Empirical research pertaining to cardiopulmonary resuscitation (CPR), clinician behaviors related to do-not-resuscitate (DNR) orders and substituted judgment suggests potential contributions to medical ethics. Research quantifying the likelihood of surviving CPR points to the need for further philosophical analysis of the limitations of the patient autonomy in decision making, the nature and definition of medical futility, and the relationship between futility and professional standards. Research on DNR orders has identified barriers to the goal of patient involvement in these life and death (...) discussions. The initial data on surrogate decision making also points to the need for a reexamination of the moral basis for substituted judgment, the moral authority of proxy decision making and the second-order status of the best interests standard. These examples of empirical research suggest that an interplay between empirical research, ethical analysis and policy development may represent a new form of interdisciplinary scholarship to improve clinical medicine. The advent of deep sequencing technology has unexpectedly advanced our structural understanding of molecules composed of nucleic acids. A significant amount of progress has been made recently extrapolating the chemical methods to probe RNA structure into sequencing methods. Herein we review some of the canonical methods to analyze RNA structure, and then we outline how these have been used to probe the structure of many RNAs in parallel. The key is the transformation of structural biology problems into sequencing problems, whereby (...) sequencing power can be interpreted to understand nucleic acid proximity, nucleic acid conformation, or nucleic acid‐protein interactions. Utilizing such technologies in this way has the promise to provide novel structural insights into the mechanisms that control normal cellular physiology and provide insight into how structure could be perturbed in disease. This is the first book to evaluate the significance and usefulness of the practices of anthropomorphism and anecdotalism for understanding animals.

We discuss the necessity of conscious thinking in the single-system propositional model of learning. Research from honeybees to humans suggests that associative learning can take place without the need for controlled reasoning or the development of beliefs of relationships between objects or events. We conclude that a single learning system is possible, but not if it depends on complex thinking.

Heyes correctly points out some problems in primate theory of mind, but lacks a critical approach to children's theory of mind, and at times implies meta-awareness when discussing theory of mind. Also, in selecting pure experimental designs, she ignores its limitations, as well as the merits, and at times the necessity, of other methodologies.

Wilkins & Wakefield's intriguing model of language evolution is deficient in evidence of human uniqueness in metaphorical matching, amodal representation, reference, conceptual structure, hierarchical organization, linguistic comprehension, sign use, laterality, and handedness. Primates show communicative reference, laterality, and handedness, and apes in particular show hierarchical organization, conceptual structure, cross-modal abilities, sign use, and displaced reference.

When my family discusses how we should spend a summer holiday, we start from certain common understandings about our preferences. We prefer self-catering accommodation to hotels, and hotels to campsites. We prefer walking and looking at scenery and wildlife to big-city sightseeing and shopping. When it comes to walks, we prefer walks of six miles or so to ones which are much shorter or much longer, and prefer well-marked but uncrowded paths to ones which are either more rugged or (...) more popular. And so on. These common understandings greatly simplify the task of choosing between holiday destinations and activities, by allowing us quickly to eliminate many options. Bender, Robert Review of: Income inequality, equality of opportunity and intergenerational mobility, by Miles Corak, Institute for the Study of Labor, Bonn University, 2013, 32 pages.

Robert F. is an eighty-five-year-old who suffered a heart attack at home in a rural location some thirty minutes from any major hospital. By the time the paramedics arrived, he was unconscious and nonresponsive. After spontaneous return of circulation, they began their standard procedure of therapeutic hypothermia. Robert's core temperature was lowered using ice packs, and cold intravenous fluids were initiated. Soon afterward, Robert started to shiver when his body temperature reached 35.6° Celsius. He was then given (...) a bolus of vecuronium as a neuromuscular blockade, sedated, and intubated. He was also given a low-dose vasopressin for blood-pressure control. Shortly after Robert arrived in the emergency room, his daughter, his medical decision-maker, produced an advance directive documenting that her father has a do-not-resuscitate order, and she demanded that the breathing tube and any other life-sustaining treatments be withdrawn immediately. The medical staff is very reluctant to comply with this demand for immediate action. Until the neuromuscular blockade wears off, removing the ventilator will prevent Robert from breathing. Furthermore, it may take some time to reverse the therapeutic hypothermia procedure to the point that the patient is at normal temperature. In addition, therapeutic hypothermia itself often causes arrest, so the patient may need to be resuscitated again. Should the staff wait until the patient is warm or honor the decision of his daughter, who holds his medical power of attorney? (shrink)

InLeibniz: Perception, Apperception, and Thought, Robert McRae alleges a flat “contradiction” at the heart of Leibniz's doctrine of three grades of monads: bare entelechies characterized by perception; animal souls capable both of perception and of sensation; and rational souls, minds or spirits endowed not only with capacities for perception and sensation but also with consciousness of self or what Leibniz calls “apperception.” Apperception is a necessary condition of those distinctively human mental processes associated with understanding and with reason. Insofar (...) as it is also a sufficient condition of rationality, it is not ascribable to animals. But apperception is a necessary condition of sensation or feeling as well; and animals are capable of sensation, according to Leibniz, who decisively rejected the Cartesian doctrine that beasts are nothing but material automata. “On the one hand,” writes McRae, “what distinguishes animals from lower forms of life is sensation or feeling, but on the other hand apperception is a necessary condition of sensation, and apperception distinguishes human beings from animals”. “We are thus left with an unresolved inconsistency in Leibniz's account of sensation, so far as sensation is attributable both to men and animals”. In two separate studies, published some four years apart, Robert McRae has argued the provocative thesis that the idea of extension is not to be numbered among the ideas accounted innate by Descartes, but among the adventitious. He has defended this view despite explicit statements to the contrary by Descartes both in the Correspondence and in the second part of the Principles of Philosophy. Against such evidence McRae has urged the overriding importance of the sixth Meditation, where, he alleges, (...) Descartes asserts “unequivocally… that the idea of extension is produced in us by bodies and is therefore not innate”. It is only in the later of the two studies that McRae's reasons for regarding the testimony of the sixth Meditation as authoritative are fully spelt out. Express statements to the contrary “must give away”, he writes, “for the proof of the existence of body by the adventitious idea of it is absolutely crucial to the Meditations”. In this paper I propose to challenge McRae's interpretation of the extension of the concept “innate idea”, while acknowledging his signal contribution to the clearing up of the intensional meaning, or rather meanings, of the concept of innateness. My thesis is that “extension” is an innate idea, though constraints of time will allow me to do little more than try to show that McRae's stated reasons for rejecting this view are not compelling. This paper addresses a question which is fundamental to the perceived legitimacy of the distribution of resources today: to what extent does unfairness in how assets came to be acquired in the past affect incomes and wealth now? To answer that question requires two things: first, a principle to determine what is, and what is not, a just acquisition of wealth or a just source of income; second, a means of using that principle to estimate what fraction of wealth and (...) income is now unjust. I use a principle put forward by Robert Nozick to provide the first of these things and then use a model of wealth accumulation and economic growth to illustrate its implications for the scale of unfairness today. The greater is depreciation of assets, the higher are saving rates out of labour income and the less important is human capital the more transient are the effects of past economic injustices. I use data on the perceived unfairness of economic outcomes to see if there is any evidence that those features which the model implies should influence the durability of injustice help explain cross-country differences in attitudes towards unfairness. Consideration of the amazing organized intricacy of human cortical anatomy entails a deeper appreciation of nature that is fully consistent with a mature religious spirit. A brain seems at first glance to be a mere lump of grayish claylike stuff, but facts of basic neuroanatomy compel us to consider that this particular kind of stuff may really contain all the richly tangible and richly ghostly inner essences of emotion, thought, and behavior. Humans are the “college graduates” of evolution. The human (...) cortex is 3,400 times the volume of, yet only slightly thicker (about 3 millimeters) than, that of the mouse. This remarkable sheet is as thin as a graduation‐day “mortarboard” cap, but its 2,600 square centimeter area is four times as large (about 20 × 20 inches if a square; both metric and English units used deliberately). Zooming in, there are about 50 billion cortical neurons; though named after “pyramids,” they are more like tiny “magic trees,” with branches and roots so long and fine that there are 1 or 2 miles of these electrically scintillating fibers within each cubic millimeter of cortex. Cortical neurons communicate intimately: viewed from above, beneath a single square millimeter 100,000 nerve cells intertwine; each such neuron makes 5,000 or more connections with others. These and many additional amazing facts about brain tissue, together with some conjectures about dense connectedness in the mathematics of graph theory, help to bear out the groundwork prepared by such pioneers as Ralph Wendell Burhoe that the spirit and knowledge of science might rejoin that of religion. If it takes enchanted matter to contain consciousness, this is a kind of enchantment that science may well be able to penetrate for eventual thoroughgoing understanding. Inevitable by‐products will be greater reverence for nature and greater awe at the mystery of nature's origin. (shrink)

The predicates 'is outgrown by Theaetetus,' 'is 300 miles west of a lemur,' and 'is such that 9 is odd' denote properties, but there is a sense in which these properties are not genuine features of the objects that have them. The fact that we find these mere-Cambridge properties odd has something to do with their relational character. But relationality in itself is not an adequate criterion for property-genuineness for there are many relational properties that do not qualify as (...) mere-Cambridge. The goal of this essay is to isolate the special type of relationality needed to explain what makes a property genuine. In the process, the author evaluates causal accounts of property-genuineness, and shows how the analysis presented is superior. (shrink)

The Significance of Taste: Kant, Aesthetic and Reflective Judgment ROBERT B. PIPPIN 1? THE FUNDAMENTAL QUESTION of the "Analytic of the Beautiful" in the "Critique of Aesthetic Judgment" is easy enough to identify. On what basis, if any, could one claim some sort of universal a priori validity for judgments of the form, "This is beautiful"? In Kant's well-known analysis of this question, the issue is reformulated as: By what right could one claim that another person ought to feel (...) pleasure in the presence of certain objects? Let us call this the "basic question."' There is controversy enough about what Kant means by this basic question and how a deduction of the validity of such judgments is supposed to work. However, shortly after Kant began serious work on a "Critique of Taste" in ~787, the whole issue became even more complicated when the proposed work became a full-blown Critique of Judgment. The question of aesthetic judg- ment was presented within the new, larger topic of reflective judgment, was presumably thereby linked to the problem of teleological judgments, and so to the great general theme of the whole of the third Critique: the purposiveness I am grateful, for comments and criticisms, to Henry Allison, Volker Gerhardt, Rudolf Makkreel, Miles Rind, and to two anonymous referees for this journal. This exploratory qualitative study investigated how doctoral students reported their personal and professional interaction experiences that they believed might facilitate or impede their academic pursuits in biomedical research. We collected 19 in-depth interviews with doctoral students in biomedical research from eight universities, and we based our qualitative analytic approach on the work of Miles and Huberman. The results indicated that among different sources and types of interaction, academic and emotional interactions from family and teachers in various stages essentially affected (...) students’ persistence in the biomedical science field. In addition, co-mentorship among peers, departmental environment, and volunteer experiences were other essential factors. This study also found related experiences among women and underrepresented minority students that were important to their academic pursuit. (shrink)

Some time in the late 1590s, the Welsh amateur mathematician John Bulkeley wrote to Thomas Harriot asking his opinion about the properties of a truly gargantuan (but totally imaginary) plano-spherical convex lens, 48 feet in diameter. While Bulkeley’s original letter is lost, Harriot devoted several pages to the optical properties of “Mr Bulkeley his Glasse” in his optical papers (now in British Library MS Add. 6789), paying particular attention to the place of its burning point. Harriot’s calculational methods in these (...) papers are almost unique in Harriot’s optical remains, in that he uses both the sine law of refraction and interpolation from Witelo’s refraction tables in order to analyze the passage of light through the glass. For this and other reasons, it is very likely that Harriot wrote his papers on Bulkeley’s glass very shortly after his discovery of the law and while still working closely with Witelo’s great Optics; the papers represent, perhaps, his very first application of the law. His and Bulkeley’s interest in this giant glass conform to a long English tradition of curiosity about the optical and burning properties of large glasses, which grew more intense in late sixteenth-century England. In particular, Thomas Digges’s bold and widely known assertions about his father’s glasses that could see things several miles distant and could burn objects a half-mile or further away may have attracted Harriot and Bulkeley’s skeptical attention; for Harriot’s analysis of the burning distance and the intensity of Bulkeley’s fantastic lens, it shows that Digges’s claims could never have been true about any real lens (and this, I propose, was what Bulkeley had asked about in his original letter to Harriot). There was also a deeper, mathematical relevance to the problem that may have caught Harriot’s attention. His most recent source on refraction—Giambattista della Porta’s De refractione of 1593—identified a mathematical flaw in Witelo’s cursory suggestion about the optics of a lens (the only place that lenses appear, however fleetingly, in the writings of the thirteenth-century Perspectivist authors). In his early notes on optics, in a copy of Witelo’s optics, Harriot highlighted Witelo’s remarks on the lens and della Porta’s criticism (which he found unsatisfactory). The most significant problem with Witelo’s theorem would disappear as the radius of curvature of the lens approached infinity. Bulkeley’s gigantic glass, then, may have provided Harriot an opportunity to test out Witelo’s claims about a plano-spherical glass, at a time when he was still intensely concerned with the problems and methods of the Perspectivist school. Gascoigne, Robert In October 1962, the world was at imminent risk of nuclear war. In response to the failed CIA backed 'Bay of Pigs' invasion, Soviet Premier Nikita Krushchev had authorized the stationing of nuclear missiles in Cuba, only ninety miles from the coast of Florida. In response, President John F. Kennedy had ordered a blockade of Cuba, which the Soviet Union regarded as an act of war. In fact, the world came much closer to a nuclear exchange (...) than has long been realized. On October 27th 1962 the Soviet submarine B59 was tracked by a group of United States warships, made up of eleven destroyers and an aircraft carrier, which were using non-lethal practice depth-charges in an attempt to bring the submarine to the surface. The submarine had lost radio contact with Moscow, which-as we will see-had very shortly beforehand ordered the freighters carrying nuclear missiles to return home. Under the intense pressure of the situation, the captain of the submarine-who thought that war may already have broken out-was determined not to allow Russia to be humiliated by the capture of his submarine, and was prepared to fire a nuclear torpedo at the surrounding American warships. Three officers had to give permission for a nuclear missile to be launched-the captain himself, the political officer, and Vasili Alexandrovich Arkhipov, who, although second-in-command of submarine B59, was commander of the flotilla of submarines to which it belonged. Arkhipov overruled his brother officers and refused to authorize the launch of the torpedo. The world owes a great debt of gratitude to this Soviet sailor who may have prevented a nuclear exchange between the Soviet Union and the United States. My father once told me how an American priest had recounted to him that, during the height of the Cuban missile crisis, the queues for confession at New York's St Patrick's cathedral stretched all the way down the nave and out onto Fifth Avenue. Given the penitential practices and beliefs of the time, and the actual state of events, there was indeed good reason for that! (shrink)

In general terms there are only three primary energy sources: fossil fuels, renewable energy, and nuclear fission. For fueling road transportation, there has been much speculation about the use of hydrogen as an energy carrier, which would usher in the “hydrogen economy.” A parallel situation would use a simple battery to store electricity directly in order to power vehicles. The efficiency of these two different approaches has been compared and shows that the hydrogen and fuel cell system would consume nearly (...) three times the primary energy required by a battery storage system. Successful introduction into the marketplace of the plug-in hybrid vehicle would eliminate the need for road vehicles to use petroleum fuels, at least for the majority of miles traveled. If electricity were to be generated primarily from sustainable primary energy sources, then road transportation would also become sustainable, resulting in an “electricity economy” rather than a “hydrogen economy.”. I met Ernie in 1965 on the wrestling mats of our high school in North Bergen, New Jersey, a township on top of the plateau overlooking Hoboken and across the Hudson River from Manhattan. Hoboken then was still the Hoboken of Elia Kazan’s “On the Waterfront” (1954).1 Even though the Hudson was less than a mile across at that point, it was a wide spiritual divide. We were Jersey boys, not New Yorkers. Ernie was as ambitious as I was about (...) wrestling, and, so, after the season was over, we used to take a bus to Journal Square in Jersey City, and then walk about eight city blocks to a gym to lift weights. In those days, high schools didn’t have weight rooms; and gyms were scarce, men only, quite filthy, and entirely devoid of cardio equipment and Nautilus machines. They were all sweat, grunts, groans, and clanking iron. By 1968, on Tuesdays and Thursdays, after a grueling wrestling practice at the high school, we would take a bus to New York City (it took about a half hour to get into “the City” by bus, less if the Lincoln Tunnel was not crowded), and then a short subway ride up to the New York Athletic Club on 59th street, across from Central Park, to spend a couple of hours working out with former university wrestling stars—guys in their mid-twenties from places like Oklahoma, Nebraska, and Iowa—who were training to make the Olympic team. Even with all of this wrestling time, we were frustrated by the fact that there was nowhere to work out on Sundays. We investigated and found out that the Jersey City YMCA had a wrestling mat and was open on Sundays. We then spent our Sunday afternoons working out there, so as not to miss a day of wrestling. Wrestling was our savior: a healthy way to get out anger.2 But it wasn’t all wrestling. We did something else too: We talked. We spent many hours together introspecting out loud, and just trying to make sense of things. Ernie has mentioned in print one early topic of discussion: “We spent years trying to solve various logical conundra like how on earth the Virgin Mary could have been a virgin.... (shrink)

During Operation Desert Shield and Operation Desert Storm, USNS Mercy was one of two American hospital ships deployed to the Persian Gulf. She arrived in the Gulf on 15 September 1990, following a 12,000-mile transit from her homeport of Oakland, California, and remained on station until 18 March 1991, when she passed through the Straits of Hormuz on her return voyage home. During the height of her deployment, Mercy was staffed with nearly 1,200 men and women, including physicians, nurses, dentists, (...) representatives of affiliated health-care disciplines, hospital corpsmen, dental technicians, healthcare administrators, chaplains, and various nonmedical support personnel.
