IntroductionBrain tumours frequently cause language impairments and are also likely to co-occur with localised abnormal slow-wave brain activity. However, it is unclear whether this applies specifically to low-grade brain tumours. We investigate slow-wave activity in resting-state electroencephalography in low-grade glioma and meningioma patients, and its relation to pre- and postoperative language functioning.MethodPatients with a glioma infiltrating the language-dominant hemisphere and patients with a meningioma with mass effect on this hemisphere underwent extensive language testing before and 1 year after surgery. EEG (...) was registered preoperatively, postoperatively, and once in healthy individuals. Slow-wave activity in delta- and theta- frequency bands was evaluated visually and quantitatively by spectral power at three levels over the scalp: the whole brain, the affected hemisphere, and the affected region.ResultsGlioma patients had increased delta activity and increased theta activity before and after surgery. In these patients, increased preoperative theta activity was related to the presence of language impairment, especially to poor word retrieval and grammatical performance. Preoperative slow-wave activity was also related to postoperative language outcomes. Meningioma patients showed no significant increase in EEG slow-wave activity compared to healthy individuals, but they presented with word retrieval, grammatical, and writing problems preoperatively, as well as with writing impairments postoperatively.DiscussionAlthough the brain-tumour pathology in low-grade gliomas and meningiomas has a different effect on resting-state brain activity, patients with low-grade gliomas and meningiomas both suffer from language impairments. Increased theta activity in glioma patients can be considered as a language-impairment marker, with prognostic value for language outcome after surgery. (shrink)

Background Genuine uncertainty on superiority of one intervention over the other is called equipoise. Physician-investigators in randomized controlled trials need equipoise at least in studies with more than minimal risks. Ideally, this equipoise is also present in patient-participants. In pediatrics, data on equipoise are lacking. We hypothesize that 1) lack of equipoise at enrolment among parents may reduce recruitment; 2) lack of equipoise during participation may reduce retention in patients assigned to a less favoured treatment-strategy. Methods We compared preferences of (...) parents/patients at enrolment, documented by a questionnaire, with preferences developed during follow-up by an interview-study to investigate equipoise of child-participants and parents in the BeSt-for-Kids-study. This trial in new-onset Juvenile Idiopathic Arthritis-patients consists of three strategies. One strategy comprises initial treatment with a biological disease-modifying-antirheumatic-drug, currently not standard-of-care. Semi-structured interviews were conducted with 23 parents and 7 patients, median 11 months after enrolment. Results Initially most parents and children were not in equipoise. Parents/patients who refused participation, regularly declined due to specific preferences. Many participating families preferred the biological-first-strategy. They participated to have a chance for this initial treatment, and would even consider stopping trial-participation when not randomized for it. Their conviction of superiority of the biological-first strategy was based on knowledge from internet and close relations. According to four parents, the physician-investigator preferred the biological-first-strategy, but the majority stated that she had no preferred strategy. In phase 2, preferences tended to change to the treatment actually received. Conclusions Lack of equipoise during enrolment did not reduce study recruitment, mainly due to the fact that preferred treatment was only available within the study. Still, when developing a trial it is important to evaluate whether the physicians’ research question is in line with preferences of the patient-group. By exploring so-called ‘informed patient-group’-equipoise, successful recruitment may be enhanced and bias avoided. In our study, lack of equipoise during trial-participation did not reduce retention in those assigned to a less favoured option. We observed a change for preference towards treatment actually received, possibly explained by comparable outcomes in all three arms. (shrink)

People’s decisions and actions are informed, influenced, and constrained by socially constructed social arrangements. Usually, these social arrangements are pre-determined, and people joining institutions or organizations may have, at least initially, little control or influence over them. Occasionally, however, but increasingly commonly in the transition to the “Digital Society,” people have an opportunity to self-determine their social arrangements “from scratch.” The issues then are: how do people gain experience in such founding processes, experiment safely with alternative social arrangements, and gain (...) expertise so that they can participate meaningfully, for example in consultation events. In this research paper, we address these issues by presenting a framework for “computer-supported social arrangements” tools which would enable people to specify, learn, apply, evaluate, and innovate social arrangements. This would contribute to empowering people and communities with the necessary experience, experimentation, and expertise for effective and sustainable self-determination of their own social arrangements. When international relations scholars think about international law they either ignore culture or offer highly deterministic accounts of its role. For the majority of scholars, international law is a rational construction, an institutional solution to the problem of order in an anarchical system, a body of rules and practices that reflect the contending interests and capabilities of major states. Issues of culture barely rate a mention. For others, culture is the deep foundation of international law, the structuring “mentality” that gives (...) law its form and content. International law, from this perspective, is a Western cultural artifact, globalized through centuries of imperialism and hegemony. These contrasting views lead to different expectations about the future of international law in today's culturally diverse international order. For rationalists, law's fate will be determined by the shifting configuration of interests that accompany new functional challenges and great power transitions. For the more culturally attuned, there are two possibilities. One is that functional utility will replace culture as law's foundation. International law may well be a Western cultural artifact, but “rational buy-in” will sustain it in a multicultural world. The other, more pessimistic, expectation is that the rule of international law will be fundamentally undermined by cultural diversity, particularly as rising non-Western powers articulate and promote markedly different cultural norms and values. Can the law benefit from an evolutionary perspective? This little book shows how the idea of survival of the fittest can help explain legal development and the rise and fall of legal institutions. The reader is invited to join in on a journey of discovery in which the world of Darwin is connected to the topics of legal change, convergence of law, legal complexity, law in hip-hop music and the adoption of the price-payment rule. This book seeks to explain why different systems of sovereign states have built different types of fundamental institutions to govern interstate relations. Why, for example, did the ancient Greeks operate a successful system of third-party arbitration, while international society today rests on a combination of international law and multilateral diplomacy? Why did the city-states of Renaissance Italy develop a system of oratorical diplomacy, while the states of absolutist Europe relied on naturalist international law and "old diplomacy"? Conventional explanations of basic (...) institutional practices have difficulty accounting for such variation. Christian Reus-Smit addresses this problem by presenting an alternative, "constructivist" theory of international institutional development, one that emphasizes the relationship between the social identity of the state and the nature and origin of basic institutional practices. Reus-Smit argues that international societies are shaped by deep constitutional structures that are based on prevailing beliefs about the moral purpose of the state, the organizing principle of sovereignty, and the norm of procedural justice. These structures inform the imaginations of institutional architects as they develop and adjust institutional arrangements between states. As he shows with detailed reference to ancient Greece, Renaissance Italy, absolutist Europe, and the modern world, different cultural and historical contexts lead to profoundly different constitutional structures and institutional practices. The first major study of its kind, this book is a significant addition to our theoretical and empirical understanding of international relations, past and present. (shrink)

In Negative Certainties, renowned philosopher Jean-Luc Marion challenges some of the most fundamental assumptions we have developed about knowledge: that it is categorical, predicative, and positive. This book opens up a new area of research by not only considering the rationality of such diverse phenomena as ordinary emotions, generalized anxiety disorder, social phobia, psychotic depression, major depressive disorder, and bipolar disorder, but also by evaluating the question whether the vagueness of these diverse disorders and emotions poses an obstacle to the rationality of these phenomena. As these emotional phenomena turn out to be vague on many different levels, an explanation is found for the millennia long dispute (...) of which kind of phenomena fall under the emotions and whether such diverse phenomena as hope and alexithymia fall under the emotions. Reprise reprend et prolonge ces nouvelles interrogations. D'abord la question de la réduction : définit-elle vraiment le principe dernier de la phénoménologie? Si tel était le cas, la formule " autant de réduction, autant de donation " peut-elle se justifier (en discussion avec la critique de Michel Henry)? Ensuite, la reconnaissance du donné comme instance (...) première et dernière de la phénoménalité peut-elle encore faire droit à l'herméneutique (en discussion avec Hans-Georg Gadamer)? Dans ce cas, comment se déplie le pli de la donation avec la manifestation? Et encore, le monde peut-il se manifester comme l'un des phénomènes donnés, et même comme la totalité du donné? Ou ne faut-il pas lui reconnaître une donation par exception, celle de la possibilité de toute donation (sur la ligne de Jan Patocka)? Jean-Luc Marion has long endeavored to broaden our view of truth. In this illuminating new book--his deepest engagement with theology to date--Marion proposes a rigorous new understanding of human and divine revelation in a deeply phenomenological key. The concept 'Experiencing Level' points to the manner in which what a person says relates to felt experience. The manner is a first person process which is quantitatively measurable. Examples of low, middle and high Experiencing are given. In a high experiencing manner a person attends directly to a bodily sense of what is implicit and allows words to emerge from that sense. The Experiencing Scale which measures the manner of process is a third person rating of a first person (...) process, according to precise linguistic and somatic characteristics. A new rating method gives high reliability. I will briefly summarize several of the more than one hundred research studies which have used the EXP Scale or other measures of high EXP process. The high end of the EXP Scale describes what came to be called 'Focusing'. Because EXP level is a variable of the manner of process, it can be applied to almost any content area. Examples from tape-recorded psychotherapy sessions, creative writing, and theory-building will be analyzed in terms of experiencing level. Having defined the variable in observable terms makes it possible to formulate exact steps for teaching high Experiencing. Two practices -- Focusing and Thinking at the Edge -- have been developed, which can be taught in precise steps. This kind of third person variable can be found only from first person process. The problems of racism and racially motivated violence in predominantly African American communities in the United States are complex, multifactorial, and historically rooted. While these problems are also deeply morally troubling, bioethicists have not contributed substantially to addressing them. Concern for justice has been one of the core commitments of bioethics. For this and other reasons, bioethicists should contribute to addressing these problems. We consider how bioethicists can offer meaningful contributions to the public discourse, research, teaching, training, policy development, and (...) academic scholarship in response to the alarming and persistent patterns of racism and implicit biases associated with it. To make any useful contribution, bioethicists will require preparation and should expect to play a significant role through collaborative action with others. (shrink)

"Schwarzen Hefte" haben umfassender und deutlicher als je zuvor die Verbindungen zwischen Philosophie und Politik bei Heidegger sichtbar werden lassen und damit eine neue Debatte uber den Rang dieses Denkens entfacht. DEnn die dort enthaltenen Texte demonstrieren, dass Philosophie, Metapolitik und Politik bei Heidegger eine Einheit bilden, zu der die Befurwortung des Nationalsozialismus und der Antisemitismus gehoren. INs Zentrum der kritischen Revision ruckt das erste Hauptwerk von 1927, "Sein und Zeit", das nahezu unangefochten als epochaler Beitrag zur Philosophie (...) des 20. JAhrhunderts gilt. DIe "Schwarzen Hefte" fordern eine "Destruktion" dieses Werks auch insofern heraus, als die dort prasentierten Selbstinterpretationen das philosophische Profil dieses Denkens scharfen und neue Gesichtspunkte fur die Diskussion seiner Substanz im philosophischen und ideologischen Kontext des 20. JAhrhunderts eroffnen. 90 Jahre nach seinem Erscheinen stellt sich der philosophischen Forschung die kritische Auseinandersetzung mit "Sein und Zeit" als unabweisbare Aufgabe. IM Vordergrund stehen hierbei zwei Fragen: Wie ist das Unternehmen einer in der "Fundamentalontologie" des Daseins begrundeten temporalen Ontologie der systematischen Intention und Durchfuhrung fachphilosophisch zu beurteilen? Human enhancement issues are not merely academic: the technologies and trends involved give rise to new needs and social demands, provide opportunities for individuals and society, and present new risks. They also challenge crucial cultural notions, social concepts and views of the human condition, and may cause new forms of social pressure and social exclusion. This chapter reports on a systematic attempt by the European Technology Assessment Group (ETAG), conducted on behalf of the Science and Technology Options Assessment (STOA) panel (...) of the European Parliament, to suggest policy options on the issue of human enhancement. It outlines and discusses a number of strategies for dealing with the issue in a European policy context, identifying a reasoned pro‐enhancement approach, a reasoned restrictive approach, and a systematic case‐by‐case approach as viable options. This essay discusses the nature of collective responsibility and explores various controversies associated with its possibility and normative value.

Natural kinds is a widely used and pivotal concept in philosophy – the idea being that the classifications and taxonomies employed by science correspond to the real kinds in nature. Natural kinds are often opposed to the idea of kinds in the human and social sciences, which are typically seen as social constructions, characterised by changing norms and resisting scientific reduction. Yet human beings are also a subject of scientific study.Does this mean humans fall into corresponding kinds of their own? (...) In The Epistemology and Morality of Human Kinds Marion Godman defends the idea of human kinds. She first examines the scientific use and nature of human kinds, considering the arguments of key philosophers whose work bears upon human kinds, such as Ian Hacking, John Searle, Richard Boyd and Ruth Millikan. Using the examples of gender, ethnic minorities and Buddhism she then argues that human kinds are a result of ongoing historical reproduction, chiefly due to pre-existing cultural models and social learning. Her novel argument shifts the focus away from the reductionism characteristic of research about human kinds. Instead, she argues that they are “multiply projectable” and deserving of scientific study not in spite of, but because of their role in explaining our identity, injustice and the emergence of group rights. Iris Marion Young (1949-2006) was one of the most influential and innovative political theorists of her generation who had a significant impact on a wide range of topics such as democratic theory, feminist theory, and justice. She bridged many longstanding divides among political theorists, engaging in Continental and critical theory, but also insisting on the importance of normative argument: her corpus stands as a testament to the fruitfulness of engaging in both abstract theory and the 'real world' of everyday (...) politics. This volume spans the several decades of her work, illustrating her intellectual development over time through three major areas of innovation: Gender: Maintaining that gender is both conceptually and politically meaningful, Young theorized gender in terms of structures that, in combination, position different people we call "women" in different ways, such that some women have some structures in common, without all women sharing all gendered structures in common. Justice: Young's early writings on a critical theory of justice evolved in her later and posthumously published works where she developed an account of justice that brought together her theorization of structure with her concern to respond to contemporary claims of injustice. The Politics of Difference: Young rejected universal and abstract theories of justice and maintained that justice instead required attending to the experiences of people marked by difference. La question de la peinture n'appartient ni d'abord, ni uniquement aux peintres ou aux esthéticiens. Elle appartient à la visibilité elle-même, donc à tous. A dire vrai ou plus exactement à tous ceux pour qui voir ne va pas de soi. Et c'est sans doute pourquoi la philosophie ne peut que se trouver, quand il y va de la peinture, à demeure. En effet, la philosophie a pris aujourd'hui une figure essentielle, la phénoménologie ; or la phénoménologie ne prétend revenir (...) aux choses mêmes que parce qu'elle entreprend d'abord de voir ce qui se donne - ce que cela donne. La visibilité exceptionnelle du tableau devient alors un cas privilégié du phénomène, éventuellement une voie vers la phénoménalité en général. Mais la phénoménologie suffit-elle à cerner la visibilité et donc tous les tableaux possibles? Le tableau n'admet-il lui-même qu'un seul statut, ou ne ménage-t-il pas d'autres ressources? En passant de l'idole à l'icône, nous poursuivons certes des recherches antérieures, mais nous suivons surtout la nécessité de la chose même : le tableau, donc le visible par excellence, s'offre au dilemme de deux figures d'apparition, inverses, adverses et pourtant indispensables, inséparables. La théologie devient, dans cette situation, une instance irrécusable de toute théorie du tableau. Pour l'avoir parfois dénié, puis simplement oublié, la pensée esthétique s'est parfois empêtrée dans de longues apories. We live today in the first global system of sovereign states in history, encompassing all of the world's polities, peoples, religions and civilizations. Christian Reus-Smit presents a new account of how this system came to be, one in which struggles for individual rights play a central role. The international system expanded from its original European core in five great waves, each involving the fragmentation of one or more empires into a host of successor sovereign states. In the most important, associated (...) with the Westphalian settlement, the independence of Latin America, and post-1945 decolonization, the mobilization of new ideas about individual rights challenged imperial legitimacy, and when empires failed to recognize these new rights, subject peoples sought sovereign independence.
