_Introducción_: la Limitación del Esfuerzo Terapéutico (LET) busca reorientar las medidas del acompañamiento terapéutico durante el final de la vida; actualmente existe legislación sobre este tema. _Objetivo: _caracterizar el conocimiento y la claridad conceptual y jurídica del personal de salud del Hospital de San José (HSJ) de Bogotá sobre la LET. _Métodos: _estudio descriptivo transversal usando encuesta de libre preparación. Se estableció claridad conceptual con resultado superior al 70% de respuestas correctas. Datos levantados con cuestionario de selección múltiple vía Formularios (...) Google, tabulados en Microsoft Excel ® y analizados con Stata ® vía estadística descriptiva. _Resultados:_ 36 % de encuestados tuvo claridad conceptual y jurídica sobre LET; el 82,3 % de encuestados diferenciaron LET de eutanasia y el 95,6 %, LET de cuidados paliativos. El 61 % de los encuestados no tuvo conflicto moral/jurídico con LET; el 60 % y el 70 % de los encuestados creen, respectivamente, que suspender el oxígeno y la ventilación mecánica no hacen parte de la LET. Finalmente, el 78,9 % expresó no haber recibido suficiente información sobre el tema. _Conclusiones_: el personal encuestado carece de claridad conceptual suficiente sobre la LET,lo que señala la necesidad de mayor educación al respecto. The present research describes a novel adaptive anomaly detection method to optimize the performance of nonlinear and time-varying systems. The proposal integrates a centroid-based approach with the real-time identification technique Recursive Least Squares. In order to find anomalies, the approach compares the present system dynamics with the average (centroid) of the dynamics found in earlier states for a given setpoint. The system labels the dynamics difference as an anomaly if it rises over a determinate threshold. To validate the proposal, two (...) different datasets obtained from a level control plant operation have been used, to which anomalies have been artificially added. The results shown have determined a satisfactory performance of the method, especially in those processes with low noise. (shrink)

Los métodos productivos de enseñanza preparan a los estudiantes para resolver problemas semejantes a los que se enfrentarán en el ejercicio laboral. Este trabajo tiene como objetivo fundamentar la pertinencia de la utilización del caso clínico como herramienta didáctica para la enseñanza en las ciencias básicas biomédicas, mediante un sistema de tareas para abordar los contenidos del tema Fisiología de la sangre de la asignatura Sangre y Sistema Inmune de la carrera de medicina. El caso clínico permite la vinculación de (...) las ciencias básicas biomédicas con las ciencias clínicas. Su utilización contribuye a la aproximación del estudiante de las ciencias básicas al estudio de estas durante su formación médica. El aprendizaje se logra a lo largo del proceso de resolución del mismo, se estimula la incorporación de conceptos necesarios para resolver el problema y se aprende un método para abordar situaciones en la práctica. Se elaboró un sistema de tareas y preguntas derivadas del problema profesional síndrome anémico. El problema docente se basó en un artículo médico que expone un caso de lesiones por empalamiento que produjo un gran sangramiento en un individuo. Se elaboraron preguntas problémicas relacionadas con la fisiología de la sangre y el tratamiento que recibió el paciente con eritropoyetina, factores de la coagulación y soluciones cristaloides en sustitución de la sangre. Se incluyó una pregunta que aborda un conflicto ético para sistematizar los conocimientos adquiridos en la asignatura Sociedad y Salud II. Productive teaching methods train students to solve problems similar to those they will face in their work life. The objective of the paper is to argue pertinence in the use of the clinical case as a didactic tool in biomedical basic sciences teaching by means of a task system that deals with the contents of the Blood Physiology subject in the Blood and Immune System subject matter of the medicine degree. Clinical cases enable the link between biomedical basic sciences and clinical sciences. Their use contributes to basic sciences students' approach to the study of the latter during their medical training. Learning is achieved through its solving process; the introduction of necessary problem-solving concepts is encouraged and a method to deal with in-practice situations is learned. A task and questions system derived from the anemic syndrome professional problem was made. The teaching problem was based on a medical article which presents a case of an individual who suffered impaling injuries which caused a great bleeding. Problem questions related to blood physiology and to the erythropoietin-based treatment the patient received, coagulation factors and crystalloid fluids used as blood substitutes, were prepared. It was included a question that deals with an ethical conflict in order to systematize the knowledge acquired in the Society and Health II subject matter. (shrink)

The black hole information problem provides important clues for trying to piece together a quantum theory of gravity. Discussions on this topic have generally assumed that in a consistent theory of gravity and quantum mechanics, quantum theory is unmodified. In this review, we discuss the black hole information problem in the context of generalisations of quantum theory. In this preliminary exploration, we examine black holes in the setting of generalised probabilistic theories, in which quantum theory and classical probability theory are (...) special cases. We are able to calculate the time it takes information to escape a black hole, assuming that information is preserved. In quantum mechanics, information should escape pure state black holes after half the Hawking photons have been emitted, but we find that this get’s modified in generalisations of quantum mechanics. Likewise the black-hole mirror result of Hayden and Preskill, that information from entangled black holes can escape quickly, also get’s modified. We find that although information exits the black hole as predicted by quantum theory, it is fairly generic that it fails to appear outside the black hole at this point—something impossible in quantum theory due to the no-hiding theorem. The information is neither inside the black hole, nor outside it, but is delocalised. (shrink)

The aim of this study is to offer a reflection on the theory of knowledge of Francisco Suárez and his nominalism in the light of Sextus Empiricus’ sceptical thought in his Outlines of Pyrrhonism. It is Sextus who allows us to compare the epistemology of Suárez with a dynamic system of knowledge. With this in view, in the first place, I examine Sextus’s scepticism and Suárez’s nominalist ontology. In the second place, I analyse the mental representation as conceived by Sextus (...) in analogy with the formal and objective concepts of Suárez, and I also discuss the concept of relation in both authors. Finally, I proceed to examine the concept of belief inasmuch as certain restrictions in the configuration of our judgements are manifest and recognized. (shrink)

Seed rematriation is a rising movement within greater efforts to improve seed and food sovereignty for Native American communities in the United States. As a feminized reframing of repatriation, rematriation seeks to heal Indigenous relationships with food, seeds, and landscapes. Since first contact, Native agricultural practices have been systematically targeted by colonization, resulting in the diminished biodiversity of cultural gardening systems. Of this vast wealth, many varieties exist today solely under the stewardship of non-Native institutions. Seed rematriation is therefore the (...) process and movement by which Native nations reclaim their cultural seed heritages. Once seeds are returned to the hands and soils of their home communities, Indigenous Nations can reestablish healthy, diverse, and sustainable seed and foodways for generations to come. This article explores the history of the term rematriation within Indigenous sovereignty scholarship as well as its evolving interpretations and applications. Considering how the seed rematriation movement has been shaped by several seed keepers in the Midwest reveals the cultural understandings and significances that underpin this work across many aspects of Indigenous lifeways. The resulting discussion from ethnographic material demonstrates why seed reclamation and seed sovereignty movements in the Midwest uphold Native nationhood through both resurgence and refusal. The Indigenous processes of recognizing and reclaiming seeds work beyond recovering agricultural knowledge to also mend severed kin relationships, rejuvenate cultural knowledge, and reestablish authority over Indigenous food systems. Reconstruction is a central notion in Dewey’s account of inquiry and in his metaphilosophical commitments. In his work, Dewey made a call for reconstruction of philosophy, in the reconstruction of central notions of the discipline, like knowledge, logic, truth, the good, reason, and experience. Inquiry itself is reconstructive, according to Dewey, involving the transformation of an indeterminate situation into one which is determinate and understood. Dewey’s philosophical views should therefore be of interest to those taking part in the recent turn (...) towards revisionary philosophical methodologies, like conceptual engineering, explication, and amelioration. In light of the recent developments in revisionary methodologies, I aim to explore Dewey’s conception of concepts in relation to the conceptual engineering literature, suggesting that it provides a useful conception of concepts for conceptual engineering, and to suggest a Deweyan model of philosophical inquiry where concepts are the objects of inquiry. In both cases, I hope to show a productive interaction between Dewey’s ideas and contemporary discussions of conceptual engineering. (shrink)

I argue that a standard formulation of hinge epistemology is host to epistemic relativism and show that two leading hinge approaches (Coliva’s acceptance account and Pritchard’s nondoxastic account) are vulnerable to a form of incommensurability that leads to relativism. Building on both accounts, I introduce a new, minimally epistemic conception of hinges that avoids epistemic relativism and rationally resolves hinge disagreements. According to my proposed account, putative cases of epistemic incommensurability are rationally resolvable: hinges are propositions that are the objects (...) of our belief-like attitudes and are rationally revisable in virtue of our overarching commitment to avoid systematic deception in our epistemic practices. An individual's health, genetic, or environmental-exposure data, placed in an online repository, creates a valuable shared resource that can accelerate biomedical research and even open opportunities for crowd-sourcing discoveries by members of the public. But these data become “immortalized” in ways that may create lasting risk as well as benefit. Once shared on the Internet, the data are difficult or impossible to redact, and identities may be revealed by a process called data linkage, in which online data sets are matched (...) to each other. Reidentification, the process of associating an individual's name with data that were considered deidentified, poses risks such as insurance or employment discrimination, social stigma, and breach of the promises often made in informed-consent documents. At the same time, re-ID poses risks to researchers and indeed to the future of science, should re-ID end up undermining the trust and participation of potential research participants. The ethical challenges of online data sharing are heightened as so-called big data becomes an increasingly important research tool and driver of new research structures. Big data is shifting research to include large numbers of researchers and institutions as well as large numbers of participants providing diverse types of data, so the participants’ consent relationship is no longer with a person or even a research institution. In addition, consent is further transformed because big data analysis often begins with descriptive inquiry and generation of a hypothesis, and the research questions cannot be clearly defined at the outset and may be unforeseeable over the long term. In this article, we consider how expanded data sharing poses new challenges, illustrated by genomics and the transition to new models of consent. We draw on the experiences of participants in an open data platform—the Personal Genome Project—to allow study participants to contribute their voices to inform ethical consent practices and protocol reviews for big-data research. Insofar as development implies economic growth, the term 'sustainable development' appears to some as a contradiction in terms. However, such conclusions still lack a thorough examination of the conceptual structure of the two terms between which there is a purported contradiction. In order to address this issue, the present paper scrutinises some of the assumptions which underwrite the ideologies of sustainability and of development. It is argued that there are key assumptions which both ideas have in common, and that sustainable (...) development is thus perfectly coherent on a conceptual level. It is alternatives which retain either term (such as 'sustainable degrowth') that are embroiled in paradox. The paper then examines two concepts for criticising the ideology of economic growth in other terms: dépense and conviviality. It is argued that the latter is preferable to the former for the purpose of developing post-sustainable critiques of growth. (shrink)

Research indicates the essentiality of dignity as a vital component for quality of life, reconfirming the emphasis on dignity preservation in the international code of nursing ethics. Applying Noblit and Hare’s meta-ethnography, the aim of the study was to develop a theory model by synthesizing 10 qualitative articles from various cultural contexts, exploring nurse and allied healthcare professional perception/practice concerning dignity-preserving dementia care. “Advocating the person’s autonomy and integrity,” which involves “having compassion for the person,” “confirming the person’s worthiness and (...) sense of self,” and “creating a humane and purposeful environment,” was identified as a primary foundation for dignity-preserving dementia care. “Balancing individual choices among persons no longer able to make sound decisions, against the duty of making choices on behalf of the person,” which involves “persuasion” and/or “mild restraint,” was considered a crucial aspect in certain situations. “Sheltering human worth—remembering those who forget” was identified as a comprehensive motive and core value within dignity-preserving dementia care. Xenophon presents his perplexing account of the conspiracy of Kinadon and its suppression in the midst of his portrait of Spartan imperial power at its zenith in the Hellenika. While the political relevance of this conspiracy has long been assumed by scholars, the labyrinthine structure of Book III obscures the centrality of the account in Xenophon’s examination of Spartan imperialism and Spartan justice. Attention to the details in the conspiracy account and its place within the narrative reveals the source of (...) corruption and decline inherent in the Spartan regime: an unexamined, and fatally deficient, view of justice as a virtue. Imperialism is not the cause but a symptom of the corruption rooted in fundamental defects of the Spartan constitution. Thus understood, the account of Kinadon’s conspiracy in the Hellenika constitutes a compelling critique of Spartan justice and helps to establish the overarching Socratic character of Xenophon’s political thought. In spite of the considerable literature nowadays existing on the issue of the moral exclusion of nonhuman animals, there is still work to be done concerning the characterization of the conceptual framework with which this question can be appraised. This paper intends to tackle this task. It starts by defining speciesism as the unjustified disadvantageous consideration or treatment of those who are not classified as belonging to a certain species. It then clarifies some common misunderstandings concerning what this means. Next, (...) it rejects the idea that there are different kinds of speciesism. Such an idea may result from confusion because there are (1) different ways in which speciesism can be defended; and (2) different speciesist positions, that is, different positions that assume speciesism among their premises. Depending on whether or not these views assume other criteria for moral consideration apart from speciesism, they can be combined or simple speciesist positions. But speciesism remains in all cases the same idea. Finally, the paper examines the concept of anthropocentrism, the disadvantageous treatment or consideration of those who are not members of the human species. This notion must be conceptually distinguished from speciesism and from misothery (aversion to nonhuman animals). Anthropocentrism is shown to be refuted because it either commits a petitio principia fallacy or it falls prey to two arguments: the argument from species overlap (widely but misleadingly known as “argument from marginal cases”) and the argument from relevance. This rebuttal identifies anthropocentrism as a speciesist view. (shrink)

Many people think we should refrain from intervening in nature as much as possible. One of the main reasons for thinking this way is that the existence of nature is a net positive. However, population dynamics teaches us that most sentient animals who come into existence in nature die shortly thereafter, mostly in painful ways. Those who survive often suffer greatly due to natural causes. If sentient beings matter, this gives us reasons to intervene to prevent such harms. This counterintuitive (...) conclusion can be opposed by arguing that we should not care about nonhuman animals; that other values, such as the existence of certain ecosystemic relations or of untouched wild areas, count for more than the interests of sentient beings; or that intervention in nature cannot succeed. There are, however, strong reasons to reject these claims and to support significant intervention in nature for the sake of animals, despite our deep-rooted intuitions to the contrary. (shrink)

Homer's Iliad, the epic poem of warfare, honour and suffering that stands at the beginning of the Western tradition, has traditionally been read as 'pre-political' and hence neglected as a substantial work of political thought. This essay argues that a close reading of the opening scenes in the Iliad reveals an exchange of public speeches which, if taken together, constitute a field of genuine political activity wherein rival claims about justice and right, as well as honour, are intensely disputed and (...) negotiated. Furthermore, this contest between 'the best of the Achaeans' is firmly situated within a framework of authority and legitimacy expressing in nascent form an essentially federal character. Thus the terms and ground of the quarrel between Agamemnon and Achilles - - and of the crisis occasioned by their political rivalry -- invite and sustain theoretical reflection upon the inherent weaknesses and strengths of federalism itself as a political form. The use of such concepts as randomness and probability is discussed and stochastic decisions occurring in the sacred texts are studied. In connection with probability, the measurements made in antiquity in order to comply with religious demands are also touched on. The cases where cause rather than randomness was recognized to be at work are indicated, and the statements contained in the Talmud and the thoughts of its later commentator, Maimonides, on hypotheses are linked with the appropriate opinions of Isaac (...) I dwell on the appearance, in the mid-19th century, of a formula describing random sampling without replacement under incomplete knowledge and discuss the statistical aspect of this formula and its applications. I also provide illustrations showing that the fairness of sampling without replacement was being questioned.
