The author, a cardiac surgeon specialising in heart transplantation, argues that excellence in medicine must always be pursued and confronts the problems of specialties and super-specialties with widely varying costs and benefit in which the pursuit of excellence results. He advocates that decisions on resource allocation should be the responsibility of the Department of Health and Social Security, acting on the advice of the public's elected representatives on the one hand and the medical profession on the other. The profession has (...) an ever-increasing responsibility to ensure that its advice is soundly based on rigorous self-audit, which should include considering the costs and benefit of its work. His commentator, Lady McCarthy, until recently chairman of an area health authority, responds by arguing that the tripartite system for resource allocation does not work. Doctors resent the imposition of any priorities other than their own. Their two-dimensional medical ethics, concerning doctors' obligations to individual patients, must be supplemented by concern for society as a whole. We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. Through discourse with international groups of physicians, we conducted a cross-cultural analysis of the types of ethical dilemmas physicians face. Qualitative analysis was used to categorise the dilemmas into seven themes, which we compared among the physicians by country of practice. These themes were a-theoretically-driven and grounded heavily within the text. We then subjected the dilemmas to an analysis of moral intensity, which represents an important theoretical perspective of ethical decision making. These constructs represent salient determinants of ethical behaviour and (...) our cross-cultural sample afforded us the opportunity to consider both the pragmatic aspects of culture, as they are perceived by physicians, as well as the theory-driven concept of moral intensity. By examining both culture and moral intensity, we hope to better elucidate the complexities of ethical decision-making determinants among physicians in their daily practice. The intent of ethics is to establish a set of standards that will provide a framework to modify, regulate, and possibly enhance moral behaviour. Eleven focus groups were conducted with physicians from six culturally distinct countries to explore their perception of formalized, written ethical guidelines (i.e., codes of ethics, credos, value and mission statements) that attempt to direct their ethical practice. Six themes emerged from the data: lack of awareness, no impact, marginal impact, other codes or value statements supersede, personal (...) codes or values dictate, and ethical guidelines are useful. Overall, codes were valued only when they were congruent with existing personal morality. The findings suggest the need to re-evaluate the purpose, content, and delivery of codes for them to improve their function in promoting ethical conduct. (shrink)

According to the priority view, or prioritarianism, it matters more to benefit people the worse off they are. But how exactly should the priority view be defined? This article argues for a highly general characterization which essentially involves risk, but makes no use of evaluative measurements or the expected utility axioms. A representation theorem is provided, and when further assumptions are added, common accounts of the priority view are recovered. A defense of the key idea behind the priority view, the (...) priority principle, is provided. But it is argued that the priority view fails on both ethical and conceptual grounds. (shrink)

Here's the article which was a 1964 Stanford AI Memo. After the original memo, several people offered different proofs of the theorem including Shmuel Winograd, Marvin Minsky and Dimitri Stefanyuk - none published, to my knowledge. Winograd claimed that his proof was non-creative, because it didn't use an extraneous idea like the colors of the squares. This set off a contest to see who could produce the most non-creative proof. Minsky's idea was to start with the diagonal next to an (...) excluded corner ssquare, note that 2 dominoes had to project from it to the diagonal with three squares, and from there 1 domino to the four square diagonal, etc. Coming from the other end also leaves only six of the eight squares in the long diagonal covered. Minsky's proof gets high points for non-creativity, because it is specific to the 8 by 8 board. (Using the colors it is easy to show that a Minsky style proof will work for any even sized board.). Elephant 2000 is a proposed programming language good for writing and verifying programs that interact with people (eg. transaction processing) or interact with programs belonging to other organizations (eg. electronic data interchange) 1. Communication inputs and outputs are in an I-O language whose sentences are meaningful speech acts identified in the language as questions, answers, offers, acceptances, declinations, requests, permissions and promises. 2. The correctness of programs is partly defined in terms of proper performance of the speech acts. Answers should (...) be truthful and responsive, and promises should be kept. Sentences of logic expressing these forms of correctness can be generated automatically from the form of the program. 3. Elephant aource programs may not need data structures, because they can refer directly to the past. Thus a program can say that an airline passenger has a reservation if he has made one and hasn't cancelled it. 4. Elephant programs themselves can be represented as sentences of logic. Their extensional properties follow from this representation without an intervening theory of programming or anything like Hoare axioms. 5. Elephant programs that interact non-trivially with the outside world can have both input-output specification, relating the programs inputs and outputs, and accomplishment specifications concerning what the program accomplishes in the world. These concepts are respectively generalizations of the philosophers' illocutionary and perlocutionary speech acts. 6. Programs that engage in commercial transactions assume obligations on behalf of their owners in exchange for obligations assumed by other entities. It may be part of the specification of an Elephant 2000 program that these obligations are exchanged as intended, and this too can be expressed by a logical sentence. 7. Human speech acts involve intelligence. Elephant 2000 is on the borderline of AI, but the article emphasizes the Elephant usages that do not require AI. (shrink)

From the AI point of view, consciousness must be regarded as a collection of interacting processes rather than the unitary object of much philosophical speculation. We ask what kinds of propositions and other entities need to be designed for consciousness to be useful to an animal or a machine. We thereby assert that human consciousness is useful to human functioning and not just and epiphenomenon. Zombies in the sense of Todd Moody's article are merely the victims of Moody's prejudices. To (...) behave like humans, zombies will need what Moody might call pseudo-consciousness, but useful pseudo-consciousness will share all the observable qualities of human consciousness including what the zombie will be able to report. Robots will require a pseudo-consciousness with many of the intellectual qualities of human consciousness but will function successfully with few if any human emotional conscious qualities if that is how we choose to build them. Then a fact about an unexpected obstacle---the traveller losing his ticket---is added without changing any of the previous facts, and the original plan can no longer be shown to work if it must take into account the new fact. However, an altered plan that includes buying a replacement ticket can now be shown to work. The formalism used (...) is a modification of one developed by Vladimir Lifschitz, and I have been informed that the modification isn't correct, and I should go back to Lifschitz's original formalism. T. A. Cavanaugh defends double-effect reasoning (DER), also known as the principle of double effect. DER plays a role in anti-consequentialist ethics (such as deontology), in hard cases in which one cannot realize a good without also causing a foreseen, but not intended, bad effect (for example, killing non-combatants when bombing a military target). This study is the first book-length account of the history and issues surrounding this controversial approach to hard cases. “Offsetting” habitat destruction has widespread appeal as an instrument for balancing economic growth with biodiversity conservation. Requiring proponents to pay the nontrivial costs of habitat loss encourages sensitive planning approaches. Offsetting, biobanking, and biodiverse carbon sequestration schemes will play an important role in conserving biodiversity under increasing human pressures. However, untenable assumptions in existing schemes are undermining their benefits. Policies that allow habitat destruction to be offset by the protection of existing habitat are guaranteed to result in further loss of (...) biodiversity. Similarly, schemes that allow trading the immediate loss of existing habitat for restoration projects that promise future habitat will, at best, result in time lags in the availability of habitat that increases extinction risks, or at worst, fail to achieve the offset at all. We detail concerns about existing approaches and describe how offsetting and trading policies can be improved to provide genuine benefits for biodiversity. Due to uncertainties about the way in which restored vegetation matures, we propose that the biodiversity bank should be a savings bank. Accrued biodiversity values should be demonstrated before they can be used to offset biodiversity losses. We provide recommendations about how this could be achieved in practice. (shrink)

How can finance be durably democratized? In the centers of financial power in both the United States and the United Kingdom, proposals now circulate to give workers and the public more say over how flows of credit are allocated. This article examines five democratization proposals: credit union franchises, public investment banks, sovereign wealth funds, inclusive ownership funds, and bank nationalization. It considers how these plans might activate worker and public engagement in decision making about finance by focusing on three modes (...) of public participation: representative democracy, direct democracy, and deliberative minipublics. It then considers the degree to which democratization plans might be resilient to de-democratization threats from business. It argues that of the five, bank nationalization goes furthest in guarding against de-democratization threats but is still pocked with pitfalls if it relies solely on representative democracy. It argues that two criteria appear necessary for democratically durable alternatives: the active direct participation of workers and citizens and the weakening of businesses’ capacity for democratic retrenchment. This article explores union attempts to control pension fund investment for the debate on financial restructuring in the United States. It puts popular control of finance into comparative and historical perspective and argues that laws and politics help explain why the flow of finance is corporate controlled. First, changes in the legal regime—the Taft-Hartley Act of 1947 and the Employee Retirement Income Security Act of 1974—put constraints on labor’s ability to influence investment decisions. This is evident when comparing single- and (...) multi-employer plans, where the laws had different consequences. Second, attempts to reform these laws failed. Had they been successful, Carter’s proposed economic revitalization plan in the run-up to his failed reelection in 1980 would have created new ways for unions to control and redirect retirement investment for social purposes. The reform failure is treated as a “suppressed historical alternative” through a comparison with a successful reform in Quebec, Canada, which gave unions broad controls over the Solidarity Fund in 1983. The findings suggest, somewhat counter-intuitively, that legal restrictions need to be loosened for democratic control of finance to be possible. For pension funds, more regulations led to more corporate control, not less. (shrink)
