The aim of this study was to evaluate terminal care among hospitalized children who died of HIV/AIDS. The design was a retrospective chart review of the terminal hospitalization. The setting was a public, secondary and tertiary children's hospital in Cape Town, South Africa (SA). The patients included a consecutive series of in‐patient deaths from HIV‐related causes. The main outcome measures included: documentation of do not resuscitate (DNR) orders and comfort care plans, intensity of diagnostic and therapeutic interventions in last 24 (...) hours of life, and presence of pain and distress in last 48 hours of life. The results are based on the review of 165 out of 167 in‐patient deaths. Of those, 79% of patients died in general wards. Median age and length of stay were 4 months and 6 days respectively. A total of 84% of patients had a DNR order. DNR orders appeared simultaneously in only 41% of medical and nursing notes. Only 44% of patients had a comfort care plan. Pain and distress in the last 48 hours was documented in 55% of patients who died in the general wards. Respiratory symptomatology and painful skin conditions accounted for most discomfort. Half (36/72) the patients with pain and distress, including 16 with a comfort care plan, received no analgesia. Conclusions drawn found that, despite clinical uncertainty, doctors made tough end of life decisions that included DNR orders and comfort care plans. The lower rate of comfort care plans suggests doctors had difficulty making the transition from curative to palliative care. Many comfort care plans were incoherent and included interventions unlikely to promote patients' comfort. In light of the HIV/AIDS pandemic in SA, reforms are needed to integrate palliative care within mainstream hospital medicine. However, without adequate human resources including trained interpreters, doctors and nurses will struggle to deliver optimal terminal care in acute hospitals. (shrink)

Biomechanical engineering is marked by highly experimental efforts to craft mechanical devices that might one day alleviate the scarcity of transplantable organs in the USA. A pronounced desire among bioengineers involves melding humans with machines, bearing the promise of perfecting the natural yet messy flaws of the ‘natal’ body. Not all bodies are considered equal within this field, however. Visual renderings of heart devices — as an unusual sort of body prosthesis — foreground a specialized aesthetic, where the well-toned male (...) form defines an idealized generic of beauty and perfection. This analysis considers the moral underpinnings of the embodied aesthetic in specific reference to engineers’ efforts to fabricate implantable ‘artificial’ hearts. Of central concern are the sociomoral consequences of a standardized male beauty, set alongside the striking absence — or invisibility — of the female form within this rarified field of science. Background A significant number of women experience childbirth as traumatic. These experiences are often characterised by a loss of control coupled with a perceived lack of support and inadequate communication with health care professionals. Little is known about the choices women make in subsequent pregnancy(s) and birth(s), or why they make these choices. This study aimed to understand these choices and explore the reasons behind them. Methods A longitudinal Grounded Theory Methods (GTM) study involving 9 women was conducted. Over half (...) of the participants had a formal diagnosis of PTSD and/or PND related to the previous birth. Interviews were carried out at 3 timepoints perinatally. These findings are from the first interviews at 12-20 weeks. Results From the first days of pregnancy, this cohort of women were focused on concerns that this birth would be a repeated traumatic experience. The women were deliberately searching out and analysing information about their choices in this pregnancy and birth, and making plans which had two aims; firstly to avoid a repeat of their previous birth experience, and secondly to avoid a loss of control to other people during the birth. The women considered a range of birth choices, from elective caesareans to freebirth. Some women felt well supported by those around them, including health care professionals, partners, friends and family. Others did not, and were anticipating conflict in trying to assert their birth choices. Many early relationships with health care professionals were characterised by fear and mistrust. Discussion Women who have previously experienced a traumatic birth become pregnant again, have a strong desire to avoid a repeat experience and to feel in control of their birth choices. Access to robust information appears to help reduce uncertainty and arm women in their discussions with professionals. Similarly making plans and seeking to have them agreed with health care professionals at an early stage is used a way to reduce the risk of having a further traumatic experience. Implications for practice include supporting women in formulating and confirming pregnancy and birth plans at an early stage to reduce uncertainty and foster a sense of control. (shrink)

"Cult of ugliness," Ezra Pound’s phrase, powerfully summarizes the ways in which modernists such as Pound, T. S. Eliot, Wyndham Lewis, and T. E. Hulme—the self-styled "Men of 1914"—responded to the "horrid or sordid or disgusting" conditions of modernity by radically changing aesthetic theory and literary practice. Only the representation of "ugliness," they protested, would produce the new, truly "beautiful" work of art. They dissociated the beautiful from its traditional embodiment in female beauty, and from its association with Walter Pater (...) and Oscar Wilde. Their cultivation of ugliness displaced misogyny and homophobia. Higgins takes in texts such as John Ruskin’s art criticism, Eliot’s literary journalism, Lewis’s pro-fascism pamphlets, and the poetry of Pound, Conrad Aiken, and Langston Hughes. Radical curriculum policy transformations are emerging as a key strategy of universities across different countries as they move to strengthen their competitive position in a global knowledge era. This paper puts forward a ?global case study? research agenda in the under-researched area of university curriculum policy. The particular curriculum policies to be investigated point to potentially new forms of liberal education, and they resonate in varying degrees with contemporary patterns in Europe as well as longer standing patterns in the United (...) States. This research agenda stands to make a unique contribution with its ?whole curriculum? approach to: the examination of the relationship between curriculum content, pedagogy and assessment; the tracking of curriculum policy borrowing across different jurisdictions extending between global and local levels; and the investigation of historical antecedents of contemporary curriculum policy patterns. In particular, the proposed agenda features a rare combination of spatial and temporal dimensions of university curriculum policy flows. This research agenda will provide a strong empirical evidence base for extending theory building about university curriculum policy development, as well as policy ?learning? for policy makers, practitioners and scholars ? globally. Honeybees show lateral asymmetry in both learning about odours associated with reward and recalling memory of these associations. We have extended this research to show that bees exhibit lateral biases in their initial response to odours: viz., turning towards the source of an odour presented on their right side and turning away from it when presented on their left side. The odours we presented were the main component of the alarm pheromone, iso-amyl acetate (IAA), and four floral scents. The significant (...) bias to turn towards IAA odour on the right and away from it on the left is, we argue, a lateralization of the fight-flight response elicited by this pheromone. It contrasts to an absence of any asymmetry in the turning response to an odour of the flowers on which the bees had been feeding prior to testing: to this odour they turned towards when it was presented on either the left or right side. Lemon and orange odours were responded to differently on the left and right sides (towards on the right, away on the left) but no asymmetry was found in responses to rose odour. Our results show that side biases are present even in the initial, orienting response of bees to certain odours. (shrink)

The average age of women nursing students in Australia is rising. With this comes the likelihood that more now begin university with family responsibilities, and with their lives structured by the roles of mother and partner. Women with more traditionally gendered ideas of these roles, such as nurturing others and self‐sacrifice, are known to be attracted to nursing as a profession; once at university, however, these students can be vulnerable to gender role stress from the competing demands of study. A (...) qualitative research design, guided by Gadamer's hermeneutic philosophy, explored the gendered behaviours and experiences of 22 women nursing students, all of whom had children and began university in a heterosexual intimate relationship. The findings reveal traditional ideas of gender were almost universal among participants, and these ideas had a significant influence on the nursing degree experience. Participants commonly prioritised family over the university and practiced maternal gatekeeping (prevention of male partner involvement in domestic work). These traditionally gendered behaviours, coupled with experiences of gender role stress, had a detrimental impact on participants' capacity to study and their personal wellbeing. The importance of these findings to the burgeoning nursing workforce shortage nursing is considered in terms of student retention and the supply of graduates into the profession. The implications to the nursing profession are also explored against the evidence that nursing students with traditional gender beliefs are less likely to develop as autonomous, critical thinking nurses compared to their gender‐egalitarian peers. The introduction of gender theory via critical pedagogy in the undergraduate nursing degree curriculum is recommended to enlighten and empower women nursing students and promote the competence, agility, and sustainability of the nursing profession. This article outlines a moral education guided by African traditional values such as ubuntu and ukama. It argues that ubuntu is not by definition speciesist, as some have claimed, but that it has strong ecocentric leanings, that is, if ubuntu is understood as a concrete expression of ukama. In fact, ubuntu deconstructs the anthropocentric?ecocentric distinction which has characterised and continues to characterise debates in environmental theory/philosophy. To become more fully human does not mean caring only for the self and other (...) human beings but also for the entire biophysical world. Some implications that this discussion has for moral education are explored, as well as some opportunities that post-apartheid curriculum frameworks offer for implementing a moral education guided by ubuntu. According to Pierce, Berkeley should be regarded as (...) the author of that method of modern pragmatism (RFB, 96): 1 Pragmaticism is... a method of thinking.... Of those who have used this way of thinking Berkeley is the clearest example.... (CP, 8.206, c.1905) Berkeley on the whole has more right to be considered the introducer of pragmatism into philosophy than any other one man.... Moreover, while Peirce cites the Essay Towards a New Theory of Vision as using the "medium of pragmatism," others have criticized Berkeley in this very work for drawing conclusions without any experimentation. 4 Since it is precisely the pivotal role of experience and experiment that distinguish the method of pragmatism, it is puzzling that Berkeley is chosen as worthy of the title that Peirce bestows on him. My hope in this paper is to shed some light on this question. I beginwith an explication of Peirce's pragmatism and then move on to offer an account ofhow Berkeley uses this method, though never formulates it. Next, I discuss the positions Peirce understood to be consequences of the view, including critical common-sensism, fallibilism, and scholastic realism.
