We consider, from a physical perspective, the case where the interface between an organism and its environment becomes large enough that it acts as a buffer regulating their matter and energy exchanges. We illustrate the physiological and evolutionary role of buffers through the example of lungfish estivation. Then we ponder the relevance of buffers of this kind to the quest for a general definition of concepts like niche construction, the extended phenotype, and related ones, whose meaning is conveyed at present (...) mostly through particular examples. When entering an environment, animals – including humans – tend to consult their memories to determine what they know about the place. This information is useful to determine: is this place safe? And what happens next? In this chapter, we argue on both empirical and conceptual grounds that memory is largely organized by space. Spatial relations determine what is recalled and which experiences are combined in generalizations. Time does not play an analogous role. We show that space and time in (...) memory are thus deeply asymmetrical. We conclude with a consequence of our view: spatial organization entails a basic form of abstraction in a way temporal organization does not, and so the spatial organization of memory reveals that memory involves generalization from the very beginning. Scholars have proposed a number of courses and programs intended to improve the ethical behavior of scientists in an attempt to maintain the integrity of the scientific enterprise. In the present study, we conducted a quantitative meta-analysis based on 26 previous ethics program evaluation efforts, and the results showed that the overall effectiveness of ethics instruction was modest. The effects of ethics instruction, however, were related to a number of instructional program factors, such as course content and delivery methods, in (...) addition to factors of the evaluation study itself, such as the field of investigator and criterion measure utilized. An examination of the characteristics contributing to the relative effectiveness of instructional programs revealed that more successful programs were conducted as seminars separate from the standard curricula rather than being embedded in existing courses. Furthermore, more successful programs were case based and interactive, and they allowed participants to learn and practice the application of real-world ethical decision-making skills. The implications of these findings for future course development and evaluation are discussed. (shrink)

The University of Pittsburgh Medical Center (UPMC) "Policy for the Management of Terminally Ill Patients Who May Become Organ Donors after Death" proposes to take organs from certain patients as soon as possible after expected cardiopulmonary death. This policy requires clear understanding of the descriptive state of the donor's critical cardiopulmonary and neurologic functional capacity at the time interventions to sustain or harvest organs are undertaken. It also requires strong consensus about the moral and legal status of the donor during (...) dying and confirmation of death. There has been no need for the clarity and precision that this policy relies upon, and thus the needed research and conceptual work has not previously been generated. The empiric base and societal consensus are both too frail to provide justification for this policy at this time. (shrink)

This paper explains how middle managers might enlist ethically engaged employees into the production of reliable, sustainable CSR. An accompanying model illustrates how those managers can encounter employee engagement in CSR and channel their enthusiasm effectively. It presents factors scaffolding organizational support for employee engagement and how they relate to the intensity of that engagement. It introduces the importance of employee voice and illustrates how associated signals might be captured.

We have conducted a retrospective study of deaths on a paediatric medical intensive care unit over a two-year period and reviewed similar series from outside the UK. There were 89 deaths out of 651 admission (13.7% mortality). In almost two-thirds of the cases death occurred with a decision to limit medical treatment or withdraw mechanical ventilation, implying that additional or further therapy was considered futile. We highlight this as a crucially important issue in the practice of intensive care. More comprehensive (...) studies are needed to help clinicians derive consensus on what constitutes a futile intervention, and therefore when such an intervention should be withheld. This will help families and society better understand the limitations of intensive care. (shrink)

The COVID-19 pandemic has highlighted the risks that can be involved in healthcare work. In this paper, we explore the issue of staff safety in clinical work using the example of personal protective equipment in the COVID-19 crisis. We articulate some of the specific ethical challenges around PPE currently being faced by front-line clinicians, and develop an approach to staff safety that involves balancing duty to care and personal well-being. We describe each of these values, and present a decision-making framework (...) that integrates the two. The aim of the framework is to guide the process of balancing these two values when staff safety is at stake, by facilitating ethical reflection and/or decision-making that is systematic, specific and transparent. It provides a structure for individual reflection, collaborative staff discussion, and decision-making by those responsible for teams, departments and other groups of healthcare staff. Overall the framework guides the decision maker to characterise the degree of risk to staff, articulate feasible options for staff protection in that specific setting and identify the option that ensures any decrease in patient care is proportionate to the increase in staff well-being. It applies specifically to issues of PPE in COVID-19, and also has potential to assist decision makers in other situations involving protection of healthcare staff. There are no data in this work. When healthcare professionals feel constrained from acting in a patient’s best interests, moral distress ensues. The resulting negative sequelae of burnout, poor retention rates, and ultimately poor patient care are well recognized across healthcare providers. Yet an appreciation of how particular disciplines, including physicians, come to be “constrained” in their actions is still lacking. This paper will examine how the application of shared decision-making may contribute to the experience of moral distress for physicians and why such distress may go under-recognized. (...) This paper articulates an account of imperatives that sensibly supports the idea of a logic of imperative inferences. We rebuke common objections to the very possibility of such a logic, from a perspective based on recent linguistic work on the morphosyntax of imperatives. Specifically, we develop the notion that the content of an imperative sentence includes both a force operator alongside an imperational content to which the force applies. We further argue that this account of the content of imperatives constitutes (...) a plausible and flexible framework to develop a logic of imperative by examining a number of reconstructions of this idea using semantical analogs of widespread modal semantics. After studying the performance of those approaches, we conclude that progress in imperative logic has been hindered by the failure to adopt conflict-tolerant and resource-sensitive semantics, but suggest that such considerations can be incorporated in this flexible framework. Finally, we also propose a simple account of the difference between operators applying to the content of imperatives and imperative operators, in a way that sheds light on some of the issues underlying the usual antinomies. The subject of medieval scribes is bound up with the question of textual authority. Scribes not only left their marks upon the manuscripts they copied, they also functioned as interpreters, editing and consequently altering the meaning of texts. Writers, however, did not simply employ scribes as copyists; they elaborated upon the figurative language associated with the book as a symbol and incorporated scribes into their texts as tropes. Such “ghostly scribes” provided authors with figures through which they could project authorial (...) personas, indicate what we would call generic categories, express a sense of community, or guide a reader's responses to a text. Though a writer like Chaucer employed scribal metaphors to signal his relative powerlessness, thereby indicating the outlines of a carefully conceived and concealed persona, women writers such as Hildegard of Bingen and Christine de Pisan exploited those same metaphors to signal both their sense of authority and their awareness of the social constraints placed upon it. The two most important English women writers of the late Middle Ages, Julian of Norwich and Margery Kempe, seem especially aware of the ways in which the deployment of a scribe could be used strategically, as a means of maintaining control over texts they profess neither to control nor to aspire to control. Alderson critiques our recent book on the basis that it overlooks children’s own views about their medical treatment. In this response, we discuss the complexity of the paediatric clinical context and the value of diverse approaches to investigating paediatric ethics. Our book focuses on a specific problem: entrenched disagreements between doctors and parents about a child’s medical treatment in the context of a paediatric hospital. As clinical ethicists, our research question arose from clinicians’ concerns in practice: What should a clinician (...) do when he or she thinks that parents are choosing a treatment pathway that does not serve the child’s best interests? Alderson’s work, in contrast, focuses on the much broader issue of children’s role in decision-making about treatment and research. We argue that these different types of work are zooming in on different aspects of paediatric ethics, with its complex mix of agents, issues and relationships. As one of the preeminent philosophers of the twentieth century, W. V. Quine made groundbreaking contributions to the philosophy of science, mathematical logic, and the philosophy of language. This collection of essays examines Quine's views, particularly his holism and naturalism, for their value to feminist theorizing today. Some contributors to this volume see Quine as severely challenging basic tenets of the logico-empiricist tradition in the philosophy of science—the analytic/synthetic distinction, verificationism, foundationalism—and accept various of his positions as potential resources for (...) feminist critique. Other contributors regard Quine as an unrepentant empiricist and, unlike feminists who seek to use or extend his arguments, they interpret his positions as far less radical and more problematic. In particular, critics and advocates of Quine's arguments that the philosophy of science should be "naturalized"—understood and pursued as an enterprise continuous with the sciences proper—disagree deeply about whether such a naturalized philosophy is "philosophy enough." Central issues at stake in these disagreements reflect current questions of special interest to feminists and also bridge the analytic and postmodern traditions. They include questions about whether and how the philosophy of science, as a form of practice, is or can be normative as well as questions concerning the implications of Quine's philosophy of language for the transparency and stability of meaning. In representing feminist philosophy centrally engaged with the analytic tradition, this volume is important not only for what it contributes to the understanding of Quine and naturalized epistemology but also for what it accomplishes in working against restrictive conceptions of the place of feminism within the discipline.
