OBJECTIVE: To describe the issues faced, and how they were addressed, by the University of Toronto Critical Care Medicine Program/Joint Centre for Bioethics Task Force on Appropriate Use of Life-Sustaining Treatment. The clinical problem addressed by the Task Force was dealing with requests by patients or substitute decision makers for life-sustaining treatment that their healthcare providers believe is inappropriate. DESIGN: Case study. SETTING: The University of Toronto Joint Centre for Bioethics/Critical Care Medicine Program Task Force on Appropriate Use of Life-Sustaining (...) Treatment. PARTICIPANTS: The 24-member Task Force included physician and nursing leaders from five critical care units, bioethicists, a legal scholar, a health administration expert, a social worker, and a hospital public relations professional. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Our specific lessons learned include a) a policy focus on process; b) use of a negotiation and mediation model, rather than a hospital ethics committee model, for this process; and c) the policy development process is itself a negotiation, so we recommend equal involvement of interested groups including patients, families, and the public. CONCLUSIONS: This article describes the key issues faced by the Task Force while developing its policy. It will provide a useful starting point for other groups developing policy on appropriate use of life-sustaining treatment. (shrink)

In this paper, I develop a metasemantics for relaxed moral realism. More precisely, I argue that relaxed realists should be inferentialists about meaning and explain that the role of evaluative moral vocabulary is to organise and structure language exit transitions, much as the role of theoretical vocabulary is to organise and structure language entry transitions.

In his original paper of 1905, "On the Electrodynamics of Moving Bodies", Einstein described a procedure for synchronizing distant clocks at rest in any inertial system K. Clocks thus synchronized may be said to be in standard signal synchrony in K. It has often been claimed that there are no logical or physical reasons for preferring standard signal synchronizations to any of a range of possible non-standard ones. In this paper, the range of consistent non-standard signal synchronizations, first for any (...) one inertial system, and second for any set of such systems, is investigated, and it is shown that the requirement of consistency leaves much less room for choice than is commonly supposed. Nevertheless consistent non-standard signal synchronizations appear to be possible. However, it is also shown that good physical reasons for preferring standard signal synchronizations exist, if the Special Theory of Relativity yields correct predictions. The thesis of the conventionality of distant simultaneity espoused particularly by Reichenbach and Grunbaum is thus either trivialized or refuted. (shrink)

The moral error theory generally does not receive good press in metaethics. This paper adds to the bad news. In contrast to other critics, though, I do not attack error theorists’ characteristic thesis that no moral assertion is ever true. Instead, I develop a new counter-argument which questions error theorists’ ability to defend their claim that moral utterances are meaningful assertions. Although Aristotle (Metaphysics, Book IV, Chapter 2) was perhaps the first person to consider the part-whole relationship to be a proper subject matter for philosophic inquiry, the Polish logician Stanislow Lesniewski [15] is generally given credit for the first formal treatment of the subject matter in his Mereology.1 Woodger [30] and Tarski [24] made use of a specific adaptation of Lesniewski's work as a basis for a formal theory of physical things and their parts. The term 'calculus of individuals' was (...) introduced by Leonard and Goodman [14] in their presentation of a system very similar to Tarski's adaptation of Lesniewski's Mereology. Contemporaneously with Lesniewski's development of his Mereology, Whitehead [27] and [28] was developing a theory of extensive abstraction based on the two-place predicate, 'x extends over y\ which is the converse of 'x is a part of y\ This system, according to Russell [22], was to have been the fourth volume of their Pήncipia Mathematica, the never-published volume on geometry. Both Lesniewski [15] and Tarski [25] have recognized the similarities between Whitehead's early work and Lesniewski's Mereology. Between the publication of Whitehead's early work and the publication of Process and Reality [29], Theodore de Laguna [7] published a suggestive alternative basis for Whitehead's theory. This led Whitehead, in Process and Reality, to publish a revised form of his theory based on the two-place predicate, 'x is extensionally connected with y\ It is the purpose of this paper to present a calculus of individuals based on this new Whiteheadian primitive predicate. Humans can recollect past events in details and/or know that an object, person, or place has been encountered before. During the last two decades, there has been intense debate about how recollection and familiarity are organized in the brain. Here, we propose an integrative memory model which describes the distributed and interactive neurocognitive architecture of representations and operations underlying recollection and familiarity. In this architecture, the subjective experience of recollection and familiarity arises from the interaction between core systems and an (...) attribution system. By integrating principles from current theoretical views about memory functioning, we provide a testable framework to refine the prediction of deficient versus preserved mechanisms in memory-impaired populations. The case of Alzheimer's disease is considered as an example because it entails progressive lesions starting with limited damage to core systems before invading step-by-step most parts of the model-related network. We suggest a chronological scheme of cognitive impairments along the course of AD, where the inaugurating deficit would relate early neurodegeneration of the perirhinal/anterolateral entorhinal cortex to impaired familiarity for items that need to be discriminated as viewpoint-invariant conjunctive entities. The integrative memory model can guide future neuropsychological and neuroimaging studies aiming to understand how such a network allows humans to remember past events, to project into the future, and possibly also to share experiences. The concept of moral distress comes from nursing ethics, and was initially defined as ‘…when one knows the right thing to do, but institutional constraints make it nearly impossible to pursue the right course of action’. There is a large body of literature associated with moral distress, yet multiple definitions now exist, significantly limiting its usefulness. We undertook a systematic review of the argument-based bioethics literature on this topic as the basis for a critical appraisal, identifying 55 papers for analysis. (...) We found that moral distress is most frequently framed around individual experiences of distress in relation to local practices and constraints, and understood in terms of power relations and workplace hierarchies. This understanding is directly derived from, and often still seen as specific to, nursing. Frequently the perspective of the morally distressed individual is privileged. Understandings of moral distress have evolved towards an ‘occupational health approach’, with the assumption that moral distress should be measured and prevented. Counter-perspectives were identified, highlighting conceptual problems. Based on our review, we propose a redefinition of moral distress: ‘Ethical unease or disquiet resulting from a situation where a clinician believes they have contributed to avoidable patient or community harm through their involvement in an action, inaction or decision that conflicts with their own values’. This definition is specific enough for research use, anchored in clinicians’ professional responsibilities and concerns about harms to patients, framed relationally rather than hierarchically, and amenable to multiple perspectives on any given morally distressing situation. Hanna proposes a version of non-conceptualism he closely associates with Kant. This paper takes issue with his proposal on two fronts. First, there are reasons to dispute whether any version of non-conceptualism can be rightly attributed to Kant. In addition to pointing out passages that conflict with Hanna's interpretation, I also suggest ways in which the Kant of the Opus Postumum could integrate key insights of non-conceptualism into a basically conceptualist framework. In Part Two of the paper, I turn to (...) a more systematically oriented critique of Hanna's nonconceptualism. Drawing on work by Gareth Evans, John McDowell, Sonia Sedivy, and Alva Noë, I argue that conceptualism is in a position to integrate the points which are taken by Hanna to speak most strongly in favor of non-conceptualism. In particular, I argue for the deep compatibility of conceptualism and direct realism. At the same time, I point to what I see as weaknesses in Hanna's defence of non-conceptualism. (shrink)

This article revisits, analyzes and critiques Bruce Chatwin’s 1987 bestseller, The Songlines,1 more than three decades after its publication. In Songlines, the book primarily responsible for his posthumous celebrity, Chatwin set out to explore the essence of Central and Western Desert Aboriginal Australians’ philosophical beliefs. For many readers globally, Songlines is regarded as a—if not the—definitive entry into the epistemological basis, religion, cosmology and lifeways of classical Western and Central Desert Aboriginal people. It is argued that Chatwin’s fuzzy, ill-defined use (...) of the word-concept “songlines”2 has had the effect of generating more heat than light. Chatwin’s failure to recognize the economic imperative underpinning Australian desert people’s walking praxis is problematic: his own treks through foreign lands were underpropped by socioeconomic privilege. Chatwin’s ethnocentric idée fixe regarding the primacy of “walking” and “nomadism,” central to his Songlines thématique, well and truly preceded his visits to Central Australia. Walking, proclaimed Chatwin, is an elemental part of “Man’s” innate nature. It is argued that this unwavering, preconceived, essentialist belief was a self-serving construal justifying Chatwin’s own “nomadic” adventures of identity. Is it thus reasonable to regard Chatwin as a “rogue author,” an unreliable narrator? And if so, does this matter? Of greatest concern is the book’s continuing majority acceptance as a measured, accurate account of Aboriginal belief systems. With respect to Aboriginal desert people and the barely disguised individuals depicted in Songlines, is Chatwin’s book a “rogue text,” constituting an act of epistemic violence, consistent with Spivak’s usage of that term? The standard means of seeking the classical limit in Bohmian mechanics is through the imposition of vanishing quantum force and quantum potential for pure states. We argue that this approach fails, and that the Bohmian classical limit can be realized only by combining narrow wave packets, mixed states, and environmental decoherence.

Environmental change requires individuals and institutions to facilitate adaptive governance. However, facilitating adaptive governance may be difficult because resource users' perceptions of desirable ways of life vary. These perceptions influence preferences related to environmental governance and may stem from the ways individuals subjectively value their work and their connections to their environment. This paper uses a value-based approach to examine individual and institutional preferences for adaptive governance in Carelmapu, Chile. We show that two groups had different value frames rooted in (...) divergent ontologies which influenced their actions related to adaptive governance, creating conflict. (shrink)

Modern bioethics, which is based on Western moral philosophy and Western biomedical perspectives, has evolved within a complex, highly individualistic culture that draws a sharp distinction between church and state and tolerates a multitude of values. This discipline defines its principles in secular and objective terms that often are bewildering to people of non-Western origin. Despite much discourse, principlism remains the fundamental framework of bioethics. Principlism is held in such high regard that many bioethicists equate autonomy with personhood, as if (...) autonomy exists independently of specific beliefs and commitments.In addition, we continue to minimize the substantial differences in the way people of different cultures perceive, experience, and explain illness, although our views of the potential cultural limitations of Western medicine have grown and expanded in recent years. At the heart of it we continue to be tied to a biomedical focus that largely neglects the context of the situation. (shrink)

The process of seeking the consent of a patient to a medical procedure is, arguably, one of the most important skills a doctor, or indeed any clinician, should learn. In fact, the very idea that doctors may institute diagnostic or treatment processes of any sort without a patient's consent is utterly counter-intuitive to the modern practice of medicine. It was not always thus, and even now it can be reliably assumed that consent is still not sought and gained appropriately in (...) every clinical encounter. To say that it should be sought and gained in this manner elevates the value of consent to a high level. It can be instructive to ask oneself why such a value might be held to be the case. The answer to this question lies in the philosophical underpinning of clinical consent, which sits within a notion of personal autonomy, and respect for autonomous decision making. (shrink)

The integrative memory model formalizes a new conceptualization of memory in which interactions between representations and cognitive operations within large-scale cerebral networks generate subjective memory feelings. Such interactions allow to explain the complexity of memory expressions, such as the existence of multiples sources for familiarity and recollection feelings and the fact that expectations determine how one recognizes previously encountered information.

There is little doubt that the development and commercialisation of nanotechnologies is challenging traditional state-based regulatory regimes. Yet governments currently appear to be taking a non-interventionist approach to directly regulating this emerging technology. This paper argues that a large regulatory toolbox is available for governing this small technology and that as nanotechnologies evolve, many regulatory advances are likely to occur outside of government. It notes the scientific uncertainties facing us as we contemplate nanotechnology regulatory matters and then examines the notion (...) of regulation itself, suggesting new ways to frame our understanding of both regulation and the regulatory tools relevant to nanotechnologies. By drawing upon three different conceptual lenses of regulation, the paper articulates a wide range of potential regulatory tools at hand. It also focuses particularly on the ways various tools are currently being used or perhaps may be employed in the future. The strengths and weaknesses characterising these tools is examined as well as the different actors involved. The paper concludes that we will increasingly face debate over what is likely to work most effectively in regulating nano technologies, the legitimacy of these different potential approaches, and the speed at which these different regimes may be employed. Quasi-realist expressivists have developed a growing liking for minimalism about truth. It has gone almost unnoticed, though, that minimalism also drives an anti-Archimedean movement which launches a direct attack on expressivists’ non-moral self-image by proclaiming that all metaethical positions are built on moral grounds. This interplay between expressivism, minimalism and anti-Archimedeanism makes for an intriguing metaethical encounter. As such, the first part of this dissertation examines expressivism’s marriage to minimalism and defends it against its critics. The second part then turns (...) to the anti-Archimedean challenge to expressivism and shows how to ward off this challenge by securing expressivism’s non-moral, metaethical status without having to abandon minimalism about truth. Identifying cognition in general with propositional knowledge exposes the cognitive value of literature to abiding skepticism. This chapter argues that German romanticism has generated two competing views of the relation between literature and the overtly truth-seeking disciplines. One is a legacy of skepticism and antirealism that is powerless to give a positive account of literary value. The other is a complementarist legacy emphasizing literature’s cognitive priority to and its role as the cognitive fulfillment of discursive knowledge. The Opening Chapter of Hegel's Phenomenology of Spirit, called "Sense Certainty," is brief: 283 lines or about seven and a half pages in the critical edition of Hegel's works . Just over half the text is devoted to a series of thought experiments1 that focus on "the Here" and "the Now" as the two basic forms of immediate sensuous particularity Hegel calls "the This." The chapter's main goal is to demonstrate that, in truth, the object of sense certainty is precisely (...) the opposite of what it purports to be: "the This" is mediated abstract universality. Childhood adversity is associated with altered or dysregulated stress reactivity; these altered patterns of physiological functioning persist into adulthood. Evidence from both preclinical animal models and human neuroimaging studies indicates that early life experience differentially influences stressor-evoked activity within central visceral neural circuits proximally involved in the control of stress responses, including the subgenual anterior cingulate cortex (sgACC), paraventricular nucleus of the hypothalamus (PVN), bed nucleus of the stria terminalis (BNST) and amygdala. However, the relationship between childhood adversity and the (...) resting-state connectivity of this central visceral network remains unclear. To this end, we examined relationships between childhood threat and childhood socioeconomic deprivation, the resting-state connectivity between our regions of interest (ROIs), and affective symptom severity and diagnoses. We recruited a transdiagnostic sample of young adult males and females (n= 100; mean age = 27.28,SD= 3.99; 59 females) with a full distribution of maltreatment history and symptom severity across multiple affective disorders. Resting-state data were acquired using a 7.2-min functional magnetic resonance imaging (fMRI) sequence; noted ROIs were applied as masks to determine ROI-to-ROI connectivity. Threat was determined by measures of childhood traumatic events and abuse. Socioeconomic deprivation (SED) was determined by a measure of childhood socioeconomic status (parental education level). Covarying for age, race and sex, greater childhood threat was significantly associated with lower BNST-PVN, amygdala-sgACC and PVN-sgACC connectivity. No significant relationships were found between SED and resting-state connectivity. BNST-PVN connectivity was associated with the number of lifetime affective diagnoses. Exposure to threat during early development may entrain altered patterns of resting-state connectivity between these stress-related ROIs in ways that contribute to dysregulated neural and physiological responses to stress and subsequent affective psychopathology. This paper attempts a deduction of Kant's concept of the highest good: that is, it attempts to prove, in accordance with Dieter Henrich.s interpretation of the notion of deduction, that the highest good is an end that is also a duty. It does this by appealing to features of practical reason that make up the legitimating facts that serve as the premises that any deduction must possess. According to Kant, the highest good consists of happiness, virtue, and relations of proportionality (...) and causation between happiness and virtue, such that happiness is proportional to and caused by virtue. I argue, by drawing on accepted Kantian notions, that Kant had compelling reasons for concluding that the highest good is in fact an end that is also a duty. The subjectivity or “purpose dependency” of measurement in biology is discussed using examples from high-dimensional medical genetic research. The human observer and study designer tacitly determine the numerical and graphical representation of biological simplicity or complexity via choice of ascertainment , numbers to measure, referential basis, statistical learning formalism and feature search, and also via the selection of display styles for all these quantifications.

In a celebrated passage in ‘Of the Standard of Taste’, Hume tells us that those readers who prefer Bunyan's writings to Addison's are merely ‘pretended critics’ whose judgment is ‘absurd and ridiculous’; this is ‘no less an extravagance, than if he had maintained a mole-hill to be as high as TENERIFFE, or a pond as extensive as the ocean’. Hume shows a decisiveness and vehemence in his judgment against Bunyan that has greater significance than that of being a mere reflection (...) of his aesthetic principles. Hume does, after all, wish to make ‘durable admiration’ the foundation of his standard of taste, and both the number of eighteenth-century reprints of The Pilgrim's Progress and Johnson's comment that this work has as ‘the best evidence of its merit, the general and continued approbation of mankind’ testify to the lasting popularity of Bunyan's work. Hume's critical judgment on Bunyan is not merely a consequence of a mechanical application of his standard of taste, but is rather a reflection of what I will term Hume's ‘epistemology of ease’. This paper describes the processes of cognitive modeling and representation of human expertise for developing an ontology and knowledge base of an expert system. An ontology is an organization and classification of knowledge. Ontological engineering in artificial intelligence has the practical goal of constructing frameworks for knowledge that allow computational systems to tackle knowledge-intensive problems and supports knowledge sharing and reuse. Ontological engineering is also a process that facilitates construction of the knowledge base of an intelligent system, which can be (...) defined as a computer program that can duplicate problem-solving capabilities of human experts in specific areas. This paper presents the processes of knowledge acquisition, analysis, and representation, which laid the basis for ontology construction. In this case, the processes are applied in ontological engineering for construction of an expert system in the domain of monitoring of a petroleum production and separation facility. The acquired knowledge was also formally represented in two knowledge acquisition tools. (shrink)

THIS ESSAY PROPOSES THAT EVERYDAY REFERENCES TO HEROES AND suicides share a lack of critical common sense, and that ethicists should initiate critical discourse on these issues to lift the level of popular reflection. The absence of critical discourse serves the interests of powerful social groups and organizations to the disadvantage of other social groups. The absence of critical discourse is further supported by broad social suspicion of decision making by ordinary individuals resulting in social preference for trusting elites, even (...) when the arguments supporting such preferences are recognized as flawed or untrue.
