In El Salvador from 1978 to 1988, contraceptive use among married women 15–44 years of age increased from 34% to 47%, and the total fertility rate declined from 6·3 to 4·6 children per woman. Most of this change took place from 1978 to 1985. Sterilization is the most prevalent method used, but nearly one-half of the women who are sterilized did not use any contraception before their operation. Few young couples use reversible methods of contraception to space births or delay (...) the start of childbearing. The originator of philosophical pluralism, McKeon made extraordinary contributions to philosophy, to international relations, and to theory-formation in the communication arts, aesthetics, the organization of knowledge, and the practical sciences. This collection, which includes a philosophical autobiography as well as the out-of-print title essay "Freedom and History" and a previously unpublished essay on "Philosophic Semantics and (...) The modus operandi of this book is contextual—throughout he demonstrates how ideas emerge from or are inspired by particular environments. And the need to put philosophical ideas in their larger historical and cultural context so as to fully understand them is, as will be illustrated below, a facet of his philosophical method. Another of its facets is fallibilism, a deep commitment to subjecting all theories and concepts (in any field) to incessant scrutiny, testing, correction, and clarification. This suggests that a (...) totality of knowledge of the world or the absolute truth about things is a pair of ideals impossible of realization and approachable at best asymptotically. If his method is contextualist and fallilbilist, then his metaphysics is pluralistic. In his view reality is not reducible to just one single substance or principle but instead is constituted irreducibly of many different kinds of thing or principles. He is thus implacably opposed to any form of ontological monism—what James designates a “block-universe”—and Hegelian absolutism. Callaway conceives of the world as a Jamesian multiverse. Contextualism, fallibilism, and pluralism, then, are the themes brought to the fore in his book and which emerge from his travels at home and abroad. (shrink)

Three eminent scientists analyze the scientific, social, and political roots of biological determinism.

Dubbed “Nietzsche without his hammer” by literary critic James Wood, the Romanian philosopher E. M. Cioran is known as much for his profound pessimism and fatalistic approach as for the lyrical, raging prose with which he communicates them. Unlike many of his other works, such as _On the Heights of Despair_ and _Tears and Saints_, _The New Gods_ eschews his usual aphoristic approach in favor of more extensive and analytic essays. Returning to many of Cioran’s favorite themes, _The New Gods (...) _explores humanity’s attachment to gods, death, fear, and infirmity, in essays that vary widely in form and approach. In “Paleontology” Cioran describes a visit to a museum, finding the relatively pedestrian destination rife with decay, death, and human weakness. In another chapter, Cioran explores suicide in shorter, impressionistic bursts, while “The Demiurge” is a shambolic exploration of man’s relationship with good, evil, and God. All the while, _The New Gods _reaffirms Cioran’s belief in “lucid despair,” and his own signature mixture of pessimism and skepticism in language that never fails to be a pleasure. It was his mother's death which allowed [Roland] Barthes to write: "I looked through…" "To write on something is to forfeit it," Barthes used to say, reciprocally, it is licit to write on what is already dead, it was Barthes himself in one of his acceptations. His mother was for Barthes the internal order, who permitted both the external other and the I to exist. Once she was dead, his life was over and could therefore become the object of writing. (...) Barthes no doubt had other books to write; but he no longer had any life to live. I find it emblematic that his last book should have been "on photography" . Eloquent or discreet, a photograph never says anything but: I was there; it leads to a gesture of monstration, to a silent deixis, and symbolizes a pre- or post-discursive world; it makes me an object, that is, a dead man. What Barthes himself calls "my last investigation" also concerned death. Tzvetan Todorov, of the Centre National de la Recherche Scientifique in Paris, has numerous books on literary theory, including Théories du symbole and Symbolisme et interprétation, which has been published in English. His previous contribution to Crtitical Inquiry, "The Verbal Age," appeared in the Winter 1977 issue. The situated potentials for action between material things in the world and the interactional processes thereby afforded need to be seen as not only constituting the possibility of agency, but thereby also comprising it. Eo ipso, agency must be de-fused from any local, "contained" subject and be understood as a situational property in which subjects and objects can both participate. Any technological artifact should thus be understood as a complex of agential capacities that function relative to any number of social (...) and material factors. Keeping in mind that we are co-constituted by webs of relations involving increasingly complex collections of artefacts, networks, niches, and communities of practice, our investigation will be guided by interrogating the functional potential of a thing that in the last fifteen years has seamlessly worked its way into the everyday life of millions of human agents. This "thing," the smartphone, is merely a nodal point in a highly complex network. Recognizing this massive "collective," we nevertheless want to show some of the ways in which something as seemingly mundane as a smartphone can reflexively alter the range of actions available to a cognitive agent (Latour 1994). Specifically, we hope to shed light on some of the social-cognitive consequences of technological mediation by looking at the complementary, if not mutually implied, domains of memory and knowledge. Employing the National Institute of Mental Health-funded Prevention of Suicide in Primary Care Elderly Collaborative Trial as a case study, we discuss 2 sets of ethical issues: obtaining informed consent for a clinic-based intervention study and using treatment as usual (TAU) as the control condition. We then address these ethical issues in the context of the debate about the quality improvement efforts of health care organizations. Our analysis reveals the tension between ethics and scientific integrity involved with using TAU as (...) a control condition and the difficulty in designing high-quality research in a community-based setting. Language and tradition have long been relegated to the sidelines as scholars have considered the role of politics, science, technology and economics in the making of the modern world. This reading of over two centuries of philosophy, political theory, anthropology, folklore and history argues that new ways of imagining language and representing supposedly premodern people - the poor, labourers, country folk, non-europeans and women - made political and scientific revolutions possible. The connections between language ideologies, privileged linguistic codes, and political (...) concepts and practices shape the diverse ways we perceive ourselves and others. This 2003 book demonstrates that contemporary efforts to make schemes of social inequality based on race, gender, class and nationality seem compelling and legitimate, rely on deeply-rooted ideas about language and tradition. Showing how critics of modernity unwittingly reproduce these foundational fictions, it suggests strategies for challenging the undemocratic influence of these voices of modernity. (shrink)

The repertory grid method was used to determine what terminology respondents use to distinguish between different applications of genetic engineering drawn from food- related, agricultural, and medical applications. Respondents were asked to react to fifteen applications phrased in general terms, and results compared with a second study where fifteen more specific applications were used as stimuli. Both sets of data were submitted to generalized Procrustes analysis. Applications associated with animals or human genetic material were described as causing ethical concern, being (...) unnatural, harmful, and dangerous. Those involving plants or microorganisms were described as beneficial, progressive, and necessary. The results were validated in survey research, which indicated that general applications ofgenetic engineering were perceived as either positive or negative, whereas specific applications were more highly differentiated in perceptual terms. The results imply that the public debate about genetic engineering must take due account of the complexity of public concerns. (shrink)

This article advocates a naturalist and realist ethics of solidarity. Specifically, it argues that human needs should be met; and that they should be met in harmony with the environment. Realism should include respect for existing cultures and the morals presently being practiced – with reasonable exceptions. Dignity must come in a form understood and appreciated by the person whose dignity is being respected. It is also argued that naturalist ethics are needed to combat liberal ethics, not least because the (...) latter supports today’s inflexible and dysfunctional institutions. In arguing for these positions, reference is made to the naturalist realist ethics of Georges Canguilhem, C.H. Waddington, John Dewey and David Sloan Wilson, all of whom embed the social order in the natural order. (shrink)

Martin Heidegger--philosophy's hidden king or leading exponent of a dangerously misguided secular mysticism? Heidegger has been acclaimed as the most powerfully original philosopher of the twentieth century, who made a deep impact on thinkers such as Sartre, Habermas, Foucault, Lacan, and Derrida.

BACKGROUND Patients with chronic pain face significant barriers in finding clinicians to manage long-term opioid therapy (LTOT). For patients on LTOT, it is increasingly common to have them sign opioid treatment agreements (OTAs). OTAs enumerate the risks of opioids, as informed consent documents would, but also the requirements that patients must meet to receive LTOT. While there has been an ongoing scholarly discussion about the practical and ethical implications of OTA use in the abstract, little is known about how clinicians (...) use them and if OTAs themselves modify clinician prescribing practices. OBJECTIVE To determine how clinicians use OTAs and the potential impacts of OTAs on opioid prescribing. DESIGN We conducted qualitative analysis of four focus groups of clinicians from a large Midwestern academic medical center. Groups were organized according to self-identified prescribing patterns: two groups for clinicians who identified as prescribers of LTOT, and two who did not. PARTICIPANTS 17 clinicians from General Internal Medicine, Family Medicine, and Palliative Care were recruited using purposive, convenience sampling. APPROACH Discussions were recorded, transcribed, and analyzed for themes using reflexive thematic analysis by a multidisciplinary team. KEY RESULTS Our analysis identified three main themes: (1) OTAs did not influence clinicians’ decisions whether to use LTOT generally but did shape clinical decision-making for individual patients; (2) clinicians feel OTAs intensify the power they have over patients, though this was not uniformly judged as harmful; (3) there is a potential misalignment between the intended purposes of OTAs and their implementation. CONCLUSION This study reveals a complicated relationship between OTAs and access to pain management. While OTAs seem not to impact the clinicians’ decisions about whether to use LTOT generally, they do sometimes influence prescribing decisions for individual patients. Clinicians shared complex views about OTAs’ purposes, which shows the need for more clarity about how OTAs could be used to promote shared decision-making, joint accountability, informed consent, and patient education. ABSTRACTThis paper strengthens Bhaskar’s case for the possibility of naturalism. Building on Bhaskar’s A Realist Theory of Science and The Possibility of Naturalism, and on more recent contributions by Douglas Porpora, it traces the evolution of Bhaskar’s concept of 'intransitive' and follows his suggestion to treat social structure as an intransitive generative mechanism analogous to the generative mechanisms of the natural sciences. It is suggested, building on Porpora, that the constitutive rules of the market are usefully regarded as generating an (...) intransitive 'basic social structure.' That this same intransitive object is reasonably regarded as continuing to exist and act under different descriptions is illustrated by citing how different scholars have approached it with different concepts and vocabularies. It expands on Bhaskar’s first example of an intransitive object of social science, the mass unemployment that provided a ‘motor’ for Keynes, and on Porpora’s examples of the causal powers of social structures.
