IntroductionYounger age at diagnosis is a risk factor for poor health-related quality of life in long-term breast cancer survivors. However, few studies have specifically addressed HRQOL in young adults with breast cancer, nor have early changes in HRQOL been fully characterized.MethodsEligible female patients with breast cancer were identified through our local cancer center. To establish HRQOL, patients completed the Functional Assessment of Cancer Therapy-Breast around diagnosis and 12 months later. Sociodemographic factors, genetic susceptibility to cancer, tumor- and treatment-related factors, and (...) comorbidities were abstracted from medical records and the local oncology registry. Mixed-effects models were used to identify changes in FACT-B scores during the first year of treatment and to determine whether any demographic/treatment-related factors modulated changes in scores.ResultsHealth-related quality of life in young patients with breast cancer was within normal limits at baseline, with a FACT-B overall well-being score of 108.5. Participants reported slight improvements over a 12-month period: FACT-B overall well-being scores increased 6.6 points, functional well-being improved 3.0 points, emotional well-being improved 1.9 points, and physical well-being improved 1.5 points, on average. Participants with anxiety/depression at baseline reported greater improvements in FACT-B overall well-being and functional well-being than participants who did not have anxiety/depression at baseline. Marital status, reconstructive surgery, and baseline clinical staging were also significantly associated with changes in aspects of HRQOL, although their impact on change was relatively minimal.ConclusionYoung women with breast cancer do not report HRQOL concerns during the first year of treatment. Improvements in HRQOL during the first year of treatment may be attributable to a sense of relief that the cancer is being treated, which, in the short run, may outweigh the negative late effects of treatment. We identify a species of experiment—Kon-Tiki experiments—used to demonstrate the competence of a cause to produce a certain effect, and we examine their role in the historical sciences. We argue that Kon-Tiki experiments are used to test middle-range theory, to test assumptions within historical narratives, and to open new avenues of inquiry. We show how the results of Kon-Tiki experiments are involved in projective inferences, and we argue that reliance on projective inferences does not provide historical scientists with any special (...) protection against the problem of unconceived alternatives. First published in 1980, this book questions many of the assumptions that have accumulated around the subject of intonation as it occurs in spontaneous speech, as well as texts read aloud. The book suggests alternative ways of examining the subject and primarily uses data derived from Edinburgh speech, which is explicitly compared with descriptions of standard southern English.The book critically examines many conventional assumptions made about the formal features of intonation, particularly 'tonic' or primary stress', and about the functions of (...) intonation, specifically rising intonation. A model of intonation is presented which demonstrates that the limited resources of intonation are exploited by several different expressive systems. This approach is justified in detailed analysis of extensive stretches of speech, supported by instrumental analysis as well as by experiments which elicit judgements by both naïve and phonetically trained judges. Recruiting adolescents into smoking cessation studies is challenging, particularly given institutional review board (IRB) requirements for research conducted with adolescents. This article provides a brief review of the federal regulations that apply to research conducted with adolescents, and describes researchers' experiences of seeking IRB approval for youth cessation research. Twenty-one researchers provided information. The most frequently reported difficulty involved obtaining parental consent. Solutions to commonly reported problems with obtaining IRB approval are also identified. Waivers of parental consent can facilitate recruitment (...) of youths into studies; however, researchers must ensure that their protocols comply with federal regulations when requesting a waiver. The purpose of this research was to understand institutional review board (IRB) challenges regarding youth-focused research submissions and to present advice from administrators. Semistructured self-report questionnaires were sent via e-mail to administrators identified using published lists of universities and hospitals and Internet searches. Of 183 eligible institutions, 49 responded. One half indicated they never granted parental waivers. Among those considering waivers, decision factors included research risks, survey content, and feasibility. Smoking and substance abuse research among children was generally considered more (...) than minimal risk. These findings are consistent with those from a study conducted by Mammel and Kaplan (1995), which investigated IRB practices concerning protocols involving adolescent participants. IRBs and investigators need to become aware of regulations' flexibility to ensure adequate participant protection. Investigators need to limit jargon and assumptions about participants' understanding of research objectives. (shrink)

BackgroundThe ARRIVE (Animal Research: Reporting of In Vivo Experiments) guidelines are widely endorsed but compliance is limited. We sought to determine whether journal-requested completion of an ARRIVE checklist improves full compliance with the guidelines.MethodsIn a randomised controlled trial, manuscripts reporting in vivo animal research submitted to PLOS ONE (March–June 2015) were randomly allocated to either requested completion of an ARRIVE checklist or current standard practice. Authors, academic editors, and peer reviewers were blinded to group allocation. Trained reviewers performed outcome adjudication (...) in duplicate by assessing manuscripts against an operationalised version of the ARRIVE guidelines that consists 108 items. Our primary outcome was the between-group differences in the proportion of manuscripts meeting all ARRIVE guideline checklist subitems.ResultsWe randomised 1689 manuscripts (control: n = 844, intervention: n = 845), of which 1269 were sent for peer review and 762 (control: n = 340; intervention: n = 332) accepted for publication. No manuscript in either group achieved full compliance with the ARRIVE checklist. Details of animal husbandry (ARRIVE subitem 9b) was the only subitem to show improvements in reporting, with the proportion of compliant manuscripts rising from 52.1 to 74.1% (X2 = 34.0, df = 1, p = 2.1 × 10−7) in the control and intervention groups, respectively.ConclusionsThese results suggest that altering the editorial process to include requests for a completed ARRIVE checklist is not enough to improve compliance with the ARRIVE guidelines. Other approaches, such as more stringent editorial policies or a targeted approach on key quality items, may promote improvements in reporting. (shrink)

Disturbance-generated biomass results from endogenous and exogenous natural and cultural disturbances that affect the health and productivity of forest ecosystems. These disturbances can create large quantities of plant biomass on predictable cycles. A systems analysis model has been developed to quantify aspects of system capacities (harvest, transportation, and processing), spatial aspects of the biomass generation process, and deterioration impacts on biomass quality in the various inventory states (field stands, field-harvested inventories, transportation prepared inventories, and production facility inventories). Optimal decision alternatives (...) can be used to guide responses to reclamation, utilization, mitigation, and control. This is particularly advantageous in insect and disease outbreaks, in which the process may last several years, with varying levels of intensity. The prescriptive system description, assuming capacities are fixed, results in a linear programming model. The time-dependent capacity decision model results in a mixed-integer programming model. The analytical model is developed in detail in this analysis. (shrink)

BackgroundSarcomas are a diverse group of neoplasms that vary greatly in clinical presentation and responsiveness to treatment. Given the differences in the sites of involvement, rarity, and treatment modality, a multidisciplinary approach is required. Previous literature suggests patients with sarcoma suffer from poorer quality of life especially physical and functional wellbeing. Adolescent and young adult patients are an underrepresented population in cancer research and have differing factors influencing QoL.MethodsRetrospective analysis of Young Adult patients enrolled in the Sarcoma Tissue Repository at (...) University of Iowa. QoL was assessed using the self-report FACT-G questionnaire at enrollment and 12 months post-diagnosis; overall scores and the 4 wellbeing subscales were calculated. Linear mixed effects models were used to measure the association between the rate of change in FACT-G subscale scores and baseline clinical, comorbidity, and treatment characteristics.Results49 patients were identified. 57.1% of patients had a malignancy involving an extremity. Mean FACT-G scores of overall wellbeing improved from baseline to 12 months. Social and emotional wellbeing did not differ significantly between baseline and 12 months. Physical wellbeing and functional wellbeing scores improved from baseline to 12 months. No difference was seen for FACT-G overall scores for age, sex, laterality, marital status, performance status, having children, clinical stage, limb surgery, chemotherapy, or tumor size. A difference was demonstrated in physical wellbeing scores for patients with baseline limitation compared to those with no baseline limitation. A difference was demonstrated in social wellbeing based on anatomical site.ConclusionYoung adults with sarcoma treated at a tertiary center had improvements in overall reported QoL at 12 months from diagnosis. Overall baseline QoL scores on FACT-G were lower than the general adult population for YA patients with sarcoma but at 12 months became in line with general population norms. The improvements seen merit further investigation to evaluate how these change over the continuum of care. Quality of life changes may be useful outcomes of interest in sarcoma trials. (shrink)

Teaching Joe Kincheloe is one of a handful of important recent books posthumously pushing Kincheloe's work further into the twenty-first century. Written and edited by former students and colleagues, the book underscores the depth and breath of his extraordinarily productive career. The text offers students and educators alike invaluable insights into transformative ways of seeing conducive to challenging the technocratic, imperialistic purpose of dominant forms education in an era marked by ruling elite desperation as U.S. power wanes globally. Through this (...) and other projects, Kincheloe would surely have been pleased to see his many postformal contributions counter-hegemonically providing critical pedagogy the theoretical and practical strength to contribute to the global uprising currently challenging the imperialist project of wealth extraction and cultural domination from Cuba; Venezuela; Peru; Chiapas, Mexico; Tunisia; Libya; Egypt; Yemen; and Iran, to regions in the United States. Teaching Joe L. Kincheloe will surely gain relevancy as the global movement against authoritarianism and domination intensifies and citizens begin to search for new ways to better understand their worlds and how those contexts interrelate with the historical development of their own identities. With the interpretation of texts, the diversity is evident, (...) the unity, universality and invariance less so. Still, we have the text itself, a sequence of determinately meaningful signs, independent of, and a constraint on, the activities of interpreters. We universalists might want more than that, but it's a start. It's a start some people are not prepared to give us. Stanley Fish is one of them. According to him we are looking for something which does not exist, and which we do not need. I shall argue that Fish is wrong on both counts. According to Stanley Fish, there are no constraints on interpretation that transcend particular, local interpretive communities. Indeed, there is no truth or validity that is community transcendent, no standard of lightness, proof or confirmation that has any warrant other than community approval. Intellectual ascendency is always ultimately a matter of rhetorical power, and the locus of power shifts as the norms of the community change. Yet we need not worry that this implies any dangerous kind ofrelativism or subjectivism. Our thoughts, intentions, and actions gain their very identity from the structure of public, communitarian relations we enter into, and the community imposes on us a tight structure of norms and intellectual options that "situate" us (a favored word in the Fish vocabulary) in such a way that it is always clear what interpretive move is appropriate.1 Fish is no epistemological anarchist in the style of Feyerabend: it is not the case, for him, that "anything goes." There is always a clear distinction between what goes and what does not; it is just that the rules, the structure, the hierarchy of options are not invariant across time or communities.2 Now we do it like this, tomorrow we may do it like that; we do it this way, they do it that way. Wherever you are, the rules are always clear, but the rules depend on where you are. I shall call this thesis "internalism." It bears some resemblance to views of the later Wittgenstein, and rather closely parallels Thomas Kuhn's theory of scientific paradigms. It has a negative and a positive aspect. The negative aspect is that community-specific constraints on interpretation are all we have. The positive aspect is that they are all we need. Is internalism correct? I say it is not. I say that the positive claim is false, because internal constraints alone can never answer the relativist's challenge, when that challenge is properly understood; more about that in a moment. But this merely serves to highlight the importance of the negative thesis: if that is correct then relativism carries the day. So my primary concern here is with the negative thesis. Fish's main argument for the negative thesis is that what seems to most of us to be an obvious external constraint on interpretation—the literal meaning of the text in question—is no such thing. And he bases this claim on some arguments which purport to show that there L· no such thing as literal meaning. So my aim, it emerges at last, is to defend the idea of literal meaning. But first, I shall say something about the question of relativism. II Fish's answer to relativism, it will be remembered, is that interpretive communities constrain the interpretive choices of their members in such away that there can always be seen tobe nonarbitrary ways ofproceeding when it comes to formulating and choosing between interpretive hypotheses. The present paper deals with the relationship of Lorenzen's notion of calculus with the notion of formal system . La teoria delle obbligazioni rappresenta un significativo esempio di come un insieme di tecniche logiche possa essere applicato, in maniera feconda, a contesti disputazionali in cui la dimensione dialogica e l’interazione tra soggetti coinvolti in un confronto dialettico hanno un ruolo di primo piano.L’attenzione che numerosi studiosi hanno manifestato nei confronti di questa parte della logica modernorum negli ultimi quarat’anni testimonia la profondità e la difficoltà dei problemi teorici che stanno alla base della teoria.Molte interpretazioni sono state avanzate circa le (...) motivazioni che spingono i logici medievali a scrivere trattati in cui il portato delle loro dottrine logiche – come, ad esempio, la teoria delle consequentiae - viene declinato ed elaboratamente applicato al gioco dialettico tra due disputanti. La gamma di tali letture comprende differenti ipotesi: la teoria delle obbligazioni potrebbe fornire un nucleo di regole per l’impostazione di un metodo assiomatico rudimentale così come rappresentare un tentativo di elaborare esercizi volti a valutare le capacità logiche degli studenti o, ancora, un gioco teorico, svincolato dall’effettiva applicazione a contesti concreti, la cui finalità sarebbe il mantenimento della coerenza logica.E’ stata sostenuta da più interpreti la funzione pedagogica svolta dalla teoria, al di là del fatto che si tratti o meno di un metodo per la conduzione effettiva di dispute reali. Un’altra linea interpretativa collega la teoria delle obbligazioni al contesto della letteratura dei sophismata e degli insolubilia, dal cui repertorio, in effetti, sono spesso tratti molti degli esempi che si incontrano nella lettura dei trattati de obligationibus. In questo senso, la teoria offrirebbe regole e schemi di risposta che un interlocutore deve utilizzare per risolvere o rispondere a enunciati problematici o paradossali. Non si può escludere, inoltre, che nella teoria si realizzi un intreccio delle differenti motivazioni sopra esposte: potrebbe trattarsi di uno strumento che viene sviluppato tanto per la risoluzione di sophismata e insolubilia quanto per indagare il rapporto tra linguaggio e metalinguaggio o altre questioni teoriche. Altri studiosi hanno infine sostenuto che lo scopo della teoria delle obbligazioni consisterebbe nell’elaborazione di un metodo adatto alla costruzione e alla discussione di esperimenti mentali e che questo costituirebbe una sorta di metametodologia della scienza medievale sulla linea del ragionamento secundum imaginationem. I shall argue that cinematic images do not have tense: not, at least, in the sense that has been ascribed to them by film theorists. This does not abolish time in cinema, for there can be temporal relations without tense, and temporal relations between cinematic images can indicate temporal relations between events depicted. But the dispensability of tense will require us to rethink our assumptions about what is sometimes called anachrony in cinema: the reordering of story-time by narrative, of which (...) the flashback is the most common example. The approach is philosophical: that is to say, the author offers an account of key concepts such as fictional truth, fictional characters, and fiction itself. The book argues that the concept of fiction can be explained partly in terms of communicative intentions, partly in terms of a condition which excludes relations of counterfactual dependence between the world (...) and the text. This communicative model is then applied to the following problems: how can something be 'true in the story' without being explicitly stated in the text? In what ways does interpreting a fictional story depend upon grasping its author's intentions? Is there always a unique best interpretation of a fictional text? What is the correct semantics for fictional names? What is the nature of our emotional response to a fictional work? In answering these questions the author explores the complex interaction between author, reader, and text. This interaction requires the reader to construct a 'fictional author' - a character in the story whose personality, beliefs and emotional states must be interpreted if the reader is to grasp the meaning of the work.
