BackgroundEverybody has the right to decide whether to receive specific medical treatment or not and to provide their free, prior and informed consent to do so. As dementia progresses, people with Alzheimer’s dementia (PwAD) can lose their capacity to provide informed consent to complex medical treatment. When the capacity to consent is lost, the autonomy of the affected person can only be guaranteed when an interpretable and valid advance directive exists. Advance directives are not yet common in Germany, and their (...) validity is often questionable. Once the dementia diagnosis has been made, it is assumed to be too late to write an advance directive. One approach used to support the completion of advance directives is ‘Respecting Choices’®—an internationally recognised, evidence-based model of Advance Care Planning (ACP), which, until now, has not been evaluated for the target group of PwAD. This study’s aims include (a) to investigate the proportion of valid advance directives in a memory clinic population of persons with suspected AD, (b) to determine the predictors of valid advance directives, and (c) to examine whether the offer of ACP can increase the proportion of valid advance directives in PwAD.MethodWe intend to recruit at leastN = 250 participants from two memory clinics in 50 consecutive weeks. Of these, the first 25 weeks constitute the baseline phase (no offer of ACP), the following 25 weeks constitute the intervention phase (offer of ACP). The existence and validity of an advance directive will be assessed twice (before and after the memory clinic appointment). Moreover, potential predictors of valid advance directives are assessed.DiscussionThe results of this study will enhance the development of consent procedures for advance directives of PwAD based on the ACP/Respecting Choices (R) approach. Food democracy is a concept with growing influence in food research. Food democracy deals with how actors may regain democratic control over the food system enabling its sustainable transformation. Following multi-level perspective framework's connotations, food democracy research has so far mainly focused on the niche level of the food system. An integrative approach that includes the perspectives of both the regime and the niche is still missing. This study addresses this research gap and proposes a new conceptual framework for food (...) democracy that includes actors from the niche and the regime level. Furthermore, we apply our conceptual framework to the urban food system of Vienna to explore the deeper meaning and practice of food democracy. Finally, we have conducted semi-structured interviews with actors at niche level and regime level within Vienna’s urban food system. Findings from this research broaden the perspective on food democracy and illustrate actors’ contributions at niche and regime level such as promoting organic food, re-localizing food provision, and procuring environmentally sustainable public food. Barriers to food democracy were also identified, e.g.: actors’ self-enhancement values, market-orientation, and capitalist alignment or lack of transparency. We conclude that actors at the niche and, to some extent, at the regime level may contribute to a process of on-going changes that fosters a transformation of established structures within the food system. L’auteure s’intéresse à ses efforts pour se différencier des violences subies mais aussi à la reproduction inconsciente des rapports d’emprise et d’abandon, manifeste dans le lien transférentiel, (...) et à sa propre paternité dans un parcours d’exil et d’errance en quête d’objets parentaux. Le travail thérapeutique se focalise sur la restauration de ses capacités de contenance et de transformation dans une clinique du lien intersubjectif et intrapsychique. (shrink)

In this paper, we introduce the concept of policy malaise, which refers to citizens’ dissatisfaction with the way political institutions and processes handle specific problems such as the refugee issue in Germany. Based on a representative online panel survey with two waves conducted in 2016 and 2017 (N = 836), we explore the occurrence of policy malaise among the German population and its relation to issue-specific media use, trust in news media, and issue fatigue. First, the results indicate that policy (...) malaise toward the refugee issue is widespread in Germany. Second, we found that media use relates differentially to policy malaise: While high exposure to public broadcasting was negatively associated with policy malaise, we found the opposite for private broadcasting. Third, policy malaise is higher for people who experience issue fatigue and lower for people who trust the news media. Finally, trust in media reinforces the negative and positive relations between media use and policy malaise. Implications concerning the associations between policy malaise and political alienation in its broader sense are discussed. (shrink)

A Teoria Crítica nasceu da reformulação da relação entre a filosofia e pesquisa social empírica, no Instituto de Pesquisa Social de Frankfurt, em 1931, quando Horkheimer defendeu a reorientação da agenda de pesquisa do Instituto, bem como a substituição de sua abordagem pelo que ficou conhecido como materialismo interdisciplinar. Ele conectou reflexão teórica, baseada no marxismo filosófico, e ciência social empírica, em resposta a outras interpretações do marxismo ortodoxo. Desde as primeiras pesquisas e elaborações teóricas do Instituto nessa época, Horkheimer (...) e Adorno manifestaram sua preocupação com o destino da humanidade – e a junção entre teoria social e pesquisa dizia respeito a tal preocupação. Horkheimer deixou isso claro no prefácio de The Authoritarian Personality, onde defendeu a articação entre a pesquisa e os interesses do Instituto e sua preocupação com a emancipação humana - desenvolvida a partir de uma análise cientificamente honesta do mundo material. Foi Adorno, contudo, no contato com a experiência científica norte-americana que, apesar de sua rejeição inicial da pesquisa empírica, reformulou seu conceito sobre ela, desenvolvendo uma teoria dialética da primazia do objeto, a partir da qual a possibilidade de uma teoria crítica como práxis se torna clara. O objetivo deste trabalho é mostrar o contexto da relação entre Teoria Crítica e pesquisa social empírica e como um compromisso com a práxis é parte integrante da mediação desta. (shrink)

This paper shares findings of a qualitative project exploring teacher candidates’ reflections on their own social studies inquiry teaching using video capture and annotation technology (VCAT). Teacher candidates’ annotated video and written reflections were collected and analyzed. Findings include important understandings of teacher candidates’ scholarship-aligned recognitions of strengths and areas for growth as well as areas where they had underdeveloped or novice negotiated conceptions of inquiry-centered instruction.

Human rights defenders (HRDs) fight for various human rights and address concerns related to corruption, employment, the environment, and other issues. They also challenge powerful state and private stakeholders and seek justice for human rights abuses. Therefore, HRDs are increasingly becoming targets of violent attacks and abuse with the aim of silencing them. This article begins by providing a brief definition of HRDs and then proceeds to outline the risks associated with their work in defending human rights. It also identifies (...) the perpetrators responsible for these violations. The article categorises the types of abuses against HRDs into two main categories, with a particular focus on the widespread tactic of using the legal system to target and silence defenders in Europe, which is also emerging globally. It introduces a taxonomy of various types of violations through the legal system. By categorising the types of violations against HRDs and establishing a taxonomy to aid in identifying these tactics, the article seeks to deepen understanding and awareness of the varied abuses experienced by HRDs, as well as their deviation from human rights standards, providing a valuable resource for academics, practitioners, and defenders. What holds together the various fields, which - considered together - are supposed to constitute the general intellectual discipline that people now call cognitive science? Some theorists identify the common subject matter as the mind, but scientists have not been able to agree on any single, satisfactory answer to the question of what the mind is. The Phaedo and the Symposium are dialogues in which Plato's conception of philosophy is not dissociated of orphic elements and mythical constructions. I propose that the themes of the pre-existence of the soul and the immortality of the soul in the Phaedo and myths told by Aristophanes and Diotima in the Symposium, examined together, provide rich material for understanding the nature and limits of knowledge and philosophy in Plato. I also suggest, before this interpretation, a reading of the "sudden vision (...) of beauty in itself," in Symposium 210e, which takes into account these limits. Knowledge is, from this reading, a dynamic search, much more than intuition of the forms. (shrink)

Humanitarian healthcare work presents a range of ethical challenges for expatriate healthcare professionals, including tragic choices requiring the selection of a least-worst option. In this paper we examine a particular set of tragic choices related to the prioritization of care and allocation of scarce resources between individuals in situations of widespread and urgent health needs. Drawing on qualitative interviews with clinicians, we examine the nature of these choices. We offer recommendations to clinical teams and aid organizations for preparing and supporting (...) frontline clinicians in their efforts to determine the least-worst option, and in their responsibility for making such choices. Scholars have explored the influence of the field of cybernetics on scientific thought and disciplines. However, from the inception of the field, ‘‘cyberneticians’’ had explicitly envisioned applications reaching beyond the purview of scientific disciplines; cybernetics was remarkable for its portability and potential application in a wide variety of contexts. This article explores connections between cybernetics and experimental music from 1950-1980, which was a period of experimentation with electronic techniques in recording, composition, and sound production and manipulation. Examples include musicians, engineers, (...) instrument builders, composers, and scientists in collaboration with musicians who invoked cybernetic themes in their work. These uses of cybernetics were more diverse than accounts of cybernetics within the sciences suggest, presenting a major difficulty in addressing cybernetics as a homogeneous or monolithic discourse. In particular, cybernetic discourse in music often exhibited themes of openness and indeterminacy, rather than the ‘‘command and control’’ of the ‘‘closed world.’’. (shrink)

Seed rematriation is a rising movement within greater efforts to improve seed and food sovereignty for Native American communities in the United States. As a feminized reframing of repatriation, rematriation seeks to heal Indigenous relationships with food, seeds, and landscapes. Since first contact, Native agricultural practices have been systematically targeted by colonization, resulting in the diminished biodiversity of cultural gardening systems. Of this vast wealth, many varieties exist today solely under the stewardship of non-Native institutions. Seed rematriation is therefore the (...) process and movement by which Native nations reclaim their cultural seed heritages. Once seeds are returned to the hands and soils of their home communities, Indigenous Nations can reestablish healthy, diverse, and sustainable seed and foodways for generations to come. This article explores the history of the term rematriation within Indigenous sovereignty scholarship as well as its evolving interpretations and applications. Considering how the seed rematriation movement has been shaped by several seed keepers in the Midwest reveals the cultural understandings and significances that underpin this work across many aspects of Indigenous lifeways. The resulting discussion from ethnographic material demonstrates why seed reclamation and seed sovereignty movements in the Midwest uphold Native nationhood through both resurgence and refusal. The Indigenous processes of recognizing and reclaiming seeds work beyond recovering agricultural knowledge to also mend severed kin relationships, rejuvenate cultural knowledge, and reestablish authority over Indigenous food systems. This paper provides an overview of the development of Jewish studies in Germany since reunification. After a brief historical review of the subject in the nineteenth century with the development of modern Reform Judaism and the science of Judaism created by Jewish religious and secular scholars, it focuses on the development of the past thirty years, in which not only the Jewish community but also Jewish studies have increased in importance. The growth of the Jewish community was largely due to (...) immigration from the Soviet Union, but also partly to young Israelis who moved to Berlin. In line with these different backgrounds, a new interest in diaspora research emerged. The paper also deals with the difference between German Jewish studies and those of most other countries, where Jewish studies are mainly designed by Jewish scholars. (shrink)

A well-worn trope used by phenomenologists is that things that remain invisible or unnoticed in the course of our everyday being in the world reveal themselves in instances of breakdown. This paper borrows this trope to explicate one instance of breakdown, that of traumatic spinal cord injury (TSCI). We use the phenomenology of Heidegger, especially his formulation of ecstatic temporality presented in Being and Time, to illuminate the temporal issues surrounding this radical rupture in Dasein’s being in the world through (...) data collected from field observations of inpatient rehabilitation, interviews with persons with TSCI, and with their rehabilitation providers. Specifically, we explore the breakdown in temporality (the rupture on thrown projection) that occurs in persons who experience TSCI across three interconnected existential dimensions – understanding, attunement (mood), and falling. We conclude by discussing the value this approach to human temporality might have for both social scientists interested in human temporality and to practitioners interested in research related to the rehabilitation process. Durch Migrationsprozesse und zunehmenden Pluralismus ist in Deutschland das Thema „kulturelle Diversität“ in der Medizin aktueller denn je. In der medizinischen Fachliteratur und im gesellschaftlichen Diskurs wird vermehrt von interkulturellen Konflikten im Kontakt zwischen Arzt und Patient berichtet, was die Frage nach der Rolle von Interkultureller Kompetenz für die klinische Praxis aufwirft. Zunächst widmet sich der Beitrag kritisch den verschiedenen Auffassungen des Begriffs „Kultur“ im medizinischen Kontext, um anschließend eine Methode der interkulturellen Philosophie als eine Möglichkeit für eine kultursensitive Ethik (...) zu skizzieren. Anschließend wird aufgezeigt, welche Schwierigkeiten mit dem Begriff der Interkulturellen Kompetenz verbunden sind und weshalb die Autoren diesen trotzdem als unerlässlich für die Wahrnehmung potentieller Konfliktfelder als auch für den Umgang mit diesen erachten. Schließlich werden derzeitige Entwicklungen in der medizinischen Ausbildung sowie konkrete Beispiele für eine kultursensible Ethikberatung illustriert. Es zeigt sich, dass international zahlreiche Ausgestaltungen Interkultureller Kompetenz in Form von Leitfäden und konkreten Schulungsmodulen existieren, aber auch, dass es in Deutschland an konkreten Umsetzungen in der Breite mangelt und die Zusammenarbeit wissenschaftlicher Disziplinen sowie der betroffenen Institutionen derzeit noch ausbaufähig ist. Is the being in an irreversible persistent vegetative state as the result of a horrible accident numerically identical to the human person, Lindsay, who existed before the accident? Many proponents of Thomistic metaphysics have argued that Aquinas’s answer to this question must be “yes.” In particular, it seems that Aquinas’s commitment to both Aristotelian hylomorphism and the unity of substantial form (viz., that each body/soul composite possesses one and only one substantial form) entails the position that the human person remains (...) alive as long as biological life persists. I argue, however, that although Aquinas does possess a deeply integrated account of human nature and is indeed committed to the claim that the person, Lindsay, exists as long as Lindsay’s body lives, there is good reason to suppose that he also holds that the body in the PVS is not Lindsay’s body in anything more than an equivocal sense. First, some theorists identify the common subject matter as the mind, but scientists and philosophers have not been able to agree on any single, satisfactory answer to the question of what the mind is. Second, those who speculate about the general characteristics that belong to cognitive science (...) tend to assume that all the particular fields falling under the rubric--psychology, linguistics, biology, and son on--are of roughly equal value in their ability to shed light on the nature of mind. This book argues that all the cognitive science disciplines are not equally able to provide answers to ontological questions about the mind, but rather that only neurophysiology and cultural psychology are suited to answer these questions. However, since the cultural account of mind has long been ignored in favor of the neurophysiological account, Erneling and Johnson bring together contributions that focus especially on different versions of the cultural account of the mind. The scholarship of Ulf Hannerz is characterized by its extraordinary breadth and visionary nature. He has contributed to the understanding of urban life and transnational networks, and the role of media, paradoxes of identity and new forms of community, suggesting to see culture in terms of flows rather than as bounded entities. Contributions honor Hannerz' legacy by addressing theoretical, epistemological, ethical and methodological challenges facing anthropological inquiry on topics from cultural diversity policies in Europe to transnational networks in Yemen, and (...) from pottery and literature to multinational corporations. Theoretical models for physician-patient communication in clinical practice are described in literature, but none of them seems adequate for solving the communication problem in clinical practice that emerges in case of factitious disorder. Theoretical models generally imply open communication and respect for the autonomy of the patient. In factitious disorder, the physician is confronted by lies and (self)destructive behaviour of the patient, who in one way or another tries to involve the physician in this behaviour. It is no longer controversial (...) that the physician should communicate his consideration of a factitious disorder without insistence that the patient accepts this diagnosis. However, the balance between patient autonomy and open communication on the one hand, and the preservation of the patient's health, physician integrity and of a constructive physician-patient relationship on the other is easily disrupted. In this article, an epistemological model is described to facilitate a positive outcome of confrontation in treatment of factitious disorder. Analysing the problem in terms of systems theory will help the physician to assess what information is appropriate to use in which phase of the patient's treatment, while preserving the physician-patient relationship. An important devotional genre in the thirteenth and fourteenth centuries, meditations invited their readers to place themselves at the scene of various moments in Christ’s life and encouraged them to have particular emotional responses—joy, sorrow, compassion, and the like—to those imaginative experiences. In its emphasis on feeling, meditation was seen as an activity particularly suited for women and their closer ties with the body. Meditation was also seen as an activity distinct from contemplation, which was portrayed as a “higher,” more (...) intellective pursuit. Yet meditation was intended to increase love to Christ, and love was widely considered to increase knowledge—knowledge of the same sort that contemplation also claimed to yield. Over time, then the widespread popularity of this spiritual exercise opens up space for women’s claims to knowledge to be heard because of (rather than despite) their association with the body. This paper analyses various approaches to the concept of a ‘safety culture’ in terms of their epistemological assumptions regarding the nature of learning. As a result of this analysis, the study proposes a relational-interpretive framework for the promotion of safety in health care, which is based on relational theories and the philosophy of conceptual pragmatism as this can be used to integrate the various strands of current safety research. In particular, the approach based on a relational-interpretive perspective can bridge the (...) apparent dualist gap that exists between the rational objectivist perspective and the relativist perspective on the role of learning in developing a safety culture. According to the relational-interpretive perspective of safety management that is proposed here, organizational members need to give continuous attention to the accepted organizational norms and values, which shape the safety culture. A case study from a health care safety project in Sweden is utilized to illustrate the ideas advanced in this paper. (shrink)
