In 1911, Drs John Freeman and Leonard Noon published an account of a novel treatment for hay fever. Their method of desensitisation consisted of injecting increasing doses of an extract of pollen subcutaneously until the hypersensitivity reaction was diminished or abolished. Over subsequent decades, desensitisation established itself as the cornerstone of clinical allergy in both England and the United States, at least until the advent of novel pharmaceutical agents in the 1950s and 1960s. Although British allergists such as Noon (...) and Freeman were aware of conceptual developments within European immunology and pathology (such as the identification of anaphylaxis by Richet and Portier or von Pirquet's coining of the term allergy), their approach to hay fever was driven by more immediate pragmatic, and indeed financial, considerations. Freeman's immersion in the problems of hay fever and asthma and his pioneering use of allergen desensitisation or immunotherapy were shaped by his adherence to the convictions and bacteriological practices of his principal at St Mary's Hospital, Almroth Wright, and by the drive to produce commercial vaccines which would help to subsidise the experimental and therapeutic work at St Mary's. The aim of this paper is to explore early twentieth-century approaches to hay fever and other allergic diseases by tracing the intellectual and institutional origins of clinical allergy in Britain. The science of animal consciousness investigates (i) which animal species are conscious (the distribution question) and (ii) how conscious experience differs in detail between species (the quality question). We propose a framework which clearly distinguishes both questions and tackles both of them. This two-tier account distinguishes consciousness along ten dimensions and suggests cognitive capacities which serve as distinct operationalizations for each dimension. The two-tier account achieves three valuable aims: First, it separates strong and weak indicators of the presence of consciousness. (...) Second, these indicators include not only different specific contents but also differences in the way particular contents are processed (by processes of learning, reasoning or abstraction). Third, evidence of consciousness from each dimension can be combined to derive the distinctive multi-dimensional consciousness profile of various species. Thus, the two-tier account shows how the kind of conscious experience of different species can be systematically compared. (shrink)

Four principal papers and a total of 43 peer commentaries on the evolutionary origins of morality. To what extent is human morality the outcome of a continuous development from motives, emotions and social behaviour found in nonhuman animals? Jerome Kagan, Hans Kummer, Peter Railton and others discuss the first principal paper by primatologists Jessica Flack and Frans de Waal. The second paper, by cultural anthropologist Christopher Boehm, synthesizes social science and biological evidence to support his theory of how our hominid (...) ancestors became moral. In the third paper philosopher Elliott Sober and evolutionary biologist David Sloan Wilson argue that an evolutionary understanding of human nature allows sacrifice for others and ultimate desires for another's good. Finally Brian Skyrms argues that game theory based on adaptive dynamics must join the social scientist's use of rational choice and classical game theory to explain cooperation. (shrink)

Solidarity is a fundamental social value in many European countries, though its precise practical and theoretical meaning is disputed. In a health care context, I agree with European writers who take solidarity normatively to mean roughly equal access to effective health care for all. That is, solidarity includes a sense of justice. Given that, I will argue that precision medicine represents a potential weakening of solidarity, albeit not a unique weakening. Precision medicine includes 150 targeted cancer therapies (mostly for metastatic (...) cancer), all of which are extraordinarily expensive. Our critical question: Must a commitment to solidarity as defined mean that all these targeted cancer therapies should be guaranteed to all within each country in the European Union, no matter the cost, no matter the degree of effectiveness? Such a commitment would imply that cancer was ethically special, rightfully commandeering unlimited resources. That in itself would undermine solidarity. I offer multiple examples of how current and future dissemination of these targeted cancer drugs threaten a commitment to solidarity. An alternative is to fund more cancer prevention efforts. However, that too proves a threat to solidarity. Solidarity, with or without a sense of justice, is too abstract a notion to address these challenges. Further, we need to accept that we can only hope to achieve “rough justice” and “supple solidarity.” The precise practical meaning of these notions needs to be worked out through a fair and inclusive process of rational democratic deliberation, which is the real and practical foundation of just solidarity. The majority of North American corporations awakened to the need for their own ethical guidelines during the late 1970s and early 1980s, even though modern corporations are subject to a surprising multiplicity of external codes of ethics or conduct. This paper provides an understanding of both internal and external codes through a discussion of the factors behind the development of the codes, an analysis of internal codes and an identification of problems with them.

Dobbs v. Jackson Women’s Health continues a trajectory of U.S. Supreme Court jurisprudence that undermines the normative foundation of public health — the idea that the state is obligated to provide a robust set of supports for healthcare services and the underlying social determinants of health. Dobbs furthers a longstanding ideology of individual responsibility in public health, neglecting collective responsibility for better health outcomes. Such an ideology on individual responsibility not only enables a shrinking of public health infrastructure for (...) reproductive health, it facilitates the rise of reproductive coercion and a criminal legal response to pregnancy and abortion. This commentary situates Dobbs in the context of a long historical shift in public health that increasingly places burdens on individuals for their own reproductive health care, moving away from the possibility of a robust state public health infrastructure. The annual cost of healthcare in the United States reached $2.5 trillion in 2009 (about 17.6% of GDP) with projections to 2019 of about $4.5 trillion (about 20% of likely GDP).

John Rawls has held up as a model of public reason the U.S. Supreme Court. I argue that the Dobbs Court is justifiably criticized for failing to respect public reason. First, the entire opinion is governed by an originalist ideological logic almost entirely incongruent with public reason in a liberal, pluralistic, democratic society. Second, Alito’s emphasis on “ordered liberty” seems completely at odds with the “disordered liberty” regarding abortion already evident among the states. Third, describing the embryo/fetus from conception until (...) birth as an “unborn human being” begs the question of the legal status of the embryo/fetus, as if an obiter dictum settled the matter. Fourth, Alito accuses the Roe court of failing to exercise judicial restraint, although Alito argued to overturn Roe in its entirety. In brief, the Dobbs opinion is an illiberal, disingenuous, ideological swamp that swallows up public reason and the reproductive rights of women. (shrink)

This essay uses the global impact of the Coronavirus as a heuristic semiotic for exploring the future of the church. Unlike the pandemic of 1918, which left few dents on the world's economic, social, and cultural systems, almost all the nations of the world have passed laws and implemented procedures that are only comparable to world wars in their impact on entire populations. Nations are acting in unison, but not in unity. This post-COVID, post-Corona world is the 'time that is (...) given' to the church. But it will not be a post-pandemic world. We may become COVID-proof, but we will never be pandemic-proof. There is no pre-COVID reset. There is only risk assessment from natural extinction risks to existential dangers of our own creation that are catching up to us. Disruption is the new status that is never quo; stability is the new abnormality; global cataclysm is the ever-present peril. The only way to prepare for a future of constant 'the end of the world as we know it?' moments is by developing a high Contextual Quotient, and deepening our Contextual Intelligence so we can choose 'the next right thing' in a world of volcanic volatility. CONTRIBUTION: This essay frames the semiotics of a missional ecclesiology in the COVIDian wake from the hermeneutics of blessings not curses. What virtues might we expect to come out of a virus that is fast-forwarding the future, virtues that will shape the contours of Christianity. What if the pandemic is a shock treatment that is putting the world, and the church, back in a new and better equilibrium? What if there are goldmines on the other side of the landmines and minefields? (shrink)

Les parlers poitevins nord-occidentaux (Noirmoutier, Marais nord vendéen) présentent une variation allophonique complexe du vocalisme. On peut distinguer plusieurs niveaux de diphtongaison qui rendent ces variétés particulièrement intéressantes pour l’analyse phonologique. L’étonnante diversité des formes phonétiques en surface peut cependant se réduire à deux grandes catégories de noyaux vocaliques, simples (monophtongues) et complexes (monophtongues longues et diphtongues sous-jacentes). Les premières sont sujettes à des contraintes d’expression liées à l’atérité, ou laxité, tandis que les deuxièmes alternent des voyelles tendues avec des (...) diphtongues décroissantes ou fermantes dans l’oral spontané. Cette contribution traite principalement de comment une enquête sur la structure phonologique d’une variété dialectale peut durer vingt ans, en faisant alterner diverses approches et divers angles de vue sur les données : transcription phonétique, spectrographie acoustique, analyse phonologique tendant à devenir une enquête phonologique amenée à remettre en cause ses prémisses et ses instruments d’observation des faits de parole pour aboutir à une description de la langue. Comparisons of Japan with Western countries have long been used to explore the relationship between technology and culture. In the 1950s and 1960s such work sought to determine if technological imperatives were diminishing cultural differences. In the 1970s, 1980s and early 1990s many sought to identify aspects of Japanese culture that might lie at the root of Japan’s technological successes. This article argues that we should now undertake more micro and more systematic comparative studies that are more directly grounded in (...) theory. Studies concentrating on engineers and the practice of engineering would seem to be an especially promising starting point. (shrink)

The debate around age-based healthcare rationing was precipitated by two books in the late 1980s, one by Daniel Callahan and the other by Norman Daniels. These books ignited a firestorm of criticism, best captured in the claim that any form of age-based healthcare rationing was fundamentally ageist, discriminatory in a morally objectionable sense. That is, the elderly had equal moral worth and an equal right to life as the nonelderly. If an elderly and nonelderly person each had essentially the same (...) medical problem requiring the same medical treatment, then they had an equal right to receive that treatment no matter what the cost of that treatment. Alternatively, if cost was an issue because the benefits of the treatment were too marginal, then both the elderly and nonelderly patients requiring that treatment ought to be denied it. If there were something absolutely scarce about the treatment, then some fair process would have to be used to make an allocation decision. (shrink)
