Checklists have been used to improve quality in many industries, including healthcare. The use of checklists, however, has not been extensively evaluated in clinical ethics consultation. This article seeks to fill this gap by exploring the efficacy of using a checklist in ethics consultation, as tested by an empirical investigation of the use of the checklist at a large academic medical system (Cleveland Clinic). The specific aims of this project are as follows: (1) to improve the quality of ethics consultations (...) by providing reminders to ethics consultants about process steps that are important for most patient-centered ethics consultations, (2) to create consistency in the ethics consultation process across the medical system, and (3) to establish an effective educational tool for trainers and trainees in clinical ethics consultation. The checklist was developed after a thorough literature review and an iterative process of revising and testing by a group of experienced ethics consultants. To pilot test the checklist, it was distributed to 46 ethics consultants. After a six-month pilot period in which ethics professionals used the checklist during their clinical activities, a survey was distributed to all of those who used the checklist. The 10-item survey examined consultants’ perceptions regarding the three aims listed above. Of the 25 survey respondents, 11 self-reported as experts in ethics consultation, nine perceived themselves to have mid-level expertise, and five self-reported as novices. The majority (68 percent) of all respondents, regardless of expertise, believed that the checklist could be a “helpful” or “very helpful” tool in the consultation process generally. Novices were more likely than experts to believe that the checklist would be useful in conducting consultations. The limitations of this study include: reduced generalizability given that this project was conducted at one medical system, utilized a small sample size, and used self-reported quality outcome measures. Despite these limitations, to the authors’ knowledge this is the first investigatation of the use of a checklist systematically to improve quality in ethics consultation. Importantly, our findings shed light on ways this checklist can be used to improve ethics consultation, including its use as an educational tool. Digital spaces are saturated with metaphor: we have pages, sites, mice, and windows. Yet, in the world of digital textuality, these metaphors no longer function as we might expect. Martin Paul Eve calls attention to the digital-textual metaphors that condition our experience of digital space, and traces their history as they interact with physical cultures. Eve posits that digital-textual metaphors move through three life phases. Initially they are descriptive. Then they encounter a moment of fracture or rupture. Finally, they (...) go on to have a prescriptive life of their own that conditions future possibilities for our text environments - even when the metaphors have become untethered from their original intent. Why is "whitespace" white? Was the digital page always a foregone conclusion? Over a series of theses, Eve addresses these and other questions in order to understand the moments when digital-textual metaphors break and to show us how it is that our textual softwares become locked into paradigms that no longer make sense. While valuable work has been done addressing clinical ethics within established healthcare systems, we anticipate that the projected growth in acquisitions of community hospitals and facilities by large tertiary hospitals will impact the field of clinical ethics and the day-to-day responsibilities of clinical ethicists in ways that have yet to be explored. Toward the goal of providing clinical ethicists guidance on a range of issues that they may encounter in the systematization process, we discuss key considerations and potential challenges in (...) implementing system-wide ethics consultation services. Specifically, we identify four models for organizing, developing, and enhancing ethics consultation activities within a system created through acquisitions: train-the-trainer, local capacity-building, circuit-riding, and consolidated accountability. We note each model’s benefits and challenges. To our knowledge, this is the first paper to consider the broader landscape of issues affected by consolidation. We anticipate that clinical ethicists, volunteer consultants, and hospital administrators will benefit from our recommendations. (shrink)

Por meio de ambientes organizacionais, diversos e inclusivos, especialmente no que se refere a gênero, as mulheres poderão experienciar a valorização social, a influência no poder de decisão, o pertencimento (...) e engajamento no grupo de trabalho, a autenticidade e reconhecimento de diversidade (sentimento de interação justa), e essa experiência impactará a vida profissional e concomitantemente a pessoal/privada. O objetivo principal é mapear como é abordado a violência contra a mulher no ambiente de trabalho na literatura nacional. O método utilizado será a revisão de literatura integrativa, definida como o compêndio de literatura existente objetivando a sumarização e discussão de um corpo de conhecimento relacionado ao tema. BackgroundInaccurate citations are erroneous quotations or instances of paraphrasing of previously published material that mislead readers about the claims of the cited source. They are often unaddressed due to underreporting, the inability of peer reviewers and editors to detect them, and editors’ reluctance to publish corrections about them. In this paper, we propose a new tool that could be used to tackle their circulation.MethodsWe provide a review of available data about inaccurate citations and analytically explore current ways of reporting and (...) dealing with these inaccuracies. Consequently, we make a distinction between publication and circulation of inaccurate citations. Sloppy reading of published items, literature ambiguity and insufficient quality control in the editorial process are identified as factors that contribute to the publication of inaccurate citations. However, reiteration or copy-pasting without checking the validity of citations, paralleled with lack of resources/motivation to report/correct inaccurate citations contribute to their circulation.Results and discussionWe propose the development of an online annotation tool called “MyCites” as means with which to mark and map inaccurate citations. This tool allows ORCID users to annotate citations and alert authors and also editors of journals where inaccurate citations are published. Each marked citation would travel with the digital version of the document and be visible on websites that host peer-reviewed articles. In the future development of MyCites, challenges such as the conditions of correct/incorrect-ness and parties that should adjudicate that, and, the issue of dealing with incorrect reports need to be addressed. Family Medicine Groups (FMGs) are the most recently developed primary care organizations in Quebec (Canada). Nurses within FMGs play a central role for patients with chronic diseases (CD). However, this complex role and the nursing activities related to this role vary across FMGs. Inadequate knowledge of nursing activities limits the implementation of exemplary nursing practices. This study aimed to describe FMG nursing activities with patients with CD and to describe the facilitators and barriers to these activities. A multiple‐case study was (...) performed with ten nurses practicing among patients with CD in FMGs. Five data sources were used to provide an in‐depth description of nursing activities and the facilitators and barriers to the development of these activities. After qualitative data analysis, findings show that nursing activities are clustered into five domains: Global assessment of the patient, Care management, Health promotion, Nurse–physician collaboration, and Planning services for patients with CD. Activities vary depending on contextual factors identified in each case. This multiple‐case study provides a clear description of nursing activities with patients with CD. There is a need for improved nursing activities and expertise in domains of activities that are less present in FMGs, such as case management and interprofessional collaboration. (shrink)

Background The prioritization protocols for accessing adult critical care in the extreme pandemic context contain tiebreaker criteria to facilitate decision-making in the allocation of resources between patients with a similar survival prognosis. Besides being controversial, little is known about the public acceptability of these tiebreakers. In order to better understand the public opinion, Quebec and Ontario’s protocols were presented to the public in a democratic deliberation during the summer of 2022. Objectives (1) To explore the perspectives of Quebec and Ontario (...) citizens regarding tiebreakers, identifying the most acceptable ones and their underlying values. (2) To analyze these results considering other public consultations held during the pandemic on these criteria. Methods This was an exploratory qualitative study. The design involved an online democratic deliberation that took place over two days, simultaneously in Quebec and Ontario. Public participants were selected from a community sample which excluded healthcare workers. Participants were first presented the essential components of prioritization protocols and their related issues (training session day 1). They subsequently deliberated on the acceptability of these criteria (deliberation session day 2). The deliberation was then subject to thematic analysis. Results A total of 47 participants from the provinces of Quebec (n = 20) and Ontario (n = 27) took part in the online deliberation. A diverse audience participated excluding members of the healthcare workforce. Four themes were identified: (1) Priority to young patients - the life cycle - a preferred tiebreaker; (2) Randomization - a tiebreaker of last resort; (3) Multiplier effect of most exposed healthcare workers - a median acceptability tiebreaker, and (4) Social value – a less acceptable tiebreaker. Conclusion Life cycle was the preferred tiebreaker as this criterion respects intergenerational equity, which was considered relevant when allocating scarce resources to adult patients in a context of extreme pandemic. Priority to young patients is in line with other consultations conducted around the world. Additional studies are needed to further investigate the public acceptability of tiebreaker criteria. In this chapter, I delineate the central trajectories of Sloterdijk’s creative reappropriation of certain Heideggerian motives. Essentially, Sloterdijk wagers that the Heideggerian climate that weighs on our contemporary thinking is not adequate for grasping the globalised, technological world. In order to show how Sloterdijk is lead to abandon or overcome the understanding of globalisation influenced by Heidegger, I first present what could be called Sloterdijk’s onto-anthropology, that is, his story of the pro-duction or the coming-to-the-world, of the human animal. There, (...) Sloterdijk shows how the “radical openness” of the human is predicated upon an act of insulation, the building of protective spaces, called greenhouses, incubators or spheres. Understanding this interplay between distance and nearness is crucial to understanding Sloterdijk’s onto-kinetics or his description of the ontological movement of existence. While Heidegger thinks existence according to the vertical movements of falling and gathering, Sloterdijk emphasises lateral movement, an expansion on the same plane [Ausbreitung in der Ebene]. Sloterdijk’s insight into the lateral spatiality of human existence sheds light onto his interpretation of the history of humankind as history of globalisation. Here, I concentrate on the transition between the second and third of these phases of globalisation because it is this third phase which, according to Sloterdijk, is no longer explicable in terms of Heideggerian enframing and the will to will. This inevitably leads us to ask whether the globalised world requires a response different from a Heideggerian meditative thinking and poetic dwelling and what form(s) our inhabitation of the globalised world might take. In various cultural and religious contexts, from West Asia to Southeast Asia, we come across a number of quite similar creation myths in which a peacock, seated on a cosmic tree, plays a central part. For the Yezidis, a sect of Sufi origins that has moved away from Islam, the Peacock Angel, who is the most glorious of the angels, is the master of the created world. This belief may be related to early Muslim cosmologies involving the Muhammadan Light, which (...) in some narratives had the shape of a peacock and participated in creation. In a different set of myths, the peacock and the Tree of Certainty play a role in Adam and Eve’s fall and expulsion from Paradise. The central myth of the South Indian Hindu cult of the god Murugan also involves a tree and a peacock. The myth is enacted in the annual ritual of Thaipusam, like the Nur Muhammad myth is still enacted annually in the Maulid festival of Cikoang in South Sulawesi. Images of the peacock, originating from South India, have moved across cultural and religious boundaries and have been adopted as representing the different communities’ peacock myths. (shrink)

Taking precautions to prevent harm. Whether principe de précaution, Vorsorgeprinzip, føre-var prinsippet, or försiktighetsprincip, etc., the precautionary principle embodies the idea that public and private interests should act to prevent harm. Furthermore, the precautionary principle suggests that action should be taken to limit, regulate, or prevent potentially dangerous undertakings even in the absence of absolute scientific proof. Such measures also naturally entail taking economic costs into account. With the environmental disasters of the 1980s, the precautionary principle established itself as an (...) operational concept. On the eve of the 1997 Climate Summit in Kyoto, precaution, as the precautionary principle is often referred to, has now become a key legal principle in environmental law, in general, and in current international climate negotiations, in particular, attempts to understand why. It examines in turn the natural affinity between the precautionary principle and climate change, reviews a series of issues which the principle raises, and discusses avenues which it opens paper, climate change fulfills the theoretical requirements set for the application of the precautionary principle. It comes as no surprise that the actual application of the precautionary principle in the context of climate change raises high political stakes. As a result, climate change science, in particular, and science, in general, is under the fire of politically-motivated scientific skeptics. Thus, by way of the counter-measures which must be put into effect, the precautionary principle calls for a greater sense of responsibility on the part of scientists and the public at large. Specifically, from scientists, it demands perseverance in rigor, excellence in communication, and committment to education. However, even if special efforts are made to implement the precautionary principle in the context of climate change, the success of climate change mitigation will constitute no test of the validity, the usefulness, or the efficiency of the precautionary principle. Indeed, the degree to which climate change mitigation succeeds only provides a measure of our kind's ability to manage responsibly the global commons which we inherited from our ancestors and which our generation enjoys, the global commons which we will pass on to today's children and to generations to come. This essay focuses on conceptual engineers who aim to improve other people's patterns of inference and attention by shaping their concepts. Such conceptual engineers sometimes engage in a form of epistemic paternalism that I call paternalistic cognitive engineering: instead of explicitly persuading, informing and educating others, the engineers non-consultatively rely on assumptions about the target agents’ cognitive systems to improve their belief forming. The target agents could reasonably regard such benevolent exercises of control as violating their sovereignty over their own (...) belief formation. This is a pro tanto reason against such engineering. In addition to the relevant projects of conceptual engineering, paternalistic cognitive engineering plausibly includes certain kinds of nudging and evidence suppression. I distinguish the sovereignty-based concern from other ethical worries about conceptual engineering and discuss how one might justify the relevant conceptual engineering projects despite the sovereignty-based reason against them. This book offers a new approach to the analysis of the multiple meanings of English modals, conjunctions, conditionals, and perception verbs. Although such ambiguities cannot easily be accounted for by feature-analyses of word meaning, Eve Sweetser's argument shows that they can be analyzed both readily and systematically. Meaning relationships in general cannot be understood independently of human cognitive structure, including the metaphorical and cultural aspects of that structure. Sweetser shows that both lexical polysemy and pragmatic ambiguity are shaped by our (...) metaphorical folk understanding of epistemic processes and of speech interaction. Similar regularities can be shown to structure the contrast among root, epistemic and speech act uses of modal verbs, multiple uses of conjunctions and conditionals, and certain processes of historical change observed in Indo-European languages. Since polysemy is typically the intermediate step in semantic change, the same regularities observable in polysemy can be extended to an analysis of semantic change. (shrink)

Erik Midelfort's translation makes this sensational book available to English-speaking readers. In Enlightenment Underground, Mulsow shows that even in (...) the late seventeenth century some thinkers in Germany ventured to express extremely dangerous ideas, but did so as part of a secret underground. Scouring manuscript collections across northern Europe, Mulsow studied the writings of countless hitherto unknown radical jurists, theologians, historians, and dissident students who pushed for the secularization of legal, political, social, and religious knowledge. Often their works circulated in manuscript, anonymously, or as clandestinely published books. Working as a philosophical microhistorian, Mulsow has discovered the identities of several covert radicals and linked them to circles of young German scholars, many of whom were connected with the vibrant radical cultures of the Netherlands, England, and Denmark. The author reveals how radical ideas and contributions to intellectual doubt came from Socinians and Jews, church historians and biblical scholars, political theorists, and unemployed university students. He shows that misreadings of humorous or ironic works sometimes gave rise to unintended skeptical thoughts or corrosively political interpretations of Christianity. This landmark book overturns stereotypical views of the early Enlightenment in Germany as cautious, conservative, and moderate, and replaces them with a new portrait that reveals a movement far more radical, unintended, and puzzling than previously suspected. "These essays advance a reinterpretation of pivotal categories such as self-knowing, moral agency, and altruism.

I will defend explication, in a Carnapian sense, as a strategy for revisionary ontologists and radical sceptics. The idea is that these revisionary philosophers should explicitly commit to using expressions like “S knows that p” and “Fs exist” differently from how these expressions are used in everyday contexts. I will first motivate this commitment for these revisionary philosophers. Then, I will address the main worries that arise for this strategy: the unintelligibility worry and the topic shift worry. I will focus (...) on the latter worry and provide a solution that makes use of a distinction between practically and theoretically oriented beliefs. On my view, the revisionary philosophers who admit to departing from the everyday language can still criticize everyday knowledge and existence claims, by arguing that while the language embedded in these claims is suitable for beliefs-1, it is not suitable for beliefs-2. Hak Joon Lee’s The Great World House: Martin Luther King Jr. and Global Ethics examines the implications of this daily global interaction as the need for a broader ethical perspective in a shrinking world. The need begins with the realization that we do indeed inhabit a shrinking world in which our local behavior has a broader global impact. Once we recognize the moral implications of our local interaction, it becomes clear that we must learn how to live together in a broader community.But how do we speak the language of a global morality? What does a global morality look like? Lee argues that King’s metaphor of a great world house provides a structure on which to frame the content of a healthy global morality in a shrinking world. The metaphor indicates a collective understanding of all humanity as a global community (ix). The notion of a single world house moves people groups beyond rival communities with well-defined in-group and out-group barriers into what Lee describes as a global civil society where we are all seen as members of a collective in-group (13). The concept of a collective in-group helps illustrate what Lee understands to be King’s argument for a global morality; “King’s idea of a great world house redefines what the boundaries and membership of a moral community are: the great world house constitutes a single inclusive ethical community for humanity because it identifies the world as a single unified community” (53). [End Page 234]But as laudable as King’s great world house metaphor is, we are still burdened with the task of imagining a universal moral language in practice. Efforts toward the formation of universal truths have historically been death-dealing for people groups caught on the wrong side of the color line. A one-size-fits-all morality has been a procrustean bed for people who have been racialized and subordinated to white European males. Yet Lee interprets King’s method of universal engagement to begin with convictions shared at home within the particularity of his African American Christian worldview before it moves outward toward a shared vision of a global civil society. The movement from particular to universal for a public morality is unlike the historically harmful practice of universal morality. King’s universal method includes attention to four components: vision, norms, virtues, and political practices. These four components belong to what Lee depicts as a careful and intricate dialectic between what is local and what is universal in a political dialogue that seeks the end of a global civil society with shared conceptions of community and human rights.Ironically, with regard to the conversation about human rights, Lee claims that King’s formative African American interpretation of human rights was primarily informed by the Enlightenment and then blended with an African American experience: “Mostly appropriated from Christianity and the Western Enlightenment tradition, these norms [justice, freedom, and equality] obtained a new significance for African Americans in the context of the sizzling cauldron of slavery and segregation” (55). Yet one would be hard pressed to identify the presence of modern Enlightenment intellectuals within the formative theological worldview of the African American Christianity that Lee identifies. Of necessity, African American Christianity, the Christian worldview that birthed Martin Luther King Jr., was formed to deflect the white Christian worldview that was shaped by the intersection of race, religion, and empire in modernity. However, the logical means which Frege, Wittgenstein, Russell, and the generation that succeeded them, had at their disposal were those of classical mathematical logic and set-theory, and these indeed are not very suited for an analysis of other (...) aspects of meaning than those which the slogan covers. A real change in view then had to await the emergence of other concepts, which in due course became available mainly under the influence of developments in computer science and cognate disciplines such as artificial intelligence. And this is one of the reasons why it took almost a century before any serious and successful challenge of the view that meaning equals truthconditions from within logical semantics could emerge. This book explores a question central to philosophy--namely, what does it take for a belief to be justified or rational? According to a widespread view, whether one has justification for believing a proposition is determined by how probable that proposition is, given one's evidence. In this book this view is rejected and replaced with another: in order for one to have justification for believing a proposition, one's evidence must normically support it--roughly, one's evidence must make the falsity of that proposition (...) abnormal in the sense of calling for special, independent explanation. This conception of justification bears upon a range of topics in epistemology and beyond. Ultimately, this way of looking at justification guides us to a new, unfamiliar picture of how we should respond to our evidence and manage our own fallibility. This picture is developed here. In this paper, the principal objections to unconditional forgiveness are canvassed, primarily that it fails to take wrongdoing seriously enough, and that it displays a lack of self-respect. It is argued that these objections stem from a mistaken understanding of what forgiveness actually involves, including the erroneous view that forgiveness involves some degree of condoning of the offence, and is incompatible with blaming the offender or punishing him. Two positive reasons for endorsing unconditional forgiveness are considered: respect for persons and (...) human solidarity; and it is argued that the latter provides more plausible grounds for it than the former. In this paper I propose a new solution to the problem of change: situationalism. According to this view, parts of reality fundamentally disagree about what is the case and reality as a whole is unsettled (i.e. metaphysically indeterminate). When something changes, parts of the world irreconcilably disagree about what properties it has. From this irreconcilable disagreement, indeterminacy arises. I develop this picture using situations, which are parts of possible worlds; this gives it the name situationalism. It allows a B-theory endurance (...) view on which there is genuine incompatibility when things change. There are costs to the view, which are explored, but it is a novel approach which offers a distinct explanation of what happens when things exist through change. (shrink)

. Science at the interface with society is regarded with mistrust among parts of the public. Scientific judgments on matters of practical concern are not infrequently suspected of being incompetent and biased. I discuss two proposals for remedying this deficiency. The first aims at strengthening the independence of science and suggests increasing the distance to political and economic powers. The drawback is that this runs the risk of locking science in an academic ivory tower. The second proposal favors “counter-politicization” in (...) that research is strongly focused on projects “in the public interest,” that is, on projects whose expected results will benefit all those concerned by these results. The disadvantage is that the future use of research findings cannot be delineated reliably in advance. I argue that the underlying problem is the perceived lack of relevance and reliability and that pluralism is an important step toward its solution. Pluralism serves to stimulate a more inclusive research agenda and strengthens the well-testedness of scientific approaches. However, pluralism also prevents the emergence of clear-cut practical suggestions. Accordingly, pluralism is part of the solution to the credibility crisis of science, but also part of the problem. In order for science to be suitable as a guide for practice, the leeway of scientific options needs to be narrowed – in spite of uncertainty in epistemic respect. This reduction can be achieved by appeal to criteria that do not focus on the epistemic credentials of the suggestions but on their appropriateness in practical respect. (shrink)
