IntroductionYounger age at diagnosis is a risk factor for poor health-related quality of life in long-term breast cancer survivors. However, few studies have specifically addressed HRQOL in young adults with breast cancer, nor have early changes in HRQOL been fully characterized.MethodsEligible female patients with breast cancer were identified through our local cancer center. To establish HRQOL, patients completed the Functional Assessment of Cancer Therapy-Breast around diagnosis and 12 months later. Sociodemographic factors, genetic susceptibility to cancer, tumor- and treatment-related factors, and (...) comorbidities were abstracted from medical records and the local oncology registry. Mixed-effects models were used to identify changes in FACT-B scores during the first year of treatment and to determine whether any demographic/treatment-related factors modulated changes in scores.ResultsHealth-related quality of life in young patients with breast cancer was within normal limits at baseline, with a FACT-B overall well-being score of 108.5. Participants reported slight improvements over a 12-month period: FACT-B overall well-being scores increased 6.6 points, functional well-being improved 3.0 points, emotional well-being improved 1.9 points, and physical well-being improved 1.5 points, on average. Participants with anxiety/depression at baseline reported greater improvements in FACT-B overall well-being and functional well-being than participants who did not have anxiety/depression at baseline. Marital status, reconstructive surgery, and baseline clinical staging were also significantly associated with changes in aspects of HRQOL, although their impact on change was relatively minimal.ConclusionYoung women with breast cancer do not report HRQOL concerns during the first year of treatment. Improvements in HRQOL during the first year of treatment may be attributable to a sense of relief that the cancer is being treated, which, in the short run, may outweigh the negative late effects of treatment. Virtual globes, like Google Earth, are increasingly being used by experts and lay publics in the process of 'defending place' against rapidly shifting energy geographies. Drawing upon the fields of visual studies, landscape architecture and geography, this article examines the use of geospatial tools by conservation campaigns challenging new developments. With a focus on wind energy, the article describes the strategic advantages and risks associated with using virtual globes in campaign work. The article suggests that conservation campaigns need to make (...) more judicious use of visualization technologies. In the drought-prone regions of Maharashtra State, a growing social movement for equitable water distribution is engaging the help of engineers to build technical projects. This movement aims to challenge the state’s agroindustrial development model favoring water-intensive sugarcane farming by redistributing water. This article examines the Baliraja Memorial Dam, located in southwestern Maharashtra. Through the dam, 400 families in the villages of Balawadi and Tandulwadi will receive a share of water for irrigation and domestic needs. This article explores the Baliraja (...) Dam as an appropriate technology project designed jointly by village farmers, engineers, and social activists. Through this unique alliance, Baliraja was designed to meet the needs of ecological restoration, community empowerment, and democratic technological design. The pioneering success of the Baliraja effort has prompted other communities to pressure the state irrigation bureaucracy to address the ecological and human costs of prolonged drought and water misuse. Sentence boundary detection (SBD) represents an important first step in natural language processing since accurately identifying sentence boundaries significantly impacts downstream applications. Nevertheless, detecting sentence boundaries within legal texts poses a unique and challenging problem due to their distinct structural and linguistic features. Our approach utilizes deep learning models to leverage delimiter and surrounding context information as input, enabling precise detection of sentence boundaries in English legal texts. We evaluate various deep learning models, including domain-specific transformer models like LegalBERT and (...) CaseLawBERT. To assess the efficacy of our deep learning models, we compare them with a state-of-the-art domain-specific statistical conditional random field (CRF) model. After considering model size, F1-score, and inference time, we identify the Convolutional Neural Network Model (CNN) as the top-performing deep learning model. To further enhance performance, we integrate the features of the CNN model into the subsequent CRF model, creating a hybrid architecture that combines the strengths of both models. Our experiments demonstrate that the hybrid model outperforms the baseline model, achieving a 4% improvement in the F1-score. Additional experiments showcase the superiority of the hybrid model over SBD open-source libraries when confronted with an out-of-domain test set. These findings underscore the importance of efficient SBD in legal texts and emphasize the advantages of employing deep learning models and hybrid architectures to achieve optimal performance. Disparate attempts exist to identify the key components that make an ethics pedagogy more effective and efficient. To integrate these attempts, a review of 408 articles published in leading journals is conducted. The key foci of extant literature are categorized into three domains labeled as approach, content, and delivery, and a comprehensive framework for ethics pedagogy developed. Within each of these domains, binaries that reflect two alternatives are identified. Approach, the philosophical standpoint, can be theory-laden or real-world connected. Content, the (...) constituencies addressed, can have a focus on breadth or depth. Delivery, the execution of the adopted pedagogy, can be traditional or innovative. The review of articles also identifies the lack of pedagogies that comprehensively focus on all the binaries across domains. The other substantive contribution of this article addresses this gap by developing a generic pedagogy—Integrative Live Case—based on the ACD framework. Based on an incident that is currently unfolding, this pedagogy allows integration of binaries across the three domains. It also allows for a modular course plan that can accommodate varied pedagogical preferences. Volkswagen Dieselgate is presented as a stylized example to showcase the significant advantages of using this pedagogy. (shrink)

After 40 years of attributing high rates of physician career dissatisfaction, attrition, alcoholism, divorce and suicide to ‘burnout’, there is growing recognition that these outcomes may instead be caused by moral injury. This has led to a debate about the relative diagnostic merits of these two terms, a recognition that interventions designed to treat burnout may be ineffective, and much perplexity about how—if at all—this changes anything. The current research seeks to develop the construct of moral injury outside military contexts, (...) generate more robust validity tests and more fully describe and measure the experiences of persons exposed to moral harms. Absent from the literature is a mechanism through which to move from the collective moral injury experience of physicians to a systematic change in the structure of medical practice. To address this, after providing a brief history, definitions and contrasts between burnout, moral distress and moral injury, we review the interplay of moral and ethical codes in the context of moral injury. We conclude by suggesting that professional associations can potentially prevent moral injury by providing protections for physicians within their codes of ethics. There are no data in this work. As part of a (bio)colonial project, the biological information of Indigenous Peoples has historically been under scientific scrutiny, with very limited benefits for communities and donors. Negative past experiences have contributed to further exclude Indigenous communities from novel developments in the field of omics research. Over the past decade, new guidelines, reflections, and projects of genetic research with Indigenous Peoples have flourished in Australia, providing opportunities to move the field into a place of respect and ethical relationships. This review explores (...) the ethical and cultural implications of the use of biological samples from Indigenous communities in biomedical research. A structured framework outlining emerging topics of interest for the development of respectful omics research partnerships with Indigenous Australians is presented. This paper highlights aspects related to Indigenous governance, community and individual consent, respectful handling of biological samples, data management, and communication in order to protect Indigenous interests and rights and to promote communities’ autonomy. This article studies the eugenic theories of Marathi sexological writer and novelist Narayan Sitaram Phadke, and his attempts to domesticate the modern ideal of the adult romantic couple as a yardstick of ‘emotional democracy’ in late colonial India. Locating Phadke's work against the backdrop of the Child Marriage Restraint Act (1929) and its eugenicist concerns, I argue that he conceptualized romantic love as an emotion and a form of sociability central to the state's biopolitical schemes of ensuring modern (...) coupledom but as exceeding the state's capacity to rationally order Indian sex life. Consequently, he crafted literary supplements like the bildungsroman to circulate ‘English’ idioms of emotional and corporeal intimacy in Marathi; his novels domesticated eugenic sexology for its ‘vernacular’ audiences by advocating caste-bound romantic love as the blueprint for Indian marital coupling. As exemplified by Phadke's work, an emerging Marathi discourse of love demarcated a space for the young couple to operate as a vehicle of interpersonal openness within the constraints of the upper-caste joint family. By outlining the parameters of this Brahmanical aesthetic discourse, I show that the couple became the locus of a self-styled ‘democratic’ form of emotional attachment aimed at developing a necessary dynamism within endogamous caste-based marital arrangements without radically transforming them. The science performed through the Marathi novel in the 1920s and 1930s consequently explains the increasing prominence of romantic love as a form of developmental ‘democratic’ discourse at a time when both romantic love and democracy-in-practice were widely experienced as absent from Indian society.
