Background Ethical decision making in intensive care is a demanding task. The need to proceed to ethical decision is considered to be a stress factor that may lead to burnout. The aim of this study is to explore the ethical problems that may increase burnout levels among physicians and nurses working in Portuguese intensive care units . A quantitative, multicentre, correlational study was conducted among 300 professionals.Results The most crucial ethical decisions made by professionals working in ICU were related to (...) communication, withholding or withdrawing treatments and terminal sedation. A positive relation was found between ethical decision making and burnout in nurses, namely, between burnout and the need to withdraw treatments , to withhold treatments and to proceed to terminal sedation . This did not apply to physicians. Emotional exhaustion was the burnout subdimension most affected by the ethical decision. The nurses' lack of involvement in ethical decision making was identified as a risk factor. Nevertheless, in comparison with nurses , it was the physicians who more keenly felt the need to proceed to ethical decisions in ICU.Conclusions Ethical problems were reported at different levels by physicians and nurses. The type of ethical decisions made by nurses working in Portuguese ICUs had an impact on burnout levels. This did not apply to physicians. This study highlights the need for education in the field of ethics in ICUs and the need to foster inter-disciplinary discussion so as to encourage ethical team deliberation in order to prevent burnout. Neoliberal ideology and exclusionary policies based on racialized identities characterize the current contexts in North America and Western Europe. Nursing knowledge cannot be abstracted from social, political and historical contexts; the task of examining the influence of race and racial ideologies on disciplinary knowledge and inquiry therefore remains an important task. Contemporary analyses of the role and responsibility of the discipline in addressing race‐based health and social inequities as a focus of nursing inquiry remain underdeveloped. In this article, we examine (...) nursing's engagement with ideas about race and racism and explore the ways in which nursing knowledge and inquiry have been influenced by race‐based ideological discourses. Drawing on Henry and Tator's framework of democratic racism, we consider how strategic discursive responses—the discourses of individualism, multiculturalism, colour‐blindness, political correctness and denial—have been deployed within nursing knowledge and inquiry to reinforce the belief in an essentially fair and just society while avoiding the need to acknowledge the persistence of racist discourses and ideologies. Greater theoretical, conceptual and methodological clarity regarding race, racialization and related concepts in nursing inquiry is needed to address health and social inequities. (shrink)

Against the standard reading of Isaiah Berlin's thought that drives a wedge between his early and subsequent work, this article suggests that his late normative anti-reductionism has roots in the early writings on meaning, semantics and truth. Berlin's anti-reductionist objection to logical positivists in the realm of semantics evince a sensitivity to reductionism, a recognition of the irreducibility of propositional meaning, a plea for the embededness of language in a temporal continuum, an anti-dualist call, and a celebration of the plural (...) and open-ended character of propositions, all of which keep broad affinity with his mature value pluralism in ethics and politics. The unity of concerns in Berlin's work brings about two implications. First, it sheds light on the fact that Berlin's ethical and political pluralism rests on views about semantics and ontology as well. Second, it offers a basis on which to deflect some current moral realist readings of his value pluralism, offering instead a history-laden understanding of his pluralism that provides more compelling grounds to understand the relationship between the latter and liberalism in Berlin's work. This paper explores the concept of ethos as a facet of the government’s rapidly growing initiative of the ‘specialist school’. Schools accepted on to the scheme are expected to create a new identity, or ethos: but what exactly is meant by that rather nebulous term? And, in reality, is something as all‐consuming as a school ethos, or culture, something that a school can readily conjure up? This discussion, one facet of the author’s case study of a Specialist Sports College, explores (...) the concept of school cultures, but also gives consideration to areas such as conflicting and ‘toxic’ cultures, and to the possibility that changing a school’s culture can have negative as well as positive effects. In this study I focus on the complementation patterns of commissive shell nouns in Ghanaian English (GhE). Commissive shell nouns are a type of illocutionary shell noun, i.e. a noun that encapsulates a content that is usually expressed in a complement or even separate clause or sentence thereby ascribing it an illocutionary force. I use the usage-based approach to the study of language and investigate the behavioral profile of these nouns in GhE. I apply descriptive statistics to data that have (...) been collected from the Corpus of Global Web-based English (GloWbE). The study provides evidence for the characterization of GhE usage norms, and thus contributes to the scholarly knowledge on this variety of English. It also sheds light on the contribution that the meaning-related and ultimately cognitive perspective can offer in describing the complementation patterns of illocutionary nouns in Postcolonial Englishes (PCEs). BackgroundIn the last 2 weeks of January 2021, Portugal was the worst country in the world in incidence of infections and deaths due to COVID-19. As a result, the pressure on the healthcare system increased exponentially, exceeding its capacities and leaving hospitals in near collapse. This scenario caused multiple constraints, particularly for hospital medical staff. Previous studies conducted at different moments during the pandemic reported that COVID-19 has had significant negative impacts on healthcare workers’ psychological health, including stress, anxiety, depression, (...) burnout, post-traumatic stress symptoms, and sleep disturbances. However, there are many uncertainties regarding the professional quality of life of hospital nurses and physicians. To address gaps in previous research on secondary traumatic stress, we focused on healthcare workers working in hospitals affected by a major traumatic event: the third wave of COVID-19.ObjectivesThe aim of the present study was to identify the contribution of personal and work-related contextual variables on professional quality of life of healthcare workers.MethodsCross-sectional study with a web-based questionnaire given to physicians and nurses working in a hospital setting. A total of 853 healthcare professionals participated in the survey assessing professional quality of life compassion satisfaction, secondary traumatic stress, and burnout. Factors of professional quality of life were assessed using regression analysis.ResultsMost of the participants showed moderate or high levels of compassion satisfaction, whereas the majority of them experienced moderate levels of burnout and secondary traumatic stress. The analyzed variables demonstrated no differences between professionals who were directly or not involved in the care of COVID-19 patients. Parental status was found to be a significant factor in compassion satisfaction. Female gender was significantly associated with more susceptibility to secondary traumatization. Factors that may potentially contribute to burnout include years of professional experience and the number of work hours per week.ConclusionThe COVID-19 pandemic has created a new challenge for the healthcare system. Burnout and secondary traumatic stress can lead to medical errors and impact standards of patient care, particularly compromising compassionate care. It is therefore recommended that hospitals develop psychoeducational initiatives to support professionals in dealing with barriers to compassion. (shrink)

Starting from the birthplace of childhood, this text elucidates, using Giorgio Agamben, Heraclitus, and Paulo Freire, the coexistence of childhood and time. These authors contribute to the idea that childhood, be it chronological or not, is an inspiration without which it is impossible for us to be inventive in relation to history, philosophy, and education. It thus bases itself upon the hypothesis that childhood and time are conceptually indefinable and dilutive of the idea of stability, which escapes the understanding of (...) continuity, since it is not an absolute event. Thus, this text in based on the reading of childhood as a fundamental condition of the existence of human history, which makes possible the movement from language to thought. To think about the understanding of childhood that is at issue in this text, we consider a certain experience of time different from the traditional one, the result of an understanding conceived as chronology and completion, which is present in various readings of childhood. Upon raising these considerations, several questions arise: is there something like a childhood of thought? Can childhood offer a way of not giving up to a new concept of a childlike experience of time? (shrink)

This collection of personal essays presents significant events in the lives of those in the pharmacy community whose experiences have been shaped by their race, ethnicity, gender or gender presentation, sexual orientation, ability, language, mental health, or other factors. The perspectives from the narratives highlight the importance of diversity, equity, and inclusion in the healthcare sector. The authors of the narratives also reflect (...) on their experience to consider what they wish would have happened in the experience, to offer advice for others that may be faced with similar situations. Access to these stories will deepen the collective understanding among pharmacy colleagues, patients, and society of the experiences that individuals from historically underrepresented groups within pharmacy have experienced. We design an agent based Monte Carlo model of antibiotics research and development to explore the effects of the policy intervention known as Market Entry Reward on the likelihood that an antibiotic entering pre-clinical development reaches the market. By means of sensitivity analysis we explore the interaction between the MER and four key parameters: projected net revenues, R&D costs, venture capitalists discount rates, and large pharmaceutical organizations' financial thresholds. We show that improving revenues may be more efficient than reducing costs, (...) and thus confirm that this pull-based policy intervention effectively stimulates antibiotics R&D. (shrink)

The reform of the health care system will include a mandate: Individuals are required to purchase health insurance provided that affordable options are available. But what is affordable health insurance? Three accounts of affordability of health coverage have been advanced. The first two accounts are empirical. The third account is needs-based. All three accounts are inadequate. I propose a fourth, the reasonable tradeoff account, according to which individuals should only be required to make reasonable tradeoffs in order to pay for (...) their health coverage. That is, health insurance is affordable if in order to pay for it one does not to have to sacrifice other benefit(s) that are comparable in importance to the benefits of health coverage. (shrink)

In ‘Parental Virtues: A New Way of Thinking about the Morality of Reproductive Actions’ Rosalind McDougall proposes a virtue-based framework to assess the morality of child selection. Applying the virtue-based account to the selection of children with impairments does not lead, according to McDougall, to an unequivocal answer to the morality of selecting impaired children. In ‘Impairment, Flourishing, and the Moral Nature of Parenthood,’ she also applies the virtue-based account to the discussion of child selection, and claims that couples with (...) an impairment are morally justified in selecting a child with the same impairment. This claim, she maintains, reveals that the flourishing of a child should be understood as requiring environment-specific characteristics. I argue that McDougall's argument begs the question. More importantly, it does not do justice to virtue ethics. I also question to what extent a virtue ethics framework can be successfully applied to discussions about the moral permissibility of reproductive actions. (shrink)

ABSTRACT As we strive for a more polyvocal history of science, historians have placed increasing emphasis on local case studies as a way to globalize the field. This tension between the local and the global extends to the practice as well as the content of the history of science, as the field has begun to pay more attention not just to local case studies, but also to local cultures of historiography. Many historians of science want multiple historiographical voices that take (...) seriously the concerns and literatures in different linguistic and national contexts. As more journals urge translation into what are perceived as the dominant languages in “global” humanities discourses and scholars are encouraged to facilitate the Englishing of their historical research, many questions remain: In what ways does it make sense to nationalize or culturally locate individual cultures of the history of science? Is translation a democratic and positive force in the discipline, and/or does it kill local difference? Will it continue to be meaningful to talk about the history of science? A collective dialogue within the field depends on finding a way to take local diversity in historiography and epistemology seriously while translating that local difference into a meaningful common conversation. This essay considers the challenges of embracing multiple ways of knowing that might fall under the purview of the history of science, while wondering what that means for the coherence of the field as we move forward into the next century of work in Isis. Metaethical constructivism is the view that insofar as there are normative truths, they are not fixed by normative facts that are independent of what rational agents would agree to under some specified conditions of choice. The appeal of this view lies in the promise to explain how normative truths are objective and independent of our actual judgments, while also binding and authoritative for us. -/- Constructivism comes in several varieties, some of which claim a place within metaethics while others claim (...) no place within it at all. In fact, constructivism is sometimes defended as a normative theory about the justification of moral principles. Normative constructivism is the view that the moral principles we ought to accept are the ones that agents would agree to or endorse were they to engage in a hypothetical or idealized process of rational deliberation. -/- Metaethical constructivist theories aim to account for the nature of normative truths and practical reasons. They bear a problematic relation to traditional classifications of metaethical theories. In particular, there are disagreements about how to situate constructivism in the realism/antirealism debate. These disagreements are rooted in further differences about the definition of metaethics, the relation between normative and metaethical claims, and the purported methods pertinent and specific to metaethical inquiry. The question of how to classify metaethical constructivism will be addressed in what follows by focusing on the distinctive questions that constructivist theories have been designed to answer. Section 1 explains the origin and motivation of constructivism. Sections 2–4 examine the main varieties of metaethical constructivism. Section 5 illustrates related constructivist views, some of which are not proposed as metaethical accounts of all normative truths, but only of moral truths. I investigate the reciprocal relationship between social accounts of knowledge production and efforts to increase the representation of women and some minorities in the academy. In particular, I consider the extent to which feminist social epistemologies such as Helen Longino’s critical contextual empiricism can be employed to argue that it is in researchers’ epistemic interests to take active steps to increase gender diversity. As it stands, critical contextual empiricism does not provide enough resources to succeed at this task. However, considering (...) this view through an employment equity lens highlights areas where such theories need to be further developed. I argue that views such as Longino’s ought to attend to nuances of community structure and cultural features that inhibit critical social interactions, if we are to maximize the epistemic as well as the ethical improvements associated with a social approach to knowing. These developments advance these epistemic theories for their own sake. They also help develop these theories into a tool that can be used by those calling for increased diversity in the academy. Here we comment on this topic by examining the interaction between emotion and moral judgment in certain psychopathological groups that are characterized by abnormalities in emotion processing, such as psychopaths and sexual offenders with paraphilic disorders.

Após o dia 08 de janeiro de 2023, quando se deu a invasão na Praça dos Três Poderes, no Distrito Federal, numa tentativa de destituir o governo legitimado pelas urnas, formou-se uma rede de resistência e de restauração democrática, seguindo manifestações anteriores como a Carta às brasileiras e aos brasileiros e a Frente Ampla pela Democracia. A resistência democrática propõe manter viva a democracia. Nesse contexto, examinam-se aspectos teóricos sobre o autoritarismo e o totalitarismo que ameaçam de morte as democracias (...) constitucionais. Analisando o processo judicial, identificamos uma relação entre falantes que permite uma abordagem sociológica desse instrumento jurídico. A partir de uma visão instrumental do processo, percebemos uma interação entre pessoas regidas por determinada gramática jurídica. Nesse sentido, autor, réu e juiz praticam atos de fala na jurisdição, formando uma comunidade particular num espaço público institucionalizado. Ao exercer o seu papel como falantes, interagindo entre si, cada um deles postula um tipo de reconhecimento em práticas intersubjetivas com o outro da relação. (...) Ao se reconhecerem como sujeitos de direitos eles legitimam o processo como instrumento discursivo, dialético e dialógico de acesso à justiça, estabelecendo entre eles uma pedagogia de reconhecimento. Falar-se-á a Nicômano como a um juiz iniciante. Em tempo longíquo, Nicômaco foi devidamente instruído sobre questões de conduta. Ao juiz iniciante, apresenta-se alguns assuntos de conduta processual, cujas lições analisa-se regras jurídicas para que o juiz não se afaste do senso de justiça, que a tanto aspira, e não confunda a objetividade processual com a subjetividade instrínseca, que ao erro soberbo da injustiça pode levá-lo a subverter o Direito em razão das paixões políticas. Com Levitsky e Ziblatt, introduz-se o (...) tema sobre a cooptação do juiz e, com Habermas, apresenta-se a ética processual, observando o processo como espécie de comunidade jurídica. Dworkin acrescenta algo mais sobre a conduta interpretativa do Direito. Em assuntos de conduta pessoal, Aristóteles alerta que toda ação e toda escolha objetiva um bem qualquer. Logo, em conduta pessoal, não admira prevalecer a subjetividade do agente no exame entre meios e fins. No entanto, em termos de conduta processual, o juiz age em conformidade com bens jurídicos normativamente considerados. Numa palavra, em termos de conduta processual impera a objetividade dos fatos, visando a aplicação imparcial das regras jurídicas, em conformidade com os princípios da dignidade da pessoa humana, em suas inviolabilidades, na presunção de inocência, na ampla defesa, no contraditório e num julgamento imparcial. As considerações finais, ancoradas em observações diretas, na conjuntura brasileira e na literatura referenciada, permitem concluir que a ordem jurídica democrática depente, dentre outros fatores, da conduta ética do juiz. A relação capital-trabalho é cada vez mais complexa. Da fábrica hierarquizada ao sistema de produção pulverizado, tecnológico e digital, a concentração do poder econômico produz uma grave externalidade negativa: a descentralização do trabalho e do seu valor social. A concentração do capital cada vez mais descarta o trabalhador em sua condição humana e de cidadania. A partir do tema do Colóquio Habermas é possível identificar o enfraquecimento do Estado como promotor e garantidor de direitos fundamentais dos trabalhadores, afetando a adesão (...) sindical como forma de lealdade da classe trabalhadora, ou em outras palavras a consciência de classe ou de solidariedade entre os trabalhadores. Por conseguinte, sindicatos enfraquecidos operam com déficit de legitimidade e de representatividade, tal como se dá com a estrutura do sindicato único por categoria profissional. Nesse contexto, refletimos sobre os efeitos dessa relação complexa e tensa, no momento de reconstrução ou de restauração da ordem democrática participativa e deliberativa, objetivando o não retrocesso social dos trabalhadores. Ancoramos o estudo na Constituição Federal, em princípios do Direito do Trabalho, em reflexões habermasianas sobre o tema. Situamos nossa pesquisa a partir de 2017, quando se deu a Reforma Trabalhista. Com essa abordagem, propomos um diálogo para além do direito positivado, uma vez que o valor social do trabalho e da livre iniciativa do empresário atravessam várias estruturas sociais que disputam espaço junto aos vários segmentos do Estado brasileiro. This article argues that cruelty, as a willingness to see or orchestrate the suffering of others, is not an unfortunate side-effect of neoliberal theories put into practice but is constitutive of the neoliberal project from its theoretical inception. Drawing on Lisa Duggan’s concept of ‘optimistic cruelty’ and treating the canonical texts of neoliberal economic theory as literary artefacts, the article develops this argument through a close reading of one of the central architects of the neoliberal project, the philosopher and economist (...) Friedrich Hayek. The first part of the article examines how Hayek attempts to justify the brutality of the market order he imagines – the catallaxy – by arguing that this brutality is the natural consequence of the spontaneous evolutionary processes that move civilisation forward. The second part brings to the fore the eugenicist undertones that suffuse this vision, despite Hayek’s apparent rejection of Social Darwinism. I analyse how Hayek’s market order operates through a series of disciplinary and biopolitical technologies that use pain, frustration, punishment and stigmatisation to eliminate bad habits, practices and subjectivities. These cruel mechanisms enable the catallaxy to sort between productive and unproductive lives to ensure that available resources are directed towards the former – even if it means that the others might be left to die.
