In the conclusion to this multi-part article I first review the discussions carried out around the six essential questions in psychiatric diagnosis – the position taken by Allen Frances on each question, the commentaries on the respective question along with Frances’ responses to the commentaries, and my own view of the multiple discussions. In this review I emphasize that the core question is the first – what is the nature of psychiatric illness – and that in some manner all (...) further questions follow from the first. Following this review I attempt to move the discussion forward, addressing the first question from the perspectives of natural kind analysis and complexity analysis. This reflection leads toward a view of psychiatric disorders – and future nosologies – as far more complex and uncertain than we have imagined. (shrink)

The six questions involve: 1) the nature of a mental disorder; 2) the definition of mental disorder; 3) the issue of whether, in the current state of psychiatric science, DSM-5 should assume a cautious, conservative posture or an assertive, transformative posture; 4) the role (...) of pragmatic considerations in the construction of DSM-5; 5) the issue of utility of the DSM – whether DSM-III and IV have been designed more for clinicians or researchers, and how this conflict should be dealt with in the new manual; and 6) the possibility and advisability, given all the problems with DSM-III and IV, of designing a different diagnostic system. Part 1 of this article took up the first two questions. Part 2 took up the second two questions. Part 3 now deals with Questions 5 & 6. Question 5 confronts the issue of utility, whether the manual design of DSM-III and IV favors clinicians or researchers, and what that means for DSM-5. When they touch the elephant to find out what it's like, the six blind men cannot agree, for each feels a different part and arrives at a different conclusion.

Spirituality is a growing research theme, especially in relation to health issues. The term is often poorly defined and one’s understanding is often so broad that it becomes a mere frame word devoid of meaning. In this study, we asked 514 adult Danes about their understanding of the word ‘spirituality’. Factor analysis of the answers resulted in six different understandings of spirituality: positive dimensions in human life and well-being; New Age ideology; an integrated part of established religious life; a (...) vague striving, opposed to religion; selfishness; and ordinary inspiration in human activities. It is concluded that a common understanding of the term spirituality does not exist, at least in a modern secular setting. Suggestion for future research is that the term spirituality is not used without an indication, notions or keywords of what is meant by the term in a specific context. (shrink)

I call these six hitches "exploding" knots because they untie easily and completely with one tug of the ripcord. Unlike slipped knots that untie with a ripcord, these knots leave absolutely no tangle. Yet they give up nothing in strength or ease of tying. To learn to tie them, jump to the illustrations and skip the commentary. In each illustration, the line labelled "R" is the ripcord ("running part"). The line labelled "S" is to be attached to the load (...) ("standing part"). Tighten the knot with the "S" line. To decide which of these knots is best for which application, see the table at the end, which presupposes some of the text you might have skipped. We argue that emotion cannot only be conceived of as a short-lived and intrapersonal phenomenon. Rather, based on five theoretical arguments, we propose that the social sharing of an emotional experience forms an integral part of the emotional processes. A series of six studies investigated different aspects of this hypothesis. Study 1 showed that an overwhelming majority of people reported sharing their emotional experiences and that the memories of these experiences tended to come back spontaneously to their consciousness. No (...) difference was found among emotions. Using a different procedure, Studies 2 and 3 replicated these findings in two different populations. In addition, these studies provided indications that women share their experiences with a wider array of individuals than do men. The first three studies did not find any differences among emotions, but they did not include shame. It could be argued that people are less inclined to socially share shame experiences which are typically elicited by breaking social rules. Study 4 specifically investigated this hypothesis. No differences among shame and other emotions were found except for the delay of the first sharing of the experience. Study 5 constituted a first investigation of whether the social sharing of emotion is also a significant process in now Western cultures. A comparison between Dutch and Surinamese people failed to reveal any significant difference. Based on the findings of the first five studies, a correlational model was designed in Study 6. It tested the interrelationships among disruptiveness of the emotion, social sharing, mental rumination, and recovery from the emotion disruption. A coherent pattern of findings emerged, showing that social sharing can be conceived as a bi-dimensional concept defined by features of amount and delay of sharing. In addition, both social sharing and mental rumination varied according to the disruptiveness of the emotion. Surprisingly, however, recovery could not be related to social sharing, mental rumination, or to the time elapsed since the episode. (shrink)

The emerging viewpoint of embodied cognition holds that cognitive processes are deeply rooted in the body's interactions with the world. This position actually houses a number of distinct claims, some of which are more controversial than others. This paper distinguishes and evaluates the following six claims: (1) cognition is situated; (2) cognition is time-pressured; (3) we off-load cognitive work onto the environment; (4) the environment is part of the cognitive system; (5) cognition is for action; (6) off-line cognition is (...) body based. Of these, the first three and the fifth appear to be at least partially true, and their usefulness is best evaluated in terms of the range of their applicability. The fourth claim, I argue, is deeply problematic. The sixth claim has received the least attention in the literature on embodied cognition, but it may in fact be the best documented and most powerful of the six claims. (shrink)

The Madhyamaka Buddhist philosopher Nāgārjuna (2nd c. CE) is best known for his works on emptiness in which he advances a program for the relinquishing of all philosophical views (_dṛṣṭi_) in light of the impossibility of establishing the true existence of any kind of entity. At the same time, he is famous also for his theory of two truths, according to which conventional or transactional language is both a legitimate and a necessary factor on the path to the ultimate abandonment (...) of views. Although more than 100 works are ascribed to Nāgārjuna in the Tibetan canon, none is specifically dedicated to an exploration of conventional or transactional truth. So it is of great interest to find that two eighth-century Madhyamaka thinkers, Śāntarakṣita and Kamalaśīla, have in their writings preserved and commented on six verses from a treatise they ascribe to Nāgārjuna entitled _Establishing the Transactional_ (_Tha snyad grub pa = *Vyavahārasiddhi_). The present article provides a translation of the six verses from the Tibetan (the Sanskrit of all these writings is lost) along with a translation for the first time in English of Śāntarakṣita’s commentary and Kamalaśīla’s subcommentary upon the verses. The multi-part translation is framed by a consideration of the nature and role of philosophical views in relation to Nāgārjuna’s other writings, based on a reading of these few tantalizing verses from an apparently lost text, the commentaries of Śāntarakṣita and Kamalaśīla, and the theories of contemporary Madhyamaka scholars. Part of the normalization of assisted reproductive technologies is the premise that the children born from in vitro fertilization are no different from their counterparts conceived spontaneously. However, interest in peri-conception health and new epigenetic understandings of biological plasticity has led to some questioning the presumed irrelevance of conception in vitro, and when doing so, describing IVF children as “apparently healthy.” Taking “apparently” and “healthy” seriously, this article explores how modes of attention—ways of naming and framing embryo potentiality—shape understandings (...) of health and normality. I contend that understanding the politics of potentiality, and how they may emerge in a postgenomic age, requires an unpacking of various modes of attention and framing. Ethnographic findings from South Africa’s fertility clinics and emerging literature on epigenetic variation in IVF conception demonstrate how, under a genetic mode of attention, IVF clinics views “abnormality” as fated, unviable, and discardable. Exploring the possibility of answering the postgenomic questions to IVF reveals structural challenges to knowing long-term health implications. Incipient attempts within the fertility clinic at managing these questions shows various strategic techniques, such as leveraging epigenetics to marketable ends and shifts to individual responsibility. Although debates continue, studies of cognition in infancy suggest that knowledge begins to emerge early in life and constitutes part of humans' innate endowment. Early-developing knowledge appears to be both domain-specific and task-specific, it appears to capture fundamental constraints on ecologically important classes of entities in the child's environment, and it appears to remain central to the commonsense knowledge systems of adults.

Cultural narratives shape how we think about the world, including how we decide when the end of life begins. Hospice care has become an integral part of the end‐of‐life care in the United States, but as it has grown, its policies and practices have also imposed cultural narratives, like those associated with the “six‐month rule” that the majority of the end of life takes place in the final six months of life. This idea is embedded in policies for a (...) range of care practices and reimbursement processes, even though six months is not always a meaningful marker. In the case of people living with advanced dementia, six months is both too early in the trajectory to facilitate conversations and too late in the trajectory to ensure decision‐making capacity. This essay encourages scholars and policy‐makers to consider how cultural narratives may limit what they think is possible in care for people living with dementia. (shrink)

The history of aesthetics, like the histories of other sciences, may be treated in a two-fold manner: as the history of the men who created the field of study, or as the history of the questions that have been raised and resolved in the course of its pursuit. The earlier History of Aesthetics (3 volumes, 1960-68, English-language edition 1970-74) by the author of the present book was a history of men, of writers and artists who in centuries past have spoken (...) up concerning beauty and art, form and crea tivity. The present book returns to the same subject, but treats it in a different way: as the history of aesthetic questions, concepts, theories. The matter of the two books, the previous and the present, is in part the same; but only in part: for the earlier book ended with the 17th century, while the present one brings the subject up to our own times. And from the 18th century to the 20th much happened in aesthetics; it was only in that period that aesthetics achieved recognition as a separate science, received a name of its own, and produced theories that early scholars and artists had never dreamed of. This paper examines six cross-sector partnerships in South Africa and Zambia. These partnerships were part of a research study undertaken between 2003 and 2005 and were selected because of their potential to contribute to poverty reduction in their respective countries. This paper examines the context in which the partnerships were established, their governance and accountability mechanisms and the engagement and participation of the partners and the intended beneficiaries in the partnerships. We argue that a partnership approach which has proven (...) successful in one context can be used as a valuable learning resource. However, a partnership's work, which includes all aspects of the partnership and its activities, cannot necessarily be transferred directly to another partnership without a thorough and locally informed analysis of the context in which it is implemented. In addition, we suggest that it is difficult to assess whether the good intentions behind partnerships were translated into real benefits for target groups as effective monitoring and evaluation procedures were not in place in the partnerships studied. Similarly, the absence of regularised governance and accountability systems in partnerships made it difficult to support partner and beneficiary participation and engagement. We conclude that there is a need to move beyond a 'one-size-fits-all' approach to partnerships and that partnership replication should focus more strongly on the transfer of learning about partnership processes instead of simply copying partnership activities. Moreover, the development of stronger mechanisms for assessing and ensuring accountability towards both partners and intended beneficiaries is required if partnerships are to meet their intended objectives. Chaotic systems with hidden multiscroll attractors have received much attention in recent years. However, most parts of hidden multiscroll attractors previously reported were repeated by the same type of attractor, and the composite of different types of attractors appeared rarely. In this paper, a memristor-based chaotic system, which can generate composite attractors with one up to six scrolls, is proposed. These composite attractors have different forms, similar to the Chua’s double scroll and jerk double scroll. Through theoretical analysis, we find (...) that the new system has no fixed point; that is to say, all of the composite multiscroll attractors are hidden attractors. Additionally, some complicated dynamic behaviors including various hidden coexisting attractors, extreme multistability, and transient transition are explored. Moreover, hardware circuit using discrete components is implemented, and its experimental results supported the numerical simulations results. (shrink)

A taxonomy of part‐whole or meronymic relations is developed to explain the ordinary English‐speaker's use of the term “part of” and its cognates. The resulting classification yields six types of meronymic relations: 1. component‐integral object (pedal‐bike), 2. member‐collection (ship‐fleet), 3. portion‐mass (slice‐pie), 4. stuff‐object (steel‐car), 5. feature‐activity (paying‐shopping), and 6. place‐area (Everglades‐Florida). Meronymic relations ore further distinguished from other inclusion relations, such as spatial inclusion, and class inclusion, and from several other semantic relations: attribution, attachment, and ownership. This (...) taxonomy is then used to explain cases of apparent intransitivity in merological syllogisms, and standard form syllogisms whose premises express different inclusion relations. The data suggest that intransitivities arise due to equivocations between different types of semantic relations. These results are then explained by means of the relation element theory which accounts for the character and behavior of semantic relations in terms of more primitive relational elements. The inferential phenomena observed are then explained by means of a single principle of element matching. (shrink)

Biobanks correspond to different situations: research and technological development, medical diagnosis or therapeutic activities. Their status is not clearly defined. We aimed to investigate human biobanking in Europe, particularly in relation to organisational, economic and ethical issues in various national contexts. Data from a survey in six EU countries were collected as part of a European Research Project examining human and non-human biobanking. A total of 147 institutions concerned with biobanking of human samples and data were investigated by questionnaires (...) and interviews. Most institutions surveyed belong to the public or private non-profit-making sectors, which have a key role in biobanking. This activity is increasing in all countries because few samples are discarded and genetic research is proliferating. Collections vary in size, many being small and only a few very large. Their purpose is often research, or research and healthcare, mostly in the context of disease studies. A specific budget is very rarely allocated to biobanking and costs are not often evaluated. Samples are usually provided free of charge and gifts and exchanges are the common rule. Good practice guidelines are generally followed and quality controls are performed but quality procedures are not always clearly explained. Associated data are usually computerised. Biobankers generally favour centralisation of data rather than of samples. Legal and ethical harmonisation within Europe is considered likely to facilitate international collaboration. We propose a series of recommendations and suggestions arising from the EUROGENBANK project. (shrink)

Background:Moral courage is defined as courage to act according to one’s own ethical values and principles even at the risk of negative consequences for the individual. In a complex nursing practice, ethical considerations are integral. Moral courage is needed throughout nurses’ career.Aim:To analyse graduating nursing students’ moral courage and the factors associated with it in six European countries.Research design:A cross-sectional design, using a structured questionnaire, as part of a larger international ProCompNurse study. In the questionnaire, moral courage was assessed (...) with a single question (visual analogue scale 0–100), the questionnaire also covered several background variables.Participants and research context:The sample comprised graduating nursing students (n = 1796) from all participating countries. To get a comprehensive view about graduating nursing students’ moral courage, the views of nurse managers (n = 538) and patients (n = 1327) from the same units in which the graduating nursing students practised were also explored, with parallel questionnaires.Ethical considerations:Ethical approvals and research permissions were obtained according to national standards in every country and all participants gave their informed consent.Results:The mean of graduating nursing students’ self-assessed moral courage was 77.8 (standard deviation 17.0; on a 0–100 scale), with statistically significant differences between countries. Higher moral courage was associated with many factors, especially the level of professional competence. The managers assessed the graduating nursing students’ moral courage lower (66.5; standard deviation 18.4) and the patients slightly higher (80.6; standard deviation 19.4) than the graduating nursing students themselves.Discussion and conclusions:In all countries, the graduating nursing students’ moral courage was assessed as rather high, with differences between countries and populations. These differences and associations between moral courage and ethics education require further research. (shrink)

Critical infrastructure services determine where people can survive and what they can do with their survival. This fact conditions political possibilities at a fundamental level but remains underexplored in the literature. Those who wish to extend the boundaries of political action, or to win protections and the possibility of a new political community for themselves and others, should focus a substantial part of their energies and attention on developing alternative infrastructure systems for supporting human life. Without such systems, political (...) action – no matter how revolutionary or ingenious – will ultimately find itself constrained by its position within the zones of survivability established by existing forms of infrastructure and by the hierarchies and configurations of power linked with those forms of infrastructure. As a result, those who wish to change current political and economic conditions should think of the capacity to take care of everyone as a condition for such change rather than its result. (shrink)
