Drug design methods have made significant new advances over the last ten years, mainly in the areas of molecular modelling. In more recent times important developments in theory have led to a different type of modelling becoming possible, the so‐called de novo or automated design algorithms. In this new method the programs perform much of the chemist's thinking, in finding appropriately sized chemical groups to fit into a target site. However this is a combinatoric problem which has no general analytical (...) solution; it is ripe for optimization. Other advances, such as combinatorial chemical synthesis and screening, will dramatically influence the search for new lead structures for target sites, which at present are poorly understood. Already these methods are being applied to peptide libraries. Peptides do not make good drug compounds because of their poor bioavailability; further, their flexibility reduces their affinity. In some cases peptide backbones can be removed and replaced with rigid non‐peptide scaffolds. (shrink)

Philip James Jones, a Fellow of the British Academy, was one of the most distinguished, complex, and challenging of medieval historians. His works on the Italian city-states of the thirteenth to fifteenth centuries and on Italy's agrarian history are monuments built to last, benchmarks that defined the field for a generation. Jones was elected a Fellow of the British Academy in 1984 and was awarded the Serena Medal for Italian studies in 1988. He won a major open scholarship in (...) Modern History at Wadham College, University of Oxford. Jones took a First in Modern History in 1945 and was appointed to a research studentship at Magdalen College. Second, in so far as Dean thinks there (...) is a uniquely American reli· gious experience or style of thought, privileging that style seems a dangerous move indeed. Dean is not unaware of the dangers. He specifically denies that he is engaging in any form of " American exceptionalism " (p. 25) ; it is simply that an " appropriately American historicism, centering on American culture, would have a greater chance of suggesting practices that work for and are true for America " (p. 24). But I am skeptical that such disclaimers address the real problem. Near the very end of the book, Dean suggests that a modi· fied naturalism might lead to " a notion of God that was more genuinely American than anything earlier," culminating in the recognition that " American thought about God is really the thought of a national com· munity" (p. 144). Descriptively that may well be the case; but then the Christian theologian must seek norms elsewhere. Arranged alphabetically according to topics, this col- 546 BOOK REVIEWS lection of statements from the papacy, the Roman Congregations, and the conferences of Catholic bishops could prove to he a very helpful reference tool. It contains a brief introduction which seeks to explain the role of these statements in the Catholic moral theology and Catholic medical ethics, a table of contents of the statements included, and then the statements themselves. In most instances it does not contain the entire document or statement hut only excerpts therefrom. There are some weaknesses in this work. First, these statements are not self-explanatory; they are in need of interpretation and explanation in many instances. But the authors present only the bald text and do not offer any explanatory comments. They apparently presume that these statements are pellucid to the reader, hut this is an unwarranted pre· sumption. For example, clarifying comments should have been added to the statement from the Declaration on Euthanasia because the allocution of Pius XII about giving lethal doses of analgesia is not entirely clear. A second difficulty with this work is the brief and somewhat insubstantial introduction on conscience and the role these statements should play in the formation of conscience. This sort of work invites a substantial and extended commentary on the nature of moral judgments, the relationship of conscience to the natural law, and the authority of these statements. The authors note that not all of the official statements issued by various organs of the Church are of equal authority, hut they do not try to identify the specific authoritative status of the various statements they include in their work. The reader is thus left to his or her own devices to determine, for example, the relative weight of statements of the administrative hoard of the NCCB versus statements of state episcopal conferences. A still further problem is that some of the statements are of virtually no significant authority for Catholics. For example, the statement of the administrative hoard of the NCCB on AIDS is of questionable... (shrink)

Conscious states—state that there is something it is like to be in—seem both rich or full of detail and ineffable or hard to fully describe or recall. The problem of ineffability, in particular, is a longstanding issue in philosophy that partly motivates the explanatory gap: the belief that consciousness cannot be reduced to underlying physical processes. Here, we provide an information theoretic dynamical systems perspective on the richness and ineffability of consciousness. In our framework, the richness of conscious experience corresponds (...) to the amount of information in a conscious state and ineffability corresponds to the amount of information lost at different stages of processing. We describe how attractor dynamics in working memory would induce impoverished recollections of our original experiences, how the discrete symbolic nature of language is insufficient for describing the rich and high-dimensional structure of experiences, and how similarity in the cognitive function of two individuals relates to improved communicability of their experiences to each other. While our model may not settle all questions relating to the explanatory gap, it makes progress toward a fully physicalist explanation of the richness and ineffability of conscious experience—two important aspects that seem to be part of what makes qualitative character so puzzling. The concept of white ignorance refers to phenomena of not-knowing that are produced by and reinforce systems of white supremacist domination and exploitation. I distinguish two varieties of white ignorance, belief-based white ignorance and practice-based white ignorance. Belief-based white ignorance consists in an information deficit about systems of racist oppression. Practice-based white ignorance consists in unresponsiveness to the political agency of persons and groups subject to racist oppression. Drawing on the antebellum political thought of Black abolitionists Frederick Douglass and Harriet (...) Jacobs, I contend that an anti-racist politics that conceives of its epistemic task in terms of combating practice-based white ignorance offers a more promising frame for liberatory struggle. A focus on practice-based white ignorance calls for a distinctive form of humility that involves recognition of the limits of one’s own political agency in relation to others, which is integral to democratic relations between free, equal, yet mutually dependent persons. In this paper I draw on Thaddeus Metz’s pioneering work in African ethics, and particularly his account of the concept described by the terms ubuntu, botho, hunhu or utu, to sketch an African normative understanding of the act of rebellion against the authority of the state. Most commonly articulated in the phrase “a person is a person through other persons”, ubuntu is interpreted by Metz as a unique communitarian moral principle which can be described in its essence as the claim (...) that “actions are right, or confer ubuntu on a person, insofar as they prize communal relationships, ones in which people identify with each other, or share a way of life, and exhibit solidarity toward one another, or care about each other’s quality of life”. On the face of it, this principle appears at odds with rebellions against state authority. Following Metz, I argue, however, that a deeper grasp of this principle does, in fact, provide a justification for instances of civilian rebellion against state authority, under appropriate circumstances. While there are no serious arguments in favor of there being no state control whatsoever over the private ownership and employment of firearms, there are significant arguments on the other extreme of the ‘gun control debate’ which contend for bans on the private ownership of firearms or some subset thereof. In this paper I argue that gun ban proponents like Jeff McMahan and Nicholas Dixon confuse the risk or likelihood of being confronted by an attacker intent on serious or lethal (...) harm with the right to defend oneself when faced by such an attacker. When this distinction is properly understood it becomes clear that arguments for the banning of all privately owned guns, or particular classes of guns, cannot stand so long as the firearms in question can be reasonably considered to be an effective means for individuals to defend themselves against attackers intent on serious or lethal harm. In his article, Pigden identifies Karl Popper as the originator of the philosophical argument that it is naïve to believe in any conspiracy theory. But Popper was not criticizing belief in conspiracy theories at all, as Pigden defined them or as they (...) have usually come to be understood since about the 1960s. Pigden has therefore fundamentally and anachronistically misinterpreted Popper. The object of Popper’s criticism was, rather, the inadequate approach to social science that is limited to the discovery of human intentions, including conspiracies, in particular the will of Great Men. Popper’s critique of the conspiracy theory of society was correct and should be rehabilitated. Pigden is correct only insofar as he concludes that we should not dismiss conspiracy theories without critical evaluation, a proposition with which Popper would likely have wholeheartedly agreed. In this paper, I defend a conception of Deleuze’s two volumes dedicated to film – Cinema I: The Movement-Image, and Cinema II: The Time-Image – as protracted expressions of his political philosophy. In this context, I will elaborate the difficult and entwined political claims Deleuze makes on behalf of cinema: that it is capable of engendering a tentative ‘belief in the world’, such as is the necessary correlate of political action; that it captures the contemporary political fact that ‘the people (...) are missing’, as a unified or coherent political agent; and finally that it might reveal those ‘impossible’ or ‘intolerable’ situations which would provoke such a people into being. In advancing this conceptual triumvirate, I will argue that the claims made here on behalf of cinema overspill the art form itself, linking up with Deleuze’s broader political ontology of thought and constituting a generalised political philosophy proper to so-called ‘late-capitalism’. They shared their research and perceptive insights on this key topic of great contemporary interest from quite different disciplines and viewpoints. Predictive processing accounts of neural function view the brain as a kind of prediction machine that forms models of its environment in order to anticipate the upcoming stream of sensory stimulation. These models are then continuously updated in light of incoming error signals. Predictive processing has offered a powerful new perspective on cognition, action, and perception. In this chapter we apply the insights from predictive processing to the study of emotions. In describing Augustine, the author captures the essence of the man in these words: "Genius he had in full measure... he is the master of the phrase or the sentence that embodies a penetrating insight, a flash of lightning that illuminates the entire sky; he is the rhetorician, the epigrammist, the polemicist, but not the patient, logical systematic philosopher.".

The purpose of this article is to explore enduring ethical vulnerabilities of the nursing profession as illustrated in historical chapters of nursing’s past. It describes these events, then explores two ethical vulnerabilities in depth: conflicting loyalties and duties, and relationships with patients as ‘other’. The article concludes with suggestions for more ethical approaches to the other in current nursing practice. The past may be one of the most fruitful sites for examining enduring ethical vulnerabilities of the nursing profession. First of (...) all, professional identity, which includes moral identity, comes in part from knowledge of the nursing profession’s past. Predictive processing has begun to offer new insights into the nature of conscious experience—but the link is not straightforward. A wide variety of systems may be described as predictive machines, raising the question: what differentiates those for which it makes sense to talk about conscious experience? One possible answer lies in the involvement of a higher-order form of prediction error, termed expected free energy. In this paper we explore under what conditions the minimization of this new quantity might underpin conscious (...) experience. Our suggestion is that the minimisation of Expected Free Energy is not in itself sufficient for the occurrence of conscious experience. Instead, it is relevant only insofar as it helps deliver what Ward et al. ( 2011 ) have previously described as a _sense of our own poise over an action space_. Perceptual experience, we will argue, is nothing other than the process that puts current actions in contact with goals and intentions, enabling some creatures to know the space of options that their current situation makes available. This proposal fits with recent work suggesting a deep link between conscious contents and contents computed at an ‘intermediate’ level of processing, apt for controlling action. (shrink)

After 40 years of attributing high rates of physician career dissatisfaction, attrition, alcoholism, divorce and suicide to ‘burnout’, there is growing recognition that these outcomes may instead be caused by moral injury. This has led to a debate about the relative diagnostic merits of these two terms, a recognition that interventions designed to treat burnout may be ineffective, and much perplexity about how—if at all—this changes anything. The current research seeks to develop the construct of moral injury outside military contexts, (...) generate more robust validity tests and more fully describe and measure the experiences of persons exposed to moral harms. Absent from the literature is a mechanism through which to move from the collective moral injury experience of physicians to a systematic change in the structure of medical practice. To address this, after providing a brief history, definitions and contrasts between burnout, moral distress and moral injury, we review the interplay of moral and ethical codes in the context of moral injury. We conclude by suggesting that professional associations can potentially prevent moral injury by providing protections for physicians within their codes of ethics. There are no data in this work. IntroductionHead and neck cancer diagnosis and treatment can be a significant life trauma. Some HNC survivors experience post-traumatic growth, which has been linked with better health-related quality-of-life. Empirical research on PTG, and theoretical models, point to the importance of being able to purposely make sense of the traumatic experience. Intrusive rumination, by contrast, is linked to poorer outcomes. This study explored HNC survivors’ experiences of rumination.MethodsTwenty HNC survivors between 9 months and 5 years post-diagnosis were recruited. They had a range (...) of HNC sub-types and cancer treatments. Participants underwent a semi-structured interview about their cancer diagnosis and treatment. Reflexive thematic analysis identified themes and sub-themes around rumination.ResultsFour themes with linked subthemes on the content and process of rumination were identified. Theme 1 was rumination and worry related to diagnosis. Here, survivors discussed how the HNC diagnosis and plans for treatment had dominated their initial thoughts. Theme 2 was processing the trauma of HNC. This theme reflected rumination on the traumatic experience of diagnosis and treatment and how the participant was reacting to it. Theme 3 was considering the impact. This theme encompassed retrospective thinking and comparisons between the participant now versus the early days after diagnosis. Theme 4 was continued rumination. This theme included ongoing intrusive and distressing rumination about the trauma and impact of cancer. Those who expressed ongoing rumination revisited fears or returned to negative experiences.ConclusionThis study uniquely describes the nature and content of rumination following HNC. Early intrusive rumination is common and may reflect perceptions of cancer as an existential threat. Over time, rumination can become more reflective and move towards deliberate meaning-making. Some HNC survivors may benefit from interventions to reduce barriers to this transition. The content of distressing and difficult to control rumination helps to identify those who may benefit from more directed psychological support. (shrink)

Alvin Plantinga's "Warranted Christian Belief" is without question one of the central texts of the Reformed epistemology movement. Critiques of Plantinga's defence have been both multiple and varied. As varied as these responses are, however, it is my contention that many of them amount to the same thing. It is the purpose of this paper to offer an overview of the main lines of attack that have been directed as Plantinga's project, and thereafter to show how many, if not most, (...) of these objections can be understood as versions or aspects of the same criticism, what I call the 'Inadequacy Thesis'. On what might be called a Marxist reading, Hegel’s analysis of civil society accurately recognizes a necessary tendency toward a polarization of classes and the pauperization of the proletariat, a problem for which Hegel, however, has no solution. Indeed, Marxists think there can be no solution short of eliminating civil society. It is not at all clear that this standard reading is correct. The present paper tries to show how it is plausible to understand Hegel as proposing a solution, one (...) that is similar to that of social democrats, and one that could actually work. (shrink)
