Surveillance is essential for communicable disease prevention and control. Traditional notification of demographic and clinical information, about individuals with selected infectious diseases, allows appropriate public health action and is protected by public health and privacy legislation, but is slow and insensitive. Big data–based electronic surveillance, by commercial bodies and government agencies, which draws on a plethora of internet- and mobile device–based sources, has been widely accepted, if not universally welcomed. Similar anonymous digital sources also contain syndromic information, which can be (...) analysed, using customised algorithms, to rapidly predict infectious disease outbreaks, but the data are nonspecific and predictions sometimes misleading. However, public health authorities could use these online sources, in combination with de-identified personal health data, to provide more accurate and earlier warning of infectious disease events—including exotic or emerging infections—even before the cause is confirmed, and allow more timely public health intervention. Achieving optimal benefits would require access to selected data from personal electronic health and laboratory records and the potential to re-identify individuals found to be involved in outbreaks, to ensure appropriate care and infection control. Despite existing widespread digital surveillance and major potential community benefits of extending its use to communicable disease control, there is considerable public disquiet about allowing public health authorities access to personal health data. Informed public discussion, greater transparency and an ethical framework will be essential to build public trust in the use of new technology for communicable disease control. (shrink)

In the past decade, numerous ethical frameworks have been developed to support public health decision‐making in challenging areas. Before the COVID‐19 pandemic began, members of the authorship team were involved in research programmes, in which the development of ethical frameworks was planned, to guide (a) the use of new technologies for emerging infectious disease surveillance; and (b) the allocation of scarce supplies of pandemic influenza vaccine. However, as the pandemic evolved, significant practical challenges emerged that led to our questioning the (...) value of these frameworks. We now believe that a normative instrument, such as a framework, cannot adequately or reliably provide the ethical guidance that needs to be incorporated into public health decision‐making during natural disasters or infectious disease emergencies. Recently it has been suggested that there are potentially more dynamic, flexible, and effective ways to navigate decisions involving complex considerations entailed in policies and practices during a public health emergency. In this paper, we first outline the key functions of a public health ethics framework, before describing why we believe it would not be fit for purpose during a crisis. We end by considering whether proposed alternative methods to promote ethical public health decision‐making goals have the potential to meet these objectives. (shrink)

Background Outbreaks of infectious disease cause serious and costly health and social problems. Two new technologies – pathogen whole genome sequencing and Big Data analytics – promise to improve our capacity to detect and control outbreaks earlier, saving lives and resources. However, routinely using these technologies to capture more detailed and specific personal information could be perceived as intrusive and a threat to privacy. Method Four community juries were convened in two demographically different Sydney municipalities and two regional cities in (...) New South Wales, Australia to elicit the views of well-informed community members on the acceptability and legitimacy of: making pathogen WGS and linked administrative data available for public health researchusing this information in concert with data linkage and machine learning to enhance communicable disease surveillance systems Fifty participants of diverse backgrounds, mixed genders and ages were recruited by random-digit-dialling and topic-blinded social-media advertising. Each jury was presented with balanced factual evidence supporting different expert perspectives on the potential benefits and costs of technologically enhanced public health research and communicable disease surveillance and given the opportunity to question experts. Results Almost all jurors supported data linkage and WGS on routinely collected patient isolates for the purposes of public health research, provided standard de-identification practices were applied. However, allowing this information to be operationalised as a syndromic surveillance system was highly contentious with three juries voting in favour, and one against by narrow margins. For those in favour, support depended on several conditions related to system oversight and security being met. Those against were concerned about loss of privacy and did not trust Australian governments to run secure and effective systems. Conclusions Participants across all four events strongly supported the introduction of data linkage and pathogenomics to public health research under current research governance structures. Combining pathogen WGS with event-based data surveillance systems, however, is likely to be controversial because of a lack of public trust, even when the potential public health benefits are clear. Any suggestion of private sector involvement or commercialisation of WGS or surveillance data was unanimously rejected. (shrink)

Mobile phone-based applications (apps) can promote faster targeted actions to control COVID-19. However, digital contact tracing systems raise concerns about data security, system effectiveness, and their potential to normalise privacy-invasive surveillance technologies. In the absence of mandates, public uptake depends on the acceptability and perceived legitimacy of using technologies that log interactions between individuals to build public health capacity. We report on six online deliberative workshops convened in New South Wales to consider the appropriateness of using the COVIDSafe app to (...) enhance Australian contact tracing systems. All groups took the position (by majority) that the protections enacted in the app design and supporting legislation were appropriate. This support is contingent on several system attributes including: the voluntariness of the COVIDSafe app; that the system relies on proximity rather than location tracking; and, that data access is restricted to local public health practitioners undertaking contact tracing. Despite sustained scepticism in media coverage, there was an underlying willingness to trust Australian governing institutions such that in principle acceptance of the new contact tracing technology was easy to obtain. However, tensions between the need to prove system effectiveness through operational transparency and requirements for privacy protections could be limiting public uptake. Our study shows that informed citizens are willing to trade their privacy for common goods such as COVID-19 suppression. But low case numbers and cautionary public discourses can make trustworthiness difficult to establish because some will only do so when it can be demonstrated that the benefits justify the costs to individuals. This volume deploys recent feminist epistemological frameworks to analyze how concepts like knowledge, authority, rationality, objectivity and testimony were constructed in Greece and Rome. The introduction serves as a field guide to feminist epistemological interpretations of classical sources, and the following sixteen chapters treat a variety of genres and time periods, from Greek poetry, tragedy, philosophy, oratory, historiography and material culture to Roman comedy, epic, oratory, letters, law and their reception. By using an intersectional approach to demonstrate how epistemic systems (...) exclude and pathologize the experiences of ancient women and other oppressed groups, these contributions aid in the recovery of non-dominant narratives and reveal issues of sex, gender, race, ethnicity, sexual identity, religion, age, class, familial status and citizenship in the ancient and modern world. The volume contributes to a more inclusive and equitable study of classical antiquity and builds transhistorical connections capable of exposing similar injustices in our own time. (shrink)

This article argues that Iris Murdoch, who was supervised by John Wisdom during her 1947–48 fellowship at Newnham College Cambridge, went on to practice philosophy in a recognizably Wisdomian manner in her earliest paper, “Thinking and Language” (1951). To do so, I first describe how Wisdom understood philosophical perplexity and paradox. One task that linguistic philosophers should take up is to investigate the concrete cases that give paradoxical philosophical statements their sense and to sift the truth they contain from the (...) distortion. I then show how this vision informed his critical reception of Gilbert Ryle's The Concept of Mind as well as his own investigation of the Hidden Stream Paradox. Finally, I trace a similar approach to Murdoch's discussions of a paradox I call the Coarse Net Paradox. Recognizing Murdoch's intellectual inheritance from Wisdom enables us to see a thus‐far overlooked connection between Murdoch and the tradition of linguistic philosophy. (shrink)

Section 1 contrasts the approaches to Plato of F.M.Cornford and Gilbert Ryle, two of the early twentieth century’s leading Plato interpreters. Then I trace and evaluate attempts to discern in Plato’s Theaetetus a recognition of the role of the proposition. Section 2 focuses on the hunting of the proposition in Socrates’ Dream in the Theaetetus. Ryle, inspired by Logical Atomism, argued that Plato there anticipated an insight about the difference between names and propositions that Russell credited to Wittgenstein. I rehearse (...) difficulties for understanding the logos of the Dream as a proposition, preferring a reading of logos there as something like definition: a reading Wittgenstein also adumbrated. Section 3 examines attempts by writers such as Burnyeat and Kahn to find Plato allocating a starring role to the proposition in the argument at Theaetetus 185–6: perception, since it cannot grasp being, cannot grasp truth and hence cannot be knowledge. On their reading a grasp of being is understood as a grasp of something propositional. Problems for the propositional reading include Socrates’ talk of grasping ‘the truth of something’ (alētheia tinos), and the way the dialogue develops after 186. (shrink)

Of the later dialogues of Plato, the Sophists stand out. This article highlights the concept of sophist as propounded by Plato. A didactic approach runs through the text. Socrates harps on the relation between sophist, philosopher and a statesman. Are they three different or they are the same. The basic idea that Plato wants to convey is, both features highlight some of the key enigmas of the dialogue: What is the relation between the outer and middle parts? How seriously are (...) we to take the outer part? And is there a genuine, and successful, attempt to say what the sophist is? The article focuses on two key problems discussed and solved in the middle part: the late-learners' problem, and the problem of false statement. Organ transfer is rich terrain to investigate—especially in the American context, where sophisticated technological interventions have significantly shaped understandings of (...) health and well-being, suffering, and death. We report on the case of a 2-year-old female, the youngest person ever to undergo ovarian tissue cryopreservation (OTC). This patient was diagnosed with a rare form of sickle cell disease, which required a bone-marrow transplant, and late effects included high risk of future infertility or complete sterility. Ethical concerns are raised, as the patient's mother made the decision for OTC on the patient's behalf with the intention that this would secure the option of biological childbearing in the future. Drawing on the recognition that questions of discourse and power are vital components in analysing the public participation in environmental governance, this paper examines the ways in which dominant scientific discourses about the Earth's climate inform the types of public talk facilitated in and by mini-publics, particularly when they are 'scaled up' to address environmental issues such as climate change. World Wide Views on Global Warming (WWViews) serves as a case study. Conceived and organised by the Danish Board of Technology, (...) WWViews was a historically unprecedented public forum in which participants, invited from a range of nations, were given the opportunity to deliberate on key themes addressed in the negotiations taking place during the United Nations Convention on Climate Change in Copenhagen (COP 15). "Cult of ugliness," Ezra Pound’s phrase, powerfully summarizes the ways in which modernists such as Pound, T. S. Eliot, Wyndham Lewis, and T. E. Hulme—the self-styled "Men of 1914"—responded to the "horrid or sordid or disgusting" conditions of modernity by radically changing aesthetic theory and literary practice. Only the representation of "ugliness," they protested, would produce the new, truly "beautiful" work of art. They dissociated the beautiful from its traditional embodiment in female beauty, and from its association with Walter Pater (...) and Oscar Wilde. Their cultivation of ugliness displaced misogyny and homophobia. Higgins takes in texts such as John Ruskin’s art criticism, Eliot’s literary journalism, Lewis’s pro-fascism pamphlets, and the poetry of Pound, Conrad Aiken, and Langston Hughes. Two cross-modal experiments provide partial support for O'Regan & Noë's (O&N's) claim that sensorimotor contingencies mediate perception. Differences in locating a target sound accompanied by a spatially disparate neutral light correlate with whether the two stimuli were perceived as spatially unified. This correlation suggests that internal representations are necessary for conscious perception, which may also mediate sensorimotor contingencies.
