The post-classical (or post-Avicennan, post-Rāzian) genre of the “protocols for dialectical inquiry and disputation” (ādāb al-baḥth wa-l-munāẓara) has its more proximate origins in the famed Risāla of Shams al-Dīn al-Samarqandī (d. 722/1322). The greater part of his conceptions and methodology, however, consists in a streamlining and universalizing of the more strictly juristic dialectic (jadal / khilāf) of his teacher Burhān al-Dīn al-Nasafī (d. 687/1288); and this in turn draws on the highly logicized dialectic of Rukn al-Dīn al-ʿAmīdī (d. 615/1218) and (...) his teacher Raḍī al-Dīn al-Nīsābūrī (d. 617/1220). At the heart of methods in this lineage, and carried forward by al-Samarqandī into the universal ādāb al-baḥth, are three truth-preserving logical relationships critical to the truth-seeking enterprise of dialectic: entailment (talāzum / mulāzama), mutual negation or exclusion (tanāfin / munāfā), and causal concomitance (dawarān). The practical elaboration of these relations reveals a logic in action—a premodern dialogical logic for living disputation praxis. In fact, so critical were these to the dialectical enterprise that al-Samarqandī devoted a specialized treatise entirely to summarizing their defining features and rules, aptly naming it the ʿAyn al-Naẓar, or “Wellspring of Rational Investigation.” In this article, and drawing upon a recently published digital critical edition, I will present an analytical outline of these core logical relations as presented in the ʿAyn al-Naẓar. The Dialectical Forge identifies dialectical disputation as a primary formative dynamic in the evolution of pre-modern Islamic legal systems, promoting dialectic from relative obscurity to a more appropriate position at the forefront of Islamic legal studies. The author introduces and develops a dialectics-based analytical method for the study of pre-modern Islamic legal argumentation, examines parallels and divergences between Aristotelian dialectic and early juridical jadal-theory, and proposes a multi-component paradigm—the Dialectical Forge Model—to account for the power of jadal in shaping Islamic (...) law and legal theory.In addition to overviews of current evolutionary narratives for Islamic legal theory and dialectic, and expositions on key texts, this work shines an analytical light upon the considerably sophisticated “proto-system” of juridical dialectical teaching and practice evident in Islam’s second century, several generations before the first “full-system” treatises of legal and dialectical theory were composed. This proto-system is revealed from analyses of dialectical sequences in the 2nd/8th century Kitāb Ikhtilāf al-ʿIrāqiyyīn / ʿIrāqiyyayn through a lens molded from 5th/11th century jadal-theory treatises. Qiyās, or “correlational inference” (often glossed as “analogy”), comprises a primary set of methodological tools recognized by a majority of premodern Sunnī jurists. Its elements, valid modes, and proper applications were the focus of continual argument and refinement. A particular area of debate was the methodology of determining or justifying the ʿilla: the legal cause (or occasioning factor, or ratio legis) giving rise to a ruling in God’s Law. This was most often discussed (and disputed) under the rubric of “the (...) modes of causal justification” (masālik al-taʿlīl). Among these modes was the much debated test of dawarān (concomitance of presumed cause and effect). In brief, proponents of dawarān employed it to justify claims that a property (waṣf) occasioned the ruling (ḥukm) in an authoritative source-case (aṣl). In concert with other considerations, the demonstrated co-presence (ṭard) and co-absence (ʿaks) of property and ruling—that is, their concomitance “in existence” (wujūdan) and “in nonexistence” (ʿadaman)— was taken as an indication that the property was the ruling’s ʿilla. Delving further into dawarān and causation (ʿilliyya), the current study interprets “in existence” and “in nonexistence” not as a kind of metaphor for true and false (within the framework of a classical truth-functional formal semantics), but as an accurate terminology vis-à-vis the meaning of causality statements, fully compatible with dominant Islamicate views on causal agency. In brief, a deeper logical and linguistic analysis of the different existential modes of dawarān strongly suggests that we should distinguish property (or phenomenon) and ruling (or effect) as types (concepts or propositions linguistically expressed by a sentence) as opposed to tokens (instantiations of the type; the real, ontological events that verify the proposition). Our reading of dawarān as shaped by a finer-grained structure not only allows us to identify the efficient occasioning process as a function which takes some particular token of the ʿilla (arguably, the property or properties which provide the ruling’s material cause) and renders a token of the general ruling type, but it allows us to elucidate the role of taʿlīl (causal justification) in shaping an epistemological theory of argument to the best explanation: a sophisticated, premodern manifestation of abductive reasoning. The Israeli army has by now been in direct and brutal military occupation of the West Bank and Gaza for twenty-one years; the intifadah, surely the most impressive and disciplined anticolonial insurrection in this century, is now in its eleventh month. The daily killings (...) of unarmed Palestinians by armed Israelis, soldiers and settlers, numbers several hundred; yesterday two more Palestinians were killed, the day before four were killed. The beatings, expulsions, wholesale collective punishments, the closure of schools and universities, as well as the imprisonment of dozens of thousands in places like Ansar III, a concentration camp, continue. A V sign flashed by a young Palestinian carries with six months in jail; a Palestinian flag can get you up to ten years; you risk burial alive by zealous Israel Defense Forces soldiers; if you are a member of a popular committee you are liable to arrest, and all professional, syndical, or community associations are now illegal. Any Palestinian can be put in jail without charge or trial for up to six months, renewable, for any offense, which needn’t be revealed to him or her. For non-Jews, approximately 1.5 million people on the West Bank and Gaza, there are thus no rights whatever. On the other hand, Jews are protected by Israeli law on the Occupied Territories. In such a state of apartheid—so named by most honest Israelis—the intifadah continues, as does the ideology of difference vainly attempting to repress and willfully misinterpret its significance. The focus of this chapter is the rise of investigations in philosophical aesthetics in the mid-1950s and continuing through to the mid-1960s. This salient issue had to do with the foundations of philosophical aesthetics in the context of the Marxist-Leninist worldview. That this became an issue was due in large part to the appearance, in 1956, of the first Russian translation of Marx’s Economic and Philosophical Manuscripts of 1844. Marx’s emphasis in these writings on the self-constituting, transformative potential of labor (...) suggested possibilities for an aesthetic that did not sit comfortably with the dogmatic principles of dialectical and historical materialism. In effect, the foundational question for a Soviet aesthetics went to the heart of Marxism-Leninism: could young Marx’s anthropocentrism be reconciled with the established Soviet doctrine beholden to, on the one hand, Engels’ metaphysics of “matter-in-motion” and, on the other hand, Lenin’s copy theory of knowledge? Among the aestheticians, the issue surfaced in a debate about the nature of beauty: is beauty relative to man or is it an objectively cognizable material property? To the degree that the parties to this debate addressed the underlying issue, they tended to remain ambivalent toward the result that neither side—those favoring creative expression versus the defenders of objective beauty—simply conceded nor rejected the views of the other. As I argue, a systematic philosophical aesthetics within the scope of Soviet philosophy never saw the light of day. By the mid-1960s, research was ramifying into a variety of adjacent considerations, for example, value theory, theories of culture and cultural artifacts, the history of aesthetic categories, the social and pedagogical functions of art, art and morality in relation to the “socialist way of life,” art in the era of the “scientific-technological revolution,” and so on—of which only some recalled the spirit, rarely the letter, of the initial “aesthetics” discussion. This book aims to provide scientists and engineers, and those interested in scientific issues, with a concise account of how the nature of scientific knowledge evolved from antiquity to a seemingly final form in the Twentieth Century that now strongly limits the knowledge that people would like to gain in the Twenty-first Century. Some might think that such issues are only of interest to specialists in epistemology (the theory of knowledge); however, today's major scientific and engineering problems--in biology, medicine, environmental (...) science, etc.--involve enormous complexity, and it is precisely this complexity that runs up against the limits of what is scientifically knowable. To understand the issue, one must appreciate the radical break with antiquity that occurred with the birth of modern science in the Seventeenth Century, the problems of knowledge and truth engendered by modern science, and the evolution of scientific thinking through the Twentieth Century. While originally aimed at practicing scientists and engineers, it is my hope that this book can provide a generally educated person with a basic understanding of how our perspective on scientific knowledge has evolved over the centuries to escape pre-Galilean commonsense thinking. Such an appreciation is not only beneficial for one's general education, but is important for non-scientists who must teach young students or make policy decisions in government or business. Buxrr HAS ADDUCED,Isaac Barrow (1630-1677) in his philosophy of space, time, and mathematical method strongly influenced the thinking of Newton: The recent publication of an early paper written by Newton (his De gravitatione et aequipondio fluidorum)2 affords evidence not known to Burtt of Newton's indebtedness in philosophy to Barrow, his teacher. Prior to its publication in 1962, this paper was (...) utilized by Alexandre Koyrd in his essay, "Newton and Descartes," 3 a study based on the third Horblit Lecture in the History of Science which he gave at Harvard University, March 8, 1961. Koyr8 maintains that there is a radical opposition in Newton's Principia not only to Descartes' purely scientific theories but also to the Cartesian philosophy. Yet, as he remarks, "we do not find in the Principia an open criticism of the philosophy of Descartes.... " He attributes this fact principally "to the very structure of the Principia: it is essentially a book on rational mechanics, which provides principles for physics and astronomy. In such a book there is a normal place for the discussion of Cartesian optics, but not of the conception of the relations of mind and body, and other such things." Koyrd asserts, nonetheless, that the criticism is not absent. It lurks in Newton's carefully worded definitions of fundamental concepts--those of space, time, motion, and matter--and becomes more apparent in the Optice of 1706 and in the second odition of the Principia. In support of this argument, he makes much of young Newton's unfini.~hed paper. First, I explore some answers to the question "How did Rhetoric get so Big?" Second, I review some of the more important criticisms of a "globalized" or "universalized" view of rhetorical studies. Finally, I contend that the critiques of Big Rhetoric (...) do not withstand scrutiny and ought to be dismissed for insufficient evidence. While there certainly are important issues for scholars of rhetorical studies to consider about how to enhance the quality and importance of our work, such issues should not include the concern that rhetoric has grown too "big."By "Big Rhetoric" I refer only to the theoretical position that everything, or virtually everything, can be described as "rhetorical." I refer to the growth of rhetorical scholarship in communication studies and other disciplines as the "popularization" of rhetorical studies. 1 Theories associated with Big Rhetoric are credited with popularizing or at least rationalizing what Herbert W. Simons (1990) calls the "rhetorical turn" in a variety of disciplines. Within the journals and conventions of members of the National Communication Association (NCA), popularization is often characterized by studies of the form "the rhetoric of X," where X could literally be anything. Outside of the NCA-defined parameters of communication studies, popularization is evidenced by the apparently ever-increasing ranks of scholars who use "rhetoric" as a relevant and important term of art within their scholarship. By either measure, it can be argued fairly convincingly that "rhetoric" has become a widely used construct in scholarship. What I wish to engage is the disputed desirability of broad definitions and the related popularization of rhetoric. [End Page 260] 1. Whence big rhetoric? There are a number of narratives on the rise of Big Rhetoric already in print in communication studies (Simons 1990; Gaonkar 1990), rhetoric and composition (Berlin 1987; Young and Goggin 1993), and interdisciplinary publications (Roberts and Good 1993; Mailloux 2000; Nelson, Megill, and McCloskey 1987). Many accounts identify the 1960s as a turning point. For better or for worse, there was a confluence of changing rhetorical practices, expanding rhetorical theories, and opportunities for rhetorical criticism. The cultural clashes of the 1960s were felt perhaps most acutely on college campuses. The sufficiency of deliberative argument and public address can be said to have been called into question, whether one was an antiwar activist who hated LBJ's war in Vietnam or a pro-establishment stalwart trying to make sense of the rhetoric of protest and demonstration. Years later, scholars would characterize war itself as rhetorical. What counted as rhetorical practice was up for grabs.At about the same time, our understanding of rhetorical theory was being expanded. Here we can identify two main strands of thought. For ease of reference I will call one the symbolic interactionist rationale and the other the epistemological rationale. The symbolic interactionist rationale can be boiled down to a syllogism: All persuasive actions are rhetorical. All symbol/language-use is persuasive. Therefore: All symbol/language-use is rhetorical. Perhaps the two most significant pronouncements of this approach--for my generation of rhetoric scholars, at any rate--are by Richard Weaver and Kenneth Burke. Weaver claims that "language is sermonic" in the sense that whenever we offer a description or label a phenomenon, we are "preaching" a particular way of making sense of it (1970, 201-25). Burke, of course, has two famous (or infamous) statements in Rhetoric of Motives: first, that rhetoric is "the use of language as a symbolic means of inducing cooperation in beings that by nature respond to symbols" (1950, 43); and second, that "something of the rhetorical motive comes to lurk in every 'meaning,' however purely 'scientific' its pretensions. Wherever there is persuasion, there is rhetoric. And wherever there is 'meaning,' there is 'persuasion'" (1950, 172). An evolutionary account of excessive crying in young infants – colic – has been elusive. A study of mothers with new infants suggests that more crying is associated with more negative emotions towards the infant, and perceptions of poorer infant health. These results undermine the hypothesis that excessive crying is an honest signal of vigor.

This paper presents the first part of an interpretative phenomenological analysis of schema modes within the single case of Linda, a young woman with anorexia nervosa. This paper presents the second part of an interpretative phenomenological analysis of schema modes within the single case of Linda, a young woman with anorexia nervosa. In this paper, the focus is on Linda’s Coping modes and on several important superordinate themes: mode dyads, (...) mode conflicts and balance of power, mode differentiation, and mode sequences. The findings support the value of the mode framework that is standard in schema therapy, based on Child modes, Parent modes, Coping modes, and the Healthy Adult. They furthermore highlight the idiosyncratic nature of schema modes within an individual case. This art inclines those who follow it to desire to know the secrets of this world. 1Although Columbus was certainly not unlearned, there is an artlessness and directness about his remark that suggest that whatever Columbus may have meant by "the secrets of the world" and the desire of navigators to know them, he was probably not speaking reconditely or mystically. 2 [End Page 593]Nor was Columbus the only early sixteenth-century practical man who spoke this way. The letters of Hernán Cortés, hardly a philosopher or a navigator, pointed out to correspondents that it was "a universal condition of man to want to know," and that he was first of all desirous of knowledge "of the secrets of these parts"; that "many new secrets" had been learned from the discoveries; of a volcano, "I wished to know the secret of this, which seemed something of a miracle." 3If practical folk could speak thus at the turn of the sixteenth century, it is not surprising that in the course of that century their rulers should appropriate the language of secrets, including the secrets of distant places, and the desire to know them and in doing so to transform their realms into information-gathering states, using detailed questionaires to assemble and organize new data just as they assembled cabinets of curiosities, libraries, zoos, observatories, and botanical gardens. As J. H. Elliott put it: But curiosity also had its due place within a wider Christian framework. At the end of the century, José de Acosta, in his great Natural and Moral History of the Indies, likened men to ants in their refusal to let themselves be deterred, once they had set out on their quest for facts. "And the high and eternal wisdom of the Creator uses this natural curiosity of men to communicate the light of His holy gospel to peoples who still live in the darkness of their errors." This assumption, that all knowledge was subordinated to a higher purpose and fitted into a providential design, was crucial for the assimilation of the New World of America by sixteenth-century Christendom. 4Elliott's characterization of an honorific Christian intellectual curiosity represented a very small part of a very complex semantic universe that centered on the Latin term curiositas and its vernacular cognates, not all of whose meanings were neutral or honorific. Just three years before Acosta wrote there had, after all, appeared the first printed history of Doctor Faustus, another sixteenth-century practical man who was also interested in the secrets of the world. 5 And not [End Page 594] too many years after Acosta wrote, another practical being, Milton's Lucifer, also went forth boldly to seek new worlds. 6 Only in a restricted sense could the desire to know the secrets of the world be understood in the way that Columbus, Cortés, Philip II, and Acosta celebrated it. In many cases it had to be extricated from a semantic universe in which it had long been identified with less honorable motives and more dangerous or ephemeral objects.The most influential exploration of part of that semantic universe appeared in Hans Blumenberg's long essay entitled, "The 'Trial' of Theoretical Curiosity," first published as part of a larger work in 1966, later slightly revised and separately reprinted, and translated into English in 1983. 7 Blumenberg's particular understanding of the history of philosophy, however, and his narrow focus on the natural sciences as the arena of "theoretical" curiosity limited both his conceptualization and his argument. In the UK and elsewhere suicide presents a major cause of death. In 2008 in the UK the topic of suicide rarely left the news. Controversy surrounding Daniel James and Debbie Purdy ensured that the problem of assisted suicide received frequent media discussion. This was fuelled also by reports of a higher than usual number of suicides by young people in South Wales. Attention attracted by cases such as that of Daniel James and Debbie Purdy can lead to a neglect (...) of the problem of how to respond to the vast majority of suicides, in which there is no obvious accompanying health problem. This paper seeks to redress that balance to some degree. Its focus is primarily on the provision of assisted suicide when the person seeking such assistance has no accompanying health problem. It is argued that a network of suicide centres (SCs) should be established, the purpose which in certain circumstances, will extend to assisting suicide in cases where a competent person has a definite, sustained intention to end their own life. Arguments in support of this proposal are provided, focusing on respect for autonomy, relief of suffering and respect for the ‘life-plan’ of a person. Considerations relating to reduction of harms to third parties are also recruited. Eight responses to the proposal are considered. But none is found compelling, nor are they found jointly compelling. _The Social Work Psychoanalyst's Casebook_ begins with an interview with Dr. Sanville, who reflects on her evolution as a social work analyst, theoretician, writer, teacher, and leader. These reminiscences are followed by accounts of nine analytic treatments, each of which offers an unusual window into what actually transpired between analyst and analysand during the treatment hours. These case studies concern particularly troubled, often traumatized patients-the very "hard to reach" or "difficult to treat" clients with whom social workers have long been (...) familiar. They include a reanalysis by the same analyst of a patient whose first therapy ended in a stalemate; an account of transference and countertransference phenomena during termination; a report on the analysis of a young woman who experienced both chronic and stress-related trauma; and an account of the special issues involved in the treatment of an aging woman. Most of the case studies reflect the influence of Dr. Sanville, whose work has long evinced the therapeutic imagination and disciplined creativity to which all the contributors aspire. Tthe contributors to this volume offer the salutary reminder that analytic work is built on a relationship of respect and empathy and that treatment success follows from the therapist's willingness to accommodate the unique needs of individual patients. In honoring Jean Sanville, _The Social Work Psychoanalyst's Casebook_ speaks to the robustness of a multidisciplinary approach to psychopathology that transcends the bounds of any single profession-an approach in which contemporary psychoanalysis is enlarged by the insights and emphases of social work just as social work is enriched by the clinical wisdom of psychoanalysis. This paper recounts the events surrounding the case of Ashley X, a severely disabled young girl whose parents opted for oestrogen therapy, a hysterectomy and breast removal – the so-called ‘Ashley treatment’ – in order to reduce her projected adult weight and improve her quality of life. Following a description of the events leading up to the procedure itself, and the worldwide debate which ensued, the main arguments in favour and against the procedures are presented. The paper also critically engages (...) with a recent defence of the treatments presented by one of those closely involved with the case – Dr D Diekema. Finally, the recommendations of a report which criticizes procedures at the hospital in which the treatments took place are described, and it is argued that the main recommendation regarding the constitution of the Hospital Ethics Committee (HEC) is ill-thought through and is unlikely to have made any difference to the decision taken by the HEC to approve the Ashley treatments. Background Over the last decades, the neurosciences, behavioral sciences, and the social sciences have all seen a rapid development of innovative research methods. The field of bioethics, however, has trailed behind in methodological innovation. Despite the so-called “empirical turn” in bioethics, research methodology for project development, data collection and analysis, and dissemination has remained largely restricted to surveys, interviews, and research papers. We have previously argued for a “Design Bioethics” approach to empirical bioethics methodology, which develops purpose-built methods for investigation (...) of bioethical concerns. In this paper we compare a research tool created using a design bioethics approach to a “methods-as-usual” approach in empirical bioethics. Methods Our study compared dimensions of engagement with a digital game we created, called “Tracing Tomorrow,” to a standard vignette survey. The two tools investigated the same subject matter, digital phenotyping for mental health, in a sample of 301 UK adolescents. Results Participants who played the game reported a greater sense of presence, emotional engagement, cognitive absorption, and mental health ethics insight, compared to participants who completed the vignette survey. Perceived authenticity and curiosity/motivation to learn more was equivalent for both methods. Conclusion The results of this study highlights the importance of purpose-built methodology for empirical bioethics research. (shrink)

The contributors to this collection employ the analytic resources of cultural-historical theory to examine the relationship between childhood and children's development under different societal conditions. In particular they attend to relationships between development, emotions, motives and identities, and the social practices in which children and young people may be learners. These practices are knowledge-laden, imbued with cultural values and emotionally freighted by those who already act in them. The book first discusses the organising principles that underpin a cultural-historical understanding of (...) motives, development and learning. The second section foregrounds children's lives to exemplify the implications of these ideas as they are played out - examining how children are positioned as learners in pre-school, primary school and play environments. The final section uses the core ideas to look at the implementation of policy aimed at enhancing children's engagement with opportunities for learning, by discussing motives in the organisations that shape children's development. Nahm. He had been giving a paper that involved Falstaff—I forget how, but it included the familiar appeal to the fat knight as the comic spirit of untrammelled life, so that the newly crowned Hal’s final repudiation—“I know thee not, old man”—chills the audience as a denial of humanity (...) in favor of impersonal administration. But I told Nahm that I could not see Falstaff that way. As a boy, if I cycled home from Gravesend to Rochester I had to pass over the North Downs at Gadshill. And it is at Gadshill, when we first meet him, that Falstaff’s gang is planning to waylay a band of innocent travelers. I could never take kindly to a man who made a living mugging and rolling people like me. Nahm was contemptuous of my incompetence to deal with literature: I was prevented from appreciating the portrait of the comic monster, appalling but breathtaking, by my inhibited and inhibiting moralism. But I insisted that I saw no reason why one should abandon one’s identity or one’s morality while one was watching a play or reading a book.Nahm would have been right if I had been insensitive to Falstaff’s outrageousness, his unscrupulous energy and vivacity, his wit and candor. But I think my appreciation was sharpened rather than nullified by my awareness of the real menace Falstaff represents to people who happen not to be his pals. And I think Nahm’s enthusiasm blinded him to something else that Shakespeare wrote into the part. At that very first meeting, we find Falstaff trying to negotiate for himself a position of power or privilege when the young prince shall succeed to the throne. And it is just this negotiation that Falstaff is trying to resume [End Page 398] from the sidewalk at the end, when the new king brushes him off. Nahm did not notice this because it did not occur to him that Henry V was going to be a real king with real power over real people, just as it did not occur to him that Falstaff’s robbed and brutalized pilgrim victims would be real people suffering real injury and loss. If my personal angle, the potential victim’s view, had so usurped my vision that it blinded me to whatever else the playwright had made of his character and its situation, it would have made me a bad viewer of the play. But it might have added depth to my understanding and my criticism if it gave me an angle that other viewers and readers could imaginatively share. 1 Since each of us has some sort of angle on everything, sharing perspectives can always be valuable.III have been reminded of my encounter with Nahm by reading Edward W. Said’s Culture and Imperialism. 2 The book appears to start from the position that imperialism is the “determining political horizon of modern Western culture” (p. 60), imperialism being defined as the policy of establishing and maintaining an empire, and an empire as “a relationship... in which one state controls the effective political sovereignty of another political society” (p. 9). Because this is the determining horizon, the right way to think of any “Western” author is as an agent of imperialism, although of course this is most easily shown in the case of authors and works in which the imperial connection is explicit.If there is to be a uniform curriculum and canon for the teaching of English literature everywhere in the world, that canon obviously should not be based on works that assume the rightness and normality of an imperialism that defines most readers of that literature as racially and culturally inferior; such a canon is itself a tacit agent of an offensive and gratuitous imperialism. The capacity of visual working memory (VWM) refers to the amount of visual information that can be maintained in mind at once, readily accessible for ongoing tasks. In healthy young adults, the capacity limit of VWM corresponds to about three simple objects. While some researchers argued that VWM capacity becomes adult-like in early years of life, others claimed that the capacity of VWM continues to develop beyond middle childhood. Here we assessed whether VWM capacity reaches adult levels in adolescence. Using (...) an adaptation of the visual change detection task, we measured visual working memory capacity estimates in 13 year-olds, 16-year-olds, and young adults. We tested whether the capacity estimates observed in early or later years of adolescence were comparable to the estimates obtained from adults. Our results demonstrated that the capacity of VWM continues to develop throughout adolescence, not reaching adult levels even in 16 year-olds. These findings suggest that VWM capacity displays a prolonged development, similar to the protracted trajectories observed in various other aspects of cognition. (shrink)

ObjectiveCochlear explantation for purely elective (e.g. psychological and emotional) reasons is not well studied. Herein, we aim to provide data and expert commentary about elective cochlear implant (CI) removal that may help to guide clinical decision-making and formulate guidelines related to CI explantation.Data sourcesWe address these objectives via three approaches: case report of a patient who desired elective CI removal; review of literature and expert discussion by surgeon, audiologist, bioethicist, CI user and member of Deaf community.Review methodsA systematic review using (...) three scientific online databases was performed. Included articles addressed the benefits and/or complications of cochlear implantation in young children, CI explantation with or without revision surgery and the ethical debate between the medical and Deaf communities on cochlear implantation and explantation.ConclusionsThe medical and audiological perspectives identify a host of risks related to implant removal without reimplantation, including risk from surgery, general anaesthesia, cochlear ossification and poor audiometric outcomes. The member of the deaf community and bioethicist argue that physicians need to guide the principles of beneficence, non-maleficence and patient autonomy. Taken together, patient desires should be seen as paramount, if the patient is otherwise fit for surgery and well informed.Implications for practiceSimilar to the case of device implantation, device explantation should be a multidisciplinary and collaborative decision with the patient and the family’s desires at the centre. While every case is different, we offer a CI explantation discussion to assist in clinical decision-making, patient counselling and education. (shrink)

Background: When adolescent boys experience close, secure relationships with their parents and peers, the implications are potentially far reaching, including lower levels of mental health problems in adolescence and young adulthood. Here we use rare prospective intergenerational data to extend our understanding of the impact of adolescent attachments on subsequent postpartum mental health problems in early fatherhood.Methods: At age 17–18 years, we used an abbreviated Inventory of Parent and Peer Attachment to assess trust, communication, and alienation reported by 270 male (...) participants in their relationships with mothers, fathers, and peers. More than a decade later, we assessed the adult males, now fathers, at 12 months postpartum for symptoms of depression, anxiety, and stress. Logistic regression was used to examine the extent to which attachment dimensions predicted paternal postpartum mental health, adjusting for potential confounding, and with assessment for interactions between parent and peer attachments.Results: Trust in mothers and peers, and good communication with fathers during adolescence, were associated with 5 to 7 percentage point reductions in postpartum mental health symptoms in early fatherhood. Weak evidence of parent-peer interactions suggested secure attachments with either parent or peer may compensate for an insecure attachment with the other.Conclusions: Our results suggest that fostering trust and communication in relationships that adolescent boys have with parents and peers may have substantial effects on rates of paternal postpartum mental health problems. The protective benefits may be preventative in intergenerational cycles of risk for mental health problems. (shrink)
