Type 1 diabetes (T1D) is a chronic illness that requires intensive lifelong management of blood glucose concentrations by means of external insulin administration. There have been substantial developments in the ways of measuring glucose levels, which is crucial to T1D self-management. Recently, continuous glucose monitoring (CGM) has allowed people with T1D to keep track of their blood glucose levels in near real-time. These devices have alarms that warn users about potentially dangerous blood glucose (...) trends, which can often be shared with ther people. CGM is consistently associated with improved glycemic control and reduced hypoglycemia and is currently recommended by doctors. However, due to the costs of CGM, only those who qualify for hospital provision or those who can personally afford it are able to use it, which excludes many people. In this paper, I argue that unequal access to CGM results in: (1) unjust health inequalities, (2) relational injustice, (3) injustice with regard to agency and autonomy, and (4) epistemic injustice. These considerations provide prima facie moral reasons why all people with T1D should have access to CGM technology. I discuss the specific case of CGM policy in the Netherlands, which currently only provides coverage for a small group of people with T1D, and argue that, especially with additional considerations of cost-effectiveness, the Dutch government ought to include CGM in basic health care insurance for all people with T1D. Forecasting blood glucose values for patients can help prevent hypoglycemia and hyperglycemia events in advance. To this end, this study proposes an intelligent ensemble deep learning system to predict BG values in 15, 30, and 60 min prediction horizons based on historical BG values collected via continuous glucose monitoring devices as an endogenous factor and carbohydrate intake and insulin administration information as exogenous factors. Although there are numerous deep learning algorithms available, this study applied five algorithms, (...) namely, recurrent neural network, which is optimized for sequence data, and RNN-based algorithms, stacked LSTM, bidirectional LSTM, and gated recurrent unit). Then, a genetic algorithm was applied to the five prediction models to optimize their weights through ensemble techniques and to yield the final predicted BG values. The performance of the proposed model was compared to that of the autoregressive integrated moving average model as a baseline. The results show that the proposed model significantly outperforms the baseline in terms of the root mean square error and continuous glucose error grid analysis. For the valid 29 diabetic patients for the multivariate models, the RMSE was 11.08, 19.25, and 31.30 mg/DL for 15, 30, and 60 min PH, respectively. When the same data were applied to univariate models, the RMSE was 11.28, 19.99, and 33.13 mg/DL for 15, 30, and 60 min PH, respectively. Both the univariate and multivariate models showed a statistically significant difference compared with the baseline at a 5% statistical significance level. Instead of using a model with a single algorithm, applying a GA based on each output of a model with multiple algorithms was found to play a significant role in improving model performance. (shrink)

In an old paper of our group in Milano a formalism was introduced for the continuous monitoring of a system during a certain interval of time in the framework of a somewhat generalized approach to quantum mechanics. The outcome was a distribution of probability on the space of all the possible continuous histories of a set of quantities to be considered as a kind of coarse grained approximation to some ordinary quantum observables commuting or not. In fact (...) the main aim was the introduction of a classical level in the context of QM, treating formally a set of basic quantities, to be considered as beables in the sense of Bell, as continuously taken under observation. However the effect of such assumption was a permanent modification of the Liouville-von Neumann equation for the statistical operator by the introduction of a dissipative term which is in conflict with basic conservation rules in all reasonable models we had considered. Difficulties were even encountered for a relativistic extension of the formalism. In this paper I propose a modified version of the original formalism which seems to overcome both difficulties. First I study the simple models of an harmonic oscillator and a free scalar field in which a coarse grain position and a coarse grained field respectively are treated as beables. Then I consider the more realistic case of spinor electrodynamics in which only certain coarse grained electric and magnetic fields are introduced as classical variables and no matter related quantities. The revelation that data obtained for the US-based National Surgical Adjuvant Breast and Bowel Project (NSABP) from subjects enrolled at Hôpital Saint-Luc in Montreal was falsified has eroded public trust in research. Institutions can educate researchers and help prevent unethical research practices by establishing procedures to monitor research involving human subjects. Research monitoring encompasses four categories of activity: annual reviews of continuing research, monitoring of informed consent, monitoring of adherence to approved protocols and monitoring of the (...) integrity of data. The authors describe characteristics of research projects that may call for monitoring procedures in each category. The form taken by such monitoring depends on the nature of the protocol. Although appropriate research monitoring requires substantial investment of personnel and financial resources, it is required under guidelines regulating research involving human subjects in Canada. Research monitoring is a step forward in re-establishing public confidence in medical research. (shrink)

The purpose of the study is to develop a monitoring methodology and its tools, taking into account the cyclical nature of the management process in the organizations of the system of continuing professional education. The article reveals the methodological features of the formation and management of the system of continuing professional education. Among the management functions, the monitoring function stands out. The possibilities of monitoring from the standpoint of the planning and management cycle implemented in the organizations (...) of continuing professional education are investigated. The scientific novelty of the work lies in the formalization of the monitoring mechanism and substantiation of the methodological prerequisites for its application in the strategic management of the system of continuing professional education. In the UK, over 70% of AIDS, including new cases, is located in a few Districts in central London where the distribution of previously occurring and new cases is essentially confined to the original risk groups of homosexual/bisexual men, drug addicts of both sexes, and some of their sexual partners and consorts. But control policy is still based on the assumption that HIV has already spread from persons in these risk groups into the general population, and that it will spread (...) hereafter at an increased rate because of heterosexual transmission to cause a widespread epidemic of AIDS.The basis and implications of this policy were investigated in the South East region adjacent to London, Analysis of demographic and epidemiological data shows that, with one exception, there is very little extension of AIDS from affected Districts in London to the surrounding region or even to the suburban fringe. Where AIDS is prevalent, as in this exception, the distribution follows the same, original pattern which relates essentially to risks arising from life-styles. There is no significant association between the prevalence or spread of AIDS and conventional markers of deprivation, economic and social disadvantage.Allocations of personnel, services and expenditure, assessed from returns required under the AIDS Control Act of 1987 and official registration data, continue to follow the original policy assumptions. They are therefore unrelated to the numbers of existing and new cases, disproportionate and unrealistic. Claims that this widespread excess of effort is justified by the fact that AIDS has not spread to the general population are falsified by the continuation of cases almost exclusively in risk groups. There is nothing in the data required under the Act or in registrations in this main locus of AIDS in the UK to suggest any change in this or to justify continuation of current expenditure and redundant activities.There is no evidence in these data that ethnic variations in the resident populations of Districts are associated with variations in the prevalence of AIDS. However, the larger figures available in some of the national data do indicate a disproportionate increase in some minority ethnic groups. A discussion of lessons learned in the first months of the COVID-19 pandemic which allowed data safety monitoring boards (DSMBs) to continue their work protecting the interests of human research participants while preserving research studies.

An embedded automaton is introduced to monitor the whole glycolysis process in pancreatic β-cell and it is a hybridization of both non-deterministic finite automaton and push-down automaton. The set of irreversible and reversible reactions in the glycolysis process are related to non-deterministic finite automaton and push-down automaton respectively. The embedded automaton is used to observe the glucose metabolism with the states of acceptance and rejection. The acceptance state of the embedded automaton depicts the normal level of glycolysis and insulin (...) secretion. The rejection state of this machine shows inhibition of glycolysis which obstructs the secretion of insulin. The subsequent low level of insulin leads to the high blood glucose level also known as hyperglycemia. In this study, the designed machine can be used to regulate the process of glycolysis through a group of regulatory glycolytic enzymes for the treatment of Diabetes mellitus at molecular level. Objective: One way to ensure timely predictions is by using a smart monitoring framework to continuously verify functional and non-functional property violations during the executions of the systems and their components. A panel held at the International Conference on Information Systems, December 5–7, 1993, addressed the importance and ethicality of several issues relating to ethics and information technology use. The substance of the debate and results of audience votes on the issues are presented in this paper as a means of initiating a broader debate on the issues, for it is with debate that we reach a group consensus on acceptable behavior and practice. With consensus, we can begin to develop codes (...) and policies that are feasible and practical for ethical computer use.Specific dilemmas debated involved the issues of privacy and ownership, including the ethicality of using company resources for personal use and monitoring compliance to company policies about computer use. In general, we found little consensus about ethicality of any of the types of conduct, although we found a high degree of consensus that the debated issues were important and should continue to be discussed. The final question concerned policies and codes. While policies and codes were believed to be necessary, they were also perceived as ineffective. Several suggestions for practical action to enhance the efficacy of ethical codes are presented. (shrink)

The Alice Books, full of illogical thoughts, words, and contradictions, were unrivaled entertainment until the publication of the medical literature promoting electronic fetal monitoring for every pregnancy. The modern-day EFM advocates acknowledge EFM’s decades long failure but simultaneously recommend EFM use for lawsuit protection and because the profession has used EFM for every pregnancy for fifty years, therefore, it must be efficacious. These self-indulgent, illogical rationalizations ignore the half century of evidence-based scientific research proving that EFM is a complete (...) failure as well as ignoring the fact that continued EFM use violates the fundamental principles of modern bioethics. This blind advocacy perpetuates four pernicious EFM harms occurring to mothers, babies, and the medical profession itself. This article sets out these four EFM harms with the goal of abolishing the misguided, illogical, contradictory, arguments used by the twenty-first century EFM Lewis Carroll mimics. (shrink)

Data Safety and Monitoring Boards have been referred to as a “growth industry,” and this trend continues to be fueled by recent FDA guidance and the NIH's requirement that DSMBs be employed in virtually all phase III clinical trials. The widening role of DSMBs has been sporadically questioned on ethical grounds, but growth has continued, despite the fact that many of the questions endure, unanswered, save for repeated references to safeguarding the scientific integrity of trials. This may be about (...) to change. The recently appointed director of the Office for Human Research Protections , Jerry Menikoff, is on record as regarding current practices — where consent forms often promise what the DSMB has been assembled to specifically not provide — as constituting fraudulent behavior. That is, a subject may inherently rely on, to their detriment, information that has been misrepresented in the consent document. In this paper, we assemble some of the enduring questions and top them off with Menikoff's tour de force to present what we hope will be a compelling argument to require that consent forms fairly represent what the DSMB will do — and not do — with trial data as they accumulate. We argue that DSMBs should be used only in rare circumstances, and question the practice of precluding principal investigators from DSMB membership, but our main thrust is to ensure that DSMBs, when used at all, are properly described in trial consent forms. In this paper, we propose an Internet of Things to enhancing the experience of personal gardening as a method of therapy for mental-health patients, given a belief in its role in a person’s mood and general positivity, The proposed prototype continuously senses and monitors the state of an indoor plant through different sensors. The user is notified of the plant’s needs for water, sunlight, etc., through generated notifications from channels over cloud in-real time. Continued concern for animal welfare may be alleviated when welfare would be monitored on farms. Monitoring can be characterized as an information system where various stakeholders periodically exchange relevant information. Stakeholders include producers, consumers, retailers, the government, scientists, and others. Valuating animal welfare in the animal-product market chain is regarded as a key challenge to further improve the welfare of farm animals and information on the welfare of animals must, therefore, be assessed objectively, for instance, through monitoring. Interviews (...) with Dutch stakeholder representatives were conducted to identify their perceptions about the monitoring of animal welfare. Stakeholder perceptions were characterized in relation to the specific perspectives of each stakeholder. While producers tend to perceive welfare from a production point of view, consumers will use visual images derived from traditional farming and from the animals’ natural environments. Scientists’ perceptions of animal welfare are affected by the need to measure welfare with quantifiable parameters. Retailers and governments (policy makers) have views of welfare that are derived from their relationships with producers, consumers, non-governmental organizations (NGOs), and scientists. All interviewed stakeholder representatives stated that animal welfare is important. They varied in the extent to which they weighted economic considerations relative to concern for the animals’ welfare. Many stakeholders emphasized the importance of communication in making a monitoring system work. Overall, the perspectives for the development of a sustainable monitoring system that substantially improves farm animal welfare were assessed as being poor in the short term. However, a reliable system could be initiated under certain conditions, such as integrated chains and with influential and motivated stakeholders. A scheme is described with attention points for the development of sustainable monitoring systems for farm animal welfare in the long term. (shrink)

Artificial intelligence (AI) has garnered tremendous attention in health care, and many hope that AI can enhance our health system’s ability to care for people with chronic and degenerative conditions, including Parkinson’s Disease (PD). This paper reports the themes and lessons derived from a qualitative study with people living with PD, family caregivers, and health care providers regarding the ethical dimensions of using AI to monitor, assess, and predict PD symptoms and progression. Thematic analysis identified ethical concerns at four intersecting (...) levels: personal, interpersonal, professional/institutional, and societal levels. Reflecting on potential benefits of predictive algorithms that can continuously collect and process longitudinal data, participants expressed a desire for more timely, ongoing, and accurate information that could enhance management of day-to-day fluctuations and facilitate clinical and personal care as their disease progresses. Nonetheless, they voiced concerns about intersecting ethical questions around evolving illness identities, familial and professional care relationships, privacy, and data ownership/governance. The multi-layer analysis provides a helpful way to understand the ethics of using AI in monitoring and managing PD and other chronic/degenerative conditions. (shrink)

Open-content communities that focus on co-creation without requirements for entry have to face the issue of institutional trust in contributors. This research investigates the various ways in which these communities manage this issue. It is shown that communities of open-source software—continue to—rely mainly on hierarchy (reserving write-access for higher echelons), which substitutes (the need for) trust. Encyclopedic communities, though, largely avoid this solution. In the particular case of Wikipedia, which is confronted with persistent vandalism, another arrangement has been pioneered instead. (...) Trust (i.e. full write-access) is ‘backgrounded’ by means of a permanent mobilization of Wikipedians to monitor incoming edits. Computational approaches have been developed for the purpose, yielding both sophisticated monitoring tools that are used by human patrollers, and bots that operate autonomously. Measures of reputation are also under investigation within Wikipedia; their incorporation in monitoring efforts, as an indicator of the trustworthiness of editors, is envisaged. These collective monitoring efforts are interpreted as focusing on avoiding possible damage being inflicted on Wikipedian spaces, thereby being allowed to keep the discretionary powers of editing intact for all users. Further, the essential differences between backgrounding and substituting trust are elaborated. Finally it is argued that the Wikipedian monitoring of new edits, especially by its heavy reliance on computational tools, raises a number of moral questions that need to be answered urgently. ?In everday language, the word ?attention? implies control of access to consciousness, and we adopt this usage here. Attention itself can be either voluntary or automatic. This can be readily modeled in the theory. Further, a contrastive analysis of spontaneously self?attributed vs. self?alien experiences suggests that ?self? can be interpreted as the more enduring, higher levels of the dominant context hierarchy, which create continuity over the changing flow of events. Since context is by definition unconscious in GW theory, self in (...) this sense is thought to be inherently unconscious as well. This proposal is consistent with a great deal of objective evidence. However, aspects of self may become known through ?conscious self-monitoring,? a process that ??is useful for self-evaluation and self?control. The results of conscious self-monitoring are combined with self?evaluation criteria, presumably of social origin, to produce a stable ?self?concept?, which functions as a supervisory system within the larger self organization. Patient self-management of disease is increasingly supported by technologies that can monitor a wide range of behavioural and biomedical parameters. Incorporated into everyday devices such as cell phones and clothes, these technologies become integral to the psychosocial aspects of everyday life. Many technologies are likely to be marketed directly to families with ill members, and families may enlist the support of clinicians in shaping use. Current ethical frameworks are mainly conceptualised from the perspective of caregivers, researchers, developers and regulators in (...) order to ensure the ethics of their own practices. This paper focuses on families as autonomous decision-makers outside the regulated context of healthcare. We discuss some morally relevant issues facing families in their decisions to monitor the health-related behaviours of loved ones. An example – remote parental monitoring of adolescent blood glucose – is presented and discussed through the lens of two contrasting accounts of ethics; one reflecting the predominant focus on health outcomes within the health technology assessment (HTA) framework and the other that attends to the broader sociocultural contexts shaping technologies and their implications. Issues discussed include the focus of assessments, informed consent and child assent, and family co-creation of system characteristics and implications. The parents’ decisions to remotely monitor their child has relational implications that are likely to influence conflict levels and thus also health outcomes. Current efforts to better integrate outcome assessments with social and ethical assessments are particularly relevant for informed decision-making about health monitoring technologies in families. For a better detection in Network information security monitoring system, the author proposes a method based on adaptive depth detection. A deep belief network was designed and implemented, and the intrusion detection system model was combined with a support vector machine. The data set adopts the NSL-KDD network communication data set, and this data set is authoritative in the security field. Redundant cleaning, data type conversion, normalization, and other processing operations are performed on the data set. Using the data (...) conversion method based on the probability mass function probability mass function coding, a standard data set with low redundancy and low dimensionality can be obtained. Research indicates that when the batch size reaches 64, the accuracy of the test set reaches its maximum value. As the batch size increases, the accuracy first increases and then decreases. When the batch size continues to increase, the model will inevitably fall into the local optimal state, resulting in the degradation of the detection performance of the system. In terms of the false alarm rate, the DBN-SVM model is also the highest; however, it is only 10.73%. Under the premise of increasing the detection rate, the false alarm rate is improved; for the overall detection performance of the model, it is within an acceptable range. In terms of accuracy, the DBN-SVM model also scored the highest. The accuracy rate is the ratio of normal and correct classification for intrusion detection. It can explain the detection ability of the model. In summary, the overall detection ability of the DBN-SVM model is the best. The good classification ability to use SVM is proved, and the classification of low-dimensional features is expected to increase the detection rate of the system. (shrink)

Summary The present work focuses on the transformations of the psychotherapeutic field through the relationship dynamics which occur within it. The first part of this article starts with a brief outline of the Gestalt psychological understanding of the field concept, also in its application to the psychotherapeutic situation, followed by a brief review of the introduction of the field concept into the psychoanalytic theory formation. After this, the first author first presents the theoretical concept underlying a new approach he has (...) developed for observing the relationship dynamics in psychotherapy. Mirroring a formation of both psychoanalytic and Gestalt theory of the main author, this new approach is based on the combination of psychoanalytic and Gestalt psychological concepts. According to the clinical experience and insights of the author, the phenomenological and relational approach of Gestalt theory fits well with the psychoanalytic approach; on this basis, a criterion for recording the progress of therapy can be developed. This criterion is the phenomenology of the development of the qualities of the relationships of the client, as they become visible in his dream narrations and the subsequent associations in the analysis room and continue to develop during the session and the further course of therapy. The relationship dynamics in the dream narration is thus compared with those which develop in the course of the subsequent associations. This is demonstrated and further elaborated in the second part of this article on the basis of a clinical case. The clinical example shows how the relationship dynamics develop in this sense in the individual therapy sessions and over a longer course of therapy. The associated transformations of the therapeutic field give a good indication of the progress of therapy. The main author gained such insights into the transformations of the therapeutic field and the progression of therapy, which are visible in the course of therapy, from the careful application of the criterion “MDAC of relational dynamics”. In the specific case, there was also a high degree of correspondence between the results of the application of this phenomenological criterion and the empirical evidence of the symptom questionnaire, a self-report measure requested by the patient himself during the course of the therapy. The article deals with the Dawson College Massacre, focusing on the story of Kimveer Gill, a 25-year-old man from Laval, Montreal who wished to murder young students in Dawson College. It is argued that the international community should continue working together to devise rules for monitoring specific Internet sites, as human lives are at stake. Preemptive measures could prevent the translation of murderous thoughts into murderous actions. Designated monitoring mechanisms of certain websites that promote violence and seek legitimacy (...) as well as adherents to the actualization of murderous thoughts and hateful messages have a potential of preventing such unfortunate events. Our intention is to draw the attention of the multifaceted international community to the shared interest and need in developing monitoring schemes for certain websites, in order to prevent hideous crimes. This study examines whether the delegated monitoring of main banks effectively decreases severe agency problems. For example, this includes accounting fraud in bank-dominated corporate governance. In this context, the fraud triangle specifies the three main factors of opportunity, incentive, and rationalization. Main banks may reduce the factor of opportunity through actions such as monitoring, which plays a moderating role by reducing the potential for managerial misconduct, whereas, the incentive factor may be enhanced through the subsequent pressure that influences (...) managers to force turnover when they do not repay debts. The potential influence that the main banks have on the fraud rationalization factor may be stronger in firms that are more dependent on main-bank borrowing. This study analyzed data from publicly listed firms in Japan for the period between 2008 and 2016 to determine how main-bank relationships affected accounting fraud, thus allowing an assessment of delegated monitoring practices. Findings showed that main-bank relationships were helpful for reducing accounting fraud, suggesting its overall effectiveness in delegated monitoring. Results also showed that large audit firms did not necessarily prevent accounting fraud, which implies that main banks can employ their own monitoring as a substitute for the roles typically played by these firms. Here, the main policy implication is that main-bank relationships may strengthen business ethics in the context of bank-dominated corporate governance. In conclusion, the continued analysis of bank monitoring may benefit stakeholders while strengthening business ethics in bank-centered economies. (shrink)

In this paper, through research and analysis of the communication network of the physical activity monitoring system, we combine wearable technology and identification technology and design a physical health monitoring bracelet that integrates multifaceted physical data collection and effective identity matching function. We match the identity through the chip and collect the physical fitness data generated in the process of exercise and centralized test by the sensor in real-time. Finally, the data transmission is realized through the WIFI communication (...) function to achieve the purpose of monitoring physical exercise and improving physical quality. To ensure the continuity and stability of information transmission, the joint transmission method of direct transmission and indirect transmission is essential. The safety sign is important in our daily life and workplace to prevent potential safety issues. However, it remains undetermined whether the safety signs would influence the cognitive control ability of the people, which serves to guide the behaviors in a goal-directed manner. Therefore, this study aimed to examine the effect of safety signs on cognitive control by uncovering the behavioral performance and neural manifestations underlying the monitoring of conflict and error. The participants performed a flanker task after watching (...) low- and high-hazard safety signs with the electroencephalogram data recorded continually. The behavioral results indicated a classic congruency effect with higher accuracy rate and faster response time under a congruent condition compared to an incongruent condition. However, no hazard effect on behavioral performances was observed. The results of event-related potentials demonstrated a more negative N2 elicited by the incongruent trials and an increased ERN difference between the error and correct responses in the high-hazard condition compared to those in the low-hazard condition, implying that the monitoring of the conflict and error were both enhanced after watching the high-hazard safety signs. This study contributes to the understanding of the relationship between safety signs and cognitive control, and further expand the measurements that can be applied to assess the effectiveness of safety signs design. The article critically considers the role of NGOs at the US naval base in Guantánamo Bay, Cuba. On the basis of observation of pre-trial hearings for the case against Khalid Sheik Mohammed et al.—those allegedly responsible for the September 11 attacks—the article analyses NGOs as trial monitors of the US military commissions set up to deal with ‘alien unprivileged enemy belligerents’. In spite of continued efforts by human rights NGOs and incremental improvements in the military commissions’ institutional arrangements and practice, (...) the article shows how NGOs have become so much a part of the everyday operation of justice at ‘Gitmo’ that they legitimate the military commissions’ claim to be delivering fair and transparent justice. As research involving gene editing continues to advance, we are headed in the direction of being able to modify the human germline. Should we reach a point where an argument can be made that the benefits of preventing unborn children and future generations from inheriting genetic conditions that cause tremendous suffering outweigh the risks associated with altering the human germline, the next step will be to design clinical trials using this technology in humans. These clinical trials will likely require careful (...) follow-up and monitoring of future generations born with altered genes. This paper addresses some of the ethical issues raised by intergenerational monitoring and sets out to show that these issues can be avoided with careful consideration and clinical trial design. Summary The present work focuses on the transformations of the psychotherapeutic field through the relationship dynamics that occur within it. The first part of this article starts with a brief outline of the Gestalt psychological understanding of the field concept, also in its application to the psychotherapeutic situation, followed by a brief review of the introduction of the field concept into the psychoanalytic theory formation. After this, the first author first presents the theoretical concept underlying a new approach he has (...) developed for observing the relationship dynamics in psychotherapy. Mirroring a formation both psychoanalytic and gestaltic of the main author, this new approach is based on the combination of psychoanalytic and Gestalt psychological concepts. According to the clinical experience and insights of the author, the phenomenological and relational approach of Gestalt theory fits well with the psychoanalytic approach; on this basis, a criterion for recording the progress of therapy can be developed. This criterion is the phenomenology of the development of the qualities of the relationships of the client, as they become visible in his dream narrations and the subsequent associations in the analysis room, and continue to develop during the session and the further course of therapy. The relationship dynamics in the dream narration is thus compared with those that develop in the course of the subsequent associations. This is demonstrated and further elaborated in the second part of this paper on the basis of a clinical case. The clinical example shows how the relationship dynamics develop in this sense in the individual therapy sessions and over a longer course of therapy. The associated transformations of the therapeutic field give a good indication of the progress of therapy. The main author gained such insights into the transformations of the therapeutic field and the progression of therapy, which are visible in the course of therapy, from the careful application of the criterion “manifest dream/associations comparison of relational dynamics”. In the specific case, there was also a high degree of correspondence between the results of the application of this phenomenological criterion and the empirical evidence of the symptom questionnaire, a self-report measure requested by the patient himself during the course of the therapy. Barzykowski and Moulin's model proposes that déjà vu and involuntary autobiographical memories are the result of a continuously active memory system that tracks the novelty of situations. Déjà vu would only have episodic content and concern interpretation of prior experiences. We argue that these aspects of the model would gain to be clarified and explored further and we suggest possible directions.

The topic of offender DNA databases and the retention of offender's DNA samples are issues that continue to generate debate between privacy advocates and forensic DNA scientists. Considered previously by both the Federal DNA Advisory Board and the National Commission on the Future of DNA Evidence, recommendations were made that the existing statutory frameworks were sufficient to protect the confidentiality and use of the DNA samples solely for forensic identification purposes and that the issue be revisited. Now, approximately five years (...) since these debates, privacy advocates and forensic scientists retain divergent views on this issue. This brief essay is not intended to fuel the debate, but rather, to describe the protections and safeguards currently in place for the protection of the offender DNA samples.Surrounding the discussion of the sample retention issue are the underlying principles for the establishment of the Combined DNA Index System Program and the National DNA Index System. Decoherence is the name for the complex of phenomena leading to appearance of classical features of quantum systems. In the present paper decoherence in continuous measurements is analyzed with the help of restricted path integrals (RPI) and (equivalently in simple cases) complex Hamiltonians. A continuous measurement results in a readout giving information in the classical form on the evolution of the measured quantum system. The quantum features of the system reveal themselves in the variation of possible measurement readouts. (...) For example, the monitoring energy of a multi-level system may visualize a transition between levels as a process evolving in time but with an unavoidable quantum noise. Decoherence of a continuously measured system is completely determined by the measurement readout, i.e., by the information recorded in its environment. It is shown that the ideology of RPI makes the Feynman version of quantum mechanics closed, contrary to the conventional operator form of quantum mechanics which needs quantum theory of measurement as a necessary additional part. Experiences of substitute decision-makers with requests for consent to non-therapeutic research participation during the dying process, including to what degree such requests are perceived as burdensome, have not been well described. In this study, we explored the lived experiences of family members who consented to non-therapeutic research participation on behalf of an imminently dying patient. We interviewed 33 family members involved in surrogate research consent decisions for dying patients in intensive care. Non-therapeutic research involved continuous physiological monitoring of (...) dying patients prior to and for 30 min following cessation of circulation. At some study centres participation involved installation of bedside computers. At one centre electroencephalogram monitoring was used with a subset of participants. Aside from additional monitoring, the research protocol did not involve deviations from usual end-of-life care. Thematic analysis of interviews suggests most family members did not perceive this minimal-risk, non-therapeutic study to affect their time with patients during the dying process, nor did they perceive research consent as an additional burden. In our analysis, consenting for participation in perimortem research offered families of the dying an opportunity to affirm the intrinsic value of patients’ lives and contributions. This opportunity may be particularly important for families of patients who consented to organ donation but did not proceed to organ retrieval. Our work supports concerns that traditional models of informed consent fail to account for possible benefits and harms of perimortem research to surviving families. Further research into consent models which integrate patient and family perspectives is needed. All data relevant to the study are included in the article. (shrink)

When information on a coma patient’s expected outcome is uncertain, a moral dilemma arises in clinical practice: if life-sustaining treatment is continued, the patient may survive with unacceptably poor neurological prospects, but if withdrawn a patient who could have recovered may die. Continuous electroencephalogram-monitoring is expected to substantially improve neuroprognostication for patients in coma after cardiac arrest. This raises expectations that decisions whether or not to withdraw will become easier. This paper investigates that expectation, exploring cEEG’s impacts when (...) it becomes part of a socio-technical network in an Intensive Care Unit. Based on observations in two ICUs in the Netherlands and one in the USA that had cEEG implemented for research, we interviewed 25 family members, healthcare professionals, and surviving patients. The analysis focuses on the way patient outcomes are constructed, the kind of decision support these outcomes provide, and how cEEG affects communication between professionals and relatives. We argue that cEEG can take away or decrease the intensity of the dilemma in some cases, while increasing uncertainty for others. It also raises new concerns. Since its actual impacts furthermore hinge on how cEEG is designed and implemented, we end with recommendations for ensuring responsible development and implementation. (shrink)

Despite a widely agreed necessity for agroecological transition, there are various substantial constraints that hinder the adoption of alternative, more sustainable, practices. We employ the niche management concept to examine the initial phases of transition in the local wine-growing niche in Israel, or specifically, the replacement of herbicides with the use of cover-crops combined with the practice of mowing of herbaceous growth using specialized trimming machines. Our goal is to uncover the triggers, drivers and agents of change in farming practices (...) within the agroecological system in which it happened, despite entrenched, widely recognized risk perceptions among farmers. The problem-oriented research design revolved around a transdiciplinary team that included ecologists and social scientists. We identified three groups of frontrunners who initiated, developed and sustained the adoption of the more sustainable agricultural practices. The two-year study focused on interviewing vineyard farmers who were transitioning to cover crops, and this qualitative research was paralleled with continuous ecological monitoring. The farmers believed that the risks inherent in the new practice, if any, were preventable and manageable. The transdisciplinary research design, which integrated farmers into the social and ecological research process, further assisted in facilitating a transition by creating a forum for stimulating communication between diverse stakeholders during the transition, increasing trust between actors from the various sectors and identifying key issues of concern (e.g., risk and associated costs) to better align the motives of farmers, wine-makers, and ecologists. The research design may have also contributed to the adoption of a more holistic view of the agroecological system by various stakeholders and the belief that one cultivation method clearly benefits the system, particularly in the long-term. We therefore conclude with key-insights on the role of the research itself in agroecological transition processes. (shrink)

I entered the room where one of the residents was seeing a patient that we will call Patty, a 32-year-old long-standing epilepsy patient. Patty was sitting in the chair with her husband by her side. She started screaming at me, complaining that she was tired of feeling sick and unable to do anything. (...) She had been in and out of emergency rooms with a variety of complaints, from passing out to dizziness with nausea and vomiting. She was visibly angry and started cursing me with direct expletives and blaming me for all her ailments.Her husband was quiet and, at times, would cover his face as if he was embarrassed. I felt an initial burst of anger. I felt my face getting red. I made a great effort to stay quiet, leaned on the exam table facing them, lowered my voice, and braced to take the barrage of insults that were coming my way. I had to make a strong conscious effort to control my emotions and let her vent. I felt that if I responded in kind, I would inflame the situation. Moreover, a resident and a student working with me that day were also present in the room, and I felt it was my duty to behave professionally, so I sat and tried to do active listening with the idea of offering solutions to her complaints. I am not sure how long the event lasted, but it seemed like an eternity. As she slowed down, I apologized for making her feel not cared for, repeated some of her complaints, and tried to offer solutions to them, but she would not have it and did not accept anything I said. She wanted to get her prescriptions and leave the clinic. I offered to have one of my colleagues do the follow-up as she really did not want to see me anymore.Patty is one of those patients that when you see the name in the schedule, you cringe and tell yourself, it is going to be a long clinic day. She could be in the group of patients who are considered very difficult. Before me, she had been seen by several other neurologists. They likely also preferred not to see her on the schedule because she is one of the 20–30% of patients that have a type of functional psychogenic disorder that becomes more complicated because of the coexistence of epileptic seizures along with non-epileptic psychogenic events. The latter is a more problematic event because it does not respond to pharmacological treatment and causes a lot of disability. The best way to establish whether an event is a true epileptic event or a non-epileptic psychogenic event is to do continuous electroencephalographic monitoring to capture as many events as possible on video. I tried different combinations of medications to no avail and with significant side effects. I tried to set her up for electroencephalogram home monitoring to tease the epileptic and psychogenic events apart. In truth, her social circumstances, insurance coverage, and economic situation made it impossible to do so. Every time she came to the clinic, she had a multitude of complaints, and she would ask for a medication to treat the problem. Any mention of the potential psychogenic nature of her problems would result in a defensive attitude "so you are saying I am crazy." We discussed multiple times the need to be careful with the medications and the potential bad side effects. I wanted to help her, but with every visit, there seemed to be a barrier, and the visit would end on bad terms. At least, that is how I felt. I was left every time, with a feeling of frustration. I believe that she left the clinic feeling the same. That day, I left the clinic upset and depressed. I tried to convince myself I had done all I could to help her. The empirical basis for self-control in Dohsa-hou as it relates to effects on cognitive processes has been explored in a few studies of the Japanese psychotherapy, but not under standardized conditions with a strong predictive theory of control. This study reports on a series of experiments with the Dual Mechanisms of Control framework to clarify the possible regulatory mechanism of Dohsa-hou by focusing on shoulder movement, a key body movement task used by practitioners across applied settings. Cognitive control was operationalized (...) with the AX version of Continuous Performance Test paradigm for proactive control and a modified Stroop task paradigm for reactive control in a 3-arm parallel group trial study design. Healthy Japanese university students were assigned to a Dohsa-hou group that performed a shoulder movement task for few minutes, an active control group that performed a similar task, or a passive control group comprised of a resting condition. A total of 55 participants performed the AX-CPT and 57 participants performed the modified Stroop task before and after the group manipulation. In the AX-CPT, an increase in the error rate of AY trial from pre- to post-test was observed in the passive control group only, and found to be marginally higher in the passive control group relative to Dohsa-hou group at post-test. This indicated that Dohsa-hou moderated the activation of proactive control by repeated AX-CPT performance. The error rate of the Proactive Behavioral Index did not differ from zero at post-test only in the Dohsa-hou group, indicating flexible cognitive control. In the modified Stroop task, there was no difference between congruent and incongruent trials at post-test for the Dohsa-hou group only, indicating the facilitation of reactive control. The evidence for a balancing effect for the Dohsa-hou-based shoulder movement task indicates that clients experience a form of continuous self-monitoring, which might reduce mind-wandering from their focus on movement execution combined with iterative verbal feedback from the therapist. Overall, the results of the present study suggest that the self-regulatory mechanism promoted in clinical Dohsa-hou emphasizes guided shifts in attention to the reactive mode toward a balance of cognitive control. The paper reviews the military policy of a new independent Republic of Tajikistan over 1991-2011. The smallest by territory Central Asian republic lived through a five-year civil war on its way to an independent sovereign democratic state which seven million people were wise enough to reach national reconciliation in 1997. The majority of Tajikistan population is on the verge of poverty, which is indicative of the inadequate social policy. The country is rich in Pamir water which is drawn from eleven (...) glaciers as well as cotton, electric energy and aluminum. Tajikistan secures its military power owing to cooperation with the CIS, the Collective Security Treaty Organization and the Shanghai Cooperation Organization. The Republic develops all-embracing cooperation with such Asian countries as India, Pakistan, Turkey, China and Iran. Among European countries it is Germany that allocated funds for strengthening the border with Afghanistan, which notably stands out. Cooperation with the NATO within the Partnership for Peace Program helps the military of the Republic to dispose of outdated ammunition as well as take part in NATO peacekeeping operations in accordance with resolutions of the United Nations Security Council. The Russian Federation having the largest foreign military alignment in the territory of the Republic remains the main strategic partner and guarantor of the national sovereignty of Tajikistan. The Republic still faces the problems of energy resources, terrorism and international drug trafficking. The internal political situation requires continuous international monitoring. UN observers left the country in 2000. The authoritarian government does not contribute to development of the democratic foundation of civil society. Today's Armed Forces of Tajikistan are of insignificant numerical strength and poorly-armed. The Army needs modernization and adaptation to a present-day environment. (shrink)

The brain's reliance on glucose as a primary fuel source is well established, but psychological models of cognitive processing that take energy supply into account remain uncommon. One exception is research on self-control depletion, where debate continues over a limited-resource model of self-control depletion. This model argues that transient reduction in self-control after exertion of prior self-control is caused by the depletion of brain glucose, and that self-control processes are special, perhaps unique, in this regard. This model has (...) been argued to be physiologically implausible in several recent reviews. This paper attempts to correct some inaccuracies that have occurred during debate over the physiological plausibility of this model. We contend that not only is such limitation of cognition by constraints on glucose supply plausible, it is well established in the neuroscience literature across several cognitive domains. Conversely, we argue that there is no evidence that self-control is special in regard to its metabolic cost. Mental processes require physical energy, and the body is limited in its ability to supply the brain with sufficient energy to fuel mental processes. This article reviews current findings in brain metabolism and seeks to resolve the current conflict in the field regarding the physiological plausibility of the self-control glucose-depletion hypothesis. Neonatal brain monitoring in the neonatal intensive care units (NICU) requires a continuous review of the spontaneous cortical activity, i.e., the electroencephalograph (EEG) background activity. This needs development of bedside methods for an automated assessment of the EEG background activity. In this paper, we present development of the key components of a neonatal EEG background classifier, starting from the visual background scoring to classifier design, and finally to possible bedside visualization of the classifier results. A dataset with 13,200 (...) 5-minute EEG epochs (8–16 channels) from 27 infants with birth asphyxia was used for classifier training after scoring by two independent experts. We tested three classifier designs based on 98 computational features, and their performance was assessed with respect to scoring system, pre- and post-processing of labels and outputs, choice of channels, and visualization in monitor displays. The optimal solution achieved an overall classification accuracy of 97% with a range across subjects of 81–100%. We identified a set of 23 features that make the classifier highly robust to the choice of channels and missing data due to artefact rejection. Our results showed that an automated bedside classifier of EEG background is achievable, and we publish the full classifier algorithm to allow further clinical replication and validation studies. Seeking the possible neural correlates of ubiquitous temporal awareness, this article reexamines fMRI data from off-task “default mode” periods in 25 healthy subjects studied by Grady et al. , 2005). “Brain reading” using support vector machines detected information specifying elapsed time, and further analysis specified distributed networks encoding implicit time. These networks fluctuate; none are (...) continuously active during DM. However, the aggregate regions of greatest variability closely resemble the default mode network. The main task of investment policy's monitoring are the organization of continuous observation of the object of study, formation of informative indicators; assessment and analysis of the information received, forecasting development and providing recommendations to avoid or reduce adverse effects on the object of study. It is proved that a monitoring system should include investment and provide flexibility and adaptability tools. There (...) is studied foreign experience of monitoring investment climate and defined principles of monitoring systems in accordance with the concept of sustainable development for an effective system of monitoring investment policy. The system of investment policy's monitoring as is a set of multiple monitoring systems, the implementation of which will accelerate the implementation of sustainable development in the domestic financial system. Continuing the debate on assessment, we argue that Andrew Davis' use of examples fails to address issues in the assessibility of specific items of knowledge, and show that one of his examples supports our view rather than his. We argue that John White's preferred replacement of assessment by monitoring, based on teachers' personal knowledge of their pupils, confuses personal and professional knowledge, oversimplifies the teacher's role and does not address the need for objectivity. Finally we argue that neither White (...) nor Davis grasps the relationship between assessment and the need for wider forms of accountability for public education systems. (shrink)
