Based on recent insight into the thalamocortical system and its role in perception and conscious experience, a unified pathophysiological framework for hallucinations in neurological and psychiatric conditions is proposed, which integrates previously unrelated neurobiological and psychological findings. Gamma-frequency rhythms of discharge activity from thalamic and cortical neurons are facilitated by cholinergic arousal and resonate in networks of thalamocortical circuits, thereby transiently forming assemblies of coherent gamma oscillations under constraints of afferent sensory input and prefrontal attentional mechanisms. If perception is based (...) on synchronisation of intrinsic gamma activity in the thalamocortical system, then sensory input to specific thalamic nuclei may merely play a constraining role. Hallucinations can be regarded as underconstrained perceptions that arise when the impact of sensory input on activation of thalamocortical circuits and synchronisation of thalamocortical gamma activity is reduced. In conditions that are accompanied by hallucinations, factors such as cortical hyperexcitability, cortical attentional mechanisms, hyperarousal, increased noise in specific thalamic nuclei, and random sensory input to specific thalamic nuclei may, to a varying degree, contribute to underconstrained activation of thalamocortical circuits. The reticular thalamic nucleus plays an important role in suppressing random activity of relay cells in specific thalamic nuclei, and its dysfunction may be implicated in the biological vulnerability to hallucinations in schizophrenia. Combined with general activation during cholinergic arousal, this leads to excessive disinhibition in specific thalamic nuclei, which may allow cortical attentional mechanisms to recruit thalamic relay cells into resonant assemblies of gamma oscillations, regardless of their actual sensory input, thereby producing an underconstrained perceptual experience. The commentators provide a wealth of additional neurobiological data that ought to be integrated in a comprehensive model. This response article, however, focuses on clarification of conceptual queries, thereby outlining the proposed theory of hallucinations more sharply, discussing its relationship with schizophrenia, and explaining why underconstrained thalamocortical activation may well be a candidate mechanism responsible for acute schizophrenic symptoms other than hallucinations.

All healthcare professionals (HCPs) have responsibilities to provide information to patients according to the duties found within UK decision-making guidance and with regards to theory about the doctor-patient relationship. While routinisation can be understood in a number of different ways, this paper is concerned with how routines might negatively affect patients in the decision-making process. Therefore, in this manuscript, medical decision making is understood as problematically routine when a medical test or procedure is framed as a standard one and—given the (...) way options are presented—it is implied that someone can decide to use that test or treatment without being given sufficient opportunities to think through their choices. Routinisation, when understood in this way, can affect the quality and amount of information provided to patients and the ways in which patients reflect upon their choices when making decisions. With the introduction of non-invasive prenatal testing (NIPT) into the UK healthcare system, bioethicists have questioned whether this new technology will be routinised and what the implications of routinisation on the decision-making process might be. While there have been numerous studies investigating the views and attitudes of pregnant women who use NIPT, there are fewer studies that look at the views and attitudes of healthcare professionals (HCPs). This study interviewed 20 UK-based HCPs who either offer NIPT or counselled pregnant women on the use of NIPT. One important finding was that many HCPs held the attitude that the NIPT decision-making process had become routine, however there was disagreement about whether routine NIPT was problematic or not. This study provides insights about the context that surrounds decision making for NIPT, and it raises important questions about how NIPT routinisation might be evaluated. (shrink)

Raimon Panikkar: A Companion to his Life and Thought is a guide to the life, work and thought of Raimon Panikkar, a self-professed Buddhist-Christian-Hindu philosopher and theologian. A man of deep and wide learning and an extremely prolific author, Panikkar is equally at home in various religious and cultural traditions and embodies in himself the ideals of intercultural, intrareligious, and interreligious dialogues. This book explicates Panikkar’s basic vision of life as the harmonious rhythm of divinity, humanity, and the cosmos, which (...) he terms "cosmotheandrism," and shows how it permeates and illumines his articulations of the central Christian doctrines. Given the complexity and difficulty of Panikkar’s thought this book is a welcome companion for a course on Panikkar and for a general reader who wishes to understand one of the most profound and orginal thinkers of our time. We suggest that in the particular context of medical education, ethics can be considered in a similar way to other kinds of knowledge that are categorised and shaped by academics in the context of wider society. Moreover, the study of medical ethics education is translational in a manner loosely analogous to the study of medical education as adjunct to translational medicine. Some have suggested there is merit in the idea that much as translational research attempts to connect the laboratory scientist's (...) work to its implications for patient care, translational ethics focuses on bringing ethics scholarship into the sphere of personal and public action. We distinguish the term ‘translational ethics’ (the study of ethics being translated between academy, classroom and clinic) from other prominent definitions in the bioethics literature. To do this, we build off a notion of knowledge translation that focuses on the nonlinear movement of information that comes to professionals through multiple competing sources. We suggest that this knowledge, and particularly knowledge about ethics, becomes embodied by the individual. It is through a reflective practice that internally embedded ethics knowledge might be modified, and this work might be best carried out with a moral community that maintains a sense of practical wisdom. Applying this translational approach to the study of medical ethics education can be both academically relevant and practically useful. This view of translation can help bridge the evident, multidirectional relationships between research, education and performance. It might also create further opportunities to develop medical ethics education theory. (shrink)

The practice of psychotherapy, or any form of counseling, inevitably requires an anthropological foundation from which to work. Defining the nature of personhood is a task which necessitates an explanation of the various aspects of the self, and the relationships between those aspects. In particular, it is crucial to delineate how the person is both an individual being and a relational being, for we intuitively experience life in both dimensions. These are two characteristics which have often been presented in opposition (...) to each other, however. What is needed is a theory which balances the individual character of personhood with the social character of personhood. The task at hand is to develop such a balance which can serve as a foundation for a Christian psychology. ;C. Stephen Evans, a philosopher and post-empirical critic of modern psychology, has emphasized the individual aspects of personhood. Soren Kierkegaard has influenced Evans' view of the self. Thus, Evans conceives of the person as fundamentally an agent, who chooses freely, acts purposefully and reasonably, and is responsible for the consequences of those choices and actions. ;Ray S. Anderson has developed a theological anthropology and on that basis has articulated a theoretical view of Christian counseling. His theory of the nature of personhood, influenced by the theologies of Karl Barth and Dietrich Bonhoeffer, is a thoroughly social perspective. The person is essentially relational, such that one is social before one is psychological. ;The present work constructs an integrative theory which balances the individual and social insights from Evans' and Anderson's views, respectively. It is accomplished through an analysis of the self across two ontological statuses and three orientations . A biblical paradigm is developed to illustrate the grounding of that theory in a Christian framework. Implications for therapeutic interventions are provided. (shrink)

This work builds on the Volterra series formalism presented in Dreisigmeyer and Young to model nonconservative systems. Here we treat Lagrangians and actions as ‘time dependent’ Volterra series. We present a new family of kernels to be used in these Volterra series that allow us to derive a single retarded equation of motion using a variational principle.

Increasing emphasis on genetic research means that growing numbers of human research projects in Australia will involve complex issues related to genetic privacy, familial information and genetic epidemiology. The Office of Population Health Genomics (Department of Health, Western Australia) hosted an interactive workshop to explore the ethical issues involved in the disclosure of genetic information, where researchers and members of human research ethics committees (HRECs) were asked to consider several case studies from an ethical perspective. Workshop participants used a variety (...) of approaches to examine the complex ethical issues encountered, but did not consistently refer to the values and principles outlined in the National Statement on Ethical Conduct in Human Research (NHMRC 2007) or apply rational ethical approaches. Overall, the data suggested that both researchers and HREC members may benefit from further education and support regarding the application of ethical frameworks to the issues encountered in genetic research. Peer review is a widely accepted instrument for raising the quality of science. Peer review limits the enormous unstructured influx of information and the sheer amount of dubious data, which in its absence would plunge science into chaos. In particular, peer review offers the benefit of eliminating papers that suffer from poor craftsmanship or methodological shortcomings, especially in the experimental sciences. However, we believe that peer review is not always appropriate for the evaluation of controversial hypothetical science. We argue that (...) the process of peer review can be prone to bias towards ideas that affirm the prior convictions of reviewers and against innovation and radical new ideas. Innovative hypotheses are thus highly vulnerable to being “filtered out” or made to accord with conventional wisdom by the peer review process. Consequently, having introduced peer review, the Elsevier journal Medical Hypotheses may be unable to continue its tradition as a radical journal allowing discussion of improbable or unconventional ideas. Hence we conclude by asking the publisher to consider re-introducing the system of editorial review to Medical Hypotheses. (shrink)

Based on a time-lag model, this study tested for changes in young children’s home access and use of digital media in the 2012–2018 period as well as in their parents’ views on such media. What it found was that in only a few years the digital devices available to children have become more mobile, more accessible, and more numerous in these children’s bedrooms, especially in single-parent households. Also, on average children have strongly increased their daily media use—up to 102 minutes. (...) This is especially true for both younger children and children in middle to highly educated households. Our findings support the complementary media adoption theory more than the displacement theory. Over the years parents have tended more and more to use digital media as ‘babysitters’, while they also found it increasingly difficult to stop their children from using such media, which may have major implications for parenting support activities. J. H. van 't Hoff's 1874 Dutch pamphlet, in which he proposed the spatial arrangement of atoms in a molecule, is one of the most significant documents in the history of chemistry. This essay presents a new narrative of Van 't Hoff's early life and places the appearance of the pamphlet within the context of the 'second golden age' of Dutch science. We argue that the combination of the reformed educational system in The Netherlands, the emergence of graphical molecular modelling (...) within the theoretical and practical culture of chemistry during the 1860s and 1870s, as well as Van 't Hoff's own personal research trajectory, formed the background to his unprecedented attribution of spatial meaning to the traditional concept of atomic 'arrangement'. We also present a new English translation of the pamphlet, for we have found that the existing translation, published by G. M. Richardson in 1901, contains many errors, changes and omissions. The new version offers a more accurate rendition in English of Van 't Hoff's style and argument. (shrink)

Our ability to rapidly distinguish new from already stored information is important for behavior and decision making, but the underlying processes remain unclear. Here, we tested the hypothesis that contextual cues lead to a preselection of information and, therefore, faster recognition. Specifically, on the basis of previous modeling work, we hypothesized that recognition time depends on the amount of relevant content stored in long-term memory, i.e., set size, and we explored possible age-related changes of this relationship in older humans. In (...) our paradigm, subjects learned by heart four different word lists written in different colors. On the day of testing, a color cue indicated with a probability of 50% that a subsequent word might be from the corresponding list or from a list of new words. The old/new status of the word had to be distinguished via button press. As a main finding, we can show in a sample of n = 49 subjects, including 26 younger and 23 older humans, that response times increased linearly and logarithmically as a function of set size in both age groups. Conversely, corrected hit rates decreased as a function of set size with no statistically significant differences between both age groups. As such, our findings provide empirical evidence that contextual information can lead to a preselection of relevant information stored in long-term memory to promote efficient recognition, possibly by cyclical top-down and bottom-up processing. Children from various countries have been acknowledged and studied for their ability to philosophize, while, unfortunately, Filipino children have not received similar recognition. In this paper, I make a rather unpopular claim that Filipino children can and already are doing philosophy in their efforts to make sense of their existential conditions. “Doing philosophy” here refers to the act of being perplexed by one's own or other people's experiences and making an effort to comprehend them. Filipino children, are a vast and (...) diverse group, coming from various backgrounds, speaking different or even multiple languages, and representing rich cultures and unique circumstances. This diversity adds richness to the wide array of experiences from which existential questions can emerge. However, to truly comprehend the philosophical thinking of Filipino children, they need opportunities and guidance to explore their existential questions and solutions, regardless of how tentative or rudimentary these may seem to adults. Without such opportunities, it remains unclear to what extent their thinking engages with philosophical themes and issues. This not only offers Filipino adults a view of children’s inner worlds but also brings fresh perspectives to commonly held assumptions. (shrink)

The UK government has recently committed to adopting a new policy—dubbed ‘Martha’s Rule’—which has been characterised as providing patients the right to rapidly access a second clinical opinion in urgent or contested cases. Support for the rule emerged following the death of Martha Mills in 2021, after doctors failed to admit her to intensive care despite concerns raised by her parents. We argue that framing this issue in terms of patient rights is not productive, and should be avoided. Insofar as (...) the ultimate goal of Martha’s Rule is the provision of a clinical service that protects patient safety, an approach that focuses on the obligations of the health system—rather than the individual rights of patients—will better serve this goal. We outline an alternative approach that situates rapid clinical review as part of a suite of services aimed at enhancing and protecting patient care. This approach would make greater progress towards addressing the difficult systemic issues that Martha’s Rule does not, while also better engaging with the constraints of clinical practice. (shrink)

In 1972, the young philosopher Peter Singer published "Famine, Affluence and Morality," which rapidly became one of the most widely discussed essays in applied ethics. Through this article, Singer presents his view that we have the same moral obligations to those far away as we do to those close to us. He argued that choosing not to send life-saving money to starving people on the other side of the earth is the moral equivalent of neglecting to save drowning children (...) because we prefer not to muddy our shoes. If we can help, we must--and any excuse is hypocrisy. Singer's extreme stand on our moral obligations to others became a powerful call to arms and continues to challenge people's attitudes towards extreme poverty. Today, it remains a central touchstone for those who argue we should all help others more than we do. As Bill and Melinda Gates observe in their foreword, in the age of today's global philanthropy, Singer's essay is as relevant now as it ever was. At a time in which labour markets are becoming increasingly globalised and precarisation processes are altering young people’s working and living conditions, a whole network of public and private agencies are developing different entrepreneurship programmes as the main mechanism to deal with youth exclusion and unemployment. Grounded in two on-going research projects conducted in Europe and Australia, this article proposes a preliminary, thought-provoking engagement with the concept of global grammars of enterprise to examine how the truth regimes are framed and (...) articulated in these networks. We argue that this concept enables us to identify, examine and analyse the shifting, unstable, but always strategic power relations between the governmental discourses on entrepreneurship and the enterprising behaviour and dispositions of persons and groups, and the particular «declensions» and local «translations» of the ideas of entrepreneurship that organisations and young people perform within a process of globalised precarisation.Al albur de la globalización de los mercados laborales, al mismo tiempo que ciertos procesos de precarización están alterando profundamente las condiciones de vida y de trabajo de las personas jóvenes, se despliega toda una red de agentes públicos y privados que buscan fomentar el emprendimiento como principal mecanismo para enfrentar la exclusión y el paro juvenil. Can young children such as 3-year-olds represent the world objectively? Some prominent developmental psychologists—such as Perner and Tomasello—assume so. I argue that this view is susceptible to a prima facie powerful objection: To represent objectively, one must be able to represent not only features of the entities represented but also features of objectification itself, which 3-year-olds cannot do yet. Drawing on Burge's work on perceptual constancy, I provide a response to this objection and motivate a distinction between three different kinds (...) of objectivity. This distinction helps advance current research on both objectivity and teleological action explanations in young children. (shrink)

: Are incorrect judgments on false belief tasks better explained within the framework of a conceptual change theory or a bias theory? Conceptual change theory posits a change in the form of reasoning from 3 to 4 years old while bias theory posits that processing factors are responsible for errors among younger children. The results from three experiments showed that children who failed a test of false belief took as long to respond as those who passed, and both groups of (...) children took longer to respond to belief questions than to questions about prior states of reality. The myth of generations of disengaged youth has been shattered by increases in youth turnout in the 2004, 2006, and 2008 primaries. Young Americans are responsive to effective outreach efforts, and this collection addresses how to best provide opportunities for enhancing civic learning and forming lasting civic identities. The thirteen original essays are based on research in schools and in settings beyond the schoolyard where civic life is experienced. One focus is on programs for those schools in poor communities that (...) tend to overlook civic education. Another chapter reports on how two city governments--Hampton, Virginia, and San Francisco--have invited youth to participate on boards and in agencies. Peter Kivy and Stephen Davies developed an influential and convincing account of what features of music cause listeners to hear it as expressive of emotion. Their view (the resemblance theory) holds that music is expressive of some emotion when it resembles human expressive behaviour. Some features of music, they believe, are expressive of emotion because of conventional associations. In recent years, Kivy has rejected the resemblance theory without adopting an alternative. This essay argues that Kivy has been unwise to (...) abandon the resemblance theory. New and compelling psychological evidence supports the theory. The essay also argues that new psychological evidence indicates that convention makes a smaller contribution to musical expressiveness than Kivy and Davies believe. (shrink)

In 1931, the young Kurt Gödel published his First Incompleteness Theorem, which tells us that, for any sufficiently rich theory of arithmetic, there are some arithmetical truths the theory cannot prove. This remarkable result is among the most intriguing in logic. Gödel also outlined an equally significant Second Incompleteness Theorem. How are these Theorems established, and why do they matter? Peter Smith answers these questions by presenting an unusual variety of proofs for the First Theorem, showing how to prove (...) the Second Theorem, and exploring a family of related results. The formal explanations are interwoven with discussions of the wider significance of the two Theorems. This book will be accessible to philosophy students with a limited formal background. Peter Kivy argues that Handel was the first composer to be regarded as a genius and that only in the eighteenth century was the philosophical apparatus in place that would enable any composer to be conceived of as a musical genius. According to Kivy, a Longinian conception of genius transformed Handel into a genius. A Platonic conception of genius was used to classify Mozart as a genius. Then Kant adopted a Longinian conception of genius and this shaped the perception (...) of Beethoven. Kivy is wrong on all counts. Composers were thought to be geniuses long before Handel. The emergence of philosophical aesthetics in the eighteenth century did little to shape conceptions of musical genius. More specifically, Kivy misrepresents Kant's conception of genius and the role that it plays in the recognition of Beethoven as a musical genius. (shrink)
