BackgroundAlthough the Life-Sustaining Treatment (LST) Decision Act was enforced in 2018 in Korea, data on whether it is well established in actual clinical settings are limited. Hospital-acquired pneumonia (HAP) is a common nosocomial infection with high mortality. However, there are limited data on the end-of-life (EOL) decision of patients with HAP. Therefore, we aimed to examine clinical characteristics and outcomes according to the EOL decision for patients with HAP.MethodsThis multicenter study enrolled patients with HAP at 16 referral hospitals retrospectively from (...) January to December 2019. EOL decisions included do-not-resuscitate (DNR), withholding of LST, and withdrawal of LST. Descriptive and Kaplan–Meier curve analyses for survival were performed.ResultsOf 1,131 patients with HAP, 283 deceased patients with EOL decisions (105 cases of DNR, 108 cases of withholding of LST, and 70 cases of withdrawal of LST) were analyzed. The median age was 74 (IQR 63–81) years. The prevalence of solid malignant tumors was high (32.4% vs. 46.3% vs. 54.3%, P = 0.011), and the ICU admission rate was lower (42.9% vs. 35.2% vs. 24.3%, P = 0.042) in the withdrawal group. The prevalence of multidrug-resistant pathogens, impaired consciousness, and cough was significantly lower in the withdrawal group. Kaplan–Meier curve analysis revealed that 30-day and 60-day survival rates were higher in the withdrawal group than in the DNR and withholding groups (log-rank P = 0.021 and 0.018). The survival of the withdrawal group was markedly decreased after 40 days; thus, the withdrawal decision was made around this time. Among patients aged below 80 years, the rates of EOL decisions were not different (P = 0.430); however, mong patients aged over 80 years, the rate of withdrawal was significantly lower than that of DNR and withholding (P = 0.001).ConclusionsAfter the LST Decision Act was enforced in Korea, a DNR order was still common in EOL decisions. Baseline characteristics and outcomes were similar between the DNR and withholding groups; however, differences were observed in the withdrawal group. Withdrawal decisions seemed to be made at the late stage of dying. Therefore, advance care planning for patients with HAP is needed. (shrink)

Changing land-use patterns and amenity-driven migration have brought agriculture back into people’s lives, but there is a disconnection between the realities of production agriculture and romantic images attached to farming. To the extent that “rurality” is attached to farming, people may desire to live in rural places, but they may be unprepared for the realities of living near a working farm. Greater numbers of communities are facing “either/or” outcomes regarding the conversion of “open space” land to residential or commercial uses (...) versus landscape preservation. This study explored the perceptions and preferences of a community regarding the conversion of a hypothetical parcel of open space to a working dairy or to a residential subdivision. Results suggest that the opportunity costs of foregoing open space for residential development are high, with implications for valuing the conservation of traditions that are tied to the land versus conversion of land solely for development purposes. In an emerging field such as social enterprise, it is important for an organization to secure legitimacy to obtain resources and sustain its business. Specifically, when a government distributing subsidies does not have adequate information to decide which organization is trustworthy, it is the legitimacy-seeking activities of social enterprise that decides who receives a subsidy; this, in turn, decides which organization will survive. One of the most effective ways to gain legitimacy is to explicitly emphasize in the public promotion that (...) the organization devotes to its social mission. In the case of Work Integration Social Enterprises (WISEs), an organization emphasizes its social employment of the disadvantaged individuals. However, we argue that social enterprises’ public promotion that emphasizes social employment can lower the expected wage, job satisfaction, and organizational commitment of the employees who are hired due to their disadvantaged social status. This is because such obvious promotional messages makes the employees more keenly aware of their disadvantaged status; as a result, this reinforces their self-prejudice that they are not competitive enough in the labor market. We test our hypotheses in the context of South Korean WISEs and found general support for our arguments. (shrink)

In this book of telling points and clear visions, Hyun-Eung discusses East Asian Buddhist traditions in light of Western-philosophical perspectives and presents his views on the theory and praxis in contemporary Buddhism in a way that Western readers can easily understand. East (...) Asian Buddhist philosophy has had little introduction to English-speaking societies compared with the philosophies of other traditions in Buddhism. This book provides penetrating insight into such major issues of East Asian Mahayana Buddhism as enlightenment, compassion, emptiness, and sudden enlightenment in the Zen traditions. Hyun-Eung s approach is creative, clear, logical, and methodologically modernized. Hyun-Eung s Buddhist philosophy of history is not a mere product of armchair philosophy. He wrote most essays in this book during the 1980 s while South Korea was undergoing serious political turmoil as it tried to overthrow a government backed by military generals. This book presents a philosophy that evolved from his contemplations on the real issues of South Korean society. Defining the ideal form of life as the life of bodhisattva in Mahayana Buddhism, Hyun-Eung summarizes the theme of this book as follows: My book Enlightenment and History is a collection of essays that interpret Buddhist doctrines under a new light. In particular, this book interprets bodhisattva as the combination of bodhi (enlightenment) and sattva (life, history), and defines the life of bodhisattva as enlightenment s becoming history and history s becoming enlightenment. Also, I classify the issue of fact judgments on how to understand facts and phenomena as the area of bodhi (enlightenment) and the issue of value judgments on how to live life as the area of sattva (history), and thereby I make their logical differences clear. However, at the same time, I claim that harmonizing these two areas into one area of life is the model of a nice and practical life, which is the bodhisattva that Mahayana Buddhism praises. The aim of this paper is to outline a version of “non-attachment” that is conceptually clear and practically viable. We turn to the Buddhist traditions for their wealth of insights into non-attachment, but our goal is not exegetical. We are not constrained by the Buddhist traditions and turn to Anglophone moral psychology as well to point out a close connection between non-attachment and adaptive preference. In this dissertation, I try to answer the following somewhat Kantian question: How is mental causation possible? For the discussion of the nature of mental properties, I accept the two tenets of functionalism: the multiple realizability thesis and the functional conception of mental properties as causal intermediaries between physical causes and effects. I then introduce the following two principles: the principle of physical realization and the principle of causal power identity. It is the principle of physical realization that dictates that (...) every mental property be physically realized for its instantiation. The principle of causal power identity ensures that the causal power of a mental property is identical with those of its physical realizers. I suggest that the realization relation between the mental and the physical be understood in such a way that a mental property has causal powers identical with those of its physical realizers, but that the mental property itself is not reduced to its physical realizers because the two kinds of properties are not identical. Mental properties are realized in physical properties, and the causal powers of mental properties are identical with those of physical properties; therefore, mental properties are as causally efficacious as their physical realizers. Since mental properties are not reducible to physical properties, they enjoy autonomy with the full causal powers of their realizers. To conclude, mental properties do not lose out to epiphenomenalism; and they have their own autonomy. (shrink)

Kripke's argument for the rigid designation of natural kind terms is fallacious because he does not distinguish natural kinds from second-order functional properties; by clarifying the concepts of natural kind and functional property, we can show that natural kind terms do designate their referents rigidly, but that functional property terms are not rigid designators. My discussions of functional property will also help dispel the worry about the alleged cases of contingent identity with regard to theoretical statements in science. ArgumentThis paper aims to show how a nineteenth-century Korean scholar's mathematical study reflects the Korean intellectual environment of his time by focusing on the rule of false double position and the method of root extraction. There were two major trends in Korean mathematics of the early nineteenth century: the first was “Tongsan,” literally “Eastern Mathematics,” which largely depended on Chinese mathematics of the Song and Yuan period adopting counting rod calculation; the second trend was Western mathematics, which was transmitted by (...) the Jesuits and their Chinese collaborators from the late sixteenth century. There was also an intellectual transition in late eighteenth-century Korea when mathematics, which had been of only minor interest for Confucian scholars, became an important part of Confucian pursuits. We can gain an insight into the history of mathematics in Korea by examining and understanding Hong Kil-chu's mathematical studies and the context of the academic world of his time. (shrink)

A model of corporate ethics and social responsibility (CESR) was developed and empirically tested among Chinese business undergraduates in Hong Kong and Singapore. As predicted, it was found that CESR beliefs were negatively related to Machiavellianism and two Confucian concepts, guanxi (interpersonal connections) and mianzi (face). CESR beliefs were also lower among Hong Kong than Singaporean youths. The negative effects of guanxi, mianzi, and Machiavellianism were more pronounced for the Hong Kong than Singapore sample. The great academic disputation of Ho-rak had been made for about 200 years since it began in 1709. An argument that the human nature and the nature of things are same or different was one of the main subjects for the great academic disputation. Gan Lee (李柬, 1677-1727) and Wonjin Han (韓元震, 1682-1751) were leading discussants of the argument. After Lee and Han, Neo-Confucian scholars in Joseon dynasty attempted comprehensively to synthesize their two views. But such Scholars as Seong-ju (...) Lim (任聖周, 1711-1788) and Jeongjin Ki (奇正鎭, 1798-1879) criticized a reformulation of One Principle and Many Differentiations. Lim argued that there was not only One Principle but also one vital breath as well as Many Differentiations of vital breath as well as One Principle. His idea of One Principle is relevant for a similarity between the natures of humans and things, and his idea of Many Differentiations is relevant for a difference between the natures of humans and things. Lim grasped that all the natures of things had two aspects of similarity and difference. But Gi criticized that their discussants were too narrowly specific in arguing this issue. He argued that One principle and Many Differences entailed each other. Like Lim's idea, his idea confirmed the sides of both, too. The nature of original substance is equivalent to the nature of existence. It seems to me that such a fruitful result of Korean philosophy, which an argument between the natures of humans and things had been distinctly made, for the future, will be an area of being made a deeper exploration from various aspects. In this essay I examine how compulsory military service and the Roman Catholic Church uphold and perpetuate an inadequate notion of masculinity in South Korea. I argue that the militaristic and Catholic definitions of masculinity significantly and pejoratively affect Korean culture. To unlearn these definitions, I propose an educational "readjusting" program that denounces any unjust discrimination on the basis of sex and gender.

According to Rettler, there are three types of control that we should consider in relation to the legitimacy of doxastic blame: Intention-based, reason-based, and influence-based control. Rettler argues that among these, influence-based control is the only type of control that is necessary and sufficient for fulfilling the control con-dition required for legitimate doxastic blame. The aim of this short discussion paper is to critically assess Rettler’s view of doxastic con-trol. By doing so, I attempt to defend the reason-responsiveness view from (...) According to two-dimensional semantics, primary intension and secondary intension can play the role of reflecting the cognitive aspect of an expression like Fregean sense does. The aim of this paper is to argue that this role is likely a myth. To argue for this, I attempt to show that cognitive aspects of fictional names cannot be explained within the framework of two-dimensional semantics. To be more specific, I consider four ontological theories about fictional characters that two-dimensional semanticists might be tempted (...) to choose. Ki Jeong-jin is the one of the most famous Neo-Confucian theorists of the late Joseon Dynasty, and he is well known as a proposer of the theory of statements that there is only one Principle although the phenomena vary. He particularly emphasizes the role and authority of Principle. His arguments related to the theory were mainly raised in the work of Napryangsaeui. He, in this work, explicitly reveals the intention to resolve the issues of the nature of humans and things (...) in the Ho-hak debate. But he has never directly participated in the debate. Instead, after his death, some debates over Napryangsaeui arose between his students and Jeon Woo. This paper aims to analyze Ki Jeong-jin’s theory by comparison with the view of Han Wonjin who represents the theory of nature of humans and things in the Ho-hak school, and with the issues of debate between Ki Jeong-jin’s students and Jeon Woo. Particularly, we focused on the matter of relation between ‘completeness and incompleteness’ and ‘original nature’. Through this analysis, we could expand the understanding of the aspects discussed in the theory on the similarities and differences in nature of humans and things, and the significance of Ki Jeong-jin’s theory. Furthermore, in the history of Neo-Confucianism in the Joseon Dynasty, we could shed light on the reconstruction process of the philosophical statements that there is only one Principle but the phenomena vary. This paper will describe the rite for the exorcism of toch'aebi and examine its symbolic significance in the wider social reality of Cheju shamanism. Toch'aebi is a stranger deity who visits Cheju randomly and tries to get on good terms with the people. However, this deity afflicts people, particularly women, wearing down their vitality and causing a kind of “madness” (turida). The exorcism ritual of toch'aebi requires a sacrificial feast of roast pig and several days of dancing by the possessed. (...) For this reason, the exorcism ritual is called a dancing rite, or ch'unun kut. It is supposedly toch'aebi himself who has the desire to dance. Shaman's songs (seouje sori) are used to induce the toch'aebi, who is hiding in the patient's body, to dance and reveal his identity. The trance dance and singing are directed toward a dialogical construction of the demonic reality, which entails a romantic entanglement of toch'aebi and the possessed. By translating a history of personal suffering into the idiom of love, the shaman's songs develop a particular cultural logic of cure. Through an analysis of curing song texts, along with various modes of performance such as dance and comic play, this paper explores the transformative process in which a particular cultural logic of illness and cure are produced and enacted to reintegrate the patient into the social world. In practice, the analysis addresses the articulation of a personal case of madness that afflicted Sunho, a twenty-one year old female factory worker, whose social values and understanding are reflective of contemporary Korea. Background Although the importance of clinical ethics in contemporary clinical environments is established, development of formal clinical ethics services in the Australia health system has, to date, been ad hoc. This study was designed to systematically follow and reflect upon the first 18 months of activity by a newly established service, to examine key barriers and facilitators to establishing a new service in an Australian hospital setting. Methods: how the study was performed and statistical tests used A qualitative case study (...) approach was utilised. The study gathered and analysed data using observations of service committee meetings, document analysis of agendas and minutes, and semi-structured interviews with committee members to generate semantic themes. By interpreting the thematic findings in reference to national capacity building resources, this study also aimed to provide practice-based reflections for other health agencies. Results: the main findings An overarching theme identified in the data was a strong commitment to supporting clinicians facing difficult patient care decisions and navigating difficult discussions with patients and families. Another key theme was the role of the new clinical ethics support service in providing clinicians with a pathway to raise system-wide issues with the organisation Executive. While there was strong clinical engagement, consumer and community participation remained a challenge, as did unresolved governance issues and a need for clearer policy relationship between the service and the organisation. Conclusions: brief summary and potential implications Considering these themes in relation to the national capacity building resources, the study identifies three areas likely to require ongoing development and negotiation. These are: the role of the clinical ethics support service as a link between the workforce and the Executive; the incorporation of consumers and patients; and ethical reasoning. To improve the effectiveness of the service, it is necessary to increase clarity on the service’s role at the governance and policy level, as well as develop strategies for engaging consumers, patients and families. Finally, the capacity of the service to reflect on complex cases may be enhanced through explicit discussions of various different ethical frameworks and ways of deliberating. (shrink)

COVID-19 often manifests with different outcomes in different patients, highlighting the complexity of the host-pathogen interactions involved in manifestations of the disease at the molecular and cellular levels. In this paper, we propose a set of postulates and a framework for systematically understanding complex molecular host-pathogen interaction networks. Specifically, we first propose four host-pathogen interaction (HPI) postulates as the basis for understanding molecular and cellular host-pathogen interactions and their relations to disease outcomes. These four postulates cover the evolutionary dispositions involved (...) in HPIs, the dynamic nature of HPI outcomes, roles that HPI components may occupy leading to such outcomes, and HPI checkpoints that are critical for specific disease outcomes. Based on these postulates, an HPI Postulate and Ontology (HPIPO) framework is proposed to apply interoperable ontologies to systematically model and represent various granular details and knowledge within the scope of the HPI postulates, in a way that will support AI-ready data standardization, sharing, integration, and analysis. As a demonstration, the HPI postulates and the HPIPO framework were applied to study COVID-19 with the Coronavirus Infectious Disease Ontology (CIDO), leading to a novel approach to rational design of drug/vaccine cocktails aimed at interrupting processes occurring at critical host-coronavirus interaction checkpoints. Furthermore, the host-coronavirus protein-protein interactions (PPIs) relevant to COVID-19 were predicted and evaluated based on prior knowledge of curated PPIs and domain-domain interactions, and how such studies can be further explored with the HPI postulates and the HPIPO framework is discussed. Despite contemporary Confucianism’s aspirations to be a world philosophy, there is an ethnocentric strand within the Confucian tradition, most glaringly exemplified in Han Yu’s attacks on Buddhism. This paper re-assesses Confucian ethnocentrism in the context of contrary practices that indicate a more pragmatic attitude among Confucians toward cross-cultural interactions. It argues that while the ethnocentric tendency serves as constant reminder of the need for vigilance, and recognition of the difficulties of crossing cultural boundaries, there are nevertheless resources within Confucianism for (...) constructing an ethics of communication that is urgently needed to deal with the moral problems of cultural pluralism. The paper analyses the role of various common Confucian virtues such as ren, yi, li, zhi in communication, and argues that a virtue of flexibility is implicit in Confucius’s insistence of bugu and could contribute significantly to a Confucian ethics of communicative virtues. Emergence is a notorious philosophical term of art. A variety of theorists have appropriated it for their purposes ever since George Henry Lewes gave it a philosophical sense in his 1875 Problems of Life and Mind. We might roughly characterize the shared meaning thus: emergent entities (properties or substances) ‘arise’ out of more fundamental entities and yet are ‘novel’ or ‘irreducible’ with respect to them. (For example, it is sometimes said that consciousness is an emergent property of the brain.) Each (...) of the quoted terms is slippery in its own right, and their specifications yield the varied notions of emergence that we discuss below.
