Geographical variations are documented for a wide range of health care services. As many such variations cannot be explained by demographical or epidemiological differences, they are problematic with respect to distributive justice, quality of care, and health policy. Despite much attention, geographical variations prevail. One reason for this can be that the ethical issues of geographical variations are rarely addressed explicitly. Accordingly, the objective of this article is to analyse the ethical aspects of geographical variations in the provision of health (...) services. Applying a principlist approach the article identifies and addresses four specific ethical issues: injustice, harm, lack of beneficence, and paternalism. Then it investigates the normative leap from the description of geographical variations to the prescription of right care. Lastly, the article argues that professional approaches such as developing guidelines, checklists, appropriateness criteria, and standards of care are important measures when addressing geographical variations, but that such efforts should be accompanied and supported by ethical analysis. Hence, geographical variations are not only a healthcare provision, management, or a policy making problem, but an ethical one. Addressing the ethical issues with geographical variations is key for handling this crucial problem in the provision of health services. (shrink)

There is thus a tension between respecting the preferences of the people and making policy based on our best scientific knowledge. Deciding how to make policy in the light of this tension requires an understanding of why citizens sometimes disagree with the experts on what is risky and what is (...) safe. In this paper, we examine two highly influential theories of how people form beliefs about risks: the theory that risk beliefs are errors caused by bounded rationality and the theory that such beliefs are part and parcel of people’s core value systems. We then discuss the implications of the psychological theories for questions regarding liberal-democratic decision making: Should policy be responsive to the preferences of citizens in the domain of risk regulation? What risk-regulation policies are legitimate? For over a century, the Danish thinker Søren Kierkegaard has been at the center of a number of important discussions, concerning not only philosophy and theology, but also, more recently, fields such as social thought, psychology, and contemporary aesthetics, especially literary theory. Despite his relatively short life, Kierkegaard was an extraordinarily prolific writer, as attested to by the 26-volume Princeton University Press edition of all of his published writings. But Kierkegaard left behind nearly as much unpublished writing, most of which (...) consists of what are called his "journals and notebooks." Kierkegaard has long been recognized as one of history's great journal keepers, but only rather small portions of his journals and notebooks are what we usually understand by the term “diaries.” By far the greater part of Kierkegaard’s journals and notebooks consists of reflections on a myriad of subjects—philosophical, religious, political, personal. Studying his journals and notebooks takes us into his workshop, where we can see his entire universe of thought. We can witness the genesis of his published works, to be sure—but we can also see whole galaxies of concepts, new insights, and fragments, large and small, of partially completed but unpublished works. Kierkegaard’s Journals and Notebooks enables us to see the thinker in dialogue with his times and with himself. Kierkegaard wrote his journals in a two-column format, one for his initial entries and the second for the extensive marginal comments that he added later. This edition of the journals reproduces this format, includes several photographs of original manuscript pages, and contains extensive scholarly commentary on the various entries and on the history of the manuscripts being reproduced. Volume 9 of this 11-volume series includes five of Kierkegaard’s important “NB” journals, which span from June 1852 to August 1854. This period was marked by Kierkegaard’s increasing preoccupation with what he saw as an unbridgeable gulf in Christianity—between the absolute ideal of the religion of the New Testament and the official, state-sanctioned culture of “Christendom,” which, embodied by the Danish People’s Church, Kierkegaard rejected with increasing vehemence. Crucially, Kierkegaard’s nemesis, Bishop Jakob Peter Mynster, died during this period and, in the months following, Kierkegaard can be seen moving inexorably toward the famous “attack on Christendom” with which he ended his life. Søren Kierkegaard (1813-55) was an almost unbelievably prolific writer. At his death he left not only a massive body of published work (25 volumes in the recently completed Princeton University Press edition), but also a sprawling mass of unpublished writings that rivaled the size of the published corpus. This book tells the story of the peculiar fate of this portion of Kierkegaard's literary remains, which flowed ceaselessly from his steel pen from his late teens to a week before his death. (...) It is the story of packets and sacks of paper covered with words and images that, after a vagabond existence in various homes, finally landed at the Royal Danish Library, where they are today guarded with great care. Readers are also introduced to a selection of this enormous body of material, including drawings and doodlings (often human profiles with high foreheads) that escaped from Kierkegaard's pen in unguarded moments and complement the allure of the philosopher's strikingly variable, elusive handwriting. The authors of this book are among the editors of a modern critical edition of Kierkegaard's oeuvre currently being produced in Copenhagen. By the end of his life Kierkegaard had become a controversial figure, engaged in a furious assault upon "Christendom." From the very moment of their discovery in the days following his death, the unpublished words and images constituted a highly problematic bonanza, an intellectual and religious hot potato (or sack of potatoes) that was passed from hand to hand, suppressed, selectively and tendentiously published and republished. Durch Auslegungen vor allem griechischer, mittelhochdeutscher und klassischer deutscher Dichtung werden zunächst die Grundbegriffe von Faktizität und Existentialität herausgearbeitet. Auf dieser Grundlage erfolgt sodann die Beschreibung von Anlass und Gestalt normativer Praxis. Diese Beschreibung wird endlich als anthropologische Grundlage philosophischer Ethik vorgeschlagen.

The emerging concept of systems medicine (or ‘P4 medicine’—predictive, preventive, personalized and participatory) is at the vanguard of the post-genomic movement towards ‘precision medicine’. It is the medical application of systems biology, the biological study of wholes. Of particular interest, P4 systems medicine is currently promised as a revolutionary new biomedical approach that is holistic rather than reductionist. This article analyzes its concept of holism, both with regard to methods and conceptualization of health and disease. Rather than representing a medical (...) holism associated with basic humanistic ideas, we find a technoscientific holism resulting from altered technological and theoretical circumstances in biology. We argue that this holism, which is aimed at disease prevention and health optimization, points towards an expanded form of medicalization, which we call ‘holistic medicalization’: Each person’s whole life process is defined in biomedical, technoscientific terms as quantifiable and controllable and underlain a regime of medical control that is holistic in that it is all-encompassing. It is directed at all levels of functioning, from the molecular to the social, continual throughout life and aimed at managing the whole continuum from cure of disease to optimization of health. We argue that this medicalization is a very concrete materialization of a broader trend in medicine and society, which we call ‘the medicalization of health and life itself’. We explicate this holistic medicalization, discuss potential harms and conclude by calling for preventive measures aimed at avoiding eventual harmful effects of overmedicalization in systems medicine (quaternary prevention). We demonstrate how to validly quantify into hyperintensional contexts involving non-propositional attitudes like seeking, solving, calculating, worshipping, and wanting to become. We describe and apply a typed extensional logic of hyperintensions that preserves compositionality of meaning, referential transparency and substitutivity of identicals also in hyperintensional attitude contexts. We specify and prove rules for quantifying into hyperintensional contexts. These rules presuppose a rigorous method for substituting variables into hyperintensional contexts, and the method will be described. We prove the following. First, it (...) is always valid to quantify into hyperintensional attitude contexts and over hyperintensional entities. Second, factive empirical attitudes validate, furthermore, quantifying over intensions and extensions, and so do non-factive attitudes, both empirical and non-empirical , provided the entity to be quantified over exists. Clausen and by the German philosopher Schelling, as well as a great many other entries on philosophical, theological, and literary topics. In this paper we present the results of a simulation study of credence developments in groups of communicating Bayesian agents, as they update their beliefs about a given proposition p. Based on the empirical literature, one would assume that these groups of rational agents would converge on a view over time, or at least that they would not polarize. This paper presents and discusses surprising evidence that this is not true. Our simulation study shows that these groups of Bayesian agents (...) show group polarization behavior under a broad range of circumstances. This is, we think, an unexpected result, that raises deeper questions about whether the kind of polarization in question is irrational. If one accepts Bayesian agency as the hallmark of epistemic rationality, then one should infer that the polarization we find is also rational. On the other hand, if we are inclined to think that there is something epistemically irrational about group polarization, then something must be off in the model employed in our simulation study. We discuss several possible interfering factors, including how epistemic trust is defined in the model. Ultimately, we propose that the notion of Bayesian agency is missing something in general, namely the ability to respond to higher-order evidence. (shrink)

In this article we explore how diagnostic and therapeutic technologies shape the lived experiences of illness for patients. By analysing a wide range of examples, we identify six ways that technology can (trans)form the experience of illness (and health). First, technology may create awareness of disease by revealing asymptomatic signs or markers (imaging techniques, blood tests). Second, the technology can reveal risk factors for developing diseases (e.g., high blood pressure or genetic tests that reveal risks of falling ill in the (...) future). Third, the technology can affect and change an already present illness experience (e.g., the way blood sugar measurement affects the perceived symptoms of diabetes). Fourth, therapeutic technologies may redefine our experiences of a certain condition as diseased rather than unfortunate (e.g. assisted reproductive technologies or symptom based diagnoses in psychiatry). Fifth, technology influences illness experiences through altering social-cultural norms and values regarding various diagnoses. Sixth, technology influences and changes our experiences of being healthy in contrast and relation to being diseased and ill. This typology of how technology forms illness and related conditions calls for reflection regarding the phenomenology of technology and health. How are medical technologies and their outcomes perceived and understood by patients? The phenomenological way of approaching illness as a lived, bodily being-in-the-world is an important approach for better understanding and evaluating the effects that medical technologies may have on our health, not only in defining, diagnosing, or treating diseases, but also in making us feel more vulnerable and less healthy in different regards. This article presents and evaluates arguments supporting that an approval procedure for genome-edited organisms for food or feed should include a broad assessment of societal, ethical and environmental concerns; so-called non-safety assessment. The core of analysis is the requirement of the Norwegian Gene Technology Act that the sustainability, ethical and societal impacts of a genetically modified organism should be assessed prior to regulatory approval of the novel products. The article gives an overview how this requirement has been implemented in the (...) regulatory practice, demonstrating that such assessment is feasible and justified. Even in situations where genome-edited organisms are considered comparable to non-modified organisms in terms of risk, the technology may have—in addition to social benefits—negative impacts that warrant assessments of the kind required in the Act. The main reason is the disruptive character of the genome editing technologies due to their potential for novel, ground-breaking solutions in agriculture and aquaculture combined with the economic framework shaped by the patent system. Food is fundamental for a good life, biologically and culturally, which warrants stricter assessment procedures than what is required for other industries, at least in countries like Norway with a strong tradition for national control over agricultural markets and breeding programs. It is widely agreed that obsessive-compulsive disorder involves irrationality. But where in the complex of states and processes that constitutes OCD should this irrationality be located? A pervasive assumption in both the psychiatric and philosophical literature is that the seat of irrationality is located in the obsessive thoughts characteristic of OCD. Building on a puzzle about insight into OCD (Taylor 2022), we challenge this pervasive assumption, and argue instead that the irrationality of OCD is located in the emotions that are (...) characteristic of OCD, such as anxiety or fear. In particular, we propose to understand the irrationality of OCD as a matter of harboring recalcitrant emotions. We argue that this account not only solves the puzzle about insight, but also makes better sense of how OCD sufferers experience and describe their condition and helps explain some otherwise puzzling features of compulsive behavior. Fairness, the notion that people deserve or have rights to certain resources or kinds of treatment, is a fundamental dimension of moral cognition. Drawing on recent evidence from economics, psychology, and neuroscience, we ask whether self-interest is always intuitive, requiring self-control to override with reasoning-based fairness concerns, or whether fairness itself can be intuitive. While we find strong support for rejecting the notion that self-interest is always intuitive, the literature has reached conflicting conclusions about the neurocognitive systems underpinning fairness. We (...) propose that this disagreement can largely be resolved in light of an extended Social Heuristics Hypothesis. Divergent findings may be attributed to the interpretation of behavioral effects of ego depletion or neurostimulation, reverse inference from brain activity to the underlying psychological process, and insensitivity to social context and inter-individual differences. To better dissect the neurobiological basis of fairness, we outline how future research should embrace cross-disciplinary methods that combine psychological manipulations with neuroimaging, and that can probe inter-individual, and cultural heterogeneities. A popular account of epistemic justification holds that justification, in essence, aims at truth. An influential objection against this account points out that it is committed to holding that only true beliefs could be justified, which most epistemologists regard as sufficient reason to reject the account. In this paper I defend the view that epistemic justification aims at truth, not by denying that it is committed to epistemic justification being factive, but by showing that, when we focus on the relevant (...) sense of ‘justification’, it isn’t in fact possible for a belief to be at once justified and false. To this end, I consider and reject three popular intuitions speaking in favor of the possibility of justified false beliefs, and show that a factive account of epistemic justification is less detrimental to our normal belief forming practices than often supposed.
