BackgroundAlthough patient advocates have developed templates for standard consent forms, evaluating patient preferences for first in human (FIH) and window of opportunity (Window) trial consent forms is critical due to their unique risks. FIH trials are the initial use of a novel compound in study participants. In contrast, Window trials give an investigational agent over a fixed duration to treatment naïve patients in the time between diagnosis and standard of care (SOC) surgery. Our goal was to determine the patient-preferred presentation (...) of important information in consent forms for these trials.MethodsThe study consisted of two phases: (1) analyses of oncology FIH and Window consents; (2) interviews of trial participants. FIH consent forms were analyzed for the location(s) of information stating that the study drug has not been tested in humans (FIH information); Window consents were analyzed for the location(s) of information stating the trial may delay SOC surgery (delay information). Participants were asked about their preferred placement of the information in their own trial’s consent form. The location of information in the consent forms was compared to the participants’ suggestions for placement. Results34 [17 FIH; 17 Window] of 42(81%) cancer patients approached participated. 25 consents [20 FIH; 5 Window] were analyzed. 19/20 FIH consent forms included FIH information, and 4/5 Window consent forms included delay information. 19/20(95%) FIH consent forms contained FIH information in the risks section 12/17(71%) patients preferred the same. Fourteen (82%) patients wanted FIH information in the purpose, but only 5(25%) consents mentioned it there. 9/17(53%) Window patients preferred delay information to be located early in the consent, before the “Risks” section. 3/5(60%) consents did this.ConclusionsDesigning consents that reflect patient preferences more accurately is essential for ethical informed consent; however, a one-size fits all approach will not accurately capture patient preferences. We found that preferences differed for FIH and Window trial consents, though for both, patients preferred key risk information early in the consent. Next steps include determining if FIH and Window consent templates improve understanding. (shrink)

Recent reports of CRISPR/Cas9 genome editing in parasitic helminths open up new avenues for research on these dangerous pathogens. However, the complex morphology and life cycles inherent to these parasites present obstacles for the efficient application of CRISPR/Cas9‐targeted mutagenesis. This is especially true with the trematode flukes where only modest levels of gene mutation efficiency have been achieved. Current major challenges in the application of CRISPR/Cas9 for study of parasitic worms thus lie in enhancing gene mutation efficiency and overcoming issues (...) involved in host passage so that mutated parasites survive. Strategies developed for CRISPR/Cas9 studies on Caenorhabditis elegans, protozoa and mammalian cells, including novel delivery methods, the choice of selectable markers, and refining mutation precision represent novel tactics whereby these impediments can be overcome. Furthermore, employing CRISPR/Cas9‐mediated gene drive to interfere with vector transmission represents a novel approach for the control of parasitic worms that is worthy of further exploration. This article discusses the Tang Wei incident, which evolved across the first half of 2008, during the run-up to the Olympic Games in Beijing. Tang Wei is a Chinese actress whose breakthrough role in Ang Lee’s film Lust, Caution caused a sensation amongst Chinese audiences. The nudity and sex scenes in the film were explicit, and as such challenged accepted norms in film content. This aspect of the film, combined with the characterization of a national traitor as a heroine, caused (...) deep concern among some parties involved in film regulation and censorship. The argument presented here is that Tang Wei, who was singled out for criticism and upon whom travel and work restraints were placed in the aftermath of the film’s release, was made a scapegoat for the disgust experienced by a masculinist political class when faced with female dissent and sexuality. The phenomenon of perceptual persistence after the motion stops in shape-from-motion displays was used to study the influence of prior knowledge on the maintenance of a percept in awareness. In SFM displays an object composed of discontinuous line segments are embedded in a background of randomly oriented lines. The object only becomes perceptible when the line segments that compose the object and the lines that compose the background move in counterphase. Critically, once the movement of the line segments stops, the (...) percept of the object persists for a short period of time. In the present study, perceptual persistence for digits exceeded that reported for nonsense shapes composed of the same line segments. This result is taken as evidence that the processes involved in the persistence of SFM, and therefore sustained perception, are sensitive to top-down influences. (shrink)

The generation of evidence is integral to the work of public health and health service providers. Traditionally, ethics has been addressed differently in research projects, compared with other forms of evidence generation, such as quality improvement, program evaluation, and surveillance, with review of non-research activities falling outside the purview of the research ethics board. However, the boundaries between research and these other evaluative activities are not distinct. Efforts to delineate a boundary – whether on grounds of primary purpose, temporality, underlying (...) legal authority, departure from usual practice, or direct benefits to participants – have been unsatisfactory. In workplace health interventions, engaging management and union decision makers is considered important for the success of the project, yet little research has described the process of making this happen. A case study of a knowledge-transfer process is presented to describe the practices and processes adopted by a knowledge broker who engaged workplace parties in discussions on research on physical and psychosocial factors important for employee health. The process included one-on-one interactions between the knowledge broker and individuals to explain the (...) research, to build trust and credibility, and to explore the applicability of the research to their work (sense making). It also included facilitated group sessions, where the groups explored how the research could solve problems within the workplace (social construction of knowledge). The workplace context offered multiple opportunities that helped and hindered the flow of research. Nevertheless, this intense, sustained, knowledge-transfer intervention noted conceptual, structural, and political knowledge use. (shrink)

Those designing sustainable development implementation schemes will inevitably look to scientists to help them understand sustainable development problems. Scientists have already made important contributions to the understanding of many serious environmental problems, such as the causal relationship between certain synthetic chemicals and destruction of the ozone layer. If scientists had not identified the relationship between upper atmospheric ozone concentrations and releases of chloroflorocarbons, government decisionmakers would not have agreed to action limiting their production. However, although causes and effects of some (...) environmental problems are understood well, others are not, such as the timing and magnitude of climate change caused by greenhouse gases. If global environmental problems are serious, there is an obvious and urgent need to increase scientific understanding of which human actions cause environmental degradation and how nations can proceed with needed development programs without causing further environmental damage. This article explores the use of intoxicants by a community of Kulina Indians in western Brazil. I suggest that Kulina intoxication through alcohol, tobacco, and ayahuasca is best understood as a form of semiotic appropriation of the identity of cosmological “others,” including animal spirits, creator beings, other Indian groups, and Brazilians. I consider how embodying practices, such as song and physical movement, enhance the experience of being an “alter,” facilitated by the alterations in consciousness produced by intoxicants.

Comparative valuation of different policy interventions often requires interpersonal comparability of benefit. In the field of health economics, the metric commonly used for such comparison, quality adjusted life years (QALYs) gained, has been criticized for failing to respect the equality of all persons’ intrinsic worth, including particularly those with disabilities. A methodology is proposed that interprets ‘full quality of life’ as the best health prospect that is achievable for the particular individual within the relevant budget constraint. This calibration is challenging (...) both conceptually and operationally as it shifts dramatically when technology or budget developments alter what can be achieved for incapacitated individuals. The proposal nevertheless ensures that the maximal achievable satisfaction of one person’s preferences can carry no more intrinsic value than that of another. This approach, which can be applied to other domains of social valuation, thus prevents implicit discrimination against the elderly and those with irremediable incapacities. (shrink)

G.A. Cohen criticizes Rawls’s account of justice because his difference principle permits inequalities that reflect the relative scarcity of different skills and natural abilities. Instead of viewing the ‘basic structure’ as the primary subject of justice, Cohen argues that individual citizens should cultivate an egalitarian ethos, which would enable a just society to dispense with the use of incentive payments to induce individuals to use their talents in socially ideal ways. This study examines Cohen’s critique, including his rejection of ‘incentives,’ (...) and vindicates Rawls’s approach. Ultimately, Cohen’s argument fails to grapple with the moral pluralism that characterizes modern, democratic societies, whereas Rawls’s theory is constructed to accommodate such pluralism. (shrink)

An assertion of high conditional probability or, more briefly, an HCP assertion is a statement of the type: The conditional probability of B given A is close to one. The goal of this paper is to construct logics of HCP assertions whose conclusions are highly likely to be correct rather than certain to be correct. Such logics would allow useful conclusions to be drawn when the premises are not strong enough to allow conclusions to be reached with certainty. This goal (...) is achieved by taking Adams' (1966) logic, changing its intended application from conditionals to HCP assertions, and then weakening its criterion for entailment. According to the weakened entailment criterion, called the Criterion of Near Surety and which may be loosely interpreted as a Bayesian criterion, a conclusion is entailed if and only if nearly every model of the premises is a model of the conclusion. The resulting logic, called NSL, is nonmonotonic. Entailment in this logic, although not as strict as entailment in Adams' logic, is more strict than entailment in the propositional logic of material conditionals. Next, NSL was modified by requiring that each HCP assertion be scaled; this means that to each HCP assertion was associated a bound on the deviation from 1 of the conditional probability that is the subject of the assertion. Scaling of HCP assertions is useful for breaking entailment deadlocks. For example, it it is known that the conditional probabilities of C given A and of - C given B are both close to one but the bound on the former's deviation from 1 is much smaller than the latter's, then it may be concluded that in all likelihood the conditional probability of C given A ∧ B is close to one. The resulting logic, called NSL-S, is also nonmonotonic. Despite great differences in their definitions of entailment, entailment in NSL is equivalent to Lehmann and Magidor's rational closure and, disregarding minor differences concerning which premise sets are considered consistent, entailment in NSL-S is equivalent to entailment in Goldszmidt and Pearl's System-Z⁺. Bacchus, Grove, Halpern, and Koller proposed two methods of developing a predicate calculus based on the Criterion of Near Surety. In their randomstructures method, which assumed a prior distribution similar to that of NSL, it appears possible to define an entailment relation equivalent to that of NSL. In their random-worlds method, which assumed a prior distribution dramatically different from that of NSL, it is known that the entailment relation is different from that of NSL. (shrink)

In a regressive tax system, lower-income taxpayers pay larger percentages of their incomes in taxes compared to higher-income taxpayers. Although most policymakers and citizens view regressive taxation as generally unfair and unethical, the U.S. tax system taxes wage, salary, and self-employment income in a manner that deliberately subjects lower-income taxpayers to marginal tax rates that are greater than those imposed on higher-income taxpayers. As a result, some lower-income taxpayers pay a larger percentage of their income in taxes than higher-income taxpayers. (...) In this essay, we argue that this regressiveness in the taxation of salaried income is unfair and unethical. We then evaluate President Obama’s social security plan, which would retain most of the current tax system’s regressive structure. Finally, we offer two simple alternative proposals that are non-regressive, and thus more fair and ethical approaches to the taxation of salaried income. (shrink)

The contribution of food and diet to health is much disputed in the background culture in the US. Many commercial or ideological advocates make claims, sometimes with health as a primary goal, but often accompanied by commercial or ideological interests. These compete culturally with authoritative recommendations made by publicly funded groups. For public policy concerning diet and health to be legitimate, not only should it not be inconsistent with the scientific evidence, but also it should not be inconsistent with the (...) political environment. Healthy Eating Policy and Political Philosophy (HEPPP), by Barnhill and Bonotti (2022), addresses how policy in this complex area might be justified. In the present essay I highlight some important strengths of their work and also make some points about how it might be clarified or enlarged in scope. The great strength of HEPPP is that it emphasizes the role of a political philosophy in evaluating the legitimacy of public policy. They presume a well-off liberal democratic state, and specifically they draw on Rawls’ political liberalism, as developed in Political Liberalism (PL). One of several interlocking technical concepts developed in this book, public reason speaks to the boundaries of what should be acceptable in public deliberation about constitutional essentials and basic justice. HEPPP explores the application of an extension of public reason to the justification of policies related to “healthy eating.“ Public reason has two components, both of which pertain to a common understanding among citizens: (1) a common understanding of the principles and ideals that are the foundation of the political society, and (2) a common understanding of the standards that govern reason and evidence, including common sense. HEPPP argues that even though for diet and health the complexity of the scientific evidence goes far beyond anything like common sense for a typical citizen, the technical idea of “accessibility” of the scientific evidence justifies reliance on it within the concept of public reason. I question this accessibility argument about scientific evidence and public reason because the nature of the population-level epidemiological evidence about diet and health is that it often includes judgments that are based on arguable interpretations, framing incidence in terms of risk and prevention; thus the issue is more than one of potential accessibility of scientific evidence. Nevertheless, evaluation and consideration of the scientific evidence is of obvious practical importance in policy deliberation concerning policy related to what Rawls call “ordinary legislation.“ Even though public reason applied to healthy eating policy may not be as pertinent as it is for constitutional essentials and basic justice, and even though it is unlikely to result in a “correct outcome,“ invoking reasons based on political values and principles held in common has important value even in these deliberations about more ordinary questions. That value applies not to the specifics of the particular issue, but to the larger context for the particular policy being deliberated, as a potential contribution to an overlapping consensus that is stable for the right reasons. If this vision of political liberalism were achieved, then contentious debate about health eating policy and its scientific basis could occur within that context and be resolved within the basic political structure. Because public health and political liberalism have a common concern for a good at the population level, they have an underlying affinity. Healthy eating policies could be evaluated in two not-unrelated ways: based on their instrumental contribution to the good of the public body of citizens as population-level health, and according to their stabilizing contribution to an overlapping consensus among citizens. In this latter way a policy would contribute to a conditional integral common good a la Sulmasy, one that has the prospect of conditioning a political system, and by doing so create the conditions of the possibility of a constitutive integral common good that is constituted on the right reasons for allegiance to an overlapping consensus. To conclude with reference to HEPPP, public reason may play an important function with respect to the political common good, even as debates about the weight and meaning of scientific evidence from epidemiology play out in deliberation of ordinary legislation concerning healthy eating. In this book Dr. Donald Bloesch sharply diverges from much traditional thinking on the relationship between theology and philosophy and suggests an alternative that is solidly anchored in biblical faith. Instead of seeing this relationship in terms of synthesis or correlation or even simple subordination, he calls for the conversion and transformation of philosophical meanings in the light of the biblical revelation. Philosophy can be of considerable aid to theologians, but they must take care not to let philosophical concepts (...) determine the meaning of faith. Reason can be enlisted in the service of revelation, but it cannot establish the truth of revelation. Against the irrationalism of contemporary existentialist theology and the rationalism that has pervaded both scholastic orthodoxy (Catholic and Protestant) and liberal philosophical theology, Bloesch proposes an evangelical theology of revelation that seeks to employ reason in the task of understanding the faith. The author upholds not an autonomous reason but an obedient reason, and he shows that this ideal has support in the history of theology as well as in the Bible. (shrink)

Where do Paul Tillich's views of the relationship between religion and science fit in Ian Barbour's four classifications of conflict, independence, dialogue, and integration? At different levels of analysis, he fits in all of them. In concrete religions and sciences, some conflict is evident, but religion and science can be thought of as having parallel perspectives, languages, and objectives. Tillich's method of correlation itself is a form of dialogue. His theology of nature in “Life and the Spirit” (Part 4 of (...) his Systematic Theology) fits the integration type. His strong “Two Types of Philosophy of Religion” (in Theology of Culture) is a latent natural theology. His system of the sciences is a form of synthesis, a type of integration. (shrink)

His award-winning textbooks are often given credit for shaping the field, and his scientific papers are widely referenced and stand as milestones of development over a wide variety of topics. The present volume, the seventh in a series of his collected papers, is devoted to his work on the design of new algorithms. Nearly thirty of (...) Knuth’s classic papers are collected in this book and brought up to date with extensive revisions and notes on subsequent developments. Throughout the philosophical tradition there usually have been those philosophers who have either denied the existence of mental entities outright, or else have claimed that they were, in some sense, reducible to physical entities. And, on this score, the twentieth century has been no exception. In the last twenty or so years, the various denials of the existence of mental entities have taken three distinct forms. First, there is the sort of behaviorism advocated by Quine and Ryle. Second, there is (...) the materialism of J. J. C. Smart and D. M. Armstrong.
