IntroductionBrain tumours frequently cause language impairments and are also likely to co-occur with localised abnormal slow-wave brain activity. However, it is unclear whether this applies specifically to low-grade brain tumours. We investigate slow-wave activity in resting-state electroencephalography in low-grade glioma and meningioma patients, and its relation to pre- and postoperative language functioning.MethodPatients with a glioma infiltrating the language-dominant hemisphere and patients with a meningioma with mass effect on this hemisphere underwent extensive language testing before and 1 year after surgery. EEG (...) was registered preoperatively, postoperatively, and once in healthy individuals. Slow-wave activity in delta- and theta- frequency bands was evaluated visually and quantitatively by spectral power at three levels over the scalp: the whole brain, the affected hemisphere, and the affected region.ResultsGlioma patients had increased delta activity and increased theta activity before and after surgery. In these patients, increased preoperative theta activity was related to the presence of language impairment, especially to poor word retrieval and grammatical performance. Preoperative slow-wave activity was also related to postoperative language outcomes. Meningioma patients showed no significant increase in EEG slow-wave activity compared to healthy individuals, but they presented with word retrieval, grammatical, and writing problems preoperatively, as well as with writing impairments postoperatively.DiscussionAlthough the brain-tumour pathology in low-grade gliomas and meningiomas has a different effect on resting-state brain activity, patients with low-grade gliomas and meningiomas both suffer from language impairments. Increased theta activity in glioma patients can be considered as a language-impairment marker, with prognostic value for language outcome after surgery. (shrink)

Background Genuine uncertainty on superiority of one intervention over the other is called equipoise. Physician-investigators in randomized controlled trials need equipoise at least in studies with more than minimal risks. Ideally, this equipoise is also present in patient-participants. In pediatrics, data on equipoise are lacking. We hypothesize that 1) lack of equipoise at enrolment among parents may reduce recruitment; 2) lack of equipoise during participation may reduce retention in patients assigned to a less favoured treatment-strategy. Methods We compared preferences of (...) parents/patients at enrolment, documented by a questionnaire, with preferences developed during follow-up by an interview-study to investigate equipoise of child-participants and parents in the BeSt-for-Kids-study. This trial in new-onset Juvenile Idiopathic Arthritis-patients consists of three strategies. One strategy comprises initial treatment with a biological disease-modifying-antirheumatic-drug, currently not standard-of-care. Semi-structured interviews were conducted with 23 parents and 7 patients, median 11 months after enrolment. Results Initially most parents and children were not in equipoise. Parents/patients who refused participation, regularly declined due to specific preferences. Many participating families preferred the biological-first-strategy. They participated to have a chance for this initial treatment, and would even consider stopping trial-participation when not randomized for it. Their conviction of superiority of the biological-first strategy was based on knowledge from internet and close relations. According to four parents, the physician-investigator preferred the biological-first-strategy, but the majority stated that she had no preferred strategy. In phase 2, preferences tended to change to the treatment actually received. Conclusions Lack of equipoise during enrolment did not reduce study recruitment, mainly due to the fact that preferred treatment was only available within the study. Still, when developing a trial it is important to evaluate whether the physicians’ research question is in line with preferences of the patient-group. By exploring so-called ‘informed patient-group’-equipoise, successful recruitment may be enhanced and bias avoided. In our study, lack of equipoise during trial-participation did not reduce retention in those assigned to a less favoured option. We observed a change for preference towards treatment actually received, possibly explained by comparable outcomes in all three arms. (shrink)

In this paper we provide an interpretation of Aristotle's rule for the universal quantifier in Topics Θ 157a34–37 and 160b1–6 in terms of Paul Lorenzen's dialogical logic. This is meant as a contribution to the rehabilitation of the role of dialectic within the Organon. After a review of earlier views of Aristotle on quantification, we argue that this rule is related to the dictum de omni in Prior Analytics A 24b28–29. This would be an indication of the dictum’s origin in (...) the context of dialectical games. One consequence of our approach is a novel explanation of the doctrine of the existential import of the quantifiers in dialectical terms. After a brief survey of Lorenzen's dialogical logic, we offer a set of rules for dialectical games based on previous work by Castelnérac and Marion, to which we add here the rule for the universal quantifier, as interpreted in terms of its counterpart in dialogical logic. The question of ontological foundation has undergone a noteworthy revival in recent years: metaphysicians today quarrel about how exactly to understand the asymmetrical and hyperintensional relationship of grounding. One of the reasons for this revival is that the old quantificationalist meta-ontology inherited from Quine has been effectively criticised by leading philosophers favourable to a meta-ontology, the aim of which is to come to know “which facts/items ground (constitute the base of) which other facts/items”, thus to examine the relation of ontological (...) dependence between beings (e.g. chemical properties depend on physical properties, the economic situation on the behaviour of individuals etc.), i.e. to explore the hierarchical structure of reality. I shall not discuss here the relationship of grounding in itself, but make some historical-formal remarks on the properties of the ultimate ontological foundational item itself and its aporetic nature. To do so I explore various more or less exotic philosophical ecosystems in the following order: Heidegger (Seyn), Plato (ἕν), Wáng Bi (道, dào). On the way I shall propose a new interpretation both of certain hypotheses in the Parmenides and of the nature of the opposition between Wáng Bì and Guō Xiàng in regard to the logical grammar of the expression “nothing (無, wú)” (transl. John Cook Wilson (1849–1915) was Wykeham Professor of Logic at New College, Oxford and the founder of ‘Oxford Realism’, a philosophical movement that flourished at Oxford during the first decades of the 20th century. Although trained as a classicist and a mathematician, his most important contribution was to the theory of knowledge, where he argued that knowledge is factive and not definable in terms of belief, and he criticized ‘hybrid’ and ‘externalist’ accounts. He also argued for direct realism in perception, (...) criticizing both empiricism and idealism, and argued for a moderate nominalist view of universals as being in rebus and only ‘apprehended’ by their particulars. His influence helped swaying Oxford away from idealism and, through figures such as H. A. Prichard, Gilbert Ryle, or J. L. Austin, his ideas were also to some extent at the origin of ‘moral intuitionism’ and ‘ordinary language philosophy’ which defined much of Oxford philosophy until the second half of the twentieth-century. Nevertheless, his name and legacy were all but forgotten for generations after World War II. Still, his views on knowledge are with us today, being in part at work in the writings of philosophers as diverse as John McDowell, Charles Travis, and Timothy Williamson. (shrink)

La raison pour laquelle il y a inadéquation de notre connaissance à la nature des corps extérieurs, mais aussi à celle de notre corps propre ainsi qu'à celle notre esprit, et donc à la nature de notre ego, c'est que nous sommes des êtres finis. Pour Descartes comme pour Spinoza la finitude de notre entendement rend impossible l'adéquation de la connaissance. À la connaissance adéquate, Descartes substitue la connaissance complète : certaine, mais non-absolue, vérifiée, mais seulement provisoire. La mise au (...) jour de cette première origine cartésienne de la théorie spinoziste de l'idée adéquate conduit l'auteur à l'hypothèse suivante : cette théorie accomplirait au sein de la métaphysique rationnelle développée dans l'Éthique la doctrine de la révélation et de la grâce présente dans la pensée théologique chrétienne. Dès lors il met en évidence la correspondance entre la vision béatifique suarézienne et la connaissance adéquate spinozienne, la première anticipant historiquement la seconde, à cette différence près que pour Thomas d'Aquin comme pour Suarez, « c'est la vision béatifique qui commande la connaissance », alors que c'est l'inverse pour Spinoza. Pour l'auteur, c'est le refus de toute création qui a conduit Spinoza à retrouver le modèle théologique de la vision béatifique comme moyen d'accès à la connaissance adéquates des choses et de Dieu. Inversion Quelle fonction pouvons-nous reconnaitre à la philosophie de la religion? Devons-nous même lui en reconnaître encore une? Les productions médiatiques contemporaines trouvent peut-être leur matrice dans le populaire, comme en atteste le développement important de la presse people. Si les personnalités qui y sont présentées n'appartiennent pas au peuple, elle sont néanmoins populaires, dans le sens où elles plaisent à un public très large, dans un jeu de tension entre accessibilité et inaccessibilité. Cette presse joue un rôle de médiation, d'intercession, dans une logique de proximité qui contamine aussi la télévision, dans les talks-shows comme les émissions de (...) télé-réalité. En dépassant le discours fondé sur l'aliénation des masses, on pourrait utiliser cette forme d'ascenseur tant descendant qu'ascendant comme outil de transformation sociale.Perhaps the contemporary media productions find their matrix in the popular one, as the significant development of the press people attests it. If the personalities which are presented there do not belong to the people, they are nevertheless popular, in the way they please a very large audience, in a play of tension between accessibility and inaccessibility. This press plays a part of mediation, of intercession, in a logic of proximity which contaminates also television, in the talks-shows like the emissions of real-tv. By exceeding the views based on the alienation of the masses, one could use this shape of elevator as well descendant as ascending like tool of social transformation. (shrink)

Les phénomènes apparaissent-ils toujours selon la calme adéquation en eux de l'intuition avec la signification, voire, plus souvent, avec un déficit d'intuition? Ou bien certains - les phénomènes saturés - n'apparaissent-ils pas plutôt grâce au surcroît irrépressible de l'intuition sur tous les concepts et toutes les significations que l'on voudrait leur assigner? Cette question avait surgi du principe " Autant de réduction, autant de donation " (dans Réduction et donation. Recherches sur Husserl, Heidegger et la phénoménologie, 1989) et conduit à (...) dégager la donation, telle qu'elle déplie ce qui se donne et ce qui se montre (avec Etant donné. Essai d'une phénoménologie de la donation, 1997). Reste, une fois ces acquis répétés, à étudier en eux-mêmes chacun des quatre types de phénomènes saturés : l'événement (saturé selon la quantité), l'idole ou tableau (saturé selon la qualité), la chair (saturée selon la relation) et enfin l'icône ou visage d'autrui (saturée selon la modalité). Il devient alors pensable d'étudier leur combinaison dans ce qu'on doit thématiser comme saturé à la puissance, un paradoxe des paradoxes - le phénomène de révélation. En l'occurrence, il s'agit de comprendre (contre une féconde critique de J. Derrida) les trois moments de la théologie mystique (affirmation, négation, hyperbole) non seulement comme l'accomplissement d'un phénomène saturé exemplaire, mais encore comme la répétition de toute phénoménalité de l'excès. After sketching an argument for radical anti-realism that does not appeal to human limitations but polynomial-time computability in its definition of feasibility, I revisit an argument by Wittgenstein on the surveyability of proofs, and then examine the consequences of its application to the notion of canonical proof in contemporary proof-theoretical-semantics.
