently been either ignored or misunderstood. Kant was criticized by contemporaries who asserted that his political ideas were idealistic and impractical. He countered this accusation by evolving a political philosophy which formed a link between the theoretical doctrine of pure law and the actualities of the real world. The idea that some things are fine in theory, but do not work in practice, was already an “old saying” when Kant wrote about it in 1793. Kant, who was annoyed that a man named Garve had criticized his ethical theory on this ground, responded by pointing out that there is always a gap between theory and practice. Theory provides general rules but it cannot tell us how to apply them--for that, practical judgment is (...) needed. “[T]he general rule,” said Kant, “must be supplemented by an act of judgement whereby the practitioner distinguishes instances where the rule applies from those where it does not.” This means that those who lack judgment might be helpless, even though they know a lot of theory. “There are doctors and lawyers,” Kant explains, “who did well during their schooling but who do not know how to act when asked to give advice.” The point is especially important for the kind of absolutist ethic that Kant defends. Kant held that moral rules have no exceptions; on his way of thinking, we may never lie, we may never break a promise, and so on. This is a clear example of an ethic that seems not to work in practice, for sensible people recognize that in extreme circumstances even very serious rules may have to be broken. But the “gap” that Kant identifies has often been exploited to soften the impact of such harsh precepts. Traditional Christian ethics, for example, says (like Kant) that suicide is always wrong; but because judgment is required to determine which acts count as suicide, casuists have been able to excuse various sorts of self-destruction (and, not coincidentally, avoid consigning the deceased to hellfire) by classifying them as something else. Thus the hero who sacrifices herself to save others is not a suicide, nor is the man who kills himself while blind with grief, as he lacks the required rational intention. It might be thought that such gaps could be closed by adding further principles to the theory--for example, principles that specify more closely what counts as suicide.... (shrink)

BackgroundClinical ethics support aims to support health care professionals in dealing with ethical issues in clinical practice. Although the prevalence of CES is increasing, it does meet challenges and pressing questions regarding implementation and organization. In this paper we present a specific way of organizing CES, which we have called integrative CES, and argue that this approach meets some of the challenges regarding implementation and organization.MethodsThis integrative approach was developed in an iterative process, combining actual experiences in a case (...) study in which we offered CES to a team that provides transgender health care and reflecting on the theoretical underpinnings of our work stemming from pragmatism, hermeneutics and organizational and educational sciences.ResultsIn this paper we describe five key characteristics of an integrative approach to CES; 1. Positioning CES more within care practices, 2. Involving new perspectives, 3. Creating co-ownership of CES, 4. Paying attention to follow up, and 5. Developing innovative CES activities through an emerging design.ConclusionsIn the discussion we compare this approach to the integrated approach to CES developed in the US and the hub and spokes strategy developed in Canada.
