Intensity profiles of emotional experience over time have been found to differ primarily in explosiveness and accumulation. However, the determinants of these temporal features remain poorly understood. In two studies, we examined whether emotion regulation strategies are predictive of the degree of explosiveness and accumulation of negative emotional episodes. Participants were asked to draw profiles reflecting changes in the intensity of emotions elicited either by negative social feedback in the lab or by negative events in daily life. (...) In addition, trait, and state usage of a set of emotion regulation strategies was assessed. Multilevel analyses revealed that trait rumination was positively associated with emotion accumulation. State rumination was also positively associated with emotion accumulation and, to a lesser extent, with emotion explosiveness. These results provide support for emotion regulation theories, which hypothesise that rumination is a central mechanism underlying the maintenance of negative emotions. (shrink)

BackgroundThe Authors have laid out an analysis of Italian COVID-19 confirmed data and fatality rates, pointing out how a dearth of health care resources in northern regions has resulted in hard, ethically challenging decisions in terms of granting patient access to intensive care units (ICU).Main textHaving to make such decisions certainly entails substantial difficulties, and that has led many health care professional to seek ethical guidance. The Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI) has attempted to meet (...) that growing need by a set of recommendations, applying “clinical soundness” as a beacon standard; that approach tends to prioritize patients with higher life expectancy, which could be characterized as a “moderately utilitarian” approach. Yet, such a selection has engendered daunting ethical quandaries. The authors believe it can only be warranted and acceptable if rooted in a transparent decision-making process and verifiable, reviewed criteria. Moreover, the authors have stressed how clinical experimentation in a pandemic setting is a subtext of great interest from an ethical perspective. In Italy, no drug therapy and trials were undertaken for COVID-19 patients for a rather long period of time. When the epidemic was already circulating, an intervention proved necessary on the system of administrative procedures, aimed at expediting the authorization and validation of protocols, then bogged down by bureaucracy. A new system has since been instituted by a government decree that was signed about one month after the first Covid-19 case was officially recorded in the country. Such a swift implementation, which took just a few weeks, is noteworthy and proves that clinical trials can be initiated in a timely fashion, even with a pandemic unfolding. The concerted, action of supportive care and RCTs is the only way to attain effective forms of treatments for COVID-19 and any other future outbreak.ConclusionsThe authors have arrived at the conclusion that the most effective and ethically sound response on the part of any national health care system would be to adequately reconfigure its organizational mechanisms, by making clinical trials and all related administrative procedures consistent with the current state of emergency. The decision-making environment in intensive care units (ICUs) is influenced by the transformation of intensive care medicine, the staffing situation and the increasing importance of patient autonomy. Normative implications of time in intensive care, which affect all three areas, have so far barely been considered. The study explores patterns of decision making concerning the continuation, withdrawal and withholding of therapies in intensive care. A triangulation of qualitative data collection methods was chosen. Data were collected through non-participant observation on a (...) surgical ICU at an academic medical centre followed by semi-structured interviews with nurses and physicians. The transcribed interviews and observation notes were coded and analysed using qualitative content analysis according to Mayring. Three themes related to time emerged regarding the escalation or de-escalation of therapies: influence of time on prognosis, time as a scarce resource and timing in regards to decision making. The study also reveals the ambivalence of time as a norm for decision making. The challenge of dealing with time-related efforts in ICU care results from the tension between the need to wait to optimise patient care, which must be balanced against the significant time pressure which is characteristic of the ICU setting. Decades of research conducted in Western, Educated, Industrialized, Rich, & Democratic (WEIRD) societies have led many scholars to conclude that the use of mental states in moral judgment is a human cognitive universal, perhaps an adaptive strategy for selecting optimal social partners from a large pool of candidates. However, recent work from a more diverse array of societies suggests there may be important variation in how much people rely on mental states, with people in some societies judging accidental harms just (...) as harshly as intentional ones. To explain this variation, we develop and test a novel cultural evolutionary theory proposing that the intensity of kin-based institutions will favor less attention to mental states when judging moral violations. First, to better illuminate the historical distribution of the use of intentions in moral judgment, we code and analyze anthropological observations from the Human Area Relations Files. This analysis shows that notions of strict liability—wherein the role for mental states is reduced—were common across diverse societies around the globe. Then, by expanding an existing vignette-based experimental dataset containing observations from 321 people in a diverse sample of 10 societies, we show that the intensity of a society's kin-based institutions can explain a substantial portion of the population-level variation in people's reliance on intentions in three different kinds of moral judgments. Together, these lines of evidence suggest that people's use of mental states has coevolved culturally to fit their local kin-based institutions. We suggest that although reliance on mental states has likely been a feature of moral judgment in human communities over historical and evolutionary time, the relational fluidity and weak kin ties of today's WEIRD societies position these populations' psychology at the extreme end of the global and historical spectrum. In de Ronde and Massri it was argued against the orthodox definition of quantum entanglement in terms of pure and separable states. In this paper we attempt to discuss how the logos categorical approach to quantum mechanics is able to provide an objective formal account of the notion of entanglement—completely independent of both purity and separability—in terms of the potential coding of intensive relations and effective relations. We will show how our novel redefinition allows us to provide an anschaulich content (...) to the notion of entanglement, erasing in this way the “spookiness” still present within its orthodox understanding in terms of space-time separated collapse particles. Summary. It is proposed to translate the mind-matter distinction into terms of mental and physical time. In the spirit of this idea, we hypothesize a relation between the intensity of mental presence and a crucial time scale (some seconds) often referred to as a measure for the duration of nowness. This duration is experimentally accessible and might, thus, offer a suitable way to characterize the intensity of mental presence. Interesting consequences with respect to the idea (...) of a generalized notion of mental presence, with human consciousness as a special case, are outlined. Researchers have begun to use response times to emotion items as an indirect measure of emotional clarity. Our first aim was to scrutinise the properties of this RT measure in more detail than previously. To be able to provide recommendations as to whether emotional intensity – as a possible confound – should be controlled for, we investigated the specific form of the relation between emotional intensity and RTs to emotion items. In particular, we assumed an inverted U-shaped (...) relation at the item level. Moreover, we analysed the RT measure’s convergent validity with respect to individuals’ confidence in their emotion ratings. As a second aim, we compared the predictive validity of emotional clarity measures with respect to daily emotion regulation. The results of three experience sampling studies showed that the association between emotional intensity and RT followed an inverted U shape. RT was in part related to confidence. Emotional clarity measures were unrelated to reappraisal. There was some evidence that lower emotional clarity was related to a greater use of suppression. The findings highlight that emotional intensity and squared emotional intensity should be controlled for when using the RT measure of emotional clarity in future research. (shrink)

We present two case studies from contemporary biology in which we observe conflicts between established and emerging approaches. The first case study discusses the relation between molecular biology and systems biology regarding the explanation of cellular processes, while the second deals with phylogenetic systematics and the challenge posed by recent network approaches to established ideas of evolutionary processes. We show that the emergence of new fields is in both cases driven by the development of high-throughput data generation technologies and (...) the transfer of modeling techniques from other fields. New and emerging views are characterized by different philosophies of nature, i.e. by different ontological and methodological assumptions and epistemic values and virtues. This results in a kind of conflict we call “epistemic competition” that manifests in two ways: On the one hand, opponents engage in mutual critique and defense of their fundamental assumptions. On the other hand, they compete for the acceptance and integration of the knowledge they provide by a broader scientific community. Despite an initial rhetoric of replacement, the views as well as the respective audiences come to be seen as more clearly distinct during the course of the debate. Hence, we observe—contrary to many other accounts of scientific change—that conflict results in the formation of new niches of research, leading to co-existence and perceived complementarity of approaches. Our model thus contributes to the understanding of the pluralization of the scientific landscape. (shrink)

Definition of the problem The COVID-19 pandemic poses a considerable challenge to the capacity and functionality of intensive care. This concerns not only resources but, above all, the physical and psychological boundaries of nursing professionals. The question of how care for others and self-care of nurses in intensive care units are related to each other in the context of the COVID-19 pandemic has not been addressed in public and scientific discourse so far. Arguments The present contribution reflects this relationship with (...) reference to the Code of Ethics of the International Council of Nurses, particularly considering principlism and the Care Ethics according to Joan Tronto. As a result, it shows a corridor of ethically justifiable care with several borders, above all: self-care must not be given up completely for the benefit of care for others, and a categorical subordination of care for others to self-care is ethically not justifiable. Conclusion The article makes an important contribution to a differentiated ethical consideration of the rights and responsibilities of intensive care nurses as moral actors within the pandemic in Germany. It, thus, provides a first starting point for a broad social and political discourse which is urgently needed not only during but also after the pandemic in order to improve the situation of intensive care nurses and those who are cared for. I offer a critique of Melissa Zinkin’s reading of Kant’s analysis of aesthetic judgment. She argues that in judgments of taste the imagination is freed from its determinate relation with the understanding because the form of intuition in which beauty is apprehended is different from the form of intuition employed in determinate judgment. By distinguishing between an extensive and intensive form of intuition, this interpretation is able to explain why the apprehension of beauty cannot be subsumed under a concept. (...) But I contest Zinkin’s identification of the sensus communis with this intensive form of intuition. I then substantiate two interrelated claims: that we can account for the genesis of the sensus communis by distinguishing between an intensive and an extensive form of time, and that we can avoid making the sensus communis atemporal by showing that it resides within an intensive form of time as a condition for its possibility, thereby structuring Kant’s account of the sensus communis securely within the critical framework. In an increasingly fragmented media environment, some issues stand out and generate huge publicity for a limited period of time. Using Scannell's categories, event and happening, the article analyses two cases – the 1994 Olympic Winter Games and the death of Princess Diana. It is argued that they, in order to account for audience involvement and the complexity of meanings, should be analyzed in relation to the discourses and narrative developments across different media that embed the main, televised, (...) moments. It is also argued that today's proliferation of media channels and the presence of new media can contribute to increasing attention and involvement of the audience. Ceremonial media events that previously dominated have moved in two directions: integrative, entertainment oriented events organized by the media and disaster events. (shrink)

At the height of the COVID-19 crisis in the Netherlands a shortness of intensive care beds was looming. Dutch professional medical organizations asked a group of ethicists for assistance in drafting guidelines and criteria for selection of patients for intensive care treatment in case of absolute scarcity, when medical selection criteria would no longer suffice. This article describes the Dutch context, the process of drafting the advice and reflects on the role of ethicists and lessons learned. We argue that timely (...) interaction between clinical and ethical expertise is necessary since the distinction between medical and non-medical considerations is not as clearcut as sometimes assumed. Furthermore, pragmatic considerations related to the specifics of an epidemic are of importance, for example, in relation to prioritizing health care workers. As a consequence, any protocol already present before the pandemic would need alterations to fit the current situation. The ‘fair innings’ criterion we proposed, rephrased as an argument of intergenerational solidarity, was considered reasonable by professionals as well as patient organizations. While it is desirable to draft ethical guidelines in ‘peacetime’ as a matter of pandemic preparedness, the pressure of an actual crisis facilitates decision-making, although it will also complicate a more democratic approach. This study aimed to synthesize literature data on male judo combat time in international competitions between 2010 and 2019. The search was carried out from May 8th to June 11th, 2021, in electronic databases using the following keywords: AND. After the selection process, 8 articles were included in the systematic review and 7 in the meta-analysis. These studies analyzed 2,562 international male judo combats over the years 2010 to 2019. We observed that the average male judo combat time (...) changed after each rule change. There was a significant difference between combats that ended up to the regular time and those that needed overtime. There were differences between 60kg and +100kg categories and the others in 2011-2012. However, no significant difference was found between the combat time by weight division after the 2017 rule changes, although there are still differences in relation to the end of the combats. There were significant changes in the male judo combat time with each rule change, and the data from the included studies point to a trend of homogeneity in the combat time spent between the weight divisions over the years, and an increase in the occurrence of Golden Score. More studies need to be carried out to identify the new temporal behaviors of athletes. (shrink)

The paediatric intensive care unit is a high-stress environment for parents, families and health care professionals alike. Family members experiencing stress or grief related to the admission of their sick child may at times exhibit challenging behaviours; these exist on a continuum from those that are anticipated in context, through to unacceptable aggression. Rare, extreme behaviours include threats, verbal or even physical abuse. Both extreme and recurrent ‘subthreshold’ behaviours can cause significant staff distress, impede optimal clinical care and compromise patient (...) outcomes. The unique PICU environment and model of care may magnify stressors for both families and staff and the family-centred approach to care central to paediatric practice, may also contribute to contextual challenges. Pervasive conflict in paediatric healthcare is harmful for patients, families, PICU staff and the institution more broadly. We propose that caring for children and caring for staff are inseparable goals and the latter has been inadvertently but detrimentally deprioritised as FCC has become a primary focus. A transparent and graded hierarchy of responses to variable levels of challenging behaviour is necessary to ensure that families are supported, while HCPs remain protected in the workplace. This requires establishing firm limits supported by all teams and levels of the institution. As such, we aim to identify and clarify the context and impact of challenging parent and family behaviour in the PICU and to offer potential, proactive mitigation strategies, based on reflections and stakeholder discussion following recent clinical challenges and experiences in our unit. Respect for autonomy is a key concept in contemporary bioethics and in ethics at the end of life in particular. An individualistic interpretation of autonomy may not incorporate the aspects of consideration that patients may have for their wider construct of personhood, which includes their love and consideration for their families. This anonymous case describes the intricacies of a patient’s decision making at the end of life, the choices she made, and how her decisions changed as her situation evolved. Recent years have seen a growing consensus in the philosophical community that the grandfather paradox and similar logical puzzles do not preclude the possibility of time travel scenarios that utilize spacetimes containing closed timelike curves. At the same time, physicists, who for half a century acknowledged that the general theory of relativity is compatible with such spacetimes, have intensely studied the question whether the operation of a time machine would be admissible in the context of the same (...) theory and of its quantum cousins. A time machine is a device which brings about closed timelike curves—and thus enables time travel—where none would have existed otherwise. The physics literature contains various no-go theorems for time machines, i.e., theorems which purport to establish that, under physically plausible assumptions, the operation of a time machine is impossible. We conclude that for the time being there exists no conclusive no-go theorem against time machines. The character of the material covered in this article makes it inevitable that its content is of a rather technical nature. We contend, however, that philosophers should nevertheless be interested in this literature for at least two reasons. First, the topic of time machines leads to a number of interesting foundations issues in classical and quantum theories of gravity; and second, philosophers can contribute to the topic by clarifying what it means for a device to count as a time machine, by relating the debate to other concerns such as Penrose's cosmic censorship conjecture and the fate of determinism in general relativity theory, and by eliminating a number of confusions regarding the status of the paradoxes of time travel. Background Supporting physicians in Intensive Care Units s as they face dying patients at unprecedented levels due to the COVID-19 pandemic is critical. Amidst a dearth of such data and guided by evidence that nurses in ICUs experience personal, professional and existential issues in similar conditions, a systematic scoping review is proposed to evaluate prevailing accounts of physicians facing dying patients in ICUs through the lens of Personhood. Such data would enhance understanding and guide the provision of better support for (...) ICU physicians. Methods An SSR adopts the Systematic Evidenced Based Approach to map prevailing accounts of caring for dying patients in ICUs. To enhance the transparency and reproducibility of this process, concurrent and independent use of tabulated summaries, thematic analysis and directed content analysis is adopted. Results Eight thousand three hundred fifty-eight abstracts were reviewed from four databases, 474 full-text articles were evaluated, 58 articles were included, and the Split Approach revealed six categories/themes centered around the Innate, Individual, Relational and Societal Rings of Personhood, conflicts in providing end of life care and coping mechanisms employed. Conclusion This SSR suggests that caring for dying patients in ICU impacts how physicians view their personhood. To resolve conflicts within individual concepts of personhood, physicians use prioritization, reframing and rely on accessible, personalized support from colleagues to steer coping strategies. An adapted form of the Ring Theory of Personhood is proposed to direct timely personalized, appropriate and holistic support. (shrink)

Background: The intensive care unit environment focuses on interventions and support therapies that prolong life. The exercise by nurses of their autonomy impacts on perception of the role they assume in the multidisciplinary team and on their function in the intensive care unit context. There is much international research relating to nurses’ involvement in end-of-life situations; however, there is a paucity of research in this area in Brazil. In the Brazilian medical scenario, life support limitation generated a certain reluctance of (...) a legal nature, which has now become unjustifiable with the publication of a resolution by the Federal Medical Council. In Brazil, the lack of medical commitments to end-of-life care is evident. Objective: To understand the process by which nurses exercise autonomy in making end-of-life decisions in intensive care units. Research design: Symbolic Interactionism and Corbin and Strauss theory methodology were used for this study. Participants and research context: Data were collected through single audio-recorded qualitative interviews with 14 critical care nurses. The comparative analysis of the data has permitted the understanding of the meaning of nurse’s experience in exercising autonomy relating to end-of-life decision-making. Ethical considerations: Institutional ethics approval was obtained for data collection. Participants gave informed consent. All data were anonymized. Findings: The results revealed that nurses experience the need to exercise autonomy in intensive care units on a daily basis. Their experience expressed by the process of increase opportunities to exercise autonomy is conditioned by the pressure of the intensive care unit environment, in which nurses can grow, feel empowered, and exercise their autonomy or else can continuously depend on the decisions made by other professionals. Conclusion: Nurses exercise their autonomy through care. They work to create new spaces at the same time that they acquire new knowledge and make decisions. Because of the complexity of the end-of-life situation, nurses must adopt a proactive attitude that inserts them into the decision-making process. (shrink)

Our time is characterized by what seems like an unprecedented process of intense global homogenization. This reality provides the context for exploring the nature and value of toleration. Hence, this essay is meant primarily as a contribution to international ethics rather than political philosophy. It is argued that because of the non-eliminability of differences in the world we should not even hope that there can be only one global religion or ideology. Further exploration exposes conceptual affinity between the concepts (...) of intolerance, ideology, and doctrinal evil. The last concept is developed in contrast to pure evil and average evil, and under the assumption of the metaphysical necessity of free will. Doctrinal evil is found to represent the main source of intolerance as a result of a mechanism that tends to confuse doctrinal evil (or the competing conceptions of the good) with pure evil. This connection between doctrinal evil and pure evil provides ideologies with their forcefulness. Tolerance cannot be properly understood in terms of a simple opposition to intolerance, however. Tolerance emerges as a sort of vigilance, conscientiousness, and non-negligence based not on a supposedly correct interpretation of the good, but rather on the acceptance of the fallibility of any such attempted definition. Conversely, the principal evil in doctrinal evil is found in arrogance that accompanies the intolerance-inducing irresponsible thoughtlessness. With this conceptual topology in mind the paper also addresses questions regarding religious tolerance, the ideology of human rights and democracy, the right to self-defense, ways to face evil, the dialectics of using old names for novel evils, and related issues. This book consists of transcripts from two lecture courses Levinas delivered in 1975-76, his last year at the Sorbonne. They cover some of the most pervasive themes of his thought and were written at a time when he had just published his most important—and difficult—book, _Otherwise than Being, or Beyond Essence._ Both courses pursue issues related to the question at the heart of Levinas's thought: ethical relation. The Foreword and Afterword place the lectures in the context of his (...) work as a whole, rounding out this unique picture of Levinas the thinker and the teacher. The lectures are essential to a full understanding of Levinas for three reasons. First, he seeks to explain his thought to an audience of students, with a clarity and an intensity altogether different from his written work. Second, the themes of God, death, and time are not only crucial for Levinas, but they lead him to confront their treatment by the main philosphers of the great continental tradition. Thus his discussions of accounts of death by Heidegger, Hegel, and Bloch place Levinas's thought in a broader context. Third, the basic concepts Levinas employs are those of _Otherwise than Being_ rather than the earlier _Totality and Infinity_: patience, obsession, substitution, witness, traumatism. There is a growing recognition that the ultimate standing of Levinas as a philosopher may well depend on his assessment of those terms. These lectures offer an excellent introduction to them that shows how they contribute to a wide range of traditional philosophical issues. (shrink)

Psychopaths appear to be ‘creatures apart’ – grandiose, shameless, callous and versatile in their violence. I discuss biological underpinnings to their pale affect, their selective inability to discern fear and sadness in others and a predatory orienting towards images that make most startle and look away. However, just because something is biologically underpinned does not mean that it is innate. I show that while there may be some genetic determination of fearlessness and callous-unemotionality, these and other features of the personality (...) may arise from developmental failures in the interpersonal reception of their emotions, needs and their sense of self. One is unlikely to be able to own inner experiences if shamed for having them, or if, having them, one does not know how to regulate and soothe. So psychopaths may learn to attend away and suppress them. Rather than a fully inherited difficulty, they may have become unable to reflect on inner states, so meta-emotions and self-reflective emotions like guilt and shame do not fully arise. They retain enough sensitivity to know their difference, and hide. I suggest that psychopaths are characterised by a nested sense of self, arising from the surprising effect of shame on these seemingly shameless characters. They do not have an integrated sense of self across context or across time or in relation to a generalised social other. With a nested sense of self, diminished intensity and scope of affective experience (in both directly experienced and vicarious forms) they lack textured access to a personal, owned and integrated past. Thus they lack the kind of access to the past required for a motivationally compelling planning of the future. They lack the emotional investment in the future that enables us to overcome the motivation to act opportunistically and myopically. These individuals live strangely in time. They have a fugitive sense of self and live nimbly among many pasts. They present an elegant and coherent mask to the person they are addressing in the moment and generate possible futures without conviction. (shrink)

The complexity of registered nurses’ work in the intensive care unit places them at risk of experiencing critical incident stress. Gadamer's philosophical hermeneutics (1960/2013) was used to expand the meanings of work‐related critical incident stress for registered nurses working with adults in the intensive care unit. Nine intensive care unit registered nurses participated in unstructured interviews. The interpretations emphasized that morally distressing experiences may lead to critical incident stress. Critical incident stress was influenced by the perception of judgment from co‐workers (...) and the organizational culture. Nurses in this study attempted to cope with critical incident stress by functioning in ‘autopilot’, temporarily altering their ability to critically think and to conceal emotions. Participants emphasized the importance of timely crisis interventions tailored to support their needs. This study highlighted that critical incident stress was transformative in how intensive care unit nurses practiced, potentially altering their professional self‐identity. Work‐related critical incident stress has implications for nurses, the discipline, and the health care system. (shrink)

Severe Traumatic Brain Injury (TBI) remains a major cause of death and disability afflicting mostly young adult males and elderly people, resulting in high economic costs to society. Therapeutic approaches focus on reducing the risk on secondary brain injury. Specific ethical issues pertaining in clinical testing of pharmacological neuroprotective agents in TBI include the emergency nature of the research, the incapacity of the patients to informed consent before inclusion, short therapeutic time windows, and a risk-benefit ratio based on concept (...) that in relation to the severity of the trauma, significant adverse side effects may be acceptable for possible beneficial treatments. Randomized controlled phase III trials investigating the safety and efficacy of agents in TBI with promising benefit, conducted in acute emergency situations with short therapeutic time windows, should allow randomization under deferred consent or waiver of consent. Making progress in knowledge of treatment in acute neurological and other intensive care conditions is only possible if national regulations and legislations allow waiver of consent or deferred consent for clinical trials. The scientific and practical fields—especially high-performance sports—increasingly request a stronger focus be placed on individual athletes in human movement science research. Machine learning methods have shown efficacy in this context by identifying the unique movement patterns of individuals and distinguishing their intra-individual changes over time. The objective of this investigation is to analyze biomechanically described movement patterns during the fatigue-related accumulation process within a single training session of a high number of repeated executions of a ballistic sports movement—specifically, the (...) frontal foot kick (mae-geri) in karate—in expert athletes. The two leading research questions presented for consideration are (1) Can characteristics of individual movement patterns be observed throughout the entire training session despite continuous changes, i.e., even as fatigue-related processes increase? and (2) How do intra-individual movement patterns change as fatigue-related processes increase throughout a training session? Sixteen expert karatekas performed 606 frontal foot kicks directed toward an imaginary target. The kicks were performed in nine sets at 80% (K-80) of the self-experienced maximal intensity. In addition, six kicks at maximal intensity (K-100) were performed after each of the nine sets. Between the sets, the participants took a 90-s break. Three-dimensional full-body kinematic data of all kicks were recorded with 10 infrared cameras. The normalized waveforms of nine upper- and lower-body joint angles were classified using a supervised machine learning method (support vector machine). The results of the classification revealed a disjunct distinction between the kinematic movement patterns of individual athletes. The identification of unique movement patterns of individual athletes was independent of the intensity and the degree of fatigue-related processes. In other words, even with the accumulation of fatigue-related processes, the unique movement patterns of an individual athlete can be clearly identified. During the training session, changes in intra-individual movement patterns could also be detected, indicating the occurrence of adaptations in individual movement patterns throughout the fatigue-related accumulation process. The results suggest that these adaptations can be modeled in terms of changes in patterns rather than increasing variance. Practical consequences are critically discussed. (shrink)

At the same time, I want to discuss what theories of relational autonomy, like Sue's, add to the bioethics literature on autonomy. This book offers a philosophical perspective on contemporary Tourette Syndrome scholarship, a field which has exploded over the last thirty years. Despite intense research efforts on this common neurodevelopmental condition in the age of the brain sciences, the syndrome’s causes and potential cures remain intriguingly elusive. How does this lack of progress relate to the tacitly operating philosophical concepts that shape our current thinking about Tourette Syndrome? This book foregrounds these tacit concepts and shows how they relate to “big topics” (...) in philosophy such as time, volition, and the self. By tracing how these topics relate to current research on Tourette’s, it invites us to re-think our approach to research and care. Such re-thinking is urgently needed: individuals and families living with Tourette Syndrome remain under-serviced as pharmacological and behavioural therapies provide relief for some but not all who need support. This book highlights what questions we ask and do not ask in contemporary scholarship, thereby surfacing invisible constraints and opportunities in the field. It is of interest to scholars, health professionals, students, and affected families who want to better understand this burgeoning field of research with its conceptual controversies, approaches to aetiology, and directions for new research and improved clinical care. Instead, we have focused our attention on the determinations that time receives when combined with the categories. Departing from the definition of the transcendental schemata as “determinations of time”, an attempt is made to establish the various determinations that time receives from each one of the categories, as these perform the determination of (...) time in schematism. The categories of quantity allow us to think of time as a series of homogeneous unities; the categories of quality show each instant of time as a receptacle able to receive the different intensities of the real; the categories of relation establish a rule-dependent order on the flow of time; finally, the categories of modality determine the whole of time forming a collective unity that gathers or embraces each one of the instants of time preserving its specificity. The first section illustrates the metaphysical significance of Kant’s introduction of the quantitative opposition between reality and negation, which informs the phenomena-noumena distinction and the attribution of intensive magnitude. The second section argues that Maimon’s speculative appropriation of differentials took up Kant’s conception of real opposition between (...) reality and negation but fundamentally revised the theory of space and time to dissolve the problem of applicability in Kant, leading to the consequence of obliterating the pure categories. The third section shows how Fichte inherited the Kantian-Maimonian quantitative opposition of reality and negation in his characterization of the interdetermination between the I and the Not-I and how he developed an immanent account of the relational categories such as causality and substantiality on that basis. This study investigates whether and how the intensity of social distancing from the Coronavirus disease 2019 (COVID-19) pandemic influences the corporate social responsibility (CSR) disclosure index. An empirical examination is carried out based on data from the Shanghai Stock Exchange from 2020 to 2022. CSR disclosure index is measured by the percentage of CSR-related press releases from the total press releases published on a certain day. The intensity of the COVID-19 pandemic is measured by the daily confirmed cases (...) among the population in China. This research uses the two-stage least squares regression model to alleviate the endogeneity issues. Findings reveal a reverse correlation between the intensity of the COVID-19 pandemic and the CSR disclosure. The results are consistent and robust to endogeneity tests and sensitivity analyses. Findings support the stakeholder–agency theory and indicate that Chinese managers tend to satisfy the shareholders' interests rather than those of other broad stakeholders. Moreover, Chinese managers tend to choose short-term survival rather than long-term development in times of social distancing. (shrink)

The Health (Regulation of Termination of Pregnancy) Act 2018 was commenced on 01/01/2019 in Ireland. The Act provides for legal termination of pregnancy under defined circumstances including for any reason at < 12 weeks gestation; and where two doctors agree there is ‘a condition affecting the foetus that is likely to lead to the death of the foetus either before, or within 28 days of, birth’. As such, abortion for congenital anomaly (CA) can occur at a number of time (...) points, depending on the adjudged severity. Infants born with CAs frequently require significant medical intervention and account for a high proportion of admissions to paediatric intensive care units (PICUs). The purpose of this paper was to evaluate the number of infants with CAs admitted to an Irish PICU in the period before and after the implementation of the Act. All PICU admissions < 1 month of age to a single Irish paediatric hospital between 2012 and 2021 were analysed. CAs were recorded, and the periods before and after the commencement of the Act compared. We found a difference in admissions involving CAs, particularly those related to congenital heart disease involving single ventricle anatomy. It is plausible that this difference was as a result of improved access to abortion services following the implementation of the Act. This article explores the legal conditions related to the abortion of a foetus with a CA in Ireland, and the possible impact of the Independent Review of the Operation of the Health ( Regulation of Termination of Pregnancy) Act 2018. BackgroundThe health sector aims to improve health outcomes and access to healthcare. At the same time, the sector relies on unsustainable environmental practices that are increasingly recognised to be catastrophic threats to human health and health inequities. As such, a moral imperative exists for the sector to address these practices. While strides are currently underway to mitigate the environmental impacts of healthcare, less is known about how health researchers are addressing these issues, if at all.MethodsThis paper uses an interview (...) methodology to explore the attitudes of UK health researchers using data-intensive methodologies about the adverse environmental impacts of their practices, and how they view the importance of these considerations within wider health goals.ResultsInterviews with 26 researchers showed that participants wanted to address the environmental and related health harms associated with their research and they reflected on how they could do so in alignment with their own research goals. However, when tensions emerged, their own research was prioritised. This was related to their own desires as researchers and driven by the broader socio-political context of their research endeavours.ConclusionTo help mitigate the environmental and health harms associated with data-intensive health research, the socio-political context of research culture must be addressed. Recent philosophical debates have moved beyond proclamations of the “death of philosophy” and the “death of the subject” to consider more positively how philosophy can be practiced and the human self can be conceptualized today. Inspired by the writings of Nietzsche, Bergson, and Deleuze, rapid changes related to globalization, and advances in evolutionary biology and neuroscience, these debates have generated a renewed focus on time as an active force of change and novelty. Rejecting simple linear models of time, (...) these strands of thought have provided creative alternatives to a traditional reliance on fixed boundaries and stable identities that has proven unable to grapple with the intense speeds and complexities of contemporary life. In this book, Nathan Widder contributes to these debates, but also goes significantly beyond them. Holding that current writings remain too focused on time’s movement, he examines more fundamentally time’s structure and its structural ungrounding, releasing time completely from its traditional subordination to movement and space. Doing this enables him to reformulate entirely the terms through which time and change are understood, leading to a radical alteration of our understandings of power, resistance, language, and the unconscious, and taking post-identity political philosophy and ethics in a new direction. Eighteen independent but interlinked reflections engage with ancient philosophy, mathematical theory, dialectics, psychoanalysis, archaeology, and genealogy. The book’s broad coverage and novel rereadings of key figures—including Aristotle, Bergson, Nietzsche, Foucault, and Deleuze—make this a unique rethinking of the nature of pluralism, multiplicity, and politics. The COVID‐19 pandemic has placed extraordinary stress on frontline healthcare providers as they encounter significant challenges and risks while caring for patients at the bedside. This study used qualitative research methods to explore nurses and respiratory therapists' experiences providing direct care to COVID‐19 patients during the first surge of the pandemic at a large academic medical center in the Northeastern United States. The purpose of this study was to explore their experiences as related to changes in staffing models and to (...) consider needs for additional support. Twenty semi‐structured interviews were conducted with sixteen nurses and four respiratory therapists via Zoom or by telephone. Interviews were transcribed verbatim, identifiers were removed, and data was coded and analyzed thematically. Five major themes characterize providers' experiences: a fear of the unknown, concerns about infection, perceived professional unpreparedness, isolation and alienation, and inescapable stress and distress. This manuscript analyzes the relationship between these themes and the concept of moral distress and finds that some, but not all, of the challenges that providers faced during this time align with previous definitions of the concept. This points to the possibility of broadening the conceptual parameters of moral distress to account for providers' experiences of treating patients with novel illnesses while encountering institutional and clinical challenges. In this study caring is shown to be a membershipbound activity to kin and gender categories with strong moral connotations. Being a daughter or being a son are good enough reasons for becoming a caregiver, more so for women than for men. Caregivers were interviewed within the research project The role of women in family care of disabled elderly conducted by the Social and Economic Research Department of INRCA, Ancona, Italy. Transcripts of the interviews were analyzed through a detailed discourse (...) analysis within an ethnomethodological framework. Interview data are treated as interactional encounters that occasion members to display relevant aspects of their identities and morally adequate images of being a caregiver. In the interview interaction, interlocutors display an orientation towards the production of a moral order in which duty and responsibilities are allocated on the basis of gender distinction. Males are generally described as not being responsible for caring tasks, except for situations in which females are absent or sick, that is, for serious reasons. Caregivers'' perception of time dedicated to caring is pervasive. Most caregivers said it occupied all their time, but gender differences were noticeable. Caring tasks are recognized as gender specific practices, thus failing to carry out these tasks is morally sanctionable when women are involved, but not so for men. Many caregivers described caring for older relatives as an intense source of stress, involving serious physical and psychological problems. The study on moral and identity issues related to caregiving highlights endangering constructions of caring. (shrink)

ABSTRACTEmotion differentiation refers to the precision with which people can identify and distinguish their emotions and has been associated with well-being in adults. This study investigated ED and its relation with emotional well-being in adolescents. We used an experience sampling method with 72 participants to assess adolescents’ positive and negative emotions at different time points over the course of two weekends and a baseline questionnaire to assess emotional well-being. Differentiating negative emotions was related to less negativity intensity (...) and propensity, and to the belief that emotions are malleable. Differentiating positive emotions was not related to any of the assessed well-being variables. Together, these results suggest that a detailed awareness of one’s negative emotional states is an important dimension of well-being, also in adolescence. (shrink)

Aphasia is a language disorder that occurs after a stroke and impairs listening, speaking, reading, writing, and calculation skills. Patients with post-stroke aphasia in Japan are increasing due to population aging and the advancement of medical treatment. Opportunities for adequate speech therapy in chronic stroke are limited due to time constraints. Recent studies have reported that intensive speech therapy for a short period of time or continuous speech therapy using high-tech equipment, including speech applications (apps, can improve aphasia (...) even in the chronic stage. However, its underlying mechanism for improving language function and its effect on other cognitive functions remains unclear. In the present study, we investigated whether intensive speech therapy using a newly developed speech support app could improve aphasia and other cognitive functions in patients with chronic stroke. Furthermore, we examined whether it can alter the brain network related to language and other cortical areas. Thus, we conducted a prospective, single-comparison study to examine the effects of a new speech support app on language and cognitive functions and used resting state functional MRI (rs-fMRI) regions of interest (ROI) to ROI analysis to determine changes in the related brain network. Two patients with chronic stroke participated in this study. They used the independent speech therapy system to perform eight sets of 20 randomly presented words/time (taking approximately 20 min), for 8 consecutive weeks. Their language, higher cognitive functions including attention function, and rs-fMRI, were evaluated before and after the rehabilitation intervention using the speech support app. Both patients had improved pronunciation, daily conversational situations, and attention. The rs-fMRI analysis showed increased functional connectivity of brain regions associated with language and attention related areas. Our results show that intensive speech therapy using this speech support app can improve language and attention functions even in the chronic stage of stroke, and may be a useful tool for patients with aphasia. In the future, we will conduct longitudinal studies with larger numbers of patients, which we hope will continue the trends seen in the current study, and provide even stronger evidence for the usefulness of this new speech support app. (shrink)

Currently, anomalous lived temporality is not included in the main diagnostic criteria or standard symptom checklists. In this article, we present the Transdiagnostic Assessment of Temporal Experience, a structured interview that can be used by researchers and clinicians without a comprehensive phenomenological background to explore abnormal time experiences in persons with abnormal mental conditions regardless of their diagnosis. When extensive data gathered by this scale are available, it will be possible to delineate well-defined anomalous lived temporality profiles for each (...) psychopathological disorder. This instrument may also prove useful for clinicians by providing a more refined assessment of relevant psychopathological symptoms and an in-depth understanding of the patient’s abnormal behaviour as related to specific types of time experience. In the first part of the article, we provide a brief overview of the phenomenological concept of temporality, including pre-phenomenal and phenomenal time, synthesis, conation and synchronization, and of abnormal time experiences in persons affected by psychopathological conditions. In the following part, we describe the basic structure of the interview that comprises seven categories corresponding to the abnormal features of lived temporality: anomalies of synchrony, of time structure, of implicit time flow, of explicit time flow, and anomalous experiences of the past, the present and the future. The paper also includes a section on administration and scoring of the TATE scale, the complete interview and a Likert table for quantifying the frequency, intensity and interference with daily life of the phenomena explored. Deleuze argued that a fundamental mutation in the concept of time occurred in Kant. In antiquity, the concept of time was subordinated to the concept of movement: time was a ‘measure’ of movement. In Kant, this relation is inverted: time is no longer subordinated to movement but assumes an autonomy of its own: time becomes "the pure and empty form" of everything that moves and changes. What is essential in the theory of time (...) is not the distinction between objective ‘clock time’ (or physical time) and the subjective experience of ‘time consciousness,’ since both of them measure movements, whether the movement of extensive objects or intensive states. What is fundamental is rather the relation between time and movement, since time can only assume its own concept when it ceases to be subordinate to movement, whether that movement is objective or subjective. This article examines how Kant inverted the relation between time and movement, and how Deleuze’s own theory of time builds on Kant’s revolution and extends it further. (shrink)

Pierre Bourdieu was the most influential sociologist of the later 20th Century. The framework he developed continues to inspire countless researchers across the globe and provokes intense debates long after his death. Novel concepts, innovative applications and countless elaborations spring up every day, bulking out and shaping a distinct, if not always entirely consistent, body of work that might be characterised as a recognisable tradition. For those coming to Bourdieu for the first time, therefore, and interested in using his (...) ideas in their own research, it no longer makes sense to confine oneself to the ideas of the man himself. An overview of the varied ways his concepts and arguments have been deepened and updated to make sense of new times or to fill certain gaps, and how insights on seemingly disconnected topics weave together into a bigger picture, is not just desirable but essential. Bourdieu and After aims to provide exactly this overview. Working closely with Bourdieu's own writings, but also covering a wide range of research and literature inspired by him, it aims to guide the reader through the key principles, the major and minor concepts and the concrete findings of Bourdieusian sociology as clearly and comprehensively as possible. It explains the difficult and often overlooked philosophical foundations, walks through the logic of famous terms like 'field', 'habitus' and 'capital' and demonstrates how they have been or can be used to provide powerful accounts of colonialism, the emergence of nation states and the rise of global social relations. It covers topics that Bourdieu was famous for analysing, like class and educational inequality, yet also traverses subjects that he said little on but which others influenced by him have tackled in depth, such as ethnicity, sexuality and family. Along the way Atkinson seeks to undermine some of the common criticisms levelled at Bourdieu while identifying remaining gaps and limitations. Rather than simply recognise the problems, however, Atkinson proposes possible solutions too - solutions that are facilitated, he argues, by characterising Bourdieusian sociology as what he calls 'relational phenomenology'. This article elaborates a theory of rhythm in relation to photography and, in particular, argues for the importance of rhythm in the theorization of photographic temporality. The approach taken breaks with a number of significant strands in contemporary photography theory, namely, Aristotelian-influenced modes of formalist criticism, dualistic formulations of representation and definitions of photographic temporality based on its difference to cinema. Through an interrogation of definitions of rhythm, the article examines, first, a formalist heritage from Aristotle to Lessing and (...) evident in Greenberg that assumes a naturalistic definition of rhythm based on linearity, regularity and anthropomorphic essentialism. The second strand is described through Frye's anticipation of a different rhythm, lyricism, that problematically compounds the subject as an entity defined in a dualistic paradigm. Against this background the article elaborates the possibilities inherent in Deleuze's notion of crystal time a rhythm of energetic intensity not dependent on linear temporality or subjectivism as a further, more credible, theorization of rhythmic temporality-as-texture. To investigate the effects of two different modes of physical activity on body composition, physical fitness, cardiometabolic risk, and psychological responses in female adolescents participating in a multi-disciplinary program. The 12-week randomized intervention included 25-adolescents with overweight divided into two groups: sports practice-SPG and functional training-FTG. The SPG intervention was divided into three sports: basketball, handball, and futsal. SPG participants performed one sport 3-times/week, over the course of 1 month. The FTG performed concurrent exercises 3-times/week. This study was registered in (...) Clinical Trials Registry Platform under number: RBR-45ywtg and registered in Local Ethics Committee number: 2,505.200/2018. The intensity of physical exercises-PE was matched between groups by the rating of perceived exertion. The primary outcome was body composition, and secondary outcomes were physical fitness, cardiometabolic risk, and psychological responses. There was a significant time-effect for body mass, body mass index, and low-density lipoprotein, all being reduced. There were increases over time for musculoskeletal mass, aerobic fitness, and high-density lipoprotein. There was a group time interaction with body fat percentage being lower post-intervention in the SPG. No significant differences were observed for the other variables. Both physical activity models were effective in improving a subset of obesity-related health parameters.
